What is Clomid, and what should I know about it?
In order for a woman to get pregnant, she needs to ovulate, or release an egg that can then be fertilized by sperm. And in order for pregnancy to happen, intercourse needs to be timed just right with ovulation. There are some medical conditions out there that affect a woman's ability to ovulate. For example, one of those is polycystic ovarian syndrome, or PCOS. If you haven't been having irregular cycles, or you're not ovulating, and you know that because you've been testing, then you should talk with your doctor, and they'll be able to recommend a plan - something that will help to stimulate ovulation and hopefully increase your chances of conceiving. One of the treatment methods used commonly for women who aren't ovulating is Clomid. It's a medication that stimulates your body to release an egg. To increase your chances of conceiving, you need to start having intercourse every other day, 5 days after your last dose of Clomid is taken, and continue with this for a week. And if you do this, then hopefully things will be timed just right. In conjunction with Clomid therapy, your doctor will probably recommend checking for a luteinizing hormone surge, or LH surge, and that's associated with ovulation. You can detect this by using ovulation kits. And as soon as you're seeing a positive, that gives you about a 1 to 2 day window. So if you weren't already going to have intercourse during this time, then you need to plan on it. There are some potential side effects associated with Clomid use, and one is the risk of multiple gestation. For some people, that's not a risk, and that would be welcome, especially if they've been trying to have babies for a while. But you should know, when you take this, there's a slightly higher chance that you could end up with twins, or maybe even triplets. The chance of twins is about 7% to 8%, and that's higher than the normal population that isn't taking Clomid. Other side effects include headache, nausea, vomiting, breast tenderness, abdominal pain, bloating, mood swings. And many of these things are actually associated with pregnancy too, so it makes women feel extra hopeful, when they've been taking Clomid, that they're pregnant, but these symptoms, if they're associated with pregnancy won't happen until about 5 to 6 weeks gestation, or about 5 to 6 weeks from your last period. So you can expect to feel things if you've been taking Clomid. Hot flashes are also common. But if you experience anything that seems abnormal to you, call your doctor, and they'll decide if it's a side effect of the medication, or if further investigation is necessary. Now as for how many doses you might take, a lot of women do successfully get pregnant and conceive when they take Clomid, but sometimes it's not effective. And sometimes it takes multiple rounds, even when it does work. So it may not work the first month, or even the second, or even the third. Your doctor may recommend trying for a few months to see if it helps, but after about 6 months, studies have shown that pregnancy rates actually fall significantly for women who have been using Clomid for that amount of time. And at that point, a doctor should look into other reasons for infertility. Maybe in addition to not ovulating, there's fallopian tube blockage. There's so many different factors that go into a woman and a man being able to conceive, so they might want to investigate other possibilities, other underlying conditions that need to be addressed. Is it possible to get pregnant when using Clomid for the first time?
You mentioned that you're taking Clomid to increase your chances of conceiving, and you're on round 1, and you're wondering how likely it is that you're going to conceive. And Clomid, in general, is given to women who have had issues with infertility, they've been to the doctor, and it's been determined that they're not ovulating. And so by giving the woman Clomid and stimulating ovulation, it increases her chances of having a baby. Studies have shown that 80% of women who take Clomid actually ovulate, and 30% to 40% of those women get pregnant. Even when intercourse is timed just right with ovulation, everything is as optimal as possible, the chance of pregnancy is only 30%. And a lot of people think that it's a 100% chance, but it's not. There's lots of other variables. And so this is right in line with the studies that show that women who take Clomid for ovulation, 30% to 40% of them go on to actually conceive a baby. Now there are some other questions that people have about Clomid, like - does it increase the rate of miscarriage or multiples? And the rate of miscarriage after taking Clomid is about 20%, and that's the same as the general population. Women who didn't need Clomid to conceive actually miscarry about 20% of the time as well. So that's no different. But there is a higher chance for multiples. About 8% to 14% of women who take Clomid to stimulate ovulation end up getting pregnant with multiples, meaning twins, sometimes triplets. And so that risk does exist, but for some people, that's welcome. The chance of that happening is great, because they've been trying so long to have a baby. So I sincerely hope the best for you during this round, but if it doesn't happen this time, then your doctor will talk to you about what should be done in the future. Sometimes multiple rounds are tried, but studies have shown that after 6 rounds, the chance of pregnancy actually goes down. So your doctor will work with you to decide if further investigation or treatment is necessary for other causes of infertility if this doesn't help you get pregnant. Dr Randy Morris MD-The BOARD CERTIFIED fertility expert with weekly TTC tips on InfertilityTV If you are trying to conceive, the fertility medication known as clomiphene citrate or Clomid may be an option for you. Clomid is used in two circumstances. First, Clomid is used for women who don’t ovulate to try to induce ovulation. But clomid is also used for women who already ovulate to try to further improve their chances for getting pregnant. The protocol for treating women with Clomid varies from doctor to doctor. I am going to tell you about the highly effective methods we use at IVF1. First, it is important to perform a baseline ultrasound of the ovaries and some baseline blood tests. Women who get their periods can do this on the 2nd or 3rd of menstrual bleeding. Women who don’t get periods can do the baseline at any time. As long as everything looks okay, a woman may then start to take the Clomid pills. The starting dose is usually 1 to 2 pills a day for five days. Women who don’t ovulate may use a higher dose up to 5 pills a day or use them for a longer period of time such as 7 or 10 days. After the last pill is taken, it is time to start monitoring for ovulation. With clomid, monitoring can be done by using a home ovulation predictor kit which allows a woman to test her urine every morning. A positive result means ovulation will follow in 12-24 hours. Another popular method for ovulation monitoring is to have it done at the doctor’s office. Eggs mature inside of a cyst known as a follicle. As the egg matures, the follicle grows larger. This can be seen and measured on ultrasound. The cells that make up the follicle produce estrogen that can also be measured using blood tests. After the last pill of clomid is taken, follicle ultrasounds and estrogen blood tests can be performed every few days. At times, the eggs will reach maturity without the body triggering ovulation. To get around this problem, doctors will instruct the patient to take an injection of a medication called hCG. Thirty six hours after the hCG is given ovulation will occur. About a week after ovulation was predicted, another blood test called progesterone can be drawn to confirm ovulation. A week after that a pregnancy test can be performed and then -- fingers crossed. Weeks after starting my newest round of medications (Seroquel & Nortryptaline) as a last ditch effort, I had my blood drawn for possible hormone imbalances and my Testosterone levels came back 200 ng/Dl and 150 ng/Dl. The average 25 year old male has 750 ng/Dl. With this discovery I for the first time had any type of possible explanation as to why I was not getting better and why I might be so so sick. The symptoms of such Low T are very similar to those of major depression. I started Testosterone replacement therapy soon after and have been checking in with the world and documenting my experience with treatment as well as giving my insight and perspective on various topics of mental health. I am blessed to say that I have slowly, over the last 4 years, been improving and becoming more stable which I never thought to be possible. My low T manifested itself in the form of Major depression, anxiety, and depersonalization/ derealization for over a year. Gaining some mental stability back is nothing short of a miracle as I was near death for what felt like forever. I do not consider myself to be totally healed yet but I am closer now then ever before and aim to use what I have been through to help or at least offer support to others in need I was able to successfully come off my Seroquel and Pamelor. I work out all the time as a part of my mental health recovery!!! Weight training and all kinds of cardio rule much of my free time!

Clomid Defined
http://fertilitypartnership.com/infertility-glossary Clomid is probably the single most used fertility drug by OBGYNs across the United States. It's a very useful drug--it is overused--but has some good clinical applications. What is Clomid? Clomid is a selective estrogen receptor modulator (SERM) that is in the class of drugs that includes drugs such as tamoxifen which is used in breast cancer. Well, what it does do, it's a drug that looks like estrogen but doesn't function like estrogen. All cells have receptors and that way they can respond to hormonal signals. So there are cells in your pituitary gland which is a gland is up behind your nose. It is responsible for stimulating your ovaries. It secretes another hormone called FSH or follicle stimulating hormone. Now the pituitary starts to secret follicle stimulating hormone and your ovaries start to respond with eggs, and estrogen starts to climb and then the pituitary sees that response. The estrogen binds to receptors on the pituitary and then there's what we call down regulation. And the levels of FSH start to drop. What Clomid does is it binds to the receptor. It sort of is like taking a key that's the same manufacturer as the key to your house but not the right key, so it fits into the lock but it doesn't turn. And so what ends up happening is your pituitary gland is blinded to the response of the ovaries so the pituitary gland continues to secrete FSH. So when you use Clomid you can trick your pituitary gland into making more FSH. Now the more advanced fertility drugs or just FSH that we inject into you, so this is an expensive way of giving you more FSH and stimulating your ovaries to make more eggs. Many years ago one of my mentors said to me that Clomid is a dirty drug. I remember thinking "Why?" Years later, after a lot of experience I realized he was right. Clomid is a difficult drug to use correctly because they're estrogen receptors all over your body and when you block the response to estrogen all over your body, then you end up with negative side effects. Most important from fertility standpoint is the lining of your uterus, your endometrium, when you use Clomid the lining of your uterus doesn't respond to the estrogen and ends up being too thin. So that when you do ovulate and you do make an embryo it doesn't have a safe place to land. So the use of Clomid requires careful surveillance and observation in order to be used correctly. Also Clomid is not a useful drug and a woman who's ovulating. Now, how does one know you've she's ovulated--and it's a trick question really because I'm sure your mind is thinking about what will, "you get the urine dipstick, your temperature chart, my doctor tells me with his ultrasound machine." But the truth is is only one way to know for sure that you've ovulated is that you get pregnant. But if a woman has regular periods and feels occasionally the twinges of ovulation and there are other factors: maybe progesterone levels later in the pregnancy and the temperature and dip urine and all that information is there and we presume that the woman is ovulating. Guess what? Clomid actually lowers the pregnancy rate. A well designed study published years ago said you have to be careful with the use of Clomid. However in the right patient population--that is people who are not ovulating--it can save them a lot of time and a lot of money and be extremely successful.

I started Clomid. How early can I test to see if I'm pregnant?
You mentioned that you're doing fertility treatments. You're taking Clomid and you started that mid-September and you're wondering when you can find out if you're pregnant. I can understand why you'd be so anxious and why you'd want to know as soon as possible. Now, as for when you'll be able to find out if you're pregnant or not, there's so many different variables that will affect this and so my recommendation is that you call and talk to your doctor. They know the specifics of your situation, like where you were in your cycle when you started the Clomid, when they would except you to ovulate based on their knowledge of your previous cycle history. They can give you the best information. But generally speaking, you can usually find out if you're pregnant about a week after you've missed a period. So if it was supposed to start on Monday, you can usually find out if you're pregnant by the end of the week, Thursday Friday, Saturday. Over-the-counter pregnancy tests are detecting a hormone called human chorionic gonadotropin, or hCG. Your body releases this hormone early in pregnancy and throughout the first trimester. The levels grow exponentially daily, especially early on in pregnancy. So each day makes a big difference in how much is in your blood. So, if you don't have enough, then the test may not detect it when you take an over-the-counter test too early. But if you give it a few days, like wait a week after your missed period, then usually an over-the-counter test will detect a positive result. But again, I think it would be best for you to talk to your doctor who knows exactly where you were in your cycle and when you could have been expected to ovulate and when you can find out if you're pregnant. I have PCOS and was given Clomid to conceive. Can I conceive without drugs or surgery?
You mentioned that your doctor has given you the diagnosis of PCOS, or polycystic ovarian syndrome. And you've been trying to have a baby for a long time now, and they recommended Clomid, which is a medication that stimulates ovulation. And you're wondering if that's going to help you get pregnant or if there are other ways. The effects of PCOS on a woman's body are very broad, and one of the side effects can be infertility, because your body doesn't ovulate. And so this is why your doctor has recommended Clomid, which will stimulate ovulation and hopefully help you have a baby. It's not possible to get pregnant if you're not ovulating, because ovulation is when your body releases an egg that can then be fertilized by sperm, and then pregnancy can happen. So I do recommend following the plan that your doctor has given to you. Take Clomid. Sometimes they try a couple of rounds, and if that doesn't work, then your doctor will decide if other treatments are necessary. Sometimes women who have PCOS go in to have surgery, and basically it's called a clean-out. They cauterize or burn all of the endometriosis in a woman's abdomen. And that can help regulate things and get rid of side effects that PCOS can cause. So your doctor can decide if you're a good candidate for that or not. Of course there are women out there with PCOS who end up getting pregnant, but it's usually spontaneous because they've been trying for so long, and not preventing, and it happens. But more often than not, women do need to help for infertility. And medications, and sometimes surgeries are necessary. Again, your doctor can decide what's best for you based on their knowledge of your circumstances. And I sincerely do hope the best for you. Should I use Clomid to help me get pregnant?
Many women are prescribed Clomid when they have issues with infertility. Clomid helps because it helps your body ovulate, which has to happen in order for you to get pregnant. There are side effects with Clomid, as there are with all other kinds of medication. So if you're concerned about certain side effects, be sure to ask your doctor about those that might apply to you. Or, if you notice something that's just feeling off to you, be sure to talk to your doctor and they can decide if the benefits outweigh the risk. Now, there are certain known side effects of Clomid, one of which is the fact that it increases your chances of having multiples, or twins or triplets. About 10% of women on Clomid will get pregnant with twins and so this is definitely a side effect that can have lasting effects for you. As I said earlier, though, it would just be best to talk with your doctor and they can help you weigh out the pros and cons and decide if it's right for you. I've ovulated on time in the last 2 months, but not this month yet. Should I use Clomid?
You mentioned that you were on birth control in the past and you were put on birth control because your doctor wanted to regulate your cycles in order to increase your chances of having a baby. Since then, you've had a couple of regular cycles. Ovulation and your periods were very predictable but into this third cycle, you haven't ovulated yet and you're a little bit concerned about it. Because you're only a couple of days past the point where you normally ovulate, continue to take your test and see if they're positive. As soon as you start to get positive results, begin having intercourse at the most every day. If you have it too frequently, that reduces sperm count. Continue to have intercourse from this point forward every other day until you start your period. Hopefully that will help to time things just right no matter what your ovulation prediction kits are saying. Now, if this cycle shows that you never ovulated, you can wait and see what happens next month. If it's unpredictable again, then talk with your doctor and share your concerns with her. Tell her that you're wanting to have a baby, as she already knows, and things haven't panned out as planned with the birth control. They can decide if further investigation or treatments are warranted. In about 20% of infertility cases, the problem has to do with ovulation. The woman isn't ovulating frequently enough or not when she thinks she's supposed to ovulate and you had some questions about Clomid. Clomid is used to stimulate ovulation. So you can talk with your doctor and they can determine if this is appropriate in your situation. There are a lot of side effects that come with it and most doctors recommend that couples try for six months to a year before trying infertility treatments like Clomid. On average, it does take most couples about 6 months. So the fact that you've been trying for only 2.5 months so far is actually pretty normal. So don't give up hope. Good luck to you in the future. I have PCOS and am taking Femara. How can I know if I'm ovulating?
You mentioned that you're taking Femara, which is an ovulation stimulation medication for women who don't ovulate, and it can give you a chance at conceiving, because without ovulation, you can't have a baby. So you want to know what signs there are that you're actually ovulating. And there are some things you can look for, but they're not absolutes - like cervical mucus changes. A lot of women, around the time of ovulation, will notice that vaginal discharge changes from being more creamy in consistency to being more slippery and clear, like egg whites. And if you're seeing this, especially if it's mid-cycle, then you might be ovulating, and it's time to have intercourse if you're trying to have a baby. There are some other things that you can do. You can monitor your basal body temperature - that's your temperature each morning before you get out of bed to even go to the bathroom, before you've even had a sip of water, check your temperature and record it. And your basal body temperature will actually go up slightly after you've ovulated, so that doesn't help you in the current month, but if you do it for several months, you can look back in retrospect and kind-of know on what day of your cycle your body usually ovulates. Your temperature will actually persistently stay high until your next period begins. Another thing you can do is use ovulation prediction kits. The manufacturers of most tests recommend that you start taking them on day 9 of your cycle, and what it's looking for is a surge in LH, or luteinizing hormone, which is associated with ovulation. And this surge is usually seen 12 to 36 hours before your body ovulates, and so basically, if you see a positive, then it's time to have intercourse again so that you give yourself a shot at getting pregnant. And then once you stop seeing positives, your body has already ovulated, and your chance to conceive during that cycle is gone until the next one. But I hope everything goes well for your during this cycle, and that you're able to conceive.
