This video will be reviewing Methylphenidate in the formulation Concerta™ Methylphenidate is the active ingredient in Ritalin™ and is the most common prescription stimulant in most parts of the world. It is used to treat Attention Deficit Hyperactivity Disorder, and occasionally excessive day time sleepiness Methylphenidate works as a dopamine-norepinephrine reuptake inhibitor, and increases the levels of norepinephrine and dopamine in the brain. Methylphenidate is a metabolic stimulant, and increases body temperature, heart rate, and often causes weight loss. Methylphenidate increases concentration, alertness/vigilance, and decreases distraction. Methylphenidate can cause dependance when used for a long period of time even as prescribed. Dependance means that abrupt discontinuation of the drug produces withdrawal symptoms. Methylphenidate withdrawal is not harmful to the body, however can cause depression, sleepiness, excessive hunger, and worsening of ADHD symptoms Methylphenidate can be habit forming Methylphenidate is however an extremely effective agent at treating ADHD in my personal experience, It allows for tasks to be completed without getting distracted, and decreases impulsivty. It does not produce the same level of reinforcement and mood changes than amphetamine, and is considered a safer drug physiologically

http://bigthink.com Koplewicz debunks the myth that Ritalin and Adderall affect those without ADHD differently than those with it. The effect is the same. Question: Do ADHD drugs affect a "normal" brain differently than a brain with ADHD?Harold Koplewicz: Well I think that's a myth.  I think there's a myth out there that if you are a hyper-active, or ADHD kid, Ritalin or Ritalin-like medications are going to work on you differently than they will on the normal population.  And that's what it is.  It's a myth; it's inaccurate.  So for all of us, whenever we would take on of these medicines, your dopamine and norepinephrine is going to be more readily available in your brain.  You will be more focused.  You will be more attentive, you will also, might be a little more uptight and a little more hyper-vigilant.  And when you take any medicine, there's a cost/benefit ratio.  Any time you take a medicine, there's a potential side effect.  So if you don't have ADHD and you're going to take this medicine, any psychostimulant, any type of drug like this is going to increase your heart rate, it may elevate your blood pressure, it will decrease your appetite.  Those things are side effects that are unnecessary if your dopamine and norepinephrine levels are normal.  In the same way that if you take too many aspirin.  It will thin your platelets, which might be good, but might be bad. And if you take too much aspirin you can actually get an ulcer.  So this is something that every time you get prescribed a medicine, a doctor is thinking about what the cost/benefit ratio is.  A little quick soundbyte.. So, but the important part for us to remember is that when someone with ADHD gets these medicines, they get closer to what a normal, typical brain response is.  So that they look less hyper-active, they look more attentive, therefore they're less impulsive.  They are able to do what they're capable of doing.  It doesn't increase your IQ points, it doesn't make you smarter, it lets you get to the IQ points that you do have.  And therefore, while you're never as attentive as a typical or normal individual, and you never stop moving around and stay as still as a typical normal, you are more in the normal range that permits you to be more contemplative, insightful, and therefore get in touch with and use your intelligence.  Question: Is there any danger of addiction or long-term chemical changes? Harold Koplewicz: Well, I think there are other side effects.  So, for instance, particularly if you take Dexedrine—which is a more potent version of Ritalin, slightly different chemical structure—there is no doubt that you will also get a slight rise in your mood and you will feel somewhat more euphoric and you will feel more expansive.  And therefore, if you start taking that medicine on a regular basis, when you don't have it you very possibly will feel a crash, or you will feel down.  And therefore, that's the potential side effect that could be quite serious, that you become used to it or become almost addicted to the sensation of feeling good when it's not necessary to cause you to focus or you don't need it for extended attention. I lost 25 lbs in 4 weeks and was in full panic or near panic for 8 weeks straight mixed with the darkest most painful depression I cold have ever imagined. I immediately could not work and had to move in with my parents who along with many siblings and friends had to watch me 24/7 as I was so suicidal. I was eventually hospitalized. Getting through each day seemed truly unbearable and I knew I would surely die. I have been put on many many different SSRI's SNRI's Tricyclics, Mood stabilizers, anti psychotics, holistic meds, acupuncture and even a form of shock therapy called RTMS. I barely saw any improvement in my condition for a full year. It was decided I had treatment resistant depression and I spent nearly every moment in tears. Weeks after starting my newest round of medications (Seroquel & Nortryptaline) as a last ditch effort, I had my blood drawn for possible hormone imbalances and my Testosterone levels came back 200 ng/Dl and 150 ng/Dl. The average 25 year old male has 750 ng/Dl. With this discovery I for the first time had any type of possible explanation as to why I was not getting better and why I might be so so sick. The symptoms of such Low T are very similar to those of major depression. I started Testosterone replacement therapy soon after and have been checking in with the world and documenting my experience with treatment as well as giving my insight and perspective on various topics of mental health. I am blessed to say that I have slowly, over the last year and 10 months, been improving and becoming more stable which I never thought to be possible. My low T manifested itself in the form of Major depression, anxiety, and depersonalization/ derealization for over a year. Gaining some mental stability back is nothing short of a miracle as I was near death for what felt like forever. I do not consider myself to be totally healed yet but I am closer now then ever before and aim to use what I have been through to help or at least offer support to others in need I was able to successfully come off my Seroquel and Pamelor. I work out all the time as a part of my mental health recovery!!! Weight training and all kinds of cardio rule much of my free time!

Wirkung & Nebenwirkungen von Antidepressiva zur Schmerztherapie insbesondere bei Erkrankungen wie Migräne, Fibromyalgie, chronischen (Gelenk-)Schmerzen und Nervenschmerzen (sog. neuropathische Schmerzen). Was sind Wirkung und Nebenwirkungen? Welche Dosierungen sind sinnvoll? Patienten sind oft irritiert und fühlen sich mitunter vor den Kopf gestoßen wenn Ihnen der Arzt Antidepressiva gegen Schmerzen vorschlägt. Wieso Antidepressiva? Machen die abhängig (NEIN!)? Und welche Nebenwirkungen haben diese? Ich erkläre wieso Antidepressiva bei Schmerzen wie Migräne, Fibromyalgie, neuropathische Schmerzen und chronischen (Gelenk-)Schmerzen Sinn machen kann. Bei chronischen Schmerzen findet man häufig sog. neurochemische Veränderungen in den schmerzverarbeitenden Arealen also im Gehirn und Rückenmark. Menschen mit chronischen Schmerzen haben einen erhöhten Bedarf an den Neurotransmittern Serotonin und Noradrenalin. Diese beeinflussen sowohl die Schmerzempfindung als auch Schlaf und Stimmung. Dauern die Schmerzen jahrelang an, erschöpfen sich die Reserven der Botenstoffe und die Schmerzfilter im Gehirn werden immer durchlässiger. Die Folge: Der kleinste Impuls genügt, um große Schmerzen auszulösen. Jeder Reiz wird dann ungefiltert an das Gehirn weitergeleitet und der Betroffene nimmt selbst kleinste Signale als extrem schmerzhaft wahr. Aus diesem Grund sind Antidepressiva sehr wichtige Medikamente nicht nur zur primären Behandlung einer Depression sondern auch im Rahmen einer Schmerztherapie. Insbesondere bei sog. neuropathischen Schmerzen werden Antidepressiva häufig verwendet. Wichtig in diesem Zusammenhang ist neben der Grund- und Begleiterkrankung (Schmerzen und ggfs. Depression) auch die Kinetik und sog. Galenik des Wirkstoffes. Beispielhaft erreicht das Antidepressivum Amitriptylin seinen maximalen Plasmaspiegel nach ca. vier Stunden. Daher wird empfohlen, dieses Medikament im Rahmen einer Schmerzbehandlung gegen 18 Uhr einzunehmen, da der betroffene Patient sonst häufig eine sog. Tagesmüdigkeit erfährt. Typische Nebenwirkungen sind: (1)Übelkeit, (2) Kopfschmerzen, (3) Gewichtszunahme und (4) Müdigkeit Tipps bei Übelkeit und Appetitlosigkeit 1. Nehmen Sie die Tabletten zu den Mahlzeiten 2. Nehmen Sie häufigere, kleinere Mahlzeiten zu sich 3. Trinken Sie ausreichend (stilles Wasser) 4. Ggfs. nehmen Sie Magenschutzpräparate Tipps bei Müdigkeit und Erschöpfung: 1. Spaziergänge an der frischen Luft (und bei Licht) 2. Nehmen Sie die Medikation 1-2 Stunden vor dem Schlafen 3. Bei depressiven Verstimmungen dann tagsüber z.B. Lichttherapie 4. Vorsicht beim Autofahren! Tipps gegen Gewichtszunahme 1. Bewusste Ernährung: Obst, Gemüse und Vollwertnahrung 2. Reduzieren Sie stark gesüßte Fruchtsäfte sowie Süßigkeiten 3. Bewegung und Sport 4. If parents decline such treatment, social services may intervene. Many patients, who’ve been taking these pills for years, insist that they do more harm than good. They have experienced disturbing side effects such as suicidal thoughts, addiction and even neurological damage. A lot of patients were put on their medication as children and by the time they were legally old enough to decide for themselves, they had already become addicted. They testify that breaking their dependency on the drugs is extremely difficult because, like any habit forming narcotic, they cause severe withdrawal symptoms. RTD meets some of the sufferers to hear their stories of battling to shake off prescribed medicines. Father of 5, Josh, was given anxiety medication, it caused Akathisia and Dystonia. Both are physical disorders causing involuntary and uncontrolled body movement and have left him disabled. Olivia’s son was prescribed psychiatric drugs to treat Attention Deficit Disorder (ADHD). Olivia soon noticed side effects and refused to continue giving him the pills. He was taken by social services and hospitalised. Denis from Russia used to work for a pharmaceutical company in the US but after meeting his wife, who had suffered from the side effects of antidepressants for most of her life, he started questioning the necessity for such widespread psychiatric prescribing. Their claims are supported by lawyers and medical professionals. Psychiatrist Dr. Peter Breggin is a vocal critic of psychiatric medication. He is adamant that the drugs are toxic and that many problems people talk to physiatrists about can be treated without resorting to drugs. His book on how to stop taking psychiatric medicines has helped thousands of patients who have struggled with the challenge. RTD meets a few of them. A new study from Indiana Univerity suggests teens and adults with attention-deficit and hyperactivity disorder may have a lower risk of developing an alcohol or drug problem if they take medications to treat their ADHD. According to UP, the risk of alcohol or drug abuse was 35 percent lower among men and 31 percent lower among women who took ADHD medications, than among those who did not take the medications.
