Symptoms of testicular cancer can vary slightly based on the place of tumor’s presence. In Germ Cell Tumors Testicular cancer, there can be a presence of lump on one of both testicles which may or may not be painful. You may also notice swelling of the testicles. Treatment of testicular cancer is surgery, radiation therapy, and chemotherapy or a combination of these procedures based on the advancement of cancer. This video is an informative animated presentation that explains Testicular Cancer- diagnosis and treatment. Testicular Cancer is a disease of abnormal cell growth in one or both of your testicles. It usually begins in the germ cells where genetic damage or changes called mutations cause the cells to grow uncontrollably. The cancerous germ cells clump together to form a tumour which continues to grow. In most cases, these mutations occur in the germ cells and are called germ cell tumours. Symptoms of testicular cancer may include painless mass on your testicle or testicular pain or swelling. If you have testicular cancer your doctor may recommend surgery, chemotherapy, radiation therapy, or a combination of treatments. Treatments for testicular cancer are based on the type and stage of the tumour. Your treatment will usually begin with the removal of the cancerous testicle through a radical inguinal orchiectomy. For this procedure your surgeon will make an incision in your lower abdomen, and then your surgeon will remove your testicle from your scrotum. Your surgeon will close the incision with sutures or surgical staples. Next your pathologist will analyse your testicle to determine which type of cancer you have and plan further treatment. Sometimes people have trauma to the area and all of a sudden it draws attention to the area, and they examine themselves or someone examines that area, and they feel a firm, solid mass within the testicle. That is the classic appearance of testicular cancer. Until proven otherwise, any solid, firm mass within the testicle should be considered as a potential cancer although there are other things that can cause similar findings. That's one of the more common presentations. Testicular cancer can spread to other parts of the body including the abdominal area, the chest, and various other areas. When it does do that, then patients may notice symptoms due to the spread of cancer to those sites. For example, when cancer spreads to the lungs, patients may have shortness of breath or difficulty breathing. Cancer can spread through the lymphatic channels to lymph nodes in the body. Sometimes if there's extensive spread, patients may feel lumps in the neck area or the groin areas, various other parts of the body, that may signify the presence of cancer. In general, most of those cases, most cases of testicular cancer will also have some form of a mass within the testicle itself. It's not always a 100% guarantee but most commonly that's the way it presents.

Feelings for Testicular Cancer
A chance occurrence changed Bill Pooley's life. "I happened to be watching a medical show and the doctor said 'you guys don't forget you should be doing self exams too. Especially you young guys, testicular cancer is on the rise'," says Pooley. So Pooley prepared to try his hand at a self-exam. "I decided next shower I would go and check it out. And I found a lump," says Pooley. Doctors are seeing it more often. From 1973 to 1992, the rate of testicular cancer increased by more than 50%. The rise continued through the last decade to roughly seven cases per 100,000. "Testicular cancer is what I call painless mass. Most of the time you don't have any sign or symptom beside the mass. So you are not going to have any achiness or fever or tiredness," says Dr. Meir Daller, urologist on Lee Memorial Health System's medical staff. It's the most common cancer found in men between the ages of 15 and 40. Easily detected through a self-exam, young men are less likely to think about it or report it. "Men are embarrassed by the cancer and present to the doctor quite late. Most men at the age of 30- 40, feel that they're immune from health issues," says Dr. Daller. The silver lining with testicular cancer is that it's very treatable. With surgery it's 90% curable when cases are caught early. If it's spread the cure rate is 85% "So we right away are going to do an orchidectomy, a removal of that testis. If it is metastasized, even if there's cancer in the brain, cancer in the lungs, cancer on the lymph nodes, it's curable." says Dr. Daller. Pooley underwent surgery and radiation. Four years of follow up finds him cancer free. "I pay a little bit more attention to the signs now," says Pooley. And grateful that his feelings paid off. Symptoms Of Testicular Cancer I'm doing this video about testicular cancer because someone in my family has experienced it, so I've had a chance to learn more than I ever wanted to know about it. I'll share what I've learned about the symptoms of testicular cancer in the next minute or so. I've also done a lot of reading and research on this topic on reliable medical websites like these. Compared with other types of cancer, testicular cancer is fairly rare. But it's most likely to occur in males between the ages of 15 and 35, when a tumor grows on a testicle. See the visual on the left. Cancer usually affects only one testicle. Symptoms include * A lump or enlargement in either testicle * A feeling of heaviness in the scrotum * A dull ache in the abdomen or groin * Pain or discomfort in a testicle or the scrotum Men may also experience enlargement or tenderness of the area of the breasts. Back pain can be a symptom when considered in conjunction with those I've listed. You should consult your doctor if you detect any pain, swelling or lumps in the testicles, especially if these symptoms continue for more than two weeks. But predominately those areas, head and neck and lung, are what we have the most data on. Very large studies have been conducted. Thousands of patients, numerous multiple sites, over several years, in some cases over several decades. Once a study was recently published in the Journal of American Medical Association. Studies out of UCLA, with Dr. Donald Tashkin who's a pulmonologist, a lung doctor, and studies out of Brown University, all of these have really tried to find a link between the use, people who use marijuana and people who aren't using marijuana, and tried to study them over a long time and see if there's an increased rate of cancer in the groups that have used marijuana when you control for all the other variables, and you just can't find it. And what's really exciting and interesting, is in some cases, Dr. Tashkin's studies published, reported that in some cases it's a protective effect. That is, people using marijuana had lower rates of certain aerodigestive cancers. That's very interesting. And that tells us that there may be some effect of marijuana against cancer or to help your body fight off cancer. Now, that doesn't mean to say that the smoke of marijuana is harmless. There is soot and polycyclic aromatic hydrocarbons, which are these pre, what are called procarcinogens. But it turns out the effects of those compounds on your system may be neutralized by some of the actual compounds in marijuana itself that may help to reduce the effects, the negative effects of those compounds. So it kind of has a built in safety system against developing those forms of cancer. There's been some studies on association with testicular cancer but those aren't very large studies. And usually, it's very hard to find strong data. You have to collect large samples over a long period of time and the lion's share of that evidence is showing us that there's no link to cancers. And interestingly, in the same studies, if you look at tobacco users, sometimes they'll compare them with tobacco, you get the increased risk of cancer in a dose-dependent fashion. The more you use, the higher rates of cancer. And those were in the same studies that looked at marijuana. So you can prove smoking and cancer linked with tobacco smoking. You can't prove it with marijuana.

Germ cell tumors (GCTs) account for 95 percent of testicular cancers. They may consist of one predominant histologic pattern, or represent a mix of multiple histologic types. For treatment purposes, two broad categories of testis tumors are recognized: pure seminoma (no nonseminomatous elements present), and all others, which together are termed nonseminomatous germ cell tumors (NSGCTs). In most series, the ratio of seminoma to NSGCT is about one. Testicular cancer has become one of the most curable of solid neoplasms because of remarkable treatment advances beginning in the late 1970s. So if you have a relative who has had it in the past, then you are at an increased risk of testicular cancer. The second most significant risk factor would be history of what's called an un-descended testicle, or Cryptorchidism. What that refers to is a failure of descent of the testicles. Normally, during the embryo-logic process as a baby is developing, the testicles start up here in the abdomen, and descend down into the scrotum. When there is a failure of descent, and the testis gets caught up a little bit higher, then that condition is called an un-descended testicle, or crypt organism. That process increases the risk of developing testicular cancer.

Notably, it affects most commonly men between 20 and 40, so of all cancers that affect men between the ages of 20 and 40, testicular cancer is the most common one. When we talk about how common it is, we talk about something called a bi-modal distribution. Which means that there's two spikes of peak incidents and one spike occurs between 20 and 40, another spike occurs around the age of 60. Men who get testicular cancer around the age of 60 tend to have a type of cancer called spermadicitic seminoma. Testicular cancer as any other cancer is basically an abnormal or uncontrolled growth of cells within the testicle, and when some of those cells grow uncontrollably, and have the ability to both invade and spread to other parts of the body, we refer to that as testicular cancer. Of the various types of testicular cancer, cancers can arise from cells within the testicle where sperm is produced or there are other types of cells in an area called the interstitium, where they have certain cell types called leydig cells and sertoli cells. Leydig cells are actually responsible for testosterone production. You can get tumors of the leydig cells, they're called leydig cell tumors. You can get tumors of the sertoli cells. Leydig and sertoli cell tumors are one type, but they actually represent the minority of testicular cancers, approximately 5.0% or less. Ninety-five percent of testicular cancers arise from the sperm producing cells, what we call the germ cell tumors. Of germ cell tumors, there are also many different types. However, we broadly classify them into two broad categories, one type being seminoma and the second type being non-seminoma. Now non-seminoma can have multiple subtypes within that group and those include things like embryonal cell carcinoma, teratoma, choriocarcinoma and yolk-sac tumor. These are all types of testicular cancer. There are other types of cancer that can affect the testicle as well. These may be things such as lymphoma, which can involve other parts of the body and actually grow within the testicle itself. There are some more rare types of very aggressive cancers that can affect the testicle such as rabdomiosarcoma, sarcomas and these are very aggressive forms of cancer. They don't necessarily originate within the testicle. However, there are also some benign conditions that affect the testicle that can often be confused for testicular cancer and there are tumors that arise in the tissue around the testicles that can be benign as well. But in general, when we refer to testicular cancer we're talking about cancers that arise from the testicles. The most common types we talk about, as I mentioned before, 95% are what we call germ-cell tumors with seminoma and non-seminoma. The reason we categorize, we put cancers into these separate, broad categories, seminoma versus non-seminoma, it really affects the way we manage the cancer, we treat the cancer. There are different algorithms and different pathways we use when evaluating, diagnosing and treating patients with these different types of cancers.

Testicular Cancer Survivor Marvin Jones
In this three minute video, 27-year-old Marvin Jones shares his story as a testicular cancer survivor. He is joined by his mother, Sophia Jones, and his surgeon, Dr. Andrew Kramer. His treatment team at the University of Maryland Greenebaum Cancer Center also included medical oncologist Dr. Arif Hussain and vascular surgeon Dr. Rajabrata Sarkar. As Dr. Kramer explains, testicular cancer often affects young men and can present as a painless mass in the testicle. Following surgery to remove Marvin's affected testicle and after several rounds of chemotherapy, tests revealed that his cancer had metastasized, or spread, to his abdomen. He underwent a delicate, six-hour operation to remove a football-sized tumor that had grown around several major blood vessels. He made a dramatic recovery and is now cancer-free.
