The GOP healthcare bill keeps some of the most popular parts of Obamacare, like letting young adults stay on their parents' health insurance until age 26 and requiring insurance companies to cover people with pre-existing conditions. But the Republican bill gets rid of the key element that made Obamacare work: the individual mandate. Now that people aren't required to have insurance, healthy people could leave insurance pools en masse, leaving sick people who are more expensive to cover. Hypocrisy is a minor sin in politics, but still, it is remarkable how much of it there is to be found in this legislation. A core Republican complaint when Obamacare was passed was that the law delayed many of its provisions in order to reduce public outcry and manipulate the CBO’s score. The GOP bill is similarly aggressive with such tricks, delaying changes to the Medicaid expansion until 2020 and pushing Obamacare’s tax on expensive insurance plans out until 2025. Because Republicans aren’t even trying to win Democratic votes, they’re stuck designing a bill that can wiggle through the budget reconciliation process (another thing they complained about Democrats doing). That means they can’t make major changes to insurance markets like repealing Obamacare’s essential benefit standards or allowing insurance to be sold across state lines. That last part is particularly striking, given that it was one of President Trump’s five demands in his speech last week. I’ve always been skeptical about the savings Republicans could wrest by changing those regulations, but now they can’t get those savings at all — which means sacrificing a key part of their theory of cost control. This bill has a lot of problems, and more will come clear as experts study its language, the Congressional Budget Office release its estimates, and industry players make themselves heard. But the biggest problem this bill has is that it’s not clear why it exists. What does it make better? What is it even trying to achieve? Democrats wanted to cover more people and reduce long-term costs, and they had an argument for how their bill did both. As far as I can tell, Republicans have neither. At best, you can say this bill makes every obvious health care metric a bit worse, but at least it cuts taxes on rich people? Is that really a winning argument in American politics? In reality, what I think we’re seeing here is Republicans trying desperately to come up with something that would allow them to repeal and replace Obamacare. This is a compromise of a compromise of a compromise aimed at fulfilling that promise. But “repeal and replace” is a political slogan, not a policy goal. This is a lot of political pain to endure for a bill that won’t improve many peoples’ lives, but will badly hurt millions. "When it comes to drugs, Europe is ‘free-riding’ and committing ‘blackmail.' Pfizer CEO Ian Read delivered remarks on health care and the pharmaceutical industry. He said he opposed any legislation that placed price controls on pharmaceutical drugs, calling it bad policy. He also outlined recommendations on how to improve America’s health care system. These recommendations included the need for more incentives in health care prevention and eliminating the approval backlog for generics. Professional Profile: Ian Read, 59, has spent his entire career at Pfizer ($PFE), joining the company in 1978 as an auditor. He job-hopped and globe-trotted, first through Latin America and then to Europe and on to corporate headquarters. Before taking over as CEO at the end of 2010, he was a corporate SVP and president of the worldwide pharma businesses. Total Compensation: $25.634 million Compensation Breakdown: $1.74 million in salary, $3.4 million bonus, $6.44 million in stock, $6.5 million in options and $7.15 million in pension and deferred compensation, plus $409,892 worth of perks, car service, company aircraft privileges and other compensation. Company Performance: Besides heading up the biggest drugmaker on the globe, Read has set the industry talking with his plans to streamline Pfizer. His back-to-pharma-basics strategy has won fans in the investment community, and analysts are hoping for more spinoffs and breakups, if not at Pfizer, then perhaps at Johnson & Johnson ($JNJ) or another diversified Big Pharma company. Read announced his intent to spin off or sell two business units back in 2011, and last year, the plan started coming to fruition. Pfizer sold its nutrition unit to Nestle for $11.9 billion and laid the legal groundwork for the spinoff of its animal health division, Zoetis. That unit's bond sale and IPO, wrapped up early this year, beat all expectations. Meanwhile, the company has been slimming down in another way: by cutting costs. The company's workforce shrank by more than 12,000 last year, and more layoffs were announced in December. An R&D restructuring also contributed to planned cost savings of $4.5 billion a year. What's left? The company's designated core operations--its R&D-based pharma business, and its "value" businesses in off-patent drugs and consumer healthcare. It was a tough year, saleswise; generic competition for key products, including Pfizer's longtime best-seller Lipitor, took a $7-billion-plus bite out of revenue. Even the company's new childhood vaccine, Prevnar 13, acquired along with Wyeth in 2009, began to falter. But the company's emerging markets sales are growing, if not as quickly as it may have hoped. More importantly, 5 new drugs won regulatory approval before year's end, including the orphan drug Elelyso, for Gaucher disease; the rheumatoid arthritis fighter Xeljanz; and the anticoagulant Eliquis, widely pegged as a major blockbuster.

When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

Costs are exploding even in single-payer systems driven by aging populations and rising expectations for better care. There is heated debate about what to do, but the focus is on the wrong question: how to cut costs? Porter contends that the only real solution is to transform the delivery of health care to dramatically improve value, or the health outcomes per dollar spent. Health care systems in every country have a value problem, and fixing it will require restructuring of care delivery itself, not incremental solutions. An amendment to the House GOP's ObamaCare repeal-and-replace bill would have allowed states to seek waivers for certain ObamaCare regulations, but lawmakers and their staff would be exempt from the changes. A new bill authored by Rep. Martha McSally (R-Ariz.) would remove the exemption. GOP staff and lawmakers said the exemption was originally added to comply with Senate rules but acknowledged that it was politically problematic. "That'll be fixed. While Republicans in the Senate continue to work day and night trying to figure out the best way to take health insurance away from more than 20 million people in this country, they have actually found one group of people who they absolutely refuse to take health insurance away from. Unfortunately, that group of people is themselves and their staff. According to the latest draft of the Republican health care plan, Republicans have slipped in a little provision that completely exempts members of Congress, members of the Senate, and their staffs from having to purchase or buy into or be covered by this horrendous Republican health care plan. That's right, folks. It's good enough for you but it's not good enough for them. Why? Because they understand that it doesn't cover most conditions. They understand that it's probably not, in its current form, going to cover preexisting conditions. They understand that if they want real insurance that actually works for them, they're going to have to get some other kind of coverage because this Republican plan, the plan they themselves came up with, isn't good enough for them. Perfectly fine for everybody else. It's perfectly fine to cut Medicaid by $800 billion and kick 14 million people immediately out of the insurance pool because they can't afford it without that Medicare money or Medicaid money. It's perfectly fine to slash Medicaid and Medicare, which is going to lead to the closing of hospices, which literally will result in the deaths of people. It will cut down on the money available for people to care for in-home care, which will lead to deaths if you do not have the around the clock care. It's going to lead to some hospitals closing down because they rely on Medicaid funding for more than 80% of their income. Republicans understand that. They're not stupid. They are, but they at least understand that. They know how horrible this plan is and that's why they don't want to buy into it. If you're out here in the United States thinking that the Republicans are actually working on a plan that's going to work for you, I suggest you take a look at the fact that even they don't want it. That's like a five-star chef saying, "Here. Eat my food. No thanks. I'm going to run to McDonald's because I don't want to eat that crap." Would you trust a chef like that? No, you wouldn't. Why would you trust a politician who won't even buy into their own policies? Call them. Call them now. Call them today. Call them all weekend. Call them next week. Make sure they know that you do not approve of the fact that they won't even buy into their own plan. Because if they're going to pass this, if they're going to force it on every one of us, then we're going to force it back on them.

Ron Paul's Texas Straight Talk 9/27/10: Healthcare Reform: A Huge Misdiagnosis

http://paul.house.gov http://CampaignForLiberty.com This week marked six months since Congress passed the healthcare reform bill in what has become all-too-typical legislative chicanery. Those in power crafted a mammoth piece of legislation and rammed it through Congress under a dire sense of emergency. Insisting on time enough to read the bill was dismissed as dangerous and crazy in a time of crisis. We were told that if we really wanted to see what was in the bill, we would have to pass it first. I cannot imagine the founding fathers intended for Congress to legislate in this manner. I would think if a Member is not absolutely certain the entire legislation meets Constitutional muster, the default vote should be "no" in accordance with our oath of office. But now that Congress has had six months to read the new law, there is a significant amount of buyer's remorse on Capitol Hill. The more constituents learn about the law, the more angry they become. 60% of Americans are now said to be in favor of repealing the entire thing. Unfortunately, it is much more difficult to repeal a law than to pass a bill. I wrote a while back about the egregious provision to require businesses to issue 1099s for all transactions over $600 as a way to partially pay for it. I have cosponsored legislation to fix this issue, yet this is just the tip of the iceberg. First of all, in spite of the administration repeating over and over that this legislation would not increase costs for Americans, they are now saying they knew all along that it would. The Congressional Budget Office (CBO) estimates that American families will see their premiums rise by an average of $2100 by 2016. The Wall Street Journal has reported that the cost of compliance is forcing some insurers to increase premiums by up to 20% as soon as next year! Also, in spite of repeated claims from the administration that we could all keep our plans and doctors if we liked them, the administration's own officials are now predicting that won't be true for up to 117 million Americans who will lose their current plans. Major insurers are also dropping child-only plans because of mandates and price-fixing on such policies, leaving parents with fewer choices for their children, not more. In addition, in spite of claiming this law would contain government costs, not increase them, administration actuaries now predict it will increase healthcare spending by over $300 billion. This additional spending comes along with doctor shortages, fewer choices and more taxes. Perhaps worst of all, increases in labor costs because of health insurance mandates are discouraging employers from hiring new workers and even triggering more layoffs. Anyone with a basic understanding of Austrian economics could have predicted the unintended consequences of these new healthcare policies. Central planning never increases choices and quality or cuts costs as promised. Price controls and government mandates always create artificial scarcity. Healthcare is not a right, nor a privilege. It is a product, like food or clothing. As with any good or service, the free market regulation of supply and demand provides the optimum quality to the maximum number of people. Once we realize the problems we are trying to solve today were created by government intervention beginning in the 1960's, we can begin to put patients and doctors back in control of healthcare, rather than third party oligopolies and government bureaucrats. The sooner, the better.
