(Inside Science) -- Chronic pain can be a debilitating condition, impacting everything from mood and health to overall well-being. Unfortunately, many treatment options aren’t very effective. Worse, commonly prescribed drugs like opioids are highly addictive. They can cause nausea, sleepiness and other side effects. And they are potentially deadly. According to the National Institutes of Health, more than 115 people die each day in the United States from opioid overdose. It’s become a public health crisis. But what about using marijuana for pain relief? Could that help address the opioid crisis? There is some evidence to suggest that cannabinoids -- a class of chemical components in marijuana plants -- can be effective in alleviating pain, either alongside or in place of opioids. As medical marijuana becomes more accessible in the U.S., it may serve as a safer option for some kinds of pain relief and help reduce the number of people becoming addicted to opioids. But some patients are receiving medical advice from their local dispensary, a trend some doctors are worried about. “Instead of their provider giving them information on the cannabis in terms of pain relief, they were actually getting their knowledge or obtaining their knowledge from people that had cannabis shops or marijuana type shops,” said Jacquelyn Bainbridge, a clinical pharmacy specialist at the University of Colorado Denver. Clinical trials are currently underway to examine dosage, side effects, methods for administering marijuana, and its effectiveness at relieving pain. Armed with clinical evidence, doctors can make reliable recommendations for its use as an alternative to opioids. “So, we don't know exactly what to prescribe, so we need to know what patients are actually doing, so that we know better what to prescribe, or physicians know or health care practitioners know what to prescribe,” said Bainbridge. “The other big piece is, really, what about drug interactions? We know that there are some drug-drug interactions that occur. We need to have a better handle on what those drug-drug interactions actually look like in order to balance the patient’s other medications that they may be taking. So that’s a big ‘if’ out there. And this kind of research is really important to have done in a well-controlled fashion because you know exactly what the patient is getting, so you know the THC component, you know how the methods are delivered,” said Bainbridge. Cannabis comes in many different forms -- patients can smoke it, eat it, vape it or use it topically on the skin. Also, different products have different amounts of THC or CBD -- the two most abundant cannabinoids found in cannabis. They both interact with the body’s endocannabinoid system but produce different effects. Doctors are now learning what works best for individual patients. “Are they consuming a THC product with a little bit of CBD? Are they utilizing a product that has more CBD and low THC? And basically, I think figuring out that ratio for a specific disorder or disease state is also really essential. So, we’ll never know unless we actually conduct the research to figure all of that out,” said Bainbridge. The bottom line -- will pot replace opioids for pain? “I don't think it will completely replace opioids for certain types of pain. But I think -- I think it probably shares a role. So, it may decrease the opioid use overall. And we might find out that it’s a safer option for patients than an opioid prescription, which obviously can be lethal for many patients,” concluded Bainbridge.

How to Fix a Bulging Disc (NO SURGERY!)

In this video, I’m going to show you how to fix a bulging disc in your lower back without requiring surgery. In just 3 simple steps you are going to be able to get rid of the pain that is wreaking havoc on your training and making your life miserable. The first thing that needs to be discussed however are the terms used to describe the condition. Some people refer to a bulging disc as a herniated disc. In both cases, we are talking about a scenario where the inner disc material has protruded from the disc but has not reached a point of no return. The point at which the disc can no longer be repaired without surgery is more accurately called a ruptured disc. Bulging discs are a very common problem and one that we all might have to some degree if we were to MRI our backs. The thing is however, just because we may have a disc that is bulging does not mean that we will have symptoms. The only time it starts to matter is when the presence of the disc starts to become symptomatic and interferes with your life or workouts. The good news about disc issues however is that 98 percent of them are non-operative and solved with a dedication to just a few simple steps. One of those steps however is not stretching the lower back. This may come as a surprise to you, especially if you feel that your low back has become tight or sore as a result of your disc issues. Far too many people are told to lay on their back and start pulling their knee or knees up to their chest to stretch out the tight muscles in the low back. They may even be told to get on their knees and sit back on their heels to stretch out all of the muscles of the back. Both of these would be horrible advice because they not only aren’t treating the cause of the problem but they are aggravating the actual cause and making the disc protrude even more. What you need to do is focus on fixing the disc and recentralizing it through extension. If you can do this, the spasm that you are experiencing in the low back muscles as a result of the herniation will resolve once the disc itself is fixed. There are 3 parts to fixing this issue however and it starts with a decompression of the spine to allow for more room for the nerves that are being compressed by the disc protrusion. This can easily be done by hanging from a pullup bar with your toes slightly in contact with the ground. Let the pelvis drop and feel the spine open up. Do this for about 30 seconds to a minute and then proceed to step two if you need it or just skip to step three if not. Step two is a list correction. This is needed if you find that you are leaning to one side because of the pain of the protrusion. This can easily be done against a wall and will help to put the spine back in the proper alignment prior to moving on to the final step. Lay on your stomach with your hands under your shoulders. Press down on the floor and lift your upper torso off of it. Be sure to keep your hips in contact with the ground to avoid overextension of the lumber spine. Do this a few times a day and always within a zone of comfort. Before you know it, your disc will be centralized and your pain and symptoms down the leg and in the lower back should subside. You will be ready to start attacking your workouts again in no time. The program covers the Guideline recommendations in the primary health care setting and focuses on short term care, long term care planning and management, and multidisciplinary co-ordination. It outlines the evidence regarding effective management of symptomatic hip and knee osteoarthritis in adults up until referral for joint replacement. Best practice management covers the optimal use of medicines; non--pharmacological conservative management; and care and referral pathways. This program is one of four in the Musculoskeletal Guideline Series.
