These sophisticated tests are performed on a small sample of cancer tissue in appropriate patients with early stage breast cancer. Genomic tests are usually ordered after surgery when the pathology report is finalized. It measures unique aspects of the tumor to determine if a patient will benefit from chemotherapy in addition to hormonal therapy. Such “genomic assays” developed over the last decade are a dramatic advance in breast cancer care. The Oncotype DX assay by Genomic Health Inc. is the most utilized genomic assay of those available in the United States. Who should consider a genomic test? Patients who have small “Estrogen receptor positive” (ER+) and “HER2 receptor negative” (HER2-) tumors and no evidence of cancer in their lymph nodes may benefit from an Oncotype DX assay. The purpose of this test is to better identify people who do and do not benefit from chemotherapy. The decision to undergo chemotherapy is a complicated one. Your medical oncologist will examine multiple factors to help determine if you will benefit from chemotherapy. The NCCN Guidelines, listed in the website links below, outline in much greater detail recommendations for the use of genomic tests. An Oncotype DX test can be instrumental in this decision for many patients. You may qualify for a Genomic Assay if… You have early stage cancer (Stage I or II) Your tumor is Estrogen receptor positive (ER+) Your tumor is Her2 receptor negative (HER2-) No cancer was found in your lymph nodes You are willing to consider having chemotherapy You are healthy enough to undergo chemotherapy How is chemotherapy tailored to patients? Genomic breast cancer tests are a leap forward in our ability to “look inside” breast cancer cells. Sophisticated breast cancer care is based upon the principle of providing maximal benefit from the least toxic therapy. Newly diagnosed breast cancer patients deserve the best information available to decide whether they need chemotherapy. Take our video lesson on “Will I Need Chemotherapy?“ (here) to understand the general concepts. Genomics is a promising and rapidly developing field. Take Home Message: Make sure to ask both your breast surgeon and medical oncologist if a genomic assay might play a role in your treatment decisions. For appropriate patients, these tests should be considered only one piece of the many “pieces of the puzzle” in deciding treatment decisions about chemotherapy and hormonal therapy.
Pill for Better Diagnosis of Breast Cancer | Michigan Engineering

Researchers from the University of Michigan have developed a diagnostic imaging pill enabling the early and more accurate diagnosis of cancerous tumors in breast tissue. The pill contains a targeting molecule attached to a near-infrared fluorophore that, when absorbed into the bloodstream, will target cancerous tumor cells in breast tissue. A doctor could then use desktop fluorescent imaging equipment to see if any of the targeting molecules bound to any cells in the breast. This new screening method would be cheaper and more effective than current approaches. The research was conducted in the lab of Greg Thurber, an assistant professor of chemical engineering, and was led by Sumit Bhatnagar, a PhD student in chemical engineering. The project was a collaboration between the College of Engineering and the College of Pharmacy. Kirti Verma, Yongjun Hu, Eshita Khera, Aaron Priluck and David Smith also contributed to the work. Scientists at the Institute of Cancer Research in London are growing mini tumours in the lab to test the effectiveness of cancer drugs. For this a biopsy of the cancer was removed from the patient. The sample then received the same treatment as the patient to compare outcomes. In 100% of cases where the drug did not work for the mini tumour, it was also not effective for the patient and in 85-90% of cases where the drug was shown to be effective for the mini tumour it also showed an effect for the patient. In the future, these mini tumours may help doctors determine the best drugs for cancer patients and help them avoid unnecessary treatments. An analysis of more than a dozen studies indicate that prostate cancer patients do better with surgery vs. radiation treatment; especially men with localized prostate cancer. Patients treated with radiation are twice as likely to die from prostate cancer, one & a half times likely to die sooner than prostate cancer patients treated with surgery & exposure to radiation may end in secondary cancers such as bladder cancer and rectal cancer. Dr. David Samadi explores the details. When men are diagnosed with prostate cancer it can be extremely confusing at times picking the right treatment option as there’s a lot of information out there. Dr David Samadi says that with any cancer, surgery should be the first line of treatment. With prostate cancer surgery patients get accurate staging – i.e. how much cancer a patient has, has the cancer spread to surrounding tissues & the PSA levels which has always got to be zero. Dr. David Samadi explains that there’s a lot of misleading advertisements indicating methods such as CyberKnife is robotic radio surgery which in fact is not but only a different form of radiation treatment. He says that patients are led to believe that radiation is a viable first line of treatment & if the cancer comes back, it is extremely difficult to perform surgery after radiation. Dr. Samadi also explain why it is extremely important to weigh in the experience of the surgeon as working with the right doctor would ensure a better and faster recovery. Dr. David Samadi’s patients have a continence percentage of 95% - 97% & regular sexual functionality in about 80% of prostate cancer patients. Some prostate cancer facts: • There are over 221,000 men diagnosed each year • Almost 21,000 die of prostate cancer • If the cancer is spread outside the prostate gland, the chances of a man living longer than 5 years becomes less than 30% • If the prostate is surgically removed, the survival rate becomes almost 100% Dr. David Samadi also clarifies if there are genetic components where certain males are predisposed to having prostate cancer, why is it important not to just look at the PSA as a number but to follow a trend pattern. It’s explained what factors have to be monitored closely & at what age / PSA levels a patient should start consider a biopsy or not. By 2030 cancer is expected to overtake heart disease as the nation’s number one cause of death, but state-of-the-art facilities like the new James Cancer Hospital and Solove Research Institute at Ohio State are helping doctors and researchers in their efforts to stay one step ahead. Important risk factors for breast cancer are age, gender, reproductive history, hormonal factors, and family history. Although a family history of breast and/or ovarian cancer is common in patients diagnosed with breast cancer, less than ten percent of all breast cancers are associated with genetic mutations. Assay of hormone receptors (estrogen [ER] and progesterone [PR] receptors) is an important component of the pathologic evaluation of breast cancer, for both prognostic and predictive purposes, as patients with hormone receptor-positive tumors benefit from the addition of endocrine treatments. The treatment of early stage breast cancer includes the treatment of locoregional disease with surgery, radiation therapy, or both, and the treatment of systemic disease with one or a combination of chemotherapy, endocrine therapy, or biologic therapy.
