DAWN PHENOMENON Researchers feel the most common reason for high blood glucose levels in the morning is the dawn phenomenon. The glucose is going up from sources other than digested food. Some of it is produced by the liver from stored starch and fatty acids. Livers that produce too much glucose are one of the main ways diabetes causes high blood glucose levels. Other organs also produce small amounts of glucose. This is called “gluconeogenesis” for those of you who like the technical stuff. Organs produce glucose to keep blood glucose from going too low at night or other times of not eating. From about 2 AM to 8 AM, most people’s bodies produce hormones, including cortisol, glucagon, and epinephrine. All these hormones increase insulin resistance and tell the liver to make more glucose. The idea is to get you enough glucose to get out of bed and start the day. Everyone has a dawn phenomenon. Otherwise they’d be too weak to get breakfast. But in people without diabetes, insulin levels also increase to handle the extra glucose. People with diabetes can’t increase insulin levels that much, so their early morning blood glucose levels can rise dramatically. Experts disagree on how many people have a dawn phenomenon. Estimates range from 3% to 50% of Type 2s and from 25% to 50% of Type 1s. Is dawn phenomenon a serious problem? It can be serious. According to the American Diabetes Association, “Some people with dawn phenomenon find that their glucose continues to rise until they eat in the morning. For others, levels will settle down a few hours after waking, regardless of whether or not they eat.” According to columnist Wil Dubois, the higher your A1C, the more likely you are to have a significant dawn phenomenon. It could be that spending a number of hours each morning out of control is having a significant effect on your overall control. Some people have high glucose levels in the morning because their medicines wear off overnight. This could be true of medicines like insulin, and metformin. If you are taking any long-acting medicine, consider asking your doctor about changing meds, doses, or times THE SOMOGYI EFFECT In some cases, medicine can be too strong. If your glucose goes too low in the night, you could have a rebound high in the morning. This is called the Somogyi effect. Pinpointing the Cause for Effective Treatment If your blood sugar is fairly even when you go to bed and at 3 a.m. but is higher in the morning, you are probably experiencing dawn phenomenon. If your blood sugar is low at 3 a.m., but high in the morning, you probably suffer from the Somogyi effect. If your blood sugar is elevated at 3 a.m. and then higher still in the morning, you probably have waning insulin. Even if you’ve identified the reason behind your high morning number, never attempt to correct it on your own. Instead, talk with your doctor. Together, you can find a treatment plan that gets you back on track in the morning. How can this situation be corrected? Even non-insulin users should check more often than usual, says Gandica. A good rule of thumb is to check your blood glucose every 2 to 3 hours. Medicate Keep taking your medications while sick, though perhaps with some dose adjustments to compensate for high blood glucose levels or to avoid low blood glucose if you can't eat. It is best to discuss exactly what your adjustments should be with your health care provider. But there are general guidelines. For insulin users, the "rule of thumb is to continue taking basal and bolus insulin," says David Dugdale, MD, FACP, professor of medicine at the University of Washington. But recognize that if you don't eat, you wouldn't take a bolus. Basal or long-acting insulin is needed throughout illness, and you may need more than usual. For ill people with type 2 diabetes, the insulin recommendations are the same as for type 1, but figuring out what to do about oral medications may actually be harder, says Dugdale. "They may take pills that are designed to offset what they eat. There is no formula for that," he says. "Some of the pills should be adjusted if [people] are sick enough that they can't eat." For example, sulfonylurea use may need to be reduced because the medication can increase the risk of hypoglycemia. People with type 2 diabetes using metformin may need to switch to insulin during a serious illness, such as gastrointestinal flu, because dehydration and the decreased kidney function that may result increase the risk for a rare complication, lactic acidosis, associated with metformin use. Dugdale suggests asking your doctor if you should take a break from amylin (Symlin) while ill. "It just adds another set of variables that might make it harder to do the right thing," he says. Mind the Ketones One of the most serious risks of sickness in people with diabetes is diabetic ketoacidosis (DKA), a life-threatening condition caused by too little insulin in the body in combination with the stress hormones associated with illness. Check urine ketones frequently while sick. Urine ketones are indicated on a test strip as trace, low, moderate, high, and very high. "Any amount should be noted and reported to the doctor," says Dugdale. "If it is more than 'low,' the patient should get specific instructions on how to turn things around and may need to get intravenous medication." Check with your own provider for how to deal with high ketones, and make a plan for when levels don't come down. For parents of infants, Gandica offers this tip: "Put some cotton balls in the diaper, and then when the child voids, you can squeeze the cotton balls out onto a urine ketone strip." Hydrate Dehydration is a risk factor for DKA, so it's critical to stay well hydrated while ill. A cup of fluid every hour should be enough, but keep in mind that vomiting and diarrhea increase the risk for dehydration. Dugdale recommends carbohydrate-free fluids, such as water, to help keep blood glucose levels from rising too high. "I've seen people with DKA who kept drinking sugary fluids without recognizing how much sugar they were taking, which just aggravated their blood sugar," he says. "If they take fluids with sugar, they need to account for that some way." For people who are sick to their stomach and not eating, Dugdale says this rule can be bent in order to take in some nourishment. In that situation, Gandica recommends alternating between sugar-free and sugary fluids or, if drinking is making you nauseous, between sugar-free and sugary Popsicles. Glucocorticoids can stop an inflammatory process like asthma, thiazide diuretics have been shown to be very successful at reducing high blood pressure and its complications, beta-blockers do protect against heart attacks, niacin does reduce coronary artery disease, and antipsychotic medicines can make the difference between hallucinating and perceiving reality. If you suspect that your blood glucose control may be adversely affected by a medicine you’re taking, speak with your health-care provider before making any changes. There may be an overall benefit to remaining on a drug that slightly increases blood glucose in some cases, or there may be alternatives that can be substituted. Let's look in depth at how to lower blood sugar immediately. First What is insulin? Insulin is a hormone that’s produced by the pancreas, that helps turn sugars from carbohydrates to energy. In this video, I wanna start off by breaking down what insulin does in the body. Then I wanna talk about the various forms of diabetes, type one and type two and then I'm gonna give you a breakdown of how the ketogenic diet and insulin plays a role when it comes down to diabetes because it's really important that we have an understanding there. So lets dive in and talk about insulin really quick. So within our pancreas, we have these cells that are known as beta cells and what beta do is they produce insulin as a response to any kind of carbohydrate that our body sees. So basically, when we have glucose that is derived from food that we eat, our beta cells produce these insulin molecules. These insulin molecules flow around through the body, they allow the cell to absorb that glucose. They allow liver cells, muscle cells and fat cells to absorb the glucose to ultimately be used for fuel. So then we take the next step to look at diabetes, we can understand how diabetes and insulin work together. So the first one I wanna talk about is type one diabetes. A type one diabetes is not traditional, type one is a little bit more rare and it is an autoimmune condition. You see, what happens with this autoimmune condition is you body is actually fighting off beta cells. So those beta cells that produce insulin, your body's own immune system is fighting them off and it's making it so that they're not able to produce insulin. That's why a type one diabetic needs to take exogenous insulin to make sure that they're blood sugar doesn't go through the roof from foods that they eat. Here's the other thing, without insulin, the body can start to essentially starve. Because it's not able to see that glucose because the insulin is never allowing the glucose into the cells, the body starts to break down proteins and fats. That's why often times, type one diabetic are a lot more thin that the type two diabetic. It's not the conventional way that we would look at diabetes as just being an issue with overweight people. Then we move in to type two diabetes. Type two diabetes is the more common one and this is sort of the opposite in a sense, it happens on the other end of the spectrum. Our body's cells are liver, our fat, our muscle cells no longer can receive insulin very well or they get desensitized to it. So they get so much insulin at one point in time that they don't respond to it nearly as well, which means that blood glucose ends up staying elevated and therefore, the pancreas tries its best to produce more insulin. Those beta cells try really hard because they see blood sugar rising but they can only do so much. So they try and try and try and eventually become exhausted and sometimes even fully shut down, which gets you to the point where you cannot produce enough insulin to handle the glucose. So you either have to take medications to lower your glucose or you have to take exogenous insulin if it gets too bad. So now lets go into the ketogenic diet and how this works because this is pretty interesting. Now, initially, on the surface, we can look at the ketogenetic diet, the reduction of carbohydrates and how it would reduce our need for insulin because we don't need as much insulin because we don't have as many carbs. So I wanted to look at a couple of studies and I found one that was extremely interesting and this one breaks down the exact effect of the ketogenic diet on type two diabetes. This study was published in the Journal of Medical Internet Research Publications and it took a look at 262 people that had type two diabetes. And what they found was that when they put them on a ketogenic diet, under 30 grams of carbohydrates per day and increased fat intake, that they started to have some pretty remarkable results. Researchers found that the extract of onion bulb, Allium cepa, strongly lowered high blood sugar and total cholesterol levels in diabetic rats when given with the antidiabetic drug metformin. Anthony Ojieh, lead investigator of the study, said in a statement, “Onion has the potential for use in treating patients with diabetes." The study found that two doses of onion extract strongly reduced fasting blood sugar levels in diabetic rats by 50 percent. Allium cepa also reportedly lowered the total cholesterol level in diabetic rats, with the two larger doses again having the greatest effects. "We need to investigate the mechanism by which onion brought about the blood glucose reduction," Ojieh said. "We do not yet have an explanation." Onions have a very low Glyemic index number at 10 which makes their glycemic load extremely favorable to those with diabetes. Onions are high in vitamin C, a good source of fiber, and with only 45 calories per serving, add abundant flavor to a wide variety of food. Onions are sodium, fat, and cholesterol free, and provide a number of other key nutrients. So, are onions good for diabetes? Yes they are. But be sure to not to confuse raw onions with a stack of onion rings at your favorite restaurant. We are talking about raw sliced or chopped onions. I don’t think many of you are going to sit down and just eat an onion like an apple, so you must be creative in how your incorporate onions into your diet. High Blood Pressure 231/119 Arthritis This caused problems doing normal things like washing my car, gardening, and even trying to get a good rest. This time last year, I couldnt bend down to zip up my boots. Diabetes when I was first diagnosed, my blood sugar was 28.5. Total Cholesterol 8.2 WHAT STEPS HAD YOU PREVIOUSLY TAKEN TO GET WELL? High Blood Pressure: I was put on Altace: 5 mg, then 10 mg. and then 15 mg. Although these medications helped somewhat, my numbers were still fluctuating. Changes to my diet helped very little. Arthritis I was put on Naprosyn, which made my stomach ache, so I was put on another medication to take 1 hour before the Naprosyn to coat my stomach. After one year of this, and not much relief, my medication was changed to Celebrex, which did not help either. In between doses, I would take Tylenol Arthritis pills, as well. Diabetes I was put on 500 mg. of Metformin, twice per day. Through the Lawson clinic, I was put on a very strict diet. I did well on this for some time, but I could not hold the numbers down. I then went to 1000 mg of Metformin, twice per day. Soon, Avandia was added to my regimen at 2 mgs, twice per day, then increased to 4 mgs, twice per day. All this medication failed to keep my blood sugar numbers down, so my doctor added Apo-Glyburide 5 mgs (twice per day), and suggested I would need insulin needles. I should note that through all this, I was following the diet I had been provided, and taking my prescribed medication, but my morning blood sugar numbers were still running at around 13. Cholesterol I had been prescribed Lipitor 40mg and Ezetrol 10mg. HOW DID YOU GET STARTED WITH CHIROPRACTIC? My son and his fiancé had spent sometime with us as they were having work done at their home and seen how I had no energy, get-up and go and was irritable. They asked me to attend a session with Dr. B.J. Hardick. I was very skeptical, even after the initial examination and some early treatments. As mentioned early, high levels of sugar concentration in the blood can damage the blood vessels and organs, in addition to increasing the risk of Type 1 and Type 2 diabetes. It can also lead to increased risk of oxidation and inflammation through out the entire body, stroke, kidney disease and heart attacks. Through bitter gourd’s tremendous antioxidant properties, it may help prevent these risks and lower blood sugar levels. You should try to consume bitter gourd each morning consistently on an empty stomach before you consume your breakfast. Wash the bitter gourd thoroughly. Cut it into fine, small pieces. Put the pieces into a bowl and add a small amount of salt to them. Mix well and let it sit for 10 to 15 minutes. Salt is optional. Holding the bitter gourd pieces in your hands, you can squeeze and extract the juices. You can now either drink this juice as it, adding some lemon juice and turmeric, or you can add it to your favourite healthy green smoothie recipe. If you are on medication, talk to your MEDICAL DOCTOR. This is particularly important because he or she should be involved in monitoring and tapering off your medicines. It’s also essential that you discuss the diet with your doctor if any of the following apply: ■ you are on insulin or any diabetic medication other than metformin. You should have extra screenings within six months of reducing diabetes. ■ You are pregnant or breastfeeding. ■ You have a psychiatric or eating disorder. ■ You are taking warfarin. ■ You have epilepsy. ■ You have a significant medical condition. ■ You are unwell or have a fever. Update 19th of May, 2018: My doctor told me yesterday to take the whole 10mg tablet a day, in the morning, so I tried that. It has lessened the night time trips to the toilet, and I sleep now better. Blood sugars seem good to HBA1C of 7% Well done doc! I tried Berberine and Jardiance combination for my diabetes II, seems to help.
