DVA skin cancer management  There are many types of skin cancers including BCCs or basal cell cancers, SCC or squamous cell cancers as well as melanomas. Todays review is on Bowen disease or Intraepidermal Squamous Cell Carcinoma. This is a common non-invasive cancer of the skin affecting the epidermal layers of skin. IECs most frequently occur on sun-exposed areas and present as red, scaly and persistent patches. IECs may arise de-novo, or from lesions of solar keratosis. If left untreated, Bowen disease may progress to an invasive Squamous Cell Carcinoma. This is one of the most common skin cancers I see in DVA or Department of Defence patients in Australia, and is due to the high UV radiation we have in cities like Brisbane.  Clinical Features Bowen disease most frequently presents as a persistent red scaly patch that slow grows over time. The most commonly affect areas include the legs, arms, and face- areas of sun exposure. Bowen disease or IECs can also affect the lips, oral mucosa, or even genital areas. As Bowen Disease is asymptomatic there is often a delay in the diagnosis, as it may mimic conditions such as dermatitis, psoriasis, tinea, and sun-spots.  Associations Most cases of Bowen disease arise from sun-damaged skin, including areas of actinic keratosis. The most common associations include- UV radiation. This plays a major contributing factor in the development of IECs. Immunosuppression. Patients on certain immunosuppressive medications are more likely to develop IECs.  Arsenic exposure. This was a common cause of Bowen Disease in the past, as arsenic was frequently used in medicines during the 60s and 70s including Bells Asthma Mixture, sheep dip, and as pesticides. Arsenic induced IECs-Bowen disease is a rare event today, as occupational safety has increased. A time lag of 10-20 years is seen between exposure and cancer.  Human Papilloma virus infections. Other associations include genetic diseases, radiotherapy and skin lesions such as porokeratosis, and solar keratosis. Investigations Bowen’s Disease can be suspected clinically, however in the majority of cases, a biopsy should be performed to confirm the diagnosis.  Treatment  There are multiple ways to manage Bowens/IECs. Treatment modalities will depend on the location and size of the cancer, the number of lesions, the degree of functional impairment.  Each modality has both advantages and disadvantages.  Cryotherapy or liquid nitrogen  Liquid nitrogen can be an excellent method of treating IECs. Most cases will require a double –freeze cycle of cryotherapy. This treatment is not recommended for IECs below the knee, as poor healing may occur.  Creams  Imiquimod, or 5-fluorouracil are effective in the treatment of Bowen disease. Clinical studies have shown cure rates approaching 80-90%. Treatment appears to be most successful when these creams are applied for 6-16 weeks.   Curettage and Electrocautery When small patches of IECs are present they may be treated via curettage and cautery. A local anaesthetic is given prior to the procedure to prevent pain and discomfort.  Photodynamic therapy  Photodynamic therapy or PDT can be an excellent non-surgical method of treating skin cancers such as Bowen’s/ IEC’s. PDT uses  a photosensitive cream called 5-aminolevulinic acid or methyl aminolevulinate, that is applied to the skin cancer. The cancerous cells of Bowens/ IEC take up this chemical over a period of 3 hours. A low-level laser light then activates the chemical, and destroys the skin cancer cells. Patients will require two treatments spaced a week or two apart. DVA or the Department of Veteran Affairs in Brisbane, covers PDT for both sunspots on the face and also IECs on all areas. PDT can also be used for multiple or large IECs. The benefit of PDT is that it selectively targets neoplastic cells without affecting the surrounding epidermis. This results in improved cosmetic outcomes, and faster healing times.  Surgery  Surgery is the ideal method of treating IECs in certain areas including recurrent IEC or ones that dwell in hair follicles. Surgery has the highest cure rate for Bowens/ IECs. For the majority of patients, especially DVA patients in Australia, non surgical methods, including the use of photodynamic therapy, is the future. Dr Davin Lim, Dermatologist. If it were a melanocyte which is the pigment making cell, you should be looking for dark spots, multiples spots, spots that seem to be growing in one direction whereas if it's a keratinocyte forming the skin cancer, you should look at sun exposed area. Examine your eyelids, examine your nose, your ears. If you see a bump that doesn't look like anything else, if it's red, if there are blood vessel growing on the bump, if it feels really rough and scaly, it could be a concern for skin cancer. It's important to know though, precancerous lesions may look very similar to skin cancer, and in most cases, a cancer is preceded by a long period of time of a spot that is actually precancerous and easily treated. The best way to find out whether you have a precancerous lesion or a cancerous lesion is to see a dermatologist. A dermatologist can perform a very simple skin biopsy to be analyzed under the microscope, that can tell you definitively, whether it's precancerous or cancerous.

Too much sun is often the cause, but it is not the only cause. SCC can appear on skin that was badly burned, had lots of radiation (such as x-rays), or was exposed to strong chemicals. SCC often has a reddish color. Without treatment, it can grow deeply. If this happens, the cancer can spread to other parts of the body. This can be deadly. SCC often looks like a: Hard (scaly or crusty) reddish bump, patch, or pearl-shaped growth Open sore that itches and bleeds; it can heal and return Scaly patch on the lip; skin on the lip can get thick Melanoma Know where you have moles on your skin! Melanoma often appears in an existing mole or looks like a new mole. By knowing where you have moles on your skin, you can find melanoma when it first appears. Skin cancer symptoms . This type of cancer is not usually spread to other parts of your body . Cancer begins as a small hard painless lump or patch. The most common sites are the lips , ears or the back of your hand . Squamous cell cancer grows very slowly and looks like a wart or ulcer . Symptoms of MelanomaMelanoma is considered cancer the most dangerous and serious skin and is responsible for most deaths from skin cancer . Skin cancer symptoms . Moles are supposed to be closely monitored for changes or cancer because the third type of skin cancer non-melanoma can develop in normal skin or a mole that turns malignant. The most dangerous type is most commonly affects the upper or the men and women of the face, but can occur in any area . Symptoms of skin cancer nonmelanoma are:The small lesion with an irregular border and red spots , white, blue or blue - black on the trunk or branchesThe large patch of brown with dark spots in any part of the bodyThe simple mole located anywhere on the body that changes color , size or feel or that bleeds ( Skin cancer symptoms ) Checking your skin for new growths or changes is a good idea. So this episode is going to be about a trick for doing better self skin cancer exams. Generally speaking, in most medical issues, symmetry is a reliable indicator that something isn't dangerous. So, in doing a self skin exam, if you find a mole or growth that is symmetric, then it is probably normal and not cancerous or precancerous. And just to remind you, a growth is symmetrical if you can fold it down the middle and the two halves are identical. Just like when I fold this circle in half. Yet in all of medicine there is only one situation that I know of where symmetry actually is a bad sign and usually indicates that something is abnormal and in fact at very least, precancerous. That situation occurs when two moles occur symmetrically to each other on your body. So imagine a line drawn down the center of your body, which of course goes right through your belly button. If you have two moles on your stomach, both about equal distance to the left and right of your bellybutton, then there is a very high probability that both of those moles are precancerous and each have a 20 to 25% life time probability of becoming melanomas. They should be shown to a dermatologist and probably be removed. The bottom line is that moles which are symmetric to each other with respect to the midline of your body are usually very dangerous, precancerous moles which if not removed have an unacceptably high probability of becoming deadly melanomas, a cancer that sadly claims the life of one American every hour. This is a truly rare medical example of symmetry being very dangerous, so if you come across it, show it to your doctor. A hot red painful lump usually means an infection, rather than a cancer. Lumps that come and go are not usually due to cancer. Cancer usually forms a lump that slowly gets bigger. Bad breath. Most people have bad breath at some point in their life and it is not cancer. But if you have cancer, bad breath might be worse and happen more often.

Recently, a viewer wrote in, "I have a mole that's uniform in color and shape but it itches. Can a non cancerous mole itch? Or does that mean it's surely cancerous?" Well. What do you think? Does the way a mole or growth feels really help you determines if it's normal or abnormal? The answer is usually not. The way your moles or growths feel offers very little reliable info about whether it's normal or not because both normal moles and growths, and precancerous and cancerous ones usually don't have any symptoms you can feel. They don't itch, burn, hurt or bother you.They don't feel like anything! But occasionally, regardless of whether they're normal or abnormal, if they become injured or inflamed, then they can itch, hurt or burn. Actually, the symptoms of itching, hurting or burning are usually typical of inflammation in general. That can occur if it gets scratched, rubbed, hit or just bumped into. When that happens, it may temporarily, rapidly swell, get red or even become warm, somewhat like a twisted ankle, but again, has nothing to do with normalcy. Because your body repairs the inflammation or injury that causes these symptoms, they are usually temporary, lasting only a few days or weeks. If they persist longer, then it's time to get it checked out, just to be sure. These harmless temporary symptoms of inflammation should never be confused with the actual dangerous warning signs of abnormal growths. Those include bleeding, crusting, gradual increase in size, change in shape or changes in non-red colors like brown or black. Each of these alone or together are always warning signs of abnormal growth and should be checked out immediately! Itchy Mole: Causes, Treatment, Symptoms and More Overview Moles are a common kind of skin growth. Almost everyone has at least a few of them, and some people have up to 40 moles. Moles can form on any part of your body, including on your scalp, the soles of your feet, and the palms of your hands. But often they appear on areas of your skin that have been exposed to the sun. A mole looks like round spot on your skin. Usually moles are brown or black, but they can also be tan, red, pink, blue, or skin-toned. They may darken or lighten as you get older and during certain times of life — like during pregnancy. Moles can be raised or flat. Raised moles can rub against your clothing and become irritated. This irritating can make them itch. Most moles are normal, and they’re usually harmless. But sometimes they can turn cancerous. An itchy mole, along with other changes like crusting and bleeding, could be a sign of melanoma. That’s why it’s important to keep an eye on any moles you find on your body, and report any changes to your dermatologist right away. Possible causes Moles form from cells called melanocytes. These are the pigment cells that give your skin its color. When the cells cluster together, they form a dark spot. There are a few different types of moles: Common moles are the kind most people have. Atypical moles can turn into melanoma. These are the types of moles that crust, bleed, and itch. Moles that you’re born with are called congenital moles. Moles that grow after birth are called acquired moles. Moles usually first appear in childhood or adolescence. You can continue to get new moles until middle age, and then they may start to fade. Moles get darker when your skin is exposed to the sun. Sometimes they also darken during pregnancy. Raised moles can rub against clothing and get irritated. This irritation can make them itch. Moles can turn cancerous, especially if you have a lot of them. People with more than 50 moles are at increased risk for melanoma. Itching can also be a sign of melanoma. But itching alone doesn’t mean you have cancer. You need to look at other symptoms that come with the itch. Signs it might be melanoma If your mole is raised, your clothing often rubs against it, and you have no other symptoms, the cause of your itching is probably just irritation. Signs that your mole might be melanoma can be summarized by ABCDE. Asymmetry: The two halves of the mole are uneven. Border: The mole has irregular or ragged borders. Color: It’s two or more different colors. Diameter: It’s bigger than 1/4-inch across (about the size of a pencil eraser). Evolving/Elevating: The mole is changing size, shape, or color, or it’s becoming raised above the rest of the skin. In addition to itching, look for these changes in the mole: bleeding oozing crusting over pain hardening Treating an itchy mole Most moles don’t need to be treated. If the mole itches enough to bother you, or if your dermatologist thinks it might be cancer, you can have it removed. Dermatologists use one of two procedures to remove moles: Surgical excision: The dermatologist numbs your skin and then cuts out the whole mole. Your skin is typically closed with stitches. Surgical shave: Your dermatologist can do this procedure if your mole is small. After your skin is numbed, they use a small blade to remove the top part of the mole that is raised above the rest of your skin. You shouldn’t need stitches afterward. Your dermatologist may do a biopsy. In this test, they remove a small sample of the mole or the whole mole and send it to a laboratory. There, a technician looks at the sample under a microscope to check for cancer. Your dermatologist will discuss the results of a biopsy with you. Outlook Some moles stay with you for your entire life. Others fade once you reach middle age and beyond. Most moles are harmless and don’t need to be treated. If you have melanoma, your outlook depends on the stage at which your cancer was diagnosed. Five-year survival rates for the earliest melanomas (stage 1) are around 92 to 97 percent. For a stage 4 melanoma that has spread (metastasis from the primary site) to other parts of your body, the five-year survival rate is 15 to 20 percent. It’s important to be alert for any mole changes, including itching, and report them to your dermatologist right away. The earlier you get diagnosed with any type of skin cancer, the better your outlook will be.

Feeling full and unable to eat This is a sign of ovarian cancer. A loss of appetite even when you haven't eaten for a while is a tip-off. 6. Anorexia Could be an early sign of pancreatic cancer. A sudden disgust for coffee, wine or smoking can be linked to this as well. 7. Rectal bleeding or blood in stool Common sign of colorectal cancer. Blood in the toilet should be a big enough sign to see a doctor immediately. 8. Lumps in the neck, underarm or groin area Swollen lymph nodes indicate changes in the lymphatic system which can lead to cancer. 9. Excessive bruising or bleeding that doesn't stop Suggests abnormalities with the platelets and red blood cells, which can be a sign of leukemia. Leukemia cells crowd out red blood cells impairing your blood's ability to carry oxygen leading to clots. 10. Bloating or abdominal weight gain Ovarian cancer patients reported unexplained bloating that came on suddenly and continued for a significant amount of time. 11. Unexplained weight loss An early sign of colon and other digestive cancers. Also a sign of cancer that has spread to the liver, affecting your appetite and the body's ability to get rid of waste. 12. A red, sore, or swollen breast Indicates inflammatory breast cancer, unexplained changes should be told to a doctor immediately. A nipple that appears flattened, inverted or turned sideways had also been noticed by patients diagnosed with breast cancer. 13. Swelling of facial features Lung cancer patients have reported noticing puffiness, swelling or redness of the face. Small cell lung tumors commonly block blood vessels in the chest preventing blood from flowing freely to the head. 14. Sore/lump on the skin that bleeds, becomes crusty or doesn't heal Different types of skin cancer (melanoma, basal cell carcinoma and squamous cell carcinoma) can cause these, so be aware of any odd-looking growths or spots. 15. Changes in nails A brown/black dot under the nail can indicate skin cancer. Pale or white nails can be a sign of liver cancer. "Clubbing", which is the enlargement of the fingers with nails that curve down over the tips, can be a sign of lung cancer. 16. Unusually heavy or painful periods, bleeding between periods A common sign of endometrial or uterine cancer. A transvaginal ultrasound should be considered if the problem persists. 17. Chronic cough or chest pain Leukemia and lung tumors (among others), show symptoms that can mimic a bad cough or bronchitis. Some patients also reported a chest pain that extended to the shoulder and down their arm. 18. Pelvic/Abdominal pain Pain and cramping in the pelvis can be linked with the bloating signs of ovarian cancer. Leukemia also does this because it enlarges the spleen. 19. Frequent fevers or infections Frequently a sign of leukemia, a cancer of the blood cells that starts in bone marrow. It causes the marrow to produce abnormal white blood cells that hinder your body’s infection-fighting abilities. 3. Difficulty swallowing Most commonly associated with throat cancer, but can be a good indicator of lung cancer too. 4. Weakness and fatigue Such a common symptom of cancer that it should be looked at in combination with other symptoms to determine which it is. 5. Feeling full and unable to eat This is a sign of ovarian cancer. A loss of appetite even when you haven’t eaten for a while is a tip-off. 6. Anorexia Could be an early sign of pancreatic cancer. What started off as a dry spot of skin on Dan Gillespie’s nose turned out to be something far more serious – basal cell carcinoma. “It would go away for a little bit and then come back,” he said. When it appeared to be growing larger, Dan knew he needed to get it checked. The results of the biopsy revealed that Dan’s diagnosis was basal cell carcinoma, a very slow growing cancer. “However, the quicker we treat it, the better the treatment will ultimately be,” said Michelle Draznin, MD, Dan’s dermatologist. “I never felt, when I was going and talking to the nursing staff or doctors, that it was something that couldn’t be handled and taken care of,” said Dan. Due to its growth, the tumor encompassed about half of his nose. His care team successfully reconstructed his nose after removing the tumor. “Everybody that I’ve come into contact with at Kaiser Permanente has been very nice, very knowledgeable. They’re really good about getting you plugged in with the right care,” said Dan. “And they take care of business.”

Melanoma: Causes and symptoms Symptoms of melanoma Causes of Melanoma Melanoma skin cancer Any changes to moles should be checked by a doctor As with other forms of cancer, the early stages of melanoma may not produce any symptoms. This is why actively checking skin to spot early signs of the disease is important. Changes in the appearance of the skin are key indicators of melanoma and are also used in the diagnostic process. The Melanoma Research Foundation has produced a web page that compares pictures of melanoma with those of normal moles. This American non-profit organization also lists the following symptoms and signs that should prompt a visit to the doctor: Skin change - a new spot or mole or a change in color, shape, or size of a current spot or mole. Skin sore that fails to heal. Spot or sore that becomes painful, itchy, tender, or bleeds. Spot or lump that looks shiny, waxy, smooth, or pale. A firm red lump that bleeds or appears ulcerated or crusty. A flat, red spot that is rough, dry, or scaly. The ABCDE examination of skin moles is also a key way to reveal suspect lesions (see below). Causes of melanoma As with all cancers, research is ongoing into the causes of melanoma. People with certain types of skin are more prone to developing melanoma, with the following factors associated with an increased incidence of skin cancer: High freckle density or tendency to develop freckles after sun exposure. High number of moles. Five or more atypical moles. Presence of actinic lentigines (small gray-brown spots also known as liver spots, sun spots, or age spots). Giant congenital melanocytic nevus (brown skin marks present at birth - birth marks). Pale skin that does not tan easily and burns; light-colored eyes. Red or light-colored hair. High sun exposure, particularly if it produces blistering sunburn, and especially if sun exposure is intermittent rather than regular. Age - risk increases with age. Family or personal history of melanoma. Being female. Having an organ transplant.

Basal cell carcinoma is by far the most common one. Oftentimes related to sun exposure, basal cell carcinomas happen later in life, most often in Caucasians. Around the nose, around the eye and around the ear are the top three places a basal cell carcinoma can happen. Spotting a basal cell carcinoma can be a challenge. It can present as just a red spot, a pearly little bump or a persistent spot that bleeds. Only a biopsy can determine whether you have basal cell carcinoma or not. If you have any of these symptoms it's important to see a dermatologist and get a skin biopsy to give you a definitive diagnosis.

How to spot if a mole is cancerous: Animation graphic reveals the ABCDE of skin cancer warning signs Skin cancer is the second most common form of the disease in people under 50 And yet, experts warn, the rate of early detection could be much higher Here, Dr David Fisher of Massachusetts General Hospital explains the benefits of early detection - and we present an animation graphic on how to check yourself As the years go by, our skin loses it springy freshness - and mysterious lumps and bumps start to appear. These blemishes can come in all shapes and sizes, and many are harmless. But slight changes could be a red flag: skin cancer is the second most common form of the disease in people under 50. Melanoma, a type of skin cancer, is one of the most treatable cancers. If caught early, the entire cancer could be removed simply by removing the mole in question. Basal cell carcinoma is even more manageable - only rare cases spread beyond the site. And yet, experts warn, the rate of early detection could be much higher. 'Many people are afraid of checking,' Dr David Fisher, director of the Melanoma Program at Massachusetts General Hospital, told Daily Mail Online. 'It's one of those fear and denial-type things. People would rather not know than find out something scary or devastating. 'But the thing is, six out of seven melanoma cases are cured just by catching it early and removing it. In other words, early detection could be life-saving.' He added: 'That statistic should help motivate people to be really proactive, to realize that there's a benefit to this. 'Not every cancer has that type of opportunity to catch it so early. This type of cancer does.' Moles are clusters of cells that produce a pigment in your skin. The more moles someone has on their body, the higher the risk of melanoma. Risk also increases if you have fair skin and red hair, if you sunbathe and if you use sunbeds. Of course, not all moles are dangerous: it's normal to have them. However, it can be easy to spot ones that are worth worrying about. Dermatologists use a simple list of five warning signs, using the rule ABCDE. This animation (above) explains how they work. A – asymmetry This means spotting an irregular shaped mole. B – borders Look to see if there are ragged edges to the mole. C – color change If a mole changes in color or is a different color in one part than in another. D – diameter Any increase in size should be checked out, but particularly more than about 6mm across. E – elevation In this graphic, the E section is classed as 'elevation', telling you to watch out for the mole raising from the surface, especially if it is irregularly raised. However, Dr Fisher explains, many dermatologists have different classifications for this. His preferred word is 'evolving'. 'Is it changing? Do you notice anything suspicious or concerning? That is key,' he says. 'You need to have a very low threshold for what counts as "concerning". I have yet to find a patient who is mad because it wasn't melanoma. It's always worth checking.' It's so common that an estimated one in five Americans will develop one of the types of skin cancer-such as melanoma, actinic keratosis, basal cell carcinoma, or squamous cell carcinoma-by age 70. People of all ages can get skin cancer, but it's most common in people older than 50, those who have light-colored skin, who have a family history of skin cancer, or have spent lots of time out in the sun. Though a funny-looking mole is the skin cancer symptom people are most familiar with, it can present with many other symptoms as well. The good news: most skin cancers can be cured if they're caught early. What causes skin cancer? You probably already know this one: spending a lot of time out in the sun increases your risk for skin cancer. Why? The sun's ultraviolet rays damage the DNA in your skin cells, and sometimes, that damaged DNA goes haywire and produces mutations, or errors. The mutations can cause your skin cells to grow uncontrollably, forming tumors. You're at an increased risk for skin cancer if you've been exposed to a lot of sun over a long period of time. But the scary truth is, you're also at risk if you've had just one blistering, peeling sunburn in your lifetime even if you've spent the bulk of your adulthood slathering on sunscreen and staying in the shade. Other skin cancer risk factors include: Having red hair and fair skin, Having lots of moles on your body, Living at a high altitude, Exposure to radiation, A family history of skin cancer. What are the types of skin cancer, and their symptoms ? Think about the parts of your body that get the most sun exposure: Your face, ears, scalp, lips, neck, chest, hands, and arms. On women, that also includes the legs. Skin cancer usually develops in those places, but can also show up in other areas even your groin or in between your toes. Keep an eye out for these signs of skin cancer, which vary depending on the type you've developed. Melanoma ! The rarest, yet deadliest form of skin cancer. It is most often characterized by asymmetrical moles with irregular borders. Learn more about the melanoma symptoms you should tell your doctor about ASAP. Actinic Keratosis ! Also called solar keratosis, actinic keratosis is a precancer or a premalignant lesion. These lesions are usually rough, red, and scaly, and vary in size, and often occur on the face, lips, ears, back of the hands, and arms. They may also be itchy or painful. Basal Cell Carcinoma ! The most common type of skin cancer. It tends to be slow growing and is very treatable. These lesions are pearly or shiny bumps or spots that often develop on the face, neck, and ears. They tend to form scabs or sores, be scaling and flaky, or bleed easily. Squamous Cell Carcinoma! The second most common type of skin cancer, squamous cell carcinoma is similar to basal cell carcinoma in that it tends to be a pink bump or patch that won't go away. Relatively easy to treat when spotted early. Merkel Cell Carcinoma! Although rare, this skin cancer is one of the most dangerous, as it can grow rapidly and spread to other parts of the body. Merkel cell carcinomas tend to be small; painless; red, purple, or pink in color; feel firm; and may be shiny. How do you prevent skin cancer? Wear broad-spectrum sunscreen with SPF30 (minimum!) every day-even when it's cloudy outside or you don't plan on spending much time outside. Stay in the shade when you can, and remember that the sun's rays are strongest between 10 a.m. and 2 p.m. Wear protective clothing like a wide-brimmed hat. Never use tanning beds.
