Local Anesthetic Infiltration - everything you need to know

Lately, I have had many questions about the local infiltration of lidocaine... when to use epinephrine? what is buffering and...does it help? how much volume is toxic? what are the symptoms of toxicity? and other similar questions...that made me realize I could use a refresher on this information, so I went to study and make this video review on the subject. We will start from the very basic to very advanced concepts, I hope you enjoy it and learn a lot from it. INFILTRATION ANESTHESIA What is it? It involves the injection of anesthetic agent directly into the tissue prior to surgical manipulation. A field block involves a subcutaneous (SQ) injection of an agent in such a manner as to create a field of anesthesia around the operative site. Indications and Contraindications It can be used for the majority of minor surgical procedures such as excision of skin lesions, incision of abscesses, and suturing of wounds. An advantage of this over nerve block and general anesthesia is that it is considered both quicker and safer. Local infiltration can also help with hemostasis (bleeding control), both because it directly distends the tissue and because one can use epinephrine. A major disadvantage of local infiltration, at least when compared to nerve blocks, is that a relatively large dose of the drug is needed to anesthetize a relatively small area. For large wounds, the amount of anesthetic required may risk systemic toxicity. The maximum allowed volume can be increased by adding epinephrine, using a lower concentration of the anesthetic, or both. Another thing to consider is that local infiltration also distorts the tissue that will be incised or repaired and may to be desirable in areas requiring precise alignment (like a cosmetic repair in the face or lip). Choosing the anesthetic agent Some of the more common local anesthetics are lidocaine, bupivacaine, and procaine. Lidocaine is by far the most commonly used because of its excellent activity profile, low allergenicity and toxicity, user familiarity, and ready availability. PROCAINE - is useful for patients who are allergic to amide anesthetics. Because of its prolonged duration of action, bupivacaine is considered by some physicians to be the preferred anesthetic, especially when postoperative analgesia is desired, for prolonged procedures, or short procedures in the ER that may be interrupted. Lidocaine generally lasts for about 1 hour, while bupivacaine can last up to 6 hours. So...if bupivacaine lasts 6X longer, then why not use it always? Well, there is the potential for injury if the patient cannot feel that part of the body, also having a numb area is also very annoying and can cause patients to 'play around' with it, therefore damaging the tissue. In addition, the pain of administration of bupivacaine is more, and the chances for cardiotoxic and CNS side effects is more. How can you increase the anesthesia time of duration? You can add epinephrine and/or bicarbonate to the lidocaine. By adding epinephrine you also decrease the peak blood level absorption of the medication, therefore decreasing the chances of a toxic reaction, and allows a greater volume to be injected in larger lacerations. However adding epinephrine to lidocaine lowers its pH (makes it more acidic) therefore increasing the pain of local injection. How much volume can be injected safely? For Lidocaine - it is 300 mg in an adult. That is 60 mL of the 0.5% concentration. 30mL of the 1% concentration, and 15mL of the 2% concentration. For Bupivacaine 0.25% it is 175 mg which comes out to 70 mL. If you add epinephrine you can add 50% to the total volume of injection. How long does the different local anesthetics take to numb? They all have the same onset to action. 2 to 5 minutes. COMPLICATIONS of local anesthetics WOUND HEALING - local anesthetics produce cytotoxic effects on cell structure and function in a dose- and time-related manner. Collagen synthesis, for example, is inhibited. At higher concentrations of the anesthetic, the tensile strength of the wound is progressively reduced. Adding epinephrine can cause a further decrease in this tensile strength. Conclusion - if tensile strength is important, don't use epinephrine. If the patient has a poorly healing wound or history of the same, then avoid local anesthetic altogether. Wound Infection - it has long been known that local anesthetics possess antimicrobial activity, at least in vitro.

i was at work when i made this video, so there is no commentary...read the description for a general step by step of what you're seeing. my mom asked me if i was doing this safely...so i took my ipod to work and took a video of myself drawing two 1.5ml syringes of lidocaine. i work in a dermatology clinic and use this stuff every single day. lidocaine is a local anesthetic used to numb patients before procedures...in my case they are biopsies, both punch and shave. At first i am fumbling around getting out equipment that wasn't on the table when i started and what not, but the procedure is pretty straight forward. the red needle tip is called a blunt fill, it's significantly less sharp than a regular needle that is used only to draw up medications. The yellowish-looking tip that i screwed on after is the actual needle, an 18g (very small) needle about a half inch long used to actually inject the medication into the patient. the yellow plastic part covers the needle at all times when it is not being directed into the patient's skin. When i started the procedure you'll notice that i uncapped the lidocaine and then wiped it down with an alcohol swab...you don't have to do so with a brand new container but i make it a habit to do it always, and thus not forget to do it when you're re-using a vial of the drug. When drawing, it's best to inject the vial of medication with air, equal to the amount of fluid you would like to withdraw first, which is what you didn't see me doing when i was taking my time on the first syringe. the second one i did not do it...it's really not entirely necessary, just helps avoid air bubbles. When you actually draw the drug out of the vial, pull out an extra half a ml or so, and then inject it back into the vial. this takes out any air bubbles that may be in the syringe. if there is an air bubble stuck on the plunger, flick it a few times as i did on the first one before injecting the extra medication back into the vial. After that you're ready to go...throw out all sharps in a sharps bin, don't uncap a needle unless you're ready to use it...and for god's sake put on gloves and swab the patient's injection site with alcohol before you administer anything with a syringe. Hey guys so in this vlog I share with you my experience with IV Lidocaine and Magnesium to help manage and control my Chronic Pain. Lidocaine is administered in an IV, the Lidocaine will help to block and freeze the pain receptors so they stop firing so many pain signails. As you would see in the video i didn't have the best experience, This IV occured in November, Its now mid December and I'm not in as much pain as usual, I am considering asking my Chronic Pain doctor to let me give it another shot for pain mangement, I think my body may have had a delayed effect to it. The burning is dependent on the rate of injection and the acidity of the solution. Hey y'all! If you get kind of squeamish towards needles or blood, I advise you to not watch this. I was recently in a car wreck, and my back was in a lot of pain. I got injections of lidocaine and saline, and it HURT! You could probably tell that I was in pain due to the groaning lol. The injections were so horribly painful, and the lidocaine has since worn off, so my back pain has returned. I hope you enjoy this, in some way, and maybe any future doctors that see this are able to get some idea of a procedure like this one. Thank you!

Injections Around The Shoulder - Everything You Need To Know - Dr. Nabil Ebraheim

Dr. Ebraheim’s educational animated video illustrates injection techniques of the shoulder rotator cuff muscles tear. Injection of the shoulder could be part of the treatment of shoulder pain, especially if the pain is more than what the patient can handle. Shoulder pain will probably hinder the progress of therapy. Which patient is the one who we should inject? 1- Patient has pain with restricted shoulder movement. 2- Patient that has nighttime pain. 3- Patient cannot lie on the shoulder due to pain. Injection is predominantly used for elderly patients with rotator cuff tears or patients with impingement syndrome. Injection usually reduces the pain and inflammation. It can be done either blind or ultrasound guided for injection around the shoulder itself. If the injection being done is for the scapulothoracic joint, give the injection either fluoroscopic or blind. The injection could be done by a posterior, lateral, or anterior approach. I use the lateral and posterior approach. The posterior approach is done 1-2 cm below the posterolateral acromion. I mark the spot for the injection, then I introduce the needle and inject the fluid. The blind injection usually not accurate. Some of the data reports that the accuracy is between 65%-75% is probably not true (probably worse than that). Make sure you do not inject the rotator cuff tendon itself with steroids. When you do blind injection, you probably are injecting to the tendon and you do not know it because you cannot see the tip of the needle. When you use ultrasound guidance, you must see the bursa and find the tip of the needle and distend the bursa. You may do manipulation of the patient’s shoulder manually after administering the injection. When do I give a blind injection? I usually do blind injection the first time that I see the patient, especially if the patient is elderly or if the patient has an impingement syndrome. I will use ultrasound guided injection when patient has severe pain and some restriction of shoulder movement, or if the patient has had previous shoulder surgery. There are multiple points of pain, especially in the shoulder itself such as biceps tendon and AC joint. I personally examine the patient before and after injection. It is a very rewarding experience to see that the patient’s condition improved after ultrasound guided injection. I usually inject steroids, 40 mg kenalog with about 10 mg lidocaine. Cortisone will give short term yet, reasonable relief in some patients. The numbing medication will cause the patient to feel less pain immediately. However, the steroids can negatively affect the tendon and cartilage, causing tendon damage and rupture. The shoulder is not flat. The shoulder is a ball and socket joint and when you inject above the ball (humeral head), you are controlled by the shape of the ball. You may be injecting the tendon despite any good intention of only injecting the subacromial area. You may be unable to reach the area of the subacromial space due to the shape of the proximal humerus. Even with ultrasound guidance, I still have to make multiple modifications to see that the needle actually in the bursa. How many times do I give the patient an injection of steroids? Usually about 3-4 times a year. PRP: has a high level of concentrated growth factors which may help in healing the tissue. A blood specimen is taken from the patient, centrifuged, and then platelet concentrate is obtained and activated before injected in the target area. PRP: is probably good for young, active patients who play sports. These patients may have partial or intrasubstance rotator cuff tears that causes pain which limits activity. PRP may help these patients to avoid surgery and allow for healing of the rotator cuff tear. PRP is usually injected with ultrasound guidance because we can’t afford to do this blindly. We need to be able to see where the problem area is and inject the PRP into the area of this problem. The most common cause of a lethal dysrhythmia is by a PVC hitting the peak of the T wave (known as R-on-T Phenomenon). This causes a pulseless shockable or non-shockable rhythm in at risk patients. 90% of cardiac arrests in adults are shockable (Pulseless VTach/Ventricular Fibrillation). This rhythm started with multifocal PVCs to a wide complex tachycardia (Pulseless VTach). This can only last from a few seconds to a few minutes before it becomes Coarse Ventricular Fibrillation (if no CPR and Defibrillatory shocks were administered). Coarse VFib can only last for about 4-5 minutes before it becomes Fine VFib. From then on, you have seconds for Fine VF to become Asystole. INTERVENTIONS: Based on AHA 2016 Guidelines For VF/pVT: - High quality CPR - Defibrillate 120-200 J biphasic or 360 J monophasic every two minutes - Give Epinephrine 1 mg every 3-5 minutes, followed by a 10-20 mL flush of NS/LR - May give Amiodarone 300 mg IV/IO bolus if refractory (if patient is not hypokalemic) - May give Lidocaine 1-1.5 mg/kg IV/IO bolus if refractory (if patient is hypokalemic) For Asystole/PEA: - High quality CPR - Give Epinephrine 1 mg every 3-5 minutes, followed by a flush of 10-20 mL NS/LR - Correct possible causes (H/Ts) Possible causes: - Hypoxemia - Hypo/Hyperkalemia - Hydrogen Ions (Acidosis) - Hypothermia - Toxins - Tamponade, Cardiac - Tension Pneumothorax - Thrombosis, Pulmonary Embolism - Thrombosis, Acute Coronary Syndrome If ROSC (Return of Spontaneous Circulation) 1. Optimize ventilation and Oxygenation - Advance airway placement - Ventilate 10-12/min - Keep O2Sat greater than 94-99%, PETCO2 35-40 mm Hg 2. Its been an extra rough week for me & my unwanted "sidekick" Cluster Headaches. I've used my Imitrex injections several days in a row & I'm trying not to use them so much. Sometimes the Lidocaine liquid up my nose & some oxygen will calm them down enough to be tolerable, but I have to be careful not to be to active once I start to feel better because that can bring it right back. Sometimes the Imitrex shots are the only thing that works, although even it fails to work in extreme Cluster Attacks & I have to suffer through them screaming in agony until I wear myself out & fall asleep from exhaustion.
