The answer to this question really depends on several factors. Download My Diabetes Diet & Management Guide: https://goo.gl/ruUJ1j The first of which is which type of diabetes do you have? So we first must break down Type 1 and Type 2 diabetes. Let’s talk about those with Type 1 diabetes first. If you have Type 1 then you are insulin dependent. If you have Type 1 Diabetes, then approximately 90% of your insulin producing cells have been destroyed. And sadly, the process that destroyed the 90% of insulin producing cells with destroy the remaining 10%. Ultimately you will be totally dependent on insulin. Type 1 diabetes cannot be managed without medication. And this is not my opinion, it is the opinion of the foremost diabetes center in the world, the Joslin Diabetes Center in Boston. Here is what they have to say on the matter: If you have type 1 diabetes, you must take daily insulin injections to keep your blood glucose in a normal range. Your body produces little or no insulin. Insulin is a vital hormone that helps your body convert food into energy. Without insulin, you would die. So the answer the answer as far as Type 1 Diabetes goes the answer is Yes, you do need your diabetes medication. But what about Type 2 diabetes? Well, here is what Joslin has to say about that. If you have type 2 diabetes, the answer to this question is much less clear. Many people can keep their blood glucose in a healthy range without medications (either oral diabetes medications or insulin injections) if they lose weight and keep their weight down, are regularly physically active, and follow a meal plan that helps them keep portion sizes under control and helps them spread the amount of carbohydrate they eat at each meal throughout the day. So yes, it is possible for people with Type 2 to maintain their blood sugar without medication. The key to managing your diabetes is to lose weight and regular exercise. And this isn’t something you try for a month. This is a life long commitment. It can be done, but many fail. Eventually, many people with type 2 diabetes find that despite their best efforts, weight control, exercise and diet aren't enough to keep their blood glucose in a healthy range. This is not unusual. One theory is that some people's insulin-producing cells just get tired out from having to produce more and more insulin because their cells are resistant to the effects of insulin. Now, if your healthcare provider tells you that you need to take oral diabetes medications or insulin injections to manage your blood glucose, it's important that you follow their instructions. Keeping your blood glucose in a healthy range is key to preventing long-term complications, such as eye disease, kidney disease, heart attacks, and other problems that poorly controlled blood glucose can cause over a period of years. If you want to try and manage your diabetes without medications, you must let your health care provider know your intentions and you need to monitor EVERYTHING very closely. Don’t just wake up one day and say, “I will eat right today and walk 2 miles, so I don’t need to take my meds.” That would not be a good move on your part. It is possible to manage your diabetes without meds, but do it the right way with the help of your healthcare team.

In another video I discovered that even though the American Diabetes Association recommends Metformin for those with prediabetes, only about 10% or less was actually prescribed the drug. Some of you may not be taking any diabetes meds while others may be taking Metformin and even insulin of some sort. So I will try to give you a brief overview thanks to my friends from the United Kingdom Diabetes Association who has done extensive studies on the topic. Metformin contains the active ingredient metformin hydrochloride (or metformin hcl). Metformin is available both in combination with other drugs, or as a single treatment (a monotherapy). Metformin was approved in 1994 in the United States. Metformin helps those with diabetes to respond normally to insulin. Like most diabetic drugs, the ultimate goals of Metformin are to lower blood sugar to a normal level and maintain this level. Metformin can be used in conjunction with other diabetic drugs, and diabetics should also use diet and exercise to help control their condition. Metformin helps the body to control blood sugar in several ways. The drug helps type 2 diabetics respond better to their own insulin, lower the amount of sugar created by the liver, and decreasing the amount of sugar absorbed by the intestines. There are other diabetic drugs, but for those of you with prediabetes or even Type 2, I suspect you take Metformin. A quick word about insulin. Even those very few with prediabetes take insulin, I think it is worth giving a brief overview. And those of you with Type 2 who take insulin, this may give you some insight as to what it does and how it works. Insulin is a hormone made in your pancreas, a gland located behind your stomach. It allows your body to use glucose for energy. Glucose is a type of sugar found in many carbohydrates. After a meal or snack, the digestive tract breaks down carbohydrates and changes them into glucose. Glucose is then absorbed into your bloodstream through the lining in your small intestine. Once glucose is in your bloodstream, insulin causes cells throughout your body to absorb the sugar and use it for energy. Insulin also helps balance your blood glucose levels. When there’s too much glucose in your bloodstream, insulin signals your body to store the excess in your liver. The stored glucose isn’t released until your blood glucose levels decrease, such as between meals or when your body is stressed or needs an extra boost of energy. People with diabetes either don’t make enough insulin or your body doesn’t use it properly. That is where insulin injections come into play. Insulin injections simply replace or supplement your body’s supply of insulin. All types of insulin produce the same effect. The makeup of different types of insulin affects how fast and how long they work to help mimic the natural increases and decreases of insulin levels throughout the body during the day. Rapid-acting insulin: This type of insulin begins working approximately 15 minutes after injection. Its effects can last between two and four hours. It's often used before a meal. Short-acting insulin: You inject this insulin before a meal. It starts working 30 to 60 minutes after you inject it and lasts five to eight hours. Intermediate-acting insulin: This type of insulin starts working in 30 minutes to an hour after injection, and its effects may last up to eight hours. Long-acting insulin: This insulin may not start working until about two hours after you inject it. However, it can last up to 24 hours. Background for Understanding Basal and Bolus Insulin When food is eaten, it is digested and converted to glucose (sugar) so it can be used for energy. Virtually every cell in the body, including your brain, needs glucose to function properly. The hormone insulin is needed to carry that glucose into cells in all parts of the body so that it can be used for energy. Some of this glucose is stored in the liver as a reserve fuel (called glycogen) that is released when glucose is not available through food. So, between the glucose that is consumed through food and what is gradually released from the liver, the body gets a constant supply of glucose. This also means that there needs to be a constant supply of insulin in the body to keep the amount of glucose in balance. Since more glucose is produced after a meal, the pancreas secretes more insulin. When the amount of glucose is lower, such as between meals or at night, there is less insulin needed -- but there is always at least a small amount of insulin present in the body at all times. Defining Basal and Bolus Insulin Basal insulin is the background insulin that is normally supplied by the pancreas and is present 24 hours a day, whether or not the person eats. Bolus insulin refers to the extra amounts of insulin the pancreas would naturally make in response to glucose taken in through food. The amount of bolus insulin produced depends on the size of the meal. In the person with type 1 diabetes, the pancreas no longer automatically makes insulin regardless of the intake of glucose. The beta cells that produce the insulin have largely shut down. Both the basal, or long-term background insulin, and the bolus, or quick bursts of insulin needed at mealtimes, must be obtained through injections or an insulin pump in order to process all of the glucose taken in through food or released by the liver. Types of Basal and Bolus Insulins Long-acting basal insulins, such as NPH, Levemir, and Lantus, begin working in 1-2 hours but are released slowly so they can last up to 24 hours, providing that background insulin that is needed around the clock. Fast-acting bolus insulins, such as NovoLog, Apidra, Humalog, and Regular, generally begin working within 15 minutes. The exception is Regular, which has an onset of about 30 minutes. Each of these bolus insulins are designed to be taken just before a meal and have a duration of up to five hours for NovoLog, Apidra, and Humalog, and seven hours for Regular.

If you have diabetes and hypertension, your physician will explain the interaction of antihypertensive medications and blood glucose levels. Because of this interaction, if you need medication, your medication and diet may be a little different from those prescribed for others with high blood pressure who do not have diabetes. Diuretics are the most widely prescribed category of antihypertensive drugs. Unfortunately, in addition to reducing your blood pressure, some diuretics may also reduce your potassium stores. Some authorities say that inadequate potassium may interfere even more with your ability to make adequate insulin. If you are advised to take a diuretic, you may also be advised to eat a diet rich in potassium (bananas, citrus fruits) or take a potassium supplement. Some diuretics also tend to increase levels of cholesterol and triglycerides, and you may be advised to offset these effects by following a diet that includes less animal fat and fewer dairy products. Many persons with hypertension take one of a category of drugs known as beta blockers, which influence one part of the sympathetic nervous system. Tex works by blocking the effects of the stimulation coming to the heart and blood vessels through an I special group of sympathetic nerve fibers called beta-adrenergic. These drugs decrease cardiac output by acting on nerve receptor sites in the heart. In this way, they reduce the amount of sympathetic nervous output into the heart. The heart rate slows the heart does not work as hard, less blood is pumped, and blood pressure goes down. However, the beta blockers also suppress the effect of hormones, including adrenaline, responsible for constricting blood vessels and maintaining salt and water levels in the body.

There are several different types of this drug on the market, the most popular being Glucotrol. These drugs work by increasing the amount of insulin released from the pancreas. Like Glucotrol, Prandin increases the amount of insulin in the body and can also cause hypoglycemia. It is very important for a patient with diabetes to work with their physician to get the right dosage of each medication and never double a dosage or cut one in half. Prandin cannot be used in women who are pregnant or nursing children. Starlix is another drug that works similar to Prandin but does not require adjustments. The dosage remains constant and is also safe to use on those with kidney problems. Starlix is yet another promising drug being used to treat people with Type II Diabetes. While most medications that treat diabetes increase insulin developed in the pancreas and decrease the glucose in the liver, newer medications are being marketed that decrease the absorption of carbohydrates in the intestines. Precose did remarkably well in trial studies in breaking down the carbohydrates in the system, making it easier to eliminate. However, this medication has not done as well as the sulfonlureas, which are considered the best possible medications that treat diabetes at this time. However, for those who are allergic to sulfur, Precose is a good alternate. Other new medications that are concentrating on controlling the glycemic control in the system include Symlin and Byetta. While these drugs have proven to be show promise, more testing is needed before they can replace traditional therapies. A diagnosis of Type II diabetes may be frightening for an individual, but there are many different medications available that can keep this disease at bay. It is very important, however, for a patient to be totally complaint in order for these medications to work effectively. It may take increased dosages, lowered dosages or different combinations of medications in order to get the right balance that will help you maintain a healthy blood glucose level. This is why it is so important for an individual to carefully monitor their blood glucose level throughout the day and keep a record for the physician.
