Here are the side effects that I am experiencing taking Letrozole / Femarafor infertility. This video is of symptoms on Day 1. WHAT IS LETROZOLE? Letrozole is an oral medicine that you take day 3 to 5 of your menstrual cycle. The medicine helps mature multiple eggs that a woman releases every month to help in fertility / infertility cases. WHY TAKE LETROZOLE OR CLOMID: Normally a woman ovulates one egg each month. If she struggles getting pregnant, or hasn't been able to get pregnant, has signs of infertility, the reproductive endocrinologist fertility specialist will prescribe Letrozole or Clomid to mature not just one but hopefully around 2 eggs. There is a higher chance of having twins or multiple births because you will hopefully ovulate more than one egg. SIDE EFFECTS: There is a list of known side effects of taking Letrozole or Clomid. Femara known generically as letrozole is a commonly prescribed drug to prevent recurrence of breast cancer and to treat localized disease. In the class of aromatase inhibitors, it functions to prevent male hormone produced in the adrenal gland from being converted into estrogen. Femara functions in breast cancers that contain estrogen hormone receptors – so called hormone receptor positive or HR positive tumors. About 80% of all breast cancers belong in this category. Femara lacks the ability to combat estrogen arising from ovarian production. As a result its utility appears limited to postmenopausal women. This once a day oral drug is routinely prescribed for 5-10 years after initial surgical treatment in women deemed at moderate risk of recurrence. Newer investigations suggest a maximum of 7 years of therapy offers optimal advantages with very limited reason to continue afterward. It has largely replaced tamoxifen. Cash price for generic letrozole remains higher than expected at about $10 a day with the brand name Femara well in excess of $700 a month. It replaced tamoxifen which still works quite well. Available only as a generic tamoxifen’s cash cost is only a modest $1 a day.

Say No!! to Aromatase Inhibitors There are two kinds of hormone therapy for breast cancer: 1. One stops estrogen from acting on breast cancer cells, like Tamoxifen. 2. The other interferes with estrogen production by aromatase inhibition (AI), like Letrozole, Anastrozole, and Exemestane. The two therapies differ in their action and side effects (toxicities) 1. The effect of the first is local. It acts only on breast cancer cells and does not interfere with hormone production. 2. Effect the other is systemic. Since interfering with the aromatase enzyme it affects processes in the body which depend on sex hormones. Aromatase inhibitors (AI) have grave side effects. Although Tamoxifen is the best drug on the market it is being replaced by inferior aromatase inhibitors that are far more toxic than tamoxifen. It turns out that AI did not undergo the scrutiny which would justify their distribution. Actually aromatase inhibitors cause a systemic disease, which evolves over time. I shall now explain in plain language how these bad drugs are distributed. These trials were approved by FDA. They represent a trend to replace Overall survival (OS) with Disease free survival (DFS). 1. FDA accepts survival statistics based on two endpoints, clinical and surrogate. 2. Clinical endpoint is always reliable since it is based on a clear cut event, death. 3. Surrogate endpoint is generally unreliable Clinicians who treat these toxicities believe that they are unavoidable. They are regarded as a reasonable price for cancer therapy. In reality these bad drugs are an outcome of a false cancer theory, which is discussed in other presentation.

I forgot to do one major thing before taking Letrozole because I was distracted by my period starting so late after taking Norethindrone. Would the genomic assay Oncotype DX be helpful? 6. What is Hormonal Therapy? Anti-estrogen (estrogen-blocking) medications, prescribed as pills, are incredibly effective at treating certain types of breast cancer. Hormonal therapy is given to about 70 to 80% of women with breast cancer. Chemotherapy, on the other hand, is a more intense cancer treatment that is generally administered intravenously. Most patients will not need chemotherapy. Many women that do need chemotherapy will also benefit from hormonal therapy. These treatment decisions are complex ones with your medical oncologist. You will make better choices when you are well informed before meeting with your medical oncologist. Do my “Receptors” suggest Hormonal Therapy? When the estrogen circulating in your blood stream interacts with a breast cancer that has “Estrogen Receptors” (ER) present on its surface, it tends to flip the ER switch to the “on” or “grow” position for ER Positive tumors. The same can be said to a lesser extent for the “Progesterone Receptor,” if your cancer is found to also be PR positive. Patients with ER+ breast cancers almost always benefit from the anti-estrogen effects of hormonal therapy. These medications can make cancer cells die, or slow down their growth. If a few cancer cells have already spread to other parts of the body, these medications are incredibly effective at preventing these cells from growing and threatening your life in the future. In other words, those who take hormonal therapy for ER+ tumors have a more successful chance at long-term survival when compared to those who do not. About 80% of all breast cancers are ER+. Even if your medical oncologist recommends chemotherapy for you, if your tumor is ER+ you will also benefit from up to 10 years of hormonal therapy after chemotherapy. Hormonal therapy is never given during chemotherapy nor during radiation therapy. What is “Tamoxifen?” Tamoxifen is an anti-estrogen medication (pills) that has been used with great success with ER+ cancers for three decades. It is now the primary hormonal therapy for younger, pre-menopausal women. It is also the primary drug for men with breast cancer. It is used for some post-menopausal women. What are “Aromatase Inhibitors?” Aromatase inhibitors (AIs) are a class of anti-estrogen medications (pills) that have proven to be slightly more effective than Tamoxifen for post-menopausal women. It is not recommended for younger, pre-menopausal women, except in certain circumstances. The three most common versions are Anastrozole (Arimidex), Letrozole (Femara), and Exemestane (Aromasin). What are the side effects of Hormonal Therapy? Side effects vary greatly from one person to the next for both types of hormonal medications. Some have no symptoms at all. Most have very tolerable side effects. Some patients will need to change hormonal therapy medications to find the best balance of cancer benefit versus side effects. Several side effects that are rather common for both tamoxifen and aromatase inhibitors are hot flashes, night sweats, joint pain, and vaginal dryness. Below, we list some of the other specific side effects for both drugs. Tamoxifen Side Effects: Increased risk of uterine (endometrial) cancer Increased risk of developing blood clots Slows normal bone loss in most women (a “good” side effect) Cannot be taken during pregnancy because of risk of birth defects or fetal death Can temporarily induce menopause in pre-menopausal women. Aromatase Inhibitor Side Effects: Can worsen bone loss (osteoporosis) in women Muscle and joint aches and pains Would an “Oncotype DX” assay help me? Patients who have a small, estrogen receptor positive, HER2 receptor negative tumor, and no evidence of cancer in their lymph nodes may benefit from an Oncotype DX genomic assay. This cutting-edge test looks deeper into your cancer cells to better identify people who may also benefit from chemotherapy with ER+ breast cancers. The decision to undergo chemotherapy, in addition to hormonal therapy, is a complicated one. Your medical oncologist uses many factors to help decide if you will benefit from chemotherapy. An Oncotype DX analysis of a portion of your breast cancer tissue can be instrumental in this decision.

Dr Mamounas speaks with ecancertv at SABCS 2016 about the impact of letrozole treatment up to, and beyond, 5 years on disease free survival of post-menopausal women with HR positive breast cancer who have completed previous adjuvant aromatase inhibitory therapy. From nearly 4000 patients enrolled, he describes a reduction in the that of distant recurrent disease and improvement in breast cancer free interval events, though notes that overall benefits on DFS did not meet statistical significance. Dr Mamounas also notes an elevated risk of thrombotic events after 2.5 for patients receiving letrozole past that point.

Heavily advertised as a treatment for advanced or metastatic breast cancer in postmenopausal women, Ibrance is prescribed in combination with either Femara or Faslodex. Ibrance belongs to the category of drugs known as CDK 4/6 inhibitors that function by preventing cancer cells from progressing through the cell cycle, manufacturing DNA and then dividing. While the drug does not cure cancer, it may prolong progression free survival – in other words, keep the cancer from causing more damage. In the process it may delay the time when other more aggressive therapies may be necessary. It appears to offer moderate advantages compared to either Femara or Faslodex alone by increasing progression free survival by about a year. Dr. Otis Brawley of the American Cancer Society among others points out that an increase in progression survival does not necessarily equate with prolonged overall survival. At present there are no studies indicating Ibrance enhances survival. Ibrance with its partners Femara and Faslodex target postmenopausal women whose tumors are positive for hormone receptors. Unfortunately the measurement and reporting of hormone status remain to be standardized. Ibrance is taken for 21 consecutive days each month followed by a 7 day rest. Toxicity is significant – hence the 7 day rest period for the blood cells to return to normal. Infection, nausea, fatigue, hair loss and sores in the mouth commonly occur. The cash price for one year of Ibrance exceeds $125,000 or more than $325 a day.

Fertility drugs are available in the form of tablets and injections. Tablets are those containing clomiphene citrate, anastrozole and letrozole. Injections are the ones known as the gonadotropin injections containing follicle stimulating hormones and luteinizing hormones either in the ratio of 1:1 or 2:1 or highly purified forms prepared from the DNA recombinant techniques. The oral medication is usually given from the 2nd day to the 5th day of the period or from the 5th day for 5 days. The doses 50 to 150 mg in the case of clomiphene citrate and 1 to 5 mg in the case of anastrozole and letrozole. The oral medications acts by inhibiting the estrogen receptors or by inhibiting some enzymes like the aromatase, thereby decreasing the estrogen level in the blood which indirectly stimulates the pituitary hyopothalamus axis to produce more follicle stimulating hormones which indirectly stimulates the ovary to form multiple eggs. These medicines are used in case of medicines with anovulatory cycles or in case of women with PCOS. Coming to the gonadotropin injections, they can be combined with the oral tablets in a low dose in case of intrauterine insemination cases to they are given purely from the day 2 in case of IVF cycles the treatment is usually given for 3 to 6 cycles in case of oral tablets and in case the woman does not get pregnant, we see stopping the oral medications or after stopping the gonadotropin injections, the woman starts producing the normal eggs and she gets pregnant naturally as the irregular hormones gets corrected the effect of the hormones lasts for 3 to 6 months even after stopping the medications, that is the oral tablets or the gonadotropin injections.

Hormonal treatments are used to help women ovulate or to help them “super-ovulate”, ovulate more than one egg. Women who do not ovulate spontaneously and have normal estradiol levels are traditionally prescribed Clomiphene Citrate, or Clomid. This medicine is a partial estrogen and partial anti-estrogen.  Since 2003, stronger anti-estrogens, the aromatase inhibitors such as Letrozole, have become increasingly popular for ovulation induction. For women who don’t ovulate spontaneously, 80% will ovulate with clomiphene and 50% will conceive. Clomiphene Citrate is associated with multiple follicle growth, so twins and triplets may occur, 20% of pregnancies may be multiples. This medicine can have side effects such as hot flashes, sensitivity to light, headaches and mood swings. Also sometimes the cervical mucus is drier and thicker and sometimes the inside of the uterus doesn’t grow as thick as it should. Letrozole seems to have less side effects, is more likely to make more than one follicle, and the cervix and endometrium are less affected. In comparison studies, letrozole is as good as and maybe more effective than clomid for inducing ovulation. Letrozole has less birth defects compared with clomid in one comparison study. Both medicines are pills taken for 5 days after the start of a period. The pills are started between day 2 and 5. Dosage starts at one pill and can be increased in subsequent cycles to 2 pills or 3 pills as needed. Ovulation typically occurs one week after the last pill but may be later than that. For heavier women a higher dose or longer dose for up to 7 days may be the initial dose.  For those with associated high testosterone levels, sometimes a low dose steroid, dexamethasone, can be added to the regimen to increase fertility. Insulin sensitizers such as metformin can also be added. When oral pills do not directly induce follicular growth, the next step has been ovulation induction with injectable pituitary hormones including FSH or a FSH/LH combination. The oral medications work by “tricking” the pituitary to produce more FSH to stimulate the ovary to make follicles. When this stimulus isn’t strong enough, the injectables provide the FSH directly to the ovary. These are usually successful but may produce multiple follicles. If there are too many follicles and the risk of multiple gestations is too high, then the cycle needs to be cancelled or converted into an IVF cycle where all the eggs can be removed and then we control how many embryos get transferred back in. Depending on the individual, doctors may use a combination of pills and shots that ultimately has a cost advantage and may work better than pills alone. Factors that may directly impact the success of these treatments are weight, diet and exercise. Weight is the biggest impediment to successful OI, especially with pills. A low calorie diet along with weight loss may help improve the odds of success. . A diet which included whole fat dairy is beneficial to fertility as is a plant based diet heavy in fruits and vegetables. Vigorous exercise may also improve a woman who was sedentary to ovulate. Of course some women don’t ovulate because they exercise too much. For some, weight loss is needed, however, for others weight gain, especially in body fat, is needed to ovulate. It is ultimately extremely important to discuss your individual situation with your doctor to create the best treatment option for you to produce your ideal results.
