Other surface veins may have a visible or palpable lump usually feels like a hard cord or pencil under the skin. Could continue into a DVT You can expect to be put on coumadin and will have only minor impairment if at all. It can be deceptively symptom free until the entire Saphenous vein becomes like a cord most of the way from your thigh down to the ankle. You most likely will not experience any leg swelling but you should see the vein become more prominent and then become hardened. 2)Deep vein-If in calf, dorsa flex the foot. If sharp pain then suspect thrombosis. 3)Deep vein- If between the thigh and knee-Femoral vein Can be difficult to detect at first because it tends to become established before symptoms develop. Symptoms may present as a lower muscle pain or pulled groin muscle. Further these pains may not always be present. They can be intermittent at first misleading you into thinking it's just a pulled muscle.Experience feelings of fluid build up upon standing and weakness in Quads at first . Later a feeling of dizzyness AFTER mild exertion. A craving for sweets for energy. DO NOT attempt to treat with anti-inflammatory NSAIDs like Aleve or Ibuprofen. Tylenol okay to use. Take a couple of aspirin a day until you have a chance to get to a doctor's offc. Do the pinch test to see if you are dehydrated...if skin does not snap back immediately start drinking fluids now. One you first feeling fluid building up and/or dizzyness upon standing you are probably within three days of requiring admission to the ER at the hospital. You will know you have reached a critical stage when your thigh muscles become weak, feel more and more pressure each time you stand and you feel dizzy after walking just a few yards. At this point you must go to the ER but try to have a bowel movement and pee before you go....you probably won't be able to for the next week or so. Just the physical swelling will cause that.DO THESE THINGS RIGHT NOW (TODAY) BEFORE GOING TO THE HOSPITAL FIRST THING: Laxative-If you even think you may have a thrombosis-Start immediately on either All-Bran cereal or Mira-Lax with each meal SECOND THING: Plan ahead-Transport (911=$900)/meals, grocery shopping, business now SECOND THING: Buy a pair of thigh high compression stockings 20-30mm Hg-no Rx nec. WHAT TO EXPECT: At the ER they will put an IV line in you to keep fluids going into your body. Your leg diameter will continue to swell for the next few days. Be sure you ask for a doppler to confirm thrombosis. Your pain levels will increase...you won't be able to walk but a few meters without passing out from orthostatic hypotension. If you are on diuretics for BP CONTINUE THEM. ASK THE HOSPITAL DOC (DAY 1) IF YOU CAN HAVE SOMEONE BRING YOU 20-30 mm thigh high COMPRESSION STOCKINGS FOR IMMEDIATE USE. You will need measurements of your ankle, calf, and thigh at the widest part. You will only use these until about two or three weeks after you get home then order some stockings at 30-40mm for the next 4-6weeks and then go to 40-50mm. As soon as you can (when the 30-40mm began to stretch and lose compression.The hospital may give you some short knee high compression socks which are useless and just slow your progress or they may use a machine with a pressure cuff to stimulate circulation---they usually don't get the cuff adjusted tight enough to do any good....take some control here and do it yourself. Each day, Ask your hospital doc if your vital signs have returned to normal (such as kidney function). If so then don't let the hospital keep putting IV fluid into your body...that just makes your leg feel like it is going to burst plus it makes it almost physically impossible for you to use the toilet in your room no matter where it is. They will argue the IV is for your benefit. Tolerate what you can but take control. You will also feel your upper body begin to retain fluid as well. This will also make going to the toilet a lot more difficult. Until you can get the fluid in your body down (ask doc to use lasix for that) you will be in great pain and pretty immobile and severely constipated and unable to pee. You will be put on Coumadin or Warfarin accompanied with a few shots of Lovenox injections for about ten days. The Coumadin you may be on forever. You will also be given medication for pain.

Meanwhile, painful swelling of the calves is seen in deep vein thrombosis. Angioedema is seen as swelling of the tongue, lips, throat and extremities. This is an uncommon, known side effect of ACE inhibitors, an antihypertensive medication. Hopefully this client had been instructed by his prescriber or nurse to call 911 if he experienced swelling of his tongue, lips, and throat; as the situation can sometimes become critical and need emergent treatment! NURSES should never take lightly the importance of their responsibility to explain side effects of the medications that they are administering to their clients. It may save a life!

Cosmetic surgery dangers, man’s heart rate and blood pressure plummet during surgery
Australian, Kevin Wong, age 27, almost died during a cosmetic surgery procedure to place a mandibular implant into his lower jaw. Three years ago Kevin underwent another type of plastic surgery called facial contouring to shave down his jawbone, because at the time he thought his face was too wide. After the procedure he thought the doctors had gone too far and that his face looked thin. So he decided to come to Taiwan to have an implant put in to continue to shape his face. During the procedure, Kevin stopped breathing and his heart rate and blood pressure plummeted dangerously low. He was rushed to emergency services at Taipei’s Chang Gung Memorial Hospital where doctors were able to revive him. Typical facial contouring surgeries involve shaving down the cheek bones, or the jawline of a patient’s face. The front of the jawbone where the chin is can also be shaved to make a smaller chin. If patients want a stronger chin or are dissatisfied with the results of their original surgery a chin implant can be put in place through the inner part of the lower lip. Patients do not have to be fully anesthetized during this procedure but should discuss options with their surgeon. Patellar bursitis •Pain and inflammation located in the front of the kneecap. •The bursa becomes inflamed and fills with fluid at the top of the knee. •Causes pain, swelling, tenderness and a lump in the area on top of the kneecap. Arthritis of the knee joint •Characterized by progressive wearing away of the cartilage of the joint. •The knee is a common part of the body that is most affected by arthritis. •Decreased joint space. Baker’s cyst •Swelling in the back of the knee filled with synovial fluid. •Cyst between the semimembranous and medial gastrocnemius muscles. Sudden Hair loss on the feet and toes What this might mean is you have Serious circulation problems You may think not having hair on your toes is a good thing especially during sandal season, but having hair on your toes is actually a good thing. Get it checked out Sudden baldness can be a sign that your feet aren’t getting enough blood flow to sustain hair growth. Expect your doctor to check for a pulse in your feet, which is an indication that your heart may not be able to pump enough blood to your feet. 2. You feel Frequent foot cramping What this may mean is your suffering from either Dehydration or nutritional deficiencies Randomly occurring cramps are extremely common in the feet so don’t get too worried. They can be as serious as circulation and nerve issues, or as harmless as a nutritional deficiency. If you’re exercising, be sure to drink plenty of water, since dehydration often leads to muscle cramping. You might also try upping your intake of potassium, magnesium, and calcium (with your doctor’s go-ahead, of course), since their deficiencies make cramps more common. Or you may like to try For relief, soaking your feet in a warm foot bath and stretching your toes toward your nose, not pointing down. If the cramps don’t let up, see your doctor who can test for circulation issues or nerve damage. 3. How to Stop Leg Muscle Cramps
Legs seem to have a mind of their own. Who hasn’t been ready to slip off to dream world when suddenly your legs begin to contract and cramp? Strangely, they don’t seem to cramp during the day. These nocturnal cramps are common and can prevent a good night’s sleep. They can also generate pain in the feet, calves and thighs. Cramps last anywhere from a few seconds to a few minutes. Occasionally, they last for days. In today’s video, we’ll explore ways to prevent leg cramps. The 3 major causes of leg cramps are: - Lack of exercise - Dehydration - Poor blood flow Let’s look at a few ways to prevent leg cramps: Magnesium deficiency Magnesium is an all-purpose mineral and works wonders in alleviating leg cramps. To add magnesium to your diet, eat plenty of dark leafy greens, bananas and dates. Other magnesium-rich foods include pumpkin seeds, yogurt, and dark chocolate. Another way to add magnesium is to use its oil form. Simply massage on your legs before turning in for the night. 10 Steps to Accurate Manual Blood Pressure Measurement/How to get accurate readings 10 Steps to Accurate Manual Blood Pressure Measurement Another observation the AHA makes is that although the auscultatory method has been the standard for blood pressure measurement for over 50 years, surveys indicate that physicians rarely follow the well published guidelines for their use. This is where we feel it is important to remind all healthcare professionals of best practices related to manual blood pressure measurement, so without further ado... Step 1 - Choose the right equipment: What you will need: 1. A quality stethoscope 2. An appropriately sized blood pressure cuff 3. A blood pressure measurement instrument such as an aneroid or mercury column sphygmomanometer or an automated device with a manual inflate mode. Step 2 - Prepare the patient: Make sure the patient is relaxed by allowing 5 minutes to relax before the first reading. The patient should sit upright with their upper arm positioned so it is level with their heart and feet flat on the floor. Remove excess clothing that might interfere with the BP cuff or constrict blood flow in the arm. Be sure you and the patient refrain from talking during the reading. Step 3 - Choose the proper BP cuff size: Most measurement errors occur by not taking the time to choose the proper cuff size. Wrap the cuff around the patient's arm and use the INDEX line to determine if the patient's arm circumference falls within the RANGE area. Otherwise, choose the appropriate smaller or larger cuff. 10 Steps to Accurate Manual Blood Pressure Measurement/How to get accurate readings Step 4 - Place the BP cuff on the patient's arm: Palpate/locate the brachial artery and position the BP cuff so that the ARTERY marker points to the brachial artery. Wrap the BP cuff snugly around the arm. Step 5 - Position the stethoscope: On the same arm that you placed the BP cuff, palpate the arm at the ante cubical fossa (crease of the arm) to locate the strongest pulse sounds and place the bell of the stethoscope over the brachial artery at this location. Step 6 - Inflate the BP cuff: Begin pumping the cuff bulb as you listen to the pulse sounds. When the BP cuff has inflated enough to stop blood flow you should hear no sounds through the stethoscope. 10 Steps to Accurate Manual Blood Pressure Measurement/How to get accurate readings Step 7 - Slowly Deflate the BP cuff . The Doctor recommends that the pressure should fall at 2 - 3 mmHg per second, anything faster may likely result in an inaccurate measurement. . Step 8 - Listen for the Systolic Reading: The first occurrence of rhythmic sounds heard as blood begins to flow through the artery is the patient's systolic pressure. This may resemble a tapping noise at first. 10 Steps to Accurate Manual Blood Pressure Measurement/How to get accurate readings Step 9 - Listen for the Diastolic Reading: Continue to listen as the BP cuff pressure drops and the sounds fade. Note the gauge reading when the rhythmic sounds stop. This will be the diastolic reading. Step 10 - Double Check for Accuracy: The AHA recommends taking a reading with both arms and averaging the readings. To check the pressure again for accuracy wait about five minutes between readings. Typically, blood pressure is higher in the mornings and lower in the evenings. Injury is found in the typical location; middle of the femoral condyle and posterior part of the tibia laterally. Posterior cruciate ligament tear (PCL) •Common cause of injury is a bent knee hitting a dashboard in a car accident. Tibial Sag Test –PCL knee exam Quadriceps active test-PCL knee exam •The examiner stabilizes the leg of the patient and then the patient is asked to actively contract the quadriceps muscle. •The tibia is seen actively reduced from the posterior subluxed position. Lachman’s test-PCL knee exam •Knee is bent 20-30 degrees. •The posterior drawer test is carried out while the patient is in a supine position and the knee is flexed to 90 degrees. •The amount of translation of the tibia relative to the femur is observed. The dial test is performed while the patient is in the supine or prone position and both knees are in 90 and 30 degrees of flexion. The hard part about these special effects pranks (sfx pranks) is that they are very hard to make look real. We had to do make it really look like real guts and real gore were bursting out of his stomach. Kind of like the chest burster scene in alien. We used CO2 cannisters and a camel pack bladder, hidden inside of a weightbelt, and triggered by wireless remote to make this prank look real. Look at the audience reactions and you'll see that people really believed a man's guts were exploding. Careful, it's gross, but at least it wasn't the weightlifter prolapsed rectum video that went around a while back.
