How Testicular Cancer Is Diagnosed | Testicular Cancer

In general, you undergo an evaluation because there is a suspicion that you may have testicular cancer, and this usually involves some sort of pathology related to the testicular. You may have a mass there. That's the most common presenting symptom, but in general you usually get referred to a urologist. The evaluation will then involve an examination and documentation of your history, family history, medical history. We will ask questions like, "Have you ever had a history of an undescended testicle? Do you have any family members with testicular cancer?" We'll then perform an examination to examine the testicles, to see if there are any firm masses. We also want to rule out other possible etiologies, things that could be representative of benign conditions. So we generally perform an examination, but we're looking for a firm area within the testicle that would be indicative of a possible cancer. In some cases, when the diagnosis is uncertain, then we may actually get imaging tests such as a scrotal ultrasound to look for masses within the testicle. We will also obtain a series of lab tests, and these are called tumor markers. There are three specific tumor markers we check for with testicular cancer, one called LDH or lactate dehydrogenase, one called beta hCG, human chorionic gonadotropin. This is the same hormone that we test for, for pregnancy tests. Then, another hormone called AFP, or alpha fetal protein. These are all called testicular tumor markers. When they're elevated it can signify the presence of testicular cancers and we can also look at these tumor markers to help determine the various possible types of testicular cancer that may be involved. But essentially, everybody with suspicious testicular cancer will get tumor markers, possibly imaging tests, and if we want to further evaluate for staging of testicular cancer... If we know someone has the cancer and we want to see how extensive it is, then we will often get imaging of the abdominal area or the chest, or various other parts of the body to see if there is any spread of cancer. The ultimate diagnosis of testicular cancer has to be based on a tissue diagnosis, and that means we have to have some tissue to evaluate to see if there is truly cancer in the testicle. The way we obtain that tissue is by removing the testicle. We do a procedure called a radical inguinal orchiectomy, whereby the testis is removed. It gets sent to a pathologist for them to examine it and evaluate what type of cancer it is and also what we call the staging of the cancer itself.

Symptoms of testicular cancer can vary slightly based on the place of tumor’s presence. In Germ Cell Tumors Testicular cancer, there can be a presence of lump on one of both testicles which may or may not be painful. You may also notice swelling of the testicles. Treatment of testicular cancer is surgery, radiation therapy, and chemotherapy or a combination of these procedures based on the advancement of cancer. This video is an informative animated presentation that explains Testicular Cancer- diagnosis and treatment. Testicular Cancer is a disease of abnormal cell growth in one or both of your testicles. It usually begins in the germ cells where genetic damage or changes called mutations cause the cells to grow uncontrollably. The cancerous germ cells clump together to form a tumour which continues to grow. In most cases, these mutations occur in the germ cells and are called germ cell tumours. Symptoms of testicular cancer may include painless mass on your testicle or testicular pain or swelling. Women do it all the time - men not so much - although both could benefit from self-screening for cancer. "Women are very comfortable doing breast examinations; men are less comfortable in a testicular examination," says Dr. Meir Daller. Dr. Daller is a urologist on staff of Lee Memorial Health System. But testicular cancer is on the rise. From 1973 to 1992, rates increased by more than 50%. There are very few symptoms other than a lump. "Testicular cancer is what I call painless mass. Most of the time you don't have any sign or symptom beside the mass. So you are not going to have any achiness or fever or tiredness," says Dr. Daller. Often called a young man's cancer, it is most common between the ages of 15 and 40. An age when men are less likely to think about cancer. "Men are embarrassed by the cancer and present to the doctor quite late. Most men at the age of 30- 40 feel that they're immune from health issues," says Dr. Daller. The self-exam comes down to knowing what feels normal for your body. Experts suggest feeling for lumps in the shower when the body is relaxed. "It's the easiest exam to do. Every man knows how his body feels, what's normal and what's abnormal," says Dr. Daller. The cure rate for this form of cancer is extremely high. If caught early the 5-year survival rate is 99%. "Very few people are dying of testicular cancer. The majority of the patients are cured. But it depends when we caught the cancer," says Dr. Daller. A chance occurrence changed Bill Pooley's life. "I happened to be watching a medical show and the doctor said 'you guys don't forget you should be doing self exams too. Especially you young guys, testicular cancer is on the rise'," says Pooley. So Pooley prepared to try his hand at a self-exam. "I decided next shower I would go and check it out. And I found a lump," says Pooley. Doctors are seeing it more often. From 1973 to 1992, the rate of testicular cancer increased by more than 50%. The rise continued through the last decade to roughly seven cases per 100,000. "Testicular cancer is what I call painless mass. Most of the time you don't have any sign or symptom beside the mass. So you are not going to have any achiness or fever or tiredness," says Dr. Meir Daller, urologist on Lee Memorial Health System's medical staff. It's the most common cancer found in men between the ages of 15 and 40. Easily detected through a self-exam, young men are less likely to think about it or report it. "Men are embarrassed by the cancer and present to the doctor quite late. Most men at the age of 30- 40, feel that they're immune from health issues," says Dr. Daller. The silver lining with testicular cancer is that it's very treatable. With surgery it's 90% curable when cases are caught early. If it's spread the cure rate is 85% "So we right away are going to do an orchidectomy, a removal of that testis. If it is metastasized, even if there's cancer in the brain, cancer in the lungs, cancer on the lymph nodes, it's curable." says Dr. Daller. Pooley underwent surgery and radiation. Four years of follow up finds him cancer free. "I pay a little bit more attention to the signs now," says Pooley. And grateful that his feelings paid off. With many types of chemotherapy and for a testicular cancer we use certain types of chemotherapy. Not everybody with testicular cancer will require chemotherapy, but in general, situations that may use chemotherapy would involve cases where testicular cancer has spread to different parts of the body. Most commonly the abdominal area or the chest. However, if there is spread of cancer to those sites then we may refer you to a medical oncologist. These are doctors who deliver chemotherapy for the purpose of curing the disease. Chemotherapy is highly effective for certain types of testicular cancers and is delivered over a course of several cycles. Then the medical oncologist will counsel you on the various types of chemotherapy, the risks associated with those types of chemotherapy. Most common situations where we would use chemotherapy would be situations where you may have what's called a non-seminomatous tumor. Which is one category of testicular cancer where that cancer has spread to the abdominal region. Even in cases where the cancer has not spread to the abdominal cavity or elsewhere, then we may look at certain features of the cancer within the testicle. If we see certain features like lymphovascular invasion, that may increase the risk that there may even be microscopic disease elsewhere in the body. Or if we see evidence of certain types of aggressive forms of cancer within the testicle, like embryonal carcinoma, which is one variant of non-seminosal (sp) tumor. In those cases, we may also offer chemotherapy. In general, whenever there is spread of cancer to other areas then we do consider chemotherapy as an option. But even when there is no spread of cancer to those areas, we may offer it to patients. Other options in those situations would include things like surveillance. Where we continue to monitor things to make sure that the patient does no develop cancer elsewhere or surgery in the form of what we call a retroperitoneal lymph node deception. This is a procedure that is done to remove the lymph nodes in the abdominal area to make sure there is no cancer that is present there. So, in some cases we would talk to patients about surveillance versus surgery by retroperitoneal deceptions versus chemotherapy. It all depends on what type of cancer you have, the stage of cancer you have, the extent of cancer you have, and also what happens to, what we call, testicular tumor markers. Testicular tumor markers are proteins produced by the testicular cancer. When they are elevated they generally signify the persistence or presence of viable testicular cancer. In many cases where patients have persistent elevation of their tumor markers, even after removal of the testicle, whether or not there is presence of cancer that's visible on a CT scan, we would recommend that patients in that situation undergo chemotherapy. There are many factors that we look at, but these are some of the general principles and concepts that we use to treat testicular cancer.

Symptoms Of Testicular Cancer I'm doing this video about testicular cancer because someone in my family has experienced it, so I've had a chance to learn more than I ever wanted to know about it. I'll share what I've learned about the symptoms of testicular cancer in the next minute or so. I've also done a lot of reading and research on this topic on reliable medical websites like these. Compared with other types of cancer, testicular cancer is fairly rare. But it's most likely to occur in males between the ages of 15 and 35, when a tumor grows on a testicle. See the visual on the left. Cancer usually affects only one testicle. Symptoms include * A lump or enlargement in either testicle * A feeling of heaviness in the scrotum * A dull ache in the abdomen or groin * Pain or discomfort in a testicle or the scrotum Men may also experience enlargement or tenderness of the area of the breasts. Back pain can be a symptom when considered in conjunction with those I've listed. You should consult your doctor if you detect any pain, swelling or lumps in the testicles, especially if these symptoms continue for more than two weeks. Alyaa Gad - Testicular Self Exam TSE or Testicular Self Examination is a way to examine your testes to find out if they have a normal or an abnormal condition. We actually check for lumps or any swellings or abnormalities in the testes and an early detection can save your life. So if you do it once a month and if you know what to look for you might be able to save your life. Testicular cancer is one of the most common cancers in young men between 15 and 35 years of age. So, you check regularly, to know what’s normal, and then if there’s something abnormal you’ll be able to detect it. So let me show you how to do that examination. Right after the shower, or even during the shower, you can do the examination by standing straight and holding the testes with one hand at the top and the other hand is checking if they’re both sides at the same level. You might find that one is higher than the other, that’s ok. But you just have to check, what is normal in your case and remember that. So, if the right one is higher, you remember, or the opposite. And then you’ll be holding with one hand, say the left hand, and with the right hand you check the right side. Initially you’ll start to fell it and then you’ll feel that something is like running, escaping from you. And gradually you’ll start to learn to hold it tight and fell what’s inside that bag. This bad is called the scrotum. So, with rolling movements between two fingers you will start to feel what’s inside. And once you finish the right side, you hold the top with the right hand and then do the left hand to check the left side, with a rolling movement, in a circle. Now, what do we actually look for? What should you be feeling inside? Let’s see. This is an example of what you should be feeling. Inside the scrotum you’ll feel on testis on the right, and the normal size is about 5x2.5cm (2.5x1inch). In the bag of this bag you’ll find yet another structure, and that is called the epididymis. You will actually find sort of on top back of the testes another mass, and that’s normal. That’s the epididymis, and on the other side as well. Sometimes you might be feeling that one testis has gotten slightly bigger. If you have any pain or feeling that it’s gotten bigger than normal you should contact your doctor. Sometimes you might also feel that one of the testes is smaller than the other, but this is abnormally small, or both have been shrinking very strangely, you’re not used to it, this is also a sign that you should see the doctor. You might also find a small swelling in the size of a pea, right on the surface, so you feel it from outside, and this is a strange swelling. This is an abnormal swelling. This needs contact with a doctor. And also maybe a swelling even this size, as small as a rice, also this is strange. So, what you need to do is get used to the normal feel of your body, of your testes, so that you would immediately know if there’s something abnormal there. Usually you would feel the surface, and its very soft and there’re no swelling or tumors. There’s only a mass in the back, and that’s the epididymis. Another thing you might feel is the sensation that there is a bag of worms at the back of the testes, and that’s a sign of varicocele. This is also a problem that demands us to seek medical help. What should you be feeling? So, let’s see what’s normal. This is a normal testis, so you see here, this is the front side and it’s a normal, soft egg-like structure, and in the back there’s a group of tubes, very small tubes, collecting together, called the epididymis, this is the two structures that you feel. So, what’s abnormal is to feel a swelling or a change in size, so, has gotten very much bigger or very much smaller than normal. Or if you notice any lumps, any tumors in the size of a pea or rice or bigger or smaller. Or if it changes in color, so the color suddenly changed to blue or very red, this is not normal. If there’s pain that you’re not used to, this is abnormal, and if the testis is not there or has never been there, please remember that, so, any change in the position of it, please tell us. And this bag of worm sensation, this is a sign of varicocele. Do it every month to save your life maybe one day. All the best! Just place your index and middle fingers under the testicle with your thumb on top. Firmly but gently, roll each testicle between your fingers. There are over 9,000 new cases of testicular cancer every year, averaging one new diagnosis every hour of every day. One in 250 men will be diagnosed with testicular cancer at some point in their life, with 50% of these cases occurring in men ages 15-35. About 6% of cases occur in children and teens, and about 8% occur in men over the age of 55. 90% of the time, testicular cancer presents as a lump in the testicle. A lump in the testicle can be detected early through regular self-exams. Best done during or after a shower when the scrotum is relaxed, place your index and middle fingers under the testicle with your thumb on top. According to a 2018 study done by CACTI, more than 1 in 3 of all men polled have never been told about the importance of a monthly testicular self- exam. Despite all of these facts and figures, testicular cancer is not talked about enough in society. My hopes are that sharing my story from beginning to end with an open attitude will stimulate more open discussion and bring a larger focus to men’s health in general. Knowing someone who is going through testicular cancer can help make it more real to men who might not otherwise be concerned about their own health. I put my face where their balls are (which is a somewhat awkward turn of phrase). The title, A Ballsy Sense of Tumor, is purposefully chosen to convey this mission is all about testicular cancer and that I talk about it in as positively and with as much humor as you can use when discussing cancer. While cancer is no laughing matter, my method is to approach it with humor, awareness, and positivity. There may be things that occur that draw the patient's attention to that mass, for example pain or discomfort in the groin area or trauma. Sometimes people have trauma to the area and all of a sudden it draws attention to the area, and they examine themselves or someone examines that area, and they feel a firm, solid mass within the testicle. That is the classic appearance of testicular cancer. Until proven otherwise, any solid, firm mass within the testicle should be considered as a potential cancer although there are other things that can cause similar findings. That's one of the more common presentations. Testicular cancer can spread to other parts of the body including the abdominal area, the chest, and various other areas. When it does do that, then patients may notice symptoms due to the spread of cancer to those sites. For example, when cancer spreads to the lungs, patients may have shortness of breath or difficulty breathing. Cancer can spread through the lymphatic channels to lymph nodes in the body. Sometimes if there's extensive spread, patients may feel lumps in the neck area or the groin areas, various other parts of the body, that may signify the presence of cancer. In general, most of those cases, most cases of testicular cancer will also have some form of a mass within the testicle itself. It's not always a 100% guarantee but most commonly that's the way it presents.
