How to choose the right over-the-counter pain medication for you

What the best over-the-counter medication to stop your pain? Emory Internist Dr. Sharon Bergquist, says you've got two main choices at the drugstore.. "One type is Acetaminophen, or Tylenol," Dr. Bergquist says. "The other type is the family we call NSAIDs, or non-steroidal anti-inflammatory drugs." Think aspirin, ibuprofen or naproxen. All NSAIDs. But there are some key differences in how the two types of drugs work. "One of the main differences is that the NSAID family are anti-inflammatories," says Dr. Bergquist. "They're not just pain relievers; they reduce inflammation. Tylenol has no anti-inflammatory properties, so it is purely a pain reliever." So, for a headache or fever, Acetaminophen or Tylenol may be your best bet. "If you sprain your ankle, if you stub your toe, inflammatory type injuries, you're better of with an anti-inflammatory pain reliever," Dr. Bergquist says. But what about possible side effects? Bergquist says there can be plenty. "I think a lot of people underestimate the risks of over-the-counter pain medications," she says. Read the label, take only the recommended dose, and Dr. Bergquist says keep in mind most side effects come with regular use. "But, if you just use them periodically, you should be fine," she says. Anti-inflammatories like ibuprofen can cause stomach pain, heartburn and stomach ulcers. "If you have a sensitive stomach, always take it with food," says Dr. Bergquist. "That is really critical." NSAIDS, especially aspirin, are also bloodthinners, so they can increase your risk of bleeding. And Dr. Berquist says they can raise blood pressure, and cause kidney problems, especially in people with underlying kidney disease. If you take Tylenol, or Acetaminophen , watch for common symptoms like nausea, stomach pain, loss of appetite, headache, and dizziness. Taking more than the recommended amount of Tylenol or Acetaminophen has also been linked to liver toxicity, and even liver failure. "Acetaminophen goes through your liver," Dr. Bergquist explains. "So if you have underlying liver disease you should steer clear of Acetaminophen. And, if you drink alcohol, which is also broken down by the liver, you should steer clear of Acetaminophen." If you regularly take a medication like Motrin or Tylenol, Bergquist says tell your doctor. Some side effects like liver and kidney problems can only be detected through bloodwork. Bottom line? Common Painkillers May Boost Blood Pressure In Arthritis Patients

A new study suggests that nonsteroidal anti-inflammatory drugs, or NSAIDs, may raise blood pressure in patients with arthritis. NSAIDs are among the most widely used drugs in the world, with almost 19 percent of Americans routinely using at least one NSAID. Warnings on the labels of these drugs caution against possible increases in blood pressure, but there is little evidence on the effects of specific drugs. Meanwhile, 30 million Americans have osteoarthritis, and 40 percent of those people also have high blood pressure. Managing high blood pressure in patients with arthritis could prevent more than 70,000 deaths from stroke and 60,000 deaths from heart disease each year. To examine any link between specific NSAIDs and high blood pressure, the researchers compared the effects of Celebrex with the NSAIDs naproxen, the active drug in Aleve, and ibuprofen, which is found in Advil and Motrin. After four months, the researchers found that the Celebrex lowered the patients' average systolic blood pressure slightly, but ibuprofen and naproxen increased it. He said that he just kind of woke up one morning, started walking and took one stride, and then he started to scream in a high falsetto tone like a six-year-old girl. Then he states that he fell to the ground and started grabbing his left, big, painful toe. What can make a strong star athlete fall to the ground and scream in pain? Gout can do it. What is gout? Gout is a type of arthritis or joint inflammation. It occurs in some people who have elevated levels of urate, or uric acid. Now, just because you have elevated levels of uric acid, that does not necessarily mean that you will have gout. In fact, two-thirds of people with elevated uric acid have no gout. But in the patients who do, this uric acid forms into crystals that deposit in the tissues and in the joints. Now with my patients who have these crystals in the joints, some of them have described it like having shards of glass inside your joint stabbing. When these uric acid crystals form in the joints, they trigger an inflammatory reaction which causes pain, swelling, and redness of the joint. Now, gout can occur in any joint, but it very typically can present in the big toe, which is what David Wells described. What are the risk factors for gout or symptoms of gout? Chronic kidney disease is a risk factor, which explains why I have so many patients with gout. Also, hypertension or high blood pressure is a risk factor. Obesity, taking certain medications that increase the levels of uric acid in the blood such as diuretics can be a risk factor, consuming excessive amounts of alcohol like beer, wine, also a risk factor, and consuming excessive amounts of meats and seafood as well as non-diet sodas with high fructose syrup. Do you know who has been portrayed in paintings eating rich meats and drinking wine? Henry the eighth of England. Yes, the same Henry the eighth who was in the royal family of Prince Harry and Duchess Meghan Markle. Now, history dictates that when Henry the eighth was not preoccupied with beheading his wives, he had multiple gout attacks. In fact, it's because of Henry the eighth that gout got the nickname the disease of kings. What is the treatment of Gout? The most immediate treatment of gout is to take care of the pain and inflammation. How do you treat inflammation? With anti-inflammatory drugs. So, your doctor can prescribe steroid-containing anti-inflammatories such as prednisone, which can be taken by mouth short-term. Also, you can be given steroid injections inside of the affected joint. You can also be prescribed nonsteroidal anti-inflammatory drugs, like Ibuprofen, Motrin, Advil, indomethacin. The key is to have a discussion with your physician. Find out if it is safe for you to take these anti-inflammatories. If you are a patient with kidney disease or ulcers, or if you are predisposed to bleeding easily, then NSAIDs may be contraindicated in you. Colchicine can be used to treat gout. Colchicine is a medication that helps to slow down or to stop inflammatory cells from migrating to that joint. So, colchicine is a drug that can be taken for the immediate treatment of gout. Colchicine does have certain side effects in certain patients. It can cause nausea, vomiting, and diarrhea. So yes, steroids, NSAIDs, and colchicine can all be used to treat the inflammation and the pain of gout. And there's something to be said about the pain that gout causes. Founding father Benjamin Franklin, who has been credited for inventing bifocals and discovering electricity also had gout. History reports that his pain from gout flares was so severe that he would actually have to miss many meetings about the Declaration of Independence. Another part of the treatment of Gout is prevention or prophylaxis. You can prevent recurrent gout attacks by decreasing the amount of uric acid in the blood. This can be done through certain medications such as ULORIC, Allopurinol, probenecid. Again, a consultation with your physician will let you know which medication is right for you. The downside(s) of NSAID use. According to the National Institutes of Health, nearly 50 million American adults have chronic pain or severe pain. The conventional medical model teaches us to reach first for medication to relieve pain, with ibuprofen and other NSAIDs (nonsteroidal anti-inflammatory drugs) at the top of the list. It is after all a pretty realistic expectation. Have Pain. Get rid of Pain. Done. However, with research mounting to show NSAIDs have a notable list of dangers, it makes sense instead to look for the root causes of pain and not just cover up the end result of whatever imbalance is causing your pain. NSAIDs promote leaky gut Another reason to avoid NSAIDs is their tendency to promote leaky gut, as I’ve mentioned before in some of my other blogs. In leaky gut, the lining of the small intestine becomes damaged and overly porous, allowing undigested food, bacteria, yeast, and other pathogens into the bloodstream. This triggers inflammation and pain throughout the body — the same thing people use NSAIDs to relieve, creating a vicious cycle that can be very difficult to get off. So, if the typical go-to medications for pain aren't an option any more, where do we turn? Functional medicine offers solutions. Address inflammation to root out pain When pain is treated with NSAIDs, it typically comes back when the medication runs out. However, in functional medicine the goal is to identify and address the cause of the inflammation and pain instead of simply putting a temporary Band-Aid on it. It’s understandable to want relief so you can feel and function better. The good news is many people find their chronic pain diminishes substantially or disappears completely when they adopt functional medicine strategies. So let me be clear on this. I’m NOT saying that functional medicine will fix all your pain problems. BUT, what I am saying is that the entire body is connected. If you have some sort of chronic pain it often becomes a vicious cycle of pain = inflammation = pain = and on and on. By addressing where that additional or extra inflammation is coming from we can often relieve or at least significantly reduce the amount of chronic pain you are in. It eve applies to acute injuries as well. If you body is balanced and has little inflammation prior to an injury, when you do get injured you will heal much faster. Following are a few ways functional medicine can relieve pain and inflammation: Anti-inflammatory diet. Remove foods that trigger inflammation, such as gluten, dairy, grains, legumes, eggs, sugar, and nightshades. This is typically done as an elimination and reintroduction protocol where you follow the diet strictly for a period of time and then customize it depending on your food sensitivities. This customization is generally done with Food allergy (IGE antibodies) and food sensitivity/intolerance (IGG antibodies) testing. Note: not all tests are created equally. Don’t just pick the cheapest or quickest test. Otherwise your results may not mean too much. Check out the Cyrex Array 10 on our Functional Medicine testing page. I will also put some links in the description below. Check em out! Avoid nightshades. Vegetables in the nightshade family can cause pain and inflammation in the joints. These include eggplant, potatoes (but not sweet potatoes or yams), peppers, tomatoes, tomatillos, hot pepper products and pepper-based spices. Simply removing nightshades from the diet has brought relief from joint pain for many, especially those with rheumatoid arthritis. But keep in mind this doesn’t mean everyone will benefit from this. Joints form the connections between bones. They provide support and help you move. Any damage to the joints from disease or injury can interfere with your movement and cause a lot of pain. Many different conditions can lead to painful joints, including osteoarthritis, rheumatoid arthritis, bursitis, gout, strains, sprains, and other injuries. Joint pain is extremely common. In one national survey, about one-third of adults reported having joint pain within the past 30 days. Knee pain was the most common complaint, followed by shoulder and hip pain, but joint pain can affect any part of your body, from your ankles and feet to your shoulders and hands. As you get older, painful joints become increasingly more common. Joint pain can range from mildly irritating to debilitating. It may go away after a few weeks (acute), or last for several weeks or months (chronic). Even short-term pain and swelling in the joints can affect your quality of life. Whatever the cause of joint pain, you can usually manage it with medication, physical therapy, or alternative treatments. Your doctor will first try to diagnose and treat the condition that is causing your joint pain. The goal is to reduce pain and inflammation, and preserve joint function. Treatment options include:Medications For moderate-to-severe joint pain with swelling, an over-the-counter or prescription nonsteroidal anti-inflammatory drug (NSAID) such as aspirin, ibuprofen (Advil, Motrin), or naproxen sodium (Aleve), can provide relief. A newer generation of NSAIDs known as Cox-2 inhibitors (celcoxib) is also good for pain relief, but all except one of these drugs (Celebrex) have been removed from the market because of an increased risk of heart attack, stroke, and other cardiovascular events. NSAIDs also can have side effects, potentially increasing your risk for gastrointestinal bleeding. If you have milder pain without any swelling, acetaminophen (Tylenol) can be effective. Be careful when taking this medicine though, especially if you drink alcohol, because high doses may cause liver damage. Because of the risks, you should take any of these pain medications with caution. If your pain is so severe that NSAIDs and Cox-2 medicines aren't effective enough, your doctor may prescribe a stronger opioid medication. Because opioid drugs can cause drowsiness, you should only use them under a doctor's care. They also can cause constipation, which you can relieve by taking laxatives. Other drugs that may help relieve pain include: Muscle relaxants to treat muscle spasms (may be used together with NSAIDs to increase the effect) Some antidepressants and antiepileptic drugs (which both interfere with pain signals) Topical Agents Capsaicin --- a substance found in chili peppers -- may relieve joint pain from arthritis and other conditions. Capsaicin blocks substance P, which helps transmit pain signals, and it triggers the release of chemicals in the body called endorphins, which block pain. Side effects of capsaicin cream include burning or stinging in the area where it is applied. Another topical option is an arthritis cream containing the ingredient, methyl salicylate (Ben Gay). Injections For people who don't find joint pain relief from oral or topical medications, the doctor can inject a steroid medication (which may be combined with a local anesthetic) directly into the joint every three months to four months. Steroid injections are most commonly used in patients with arthritis, joint disease, or tendinitis. The procedure is effective, but in most situations the effect be temporary. 7 Treatment Options for Juvenile Rheumatoid Arthritis 1. Nonsteroidal anti-inflammatory drugs (NSAIDs) Upon receiving a juvenile rheumatoid arthritis diagnosis, many patients will be prescribed medications to help them cope with the often painful condition. The first and most accessible type of medication prescribed to these individuals is nonsteroidal anti-inflammatory drugs, otherwise known as NSAIDs. As the name suggests, NSAIDs are designed to alleviate the symptoms associated with juvenile rheumatoid arthritis by reducing inflammation, in this case of the joints. Many NSAIDs are available over the counter, meaning they don’t even require a prescription. Examples include ibuprofen (Advil and Motrin) and naproxen (Aleve). Side effects, which are generally minimal, include nausea and issues with the liver. 2. Disease-modifying antirheumatic drugs (DMARDs) Should a young patient dealing with juvenile rheumatoid arthritis require something more powerful to combat the pain associated with this condition, their family doctor may prescribe disease-modifying antirheumatic drugs, or DMARDs. Examples of DMARDs include methotrexate (Trexall) and sulfasalazine (Azulfidine). Sometimes a physician will encourage their patient to take DMARDs in addition to non-steroidal anti-inflammatory drugs, or NSAIDs. And while DMARDs are generally more potent than NSAIDs, the side effects are fairly similar: upset stomach and, should they be taken in excess, liver issues. 3. Tumor necrosis factor blockers (TNFs) Tumor necrosis factor blockers are often prescribed by doctors to patients suffering from the effects of juvenile rheumatoid arthritis. That’s because TNFs can effectively restrict many of the symptoms associated with the condition, from stiffness in the morning hours to the late-day pain of swollen joints. Unfortunately, tumor necrosis factor blockers have more significant side effects than non-steroidal anti-inflammatory drugs, which can cause nausea in some patients. Unlike NSAIDs, TNFs increase the chance that a patient will suffer from even more substantial health problems, including cancers like lymphoma. For this reason, doctors may not prescribe TNFs unless the case of juvenile rheumatoid arthritis is considered particularly serious and painful. 4. Immune system suppressants You could hardly blame someone for wondering why on earth a physician would prescribe medications designed to actually suppress the activity of a person’s immune system if they’re fighting a painful condition like juvenile rheumatoid arthritis. But there’s a good reason physicians turn to these types of medication: because they can effectively limit the chance that the immune system of a juvenile rheumatoid arthritis patient will attack healthy cells and intensify the condition. There are a number of prevalent immune system suppressants, including abatacept (Orencia), rituximab (Rituxin), anakinra (Kineret) and tocilizumab (Actemra). Like TNFs, they do come with some significant potential side effects, including general infection to cancer. 5. Corticosteroids A far more potent way to counteract the symptoms of juvenile rheumatoid arthritis involves using corticosteroid medications, such as prednisone. Corticosteroids are often prescribed by a doctor while their patient waits for disease-modifying antirheumatic drugs to take effect. Should corticosteroid medications be prescribed, chances are they will be in use for only a short time. This is because these drugs have been shown to cause growth difficulties and increase the chances of infection. Juvenile rheumatoid arthritis patients using corticosteroids will receive these drugs orallyy or through an injection into a problematic joint. 6. Physical therapy Many older adults struggling with rheumatoid arthritis find that physical therapy can help in alleviating joint and muscle pain. The situation is no different for those dealing with juvenile rheumatoid arthritis, which affects children under age 17 — physical therapists and occupational therapists have a variety of exercises that can improve range of motion, enhance muscle tone, and help young patients overcome restricted flexibility and reduce pain and swelling. Additionally, a physical therapist or occupational therapist may provide a child with juvenile rheumatoid arthritis with protective equipment or exercise equipment that can help them build strength when at home; for example, joint supports and splints can help protect painful joints and improve overall functionality.

That condition is probably a little more common in women, but I see it in laborers, men as well as women. It can be from heavy use on the job or heavy use doing things you enjoy like crafting or knitting. It can be something that occurs quickly or is something that occurs over a long period of time. As far as treatment options for it, really it goes from the simple little to the more complex. Things like anti-inflammatory medications, Motrin or Aleve can be utilized, Tylenol, over-the-counter medications, rubs, creams, this sometimes help. When things like that do not help or in addition to those things bracing can sometimes help and the one common about a brace whether its from your doctor or from a pharmacy, is to make sure it includes the thumb itself, if it includes just the wrist and is cut out over the thumb that actually usually makes the arthritis hurt more. It is not an emergency, but it is not something that you should leave alone for a long time because it makes reconstruction harder. So overall there are a lot of things that somebody can do for CMC or basilar joint arthritis. Some of them are simple like over-the-counter medications or a brace. Some of them are a little more complicated like an injection. Now when those things stop working, reconstruction or rebuilding the joint works very well and the only time to get may be a little bit of urgency to see your physician is when the thumb goes into that collapsed or zigzag position. The changes announced Thursday apply to prescription non-steroidal anti-inflammatory drugs, or NSAIDs, including arthritis treatments like Celebrex. The agency said it plans similar changes to over-the-counter drugs in the same class, such as Advil and Motrin. Language on the pills currently warns that they can increase the risk of heart-related problems if used long term. However, the Food and Drug Administration's new warning states that heart attacks and strokes can occur in the first few weeks of taking the drugs. The agency also warns that the risks increase with higher doses of the drugs. The updates are based on an FDA review of recent studies and recommendations by outside advisers. "In general, patients with heart disease or risk factors for it have a greater likelihood of heart attack or stroke," the agency notes in the announcement posted to its website. The labeling change is the latest step in the FDA's ongoing safety review of the drugs, which stretches back to 2004. That's when Merck Co Inc. pulled its blockbuster pain reliever Vioxx off the market because of links to heart attack and stroke. Vioxx had been heavily advertised as a new kind of NSAID that was supposed to be easier on the stomach. But its withdrawal shook the medical establishment and ushered in a new era of drug safety at the FDA. In 2005, the agency added boxed warnings to all prescription NSAIDS, including Pfizer's Celebrex and high-dose versions of ibuprofen and naproxen. Celebrex is the only drug from the same class as Vioxx that remains on the market. The prescription NSAIDs are generally used for long-term chronic pain conditions like arthritis. The agency also added similar warnings to lower-dose, over-the-counter NSAIDs like Aleve and Advil. Those drugs currently warn patients to take the lowest dose possible for as short a period as possible. They are not intended to be used for pain longer than 10 days, according to their labels.
