The buildup of cholesterol can cause atherosclerosis and heart disease—the main cause of mortality in the Western world. [Professor Michael Aviram, Rambam Faculty of Medical Studies]: "The research is about trying to understand the mechanisms relating to the sedimentation of cholesterol in the coronary arteries—such blockages that cause heart strokes—and the carotid artery, which if blocked, causes a cerebrovascular accident." Eating antioxidants like fruits and vegetables is one of the ways to fight the buildup of cholesterol on the artery walls. So which fruits and vegetables should we eat? Professor Aviram says research found the most effective antioxidants to be in pomegranates, red wine, olive oil and licorice root. [Professor Michael Aviram, Rambam Faculty of Medical Studies]: "We chose people who have a partial blockage of the arteries leading to the brain and asked them to drink a glass of pomegranate juice every day. After one year, not only did the size of the atherosclerotic lesion not increase—the enlargement happened to people who did not drink pomegranate juice—but had also decreased by thirty percent." Similar results were obtained from red wine and olive oil. In 1993, one experiment had a group of students drink half a cup of olive oil a day. The students' cholesterol decreased remarkably after only one week. Professor Aviram says ancient culture also understood the benefits of different foods, as pomegranates, red wine and olive oil are the main ingredients often used in the traditional Mediterranean diet. People who take certain HIV medications often get high cholesterol as a side effect of the drugs. Recently my doctor told me that I have high cholesterol. But is it a result of my drug regimen? I had been taking Reyataz, Norvir and Truvada, all of which have high cholesterol as a possible adverse effect. It could be my genes or my lifestyle--or even a combination. I may never really know. But since high cholesterol increases your risk of having a heart attack or stroke, it's time for me to make some changes. Here Comes Trouble My average cholesterol is 234.1; the average of my doctor's patients is 185. My LDL, or "bad cholesterol," is 143.1, which is borderline high. My HDL, or "good" cholesterol, is 52, which is good. (To learn more about cholesterol levels, go here.) My triglycerides were also high. High cholesterol runs in my family. For years I have known that my dad has it. Over the holidays I learned that my mother does too. This double shot of high cholesterol means that I really have to be careful, especially since being male and of African American descent may increase my risk of heart disease also. Fortunately I don't smoke. Living for the Weekend Rather than immediately change my meds, which is complicated, my doctor decided that we'd first approach the problem by making lifestyle changes. There are two areas that I can take charge of on my own: eating and exercise. First I had to slow my cholesterol intake. Now that I'm 30, my body doesn't process foods at the same speed that it did when I was 19. This meant that I had to examine my food choices. During the week, my diet was relatively healthy. Here was my diet on a typical day: Breakfast: Ensure nutritional supplement Snack: ravoli Lunch: Quiznos sandwich, but sometimes McDonald's, pizza or buffalo wings Snack: banana Dinner: pasta, red meat or fish with rice, and wine, preferably red But the weekend was a whole different story: I love a good steak-and-eggs breakfast; I also eat a lot. Here is an example of a typical weekend day: Breakfast: pork bacon, ham, pancakes, biscuits, hash browns and maybe some grits (I also smothered foods with butter) Lunch: chips, pickles and soda Dinner: pizza, chicken wings with ranch sauce, or a steak with mashed potatoes Many of these foods are high in cholesterol (go here to learn more about foods to eat and foods to avoid). I also used a lot of condiments that were high in cholesterol. And I didn't work out much. Yet my waist is still a size 28--okay, 29--so until my doctor said something, I thought everything was okay. But I realized that I had to improve my eating habits, particularly on the weekend, and head to the gym. Given the choice between big steaks and a healthy life, to me the choice is clear. A Change of Plans So I've changed my eating habits. On a typical weekday I now eat: Breakfast: banana and a cinnamon-raisin bagel Snack: orange juice, an apple, water Lunch: club sandwich without bacon, iced green tea Snack: crackers Dinner: baked fish, greens, rice and wine, preferably red And on weekends I now eat: Breakfast: pancakes, muffin with honey, turkey bacon Lunch: tuna fish sandwich with mayonnaise Dinner: catfish, rice, couscous and wine I also take one multivitamin, fish oil to protect against heart disease, and vitamin D because my doctor has noticed in some of his patients who take it that their T-cell, or CD4, counts have gone up. This is on top of the three HIV meds (six pills) I take every day. And I've started hitting the gym three times a week to lift free weights. On my off days, I try to run at least one mile. Just because people look fit on the outside doesn't mean they're fit on the inside. And I may only be 30, with a 29 waist, but I have more than just HIV to worry about.

Today I'm going to answer the question, is drinking wine good for diabetes. But before we get into that, make sure you download my free diabetes management book which also includes a diabetes grocery shopping guide (foods to eat and avoid). I sure do get a lot of questions about drinking alcohol when it comes to diabetes. Alcohol does lower blood sugar levels. For several years doctors and researchers have known this but other factors played into decisions of whether or not to recommend alcohol for those with diabetes. But a 2015 study published in the Annals of Internal Medicine became the first long term study on the effects and safety of those with Type 2 drinking moderate amounts of alcohol. I came across this study from a news item produced by CBS, they did a great job reporting on the findings, so I give them a lot of credit for much of the content of that follows. The study involved 224 diabetes patients who generally abstained from alcohol. They were assigned to one of 3 groups who were told to drink 5 ounces of red wine, white wine, or mineral water with their dinner for two years. They were also assigned to eat a well-balanced diet but with no calorie restrictions. Tests were conducted at random times to determine how quickly the participants metabolized alcohol, as well as various cholesterol tests. Other tests included blood glucose levels, blood pressure, liver function tests, and medication use during the two-year follow-up. The study concluded that those in the red wine group had improvements in their lipid tests. The study also showed that in both red and white wine drinkers, sleep quality was significantly improved compared with the water control group. "One to two glasses of red wine for men and up to one glass of red wine for women, daily, at dinner," was the amount indicated by the study, according to Dr. Minisha Sood. "It's the non-ethanol components of the wine, which are present more so in red wine. It's the phenols, it's the resveratrol, it's the tannins. They all work together with the ethanol possibly to result in these positive changes." While the study shows benefits, Dr. Susan Spratt said, "I worry about the subset of my type 2 diabetes patients who drink too much, and that this may give them more ammunition to say alcohol is good for diabetes. Over-drinking can poison the pancreas. In these patients, when they stop drinking, their diabetes gets tremendously better." “I wouldn't tell someone to start drinking, but if I knew a type 2 diabetes patient was a moderate drinker, I would tell them it looks like red wine is the best choice out of all alcohols to drink, rather than white wine, beer, or hard liquor."
