I'm sorry , but I think genetics play the biggest part. There was a 35 year old Dr near where I live. He ate perfectly- was a vegetarian. Didn't drink or smoke. He went jogging every day and was in perfect shape. He was the perfect picture of health. One day he went out for a jog and dropped dead on the sidewalk.
Very good on fiber versus refined carbs, but I think she missed something on the endotoxins, or is describing a later and secondary effect when the large intestine is inundated with refined carbs.
The first route of endotoxins into the blood comes before this with ingestion of foods containing them, principally meat and animal products. A study done in 2011 showed that endotoxin load skyrocketed shortly after eating endotoxic foods--long before they could reach the large intestine where the native bacteria live which Dr. Patrick theorizes as the endotoxin source. She may be right in part here, but the smaller, secondary part.

Dude, at the age 7 i knew margarine was not a butter and was bad for you )) In fact, everyone in Russia (my home country) knows it. Margarine is only used for baking just to keep it cheaper. You can't have a meat based diet and not have chronic inflammation, they go hand in hand. The more meat you eat, the higher your c reactive protein and inflammatory markers. She is not a Medical Doctor. I wish she wouldn’t ACT like one. Example: she claims 80% of people with heart disease have it because of their diet. Not even close! Smoking, inactivity & genetics are HUGE risk factors for getting heart disease. SMH...
Did not feel she is/was acting like an MD. I believe she does address those other risks as well in the long version of the 'cast (at least, I have heard her mention this before but maybe not in this one). Have you listened to the longer version of this podcast? It's pretty good I thought. And anyways I'd say she's likely got a much deeper understanding of how the biochemistry of the body works than many MD's (who are also great and wonderful people I know :). Your point is well taken though, smoking and genetics are huge factors as well as inactivity (although I'd be hard pressed to find anyone who could look me in the eyes and say that those factors aren't unhealthy, so it's likely fairly widely understood, whereas the level of lipid knowledge we have is quite low). I think the purpose here was to really drive home the point that Saturated fats are not the evil we've been taught to believe. Peter Attia (himself an MD you'll be pleased to know) interviewed Tom Dayspring (also an MD) on his podcast 'The Drive' and did a 5 part series on lipids, it is super fascinating! I think any physician would be well served by listening to it, although it is highly technical and time consuming. Anyways, have a good day, I'm sure it is frustrating to you as a clinician to hear non-clinicans speak about these topics however she has some real gems in here. I could be wrong :)

How many persons alter their diet so as to not have 'gas', they alter their diet so as to not fart? Farting is most probable the gassing of fermentation in the gut, this is not something that in todays 'inside the box' living is likely to embrace. Cancer rates are through the roof, could it be - in rural settings, due modern farmers embracing the routine application of toxins while in cites due constricted diet and eating trace amounts of those toxins and so much more? Chew over this food for thought.
I don't know what type of doctor is she, but she is semi-educated on the subject. She is making several mistakes when trying to explain the fundamentals and also has poor understanding of lipoprotein complexes, their transport, metabolism, tissue-interaction. Its just semi-educated abra-cadabra. Any medical doctor of any speciality(like myself) can debunk this bs for you. Don't seek proper knowledge on a podcast😀
