Can nasal steroids & antihistamines help in curing rhinitis? - Dr. Sreenivasa Murthy T M
Both antihistamines and corticosteroids are both medications used in allergic rhinitis. They are not cures. If you are allergic to something you may not get cured, you may outgrow the problem or may stop reacting to the particular allergen after few years of exposure. But mediation giving you cure is unlikely. The first line of treatment for allergic Reaction is antihistamines. Antihistamines are tablets like cetirizine, levocetrizine, fexofenadine. These are antihistamines that we use to control symptoms of allegoric reaction like sneezing, itching, watery eyes, runny nose and nasal obstruction. A minority of the patients, they do well only with antihistamines. The problem with antihistamines is they have certain side effects like you can feel drowsy. There are people who feel their mouth and nose an upper airway feeling very dry. Some don’t tolerate the medicines too well and can affect their morning activities and they don’t prefer using it. Two, is they act only for a certain duration of time, that is you take a tablet for today, then it may act only for 24 hours and if you don’t take the tablet tomorrow, and at the end of the day they become symptomatic. So there is a limitation of the medication’s duration of action. So long as it is there in your blood stream, your symptoms are under control and once it is secreted out of your system within the next 6 hours if you are exposed to the same allergen you are again symptomatic. So you need to be on mediation as long as you are allergic and you are symptomatic. There is a subset of people, who don’t do well with only one antihistamine, then you need to be on other medicine like corticosteroids, which are available as nasal sprays. So certain patients a combination of antihistamines, along with steroids help. Steroids are anti-inflammatory drugs. So they also act on the allergy pathway and they act topically, that is they act only on the nasal mucosa and reduce the allergic response to whatever allergen they have exposed to and hence reverses the symptoms. How long they have to be used for what duration, what are the side effects, you need to sit with your allergy specialist and ENT surgeon and they should be able to explain.

A ketogenic diet is not a high protein diet, it's only consuming 3-6 oz of protein with each meal. There is a condition called ketoacidosis, which is different that nutritional ketosis, and your pH does not raise to any danger levels. Keto can also increase the uric acid, but if you consume enough vegetables, again, you will be fine. The potassium in vegetables is very kidney protective. But high sugar, and high insulin is damaging to the kidney more than anything else. Take a look at a diabetic - they usually end up with long term kidney damage. GMO foods can also effect the kidney, so eat organic. I had another question relating to does ketogenic diet cause kidney disease. Number 1, ketogenic diet is a very natural thing for the body to do. In fact I believe that is was the very first diet that we all were on and our bodies are very used to it. It is a much better fuel running your body on ketones. And the confusion is that a ketogenic diet is a high protein diet and of course high proteins can damage the kidney and the liver as well but it is not but it is not a high protein diet and you are only doing 3 to 6 ounces of protein with each meal. The ketogenic diet is a low carb diet and low carbs are very beneficial to the kidney. High carbohydrates and high sugars are very damaging to the kidney. Let’s take a look at the diabetic, they usually have vision problems, nerve problems and eventually kidney problems and they usually end up kidney dialysis because that amount of sugar is very damaging to the kidneys. High carbohydrates destroy the kidney and low carbohydrates are very safe for the kidney. There is a condition when you have diabetes called ketoacidosis and that means that the pH coming from those ketones and this is from the very unhealthy ketosis by the way. It is very different on the type of ketosis I’m talking about. It is more of a side effect because the body is burning up protein and is turning on ketones and the pH is coming very acid. But you don’t have to worry about that because when you are actively doing just by cutting out the carbs and you are not a diabetic type 1. I’m gonna recommend and I am recommending a lot of vegetables to get your potassium and potassium is very protective on the kidneys. Until you have complete kidney failure and you have kidney dialysis, in that situation you don’t want to consume a lot of protein because you can’t really utilize that protein that’s a completely different situation. But in general, potassium is very protective on kidney and that is why we recommend 7 to 10 cups of vegetables with the ketogenic diet plan to protect the kidney and give it the nutrients that it needs. Another problem with keto is that it increases the uric acid. It is not gonna do that if you keep your vegetables high, but some people can get uric acid stones, gout, simply because they are not consuming enough vegetables. Does Your Child Have Food Allergies?  Perspectives on giving "treats"
It can be hard enough to navigate parenthood without the added emotional and physical issues that surround parenting a child with food allergies or food intolerances. One of the things that we have to deal with as parents is our desire to not have our children feel left out or isolated when they are eating foods that are different than everyone else around them. It is so easy to fall into the habit of giving "treats" of the foods that cause our children to react so that WE feel better about it. Here are a couple of reasons why that may not be the best approach: 1. Your child will pay the price for days, sometimes weeks for even the smallest infraction in diet. We don't always make the connection to a meltdown a week after an offending food was taken in but that is exactly what can happenl 2. When your child is under the influence of a reaction to a dietary "slip" or "treat" your child many times looses the power to interpret and interact with life in a way that they normally would. They find themselves unable to "behave" in a way that we want them to. 3. By continually re-introducing the offending food into the body you prolong the body's healing process. The goal in eating a restrictive diet should not be to merely control symptoms but the allow the body to heal the cause. The body can absolutely heal from food allergies, but the road is not always easy. 4. Many times we are giving in to "treats" because of feelings of OURS that we are projecting onto our child. We make the assumption that our child feels and thinks the way that we do. 5. Your child may be sad for a moment because of not getting to eat a piece of cake or candy but they could be dealing with the consequences of doing just that for days and weeks after taking in the foods that cause reactions. I know that this video is a little rambly and covers so many different aspects of what happens when we slip out of the diet that we know that our children can safetly eat. It is a big topic and I will break down each point and talk more about them in future videos. Hydroxyzine belongs to the class of medications called antihistamines. It is used to relieve itching and other symptoms caused by allergic conditions. It can help to alleviate itching due to an adverse reaction to opiod medications. It works by blocking the actions of one of the body's natural chemicals known as histamine. Histamine is responsible for many of the symptoms seen when an allergic reaction occurs. The calming effects of hydroxyzine result from its sedating side effect. It is also used to manage anxiety and tension caused by situations such as dental procedures or acute emotional problems. Hydroxyzine is sometimes used as part of the treatment of alcoholism. Hydroxyzine is also used to control nausea and vomiting (except in pregnancy). Your doctor may have suggested this medication for conditions other than those listed in these drug information articles. As well, some forms of this medication may not be used for all of the conditions discussed here. If you have not discussed this with your doctor or are not sure why you are taking this medication, speak to your doctor. Do not stop taking this medication without consulting your doctor. Do not give this medication to anyone else, even if they have the same symptoms as you do. It can be harmful for people to take this medication if their doctor has not prescribed it. For children under 6 years of age, the daily dose when taken by mouth is 30 mg to 50 mg in divided doses. For children over 6 years of age, the daily dose is 50 mg to 100 mg in divided doses. Hydroxyzine may be taken with or without food. Many things can affect the dose of a medication that a person needs, such as body weight, other medical conditions, and other medications. If your doctor has recommended a dose different from the ones listed here, do not change the way that you are taking the medication without consulting your doctor. It is important to take this medication exactly as prescribed by your doctor. If you miss a dose, take it as soon as possible and continue with your regular schedule. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
