Type 1 diabetes, formally known as juvenile diabetes, is a disease characterized by the inability of patients to produce their own insulin hormone. It currently afflicts an estimated three million Americans. Within days after undergoing a novel transplant technique for insulin producing cells, the Diabetes Research Institute’s first patient in its BioHub pilot trial was able to discontinue insulin therapy. Wendy joined DRI researchers in announcing these remarkable results at a press conference held three weeks after her transplant. In this pilot clinical trial, DRI researchers are testing the omentum as an alternative transplant site to the liver, the traditional site. The donor islet cells were implanted onto the omentum within a biodegradable scaffold, one of the BioHub platforms. This FDA-approved Phase I/II study builds upon decades of progress in islet transplantation and is an important first step toward the development of the DRI BioHub, a bioengineered mini-organ that will mimic the native pancreas to restore natural insulin production in people with type 1 diabetes.

Some people who have type 2 diabetes can achieve their target blood sugar levels with diet and exercise alone, but many also need diabetes medications or insulin therapy. The decision about which medications are best depends on many factors, including your blood sugar level and any other health problems you have. Your doctor might even combine drugs from different classes to help you control your blood sugar in several different ways. 1. Metformin Generally, metformin is the first medication prescribed for type 2 diabetes. It works by improving the sensitivity of your body tissues to insulin so that your body uses insulin more effectively. Metformin also lowers glucose production in the liver. Metformin may not lower blood sugar enough on its own. Your doctor will also recommend lifestyle changes, such as losing weight and becoming more active. 2. Meglitinides These medications work like sulfonylureas by stimulating the pancreas to secrete more insulin, but they're faster acting, and the duration of their effect in the body is shorter. They also have a risk of causing low blood sugar, but this risk is lower than with sulfonylureas. Weight gain is a possibility with this class of medications as well. Examples include repaglinide (Prandin) and nateglinide (Starlix). 3. Thiazolidinediones Like metformin, these medications make the body's tissues more sensitive to insulin. This class of medications has been linked to weight gain and other more-serious side effects, such as an increased risk of heart failure and fractures. Because of these risks, these medications generally aren't a first-choice treatment. Rosiglitazone (Avandia) and pioglitazone (Actos) are examples of thiazolidinediones. 4. Sulfonylureas These medications help your body secrete more insulin. Examples of medications in this class include glyburide (DiaBeta, Glynase), glipizide (Glucotrol) and glimepiride (Amaryl). Possible side effects include low blood sugar and weight gain. 5. GLP-1 receptor agonists These medications slow digestion and help lower blood sugar levels, though not as much as sulfonylureas. Their use is often associated with some weight loss. This class of medications isn't recommended for use by itself. Exenatide (Byetta) and liraglutide (Victoza) are examples of GLP-1 receptor agonists. Possible side effects include nausea and an increased risk of pancreatitis. 6. SGLT2 inhibitors These are the newest diabetes drugs on the market. They work by preventing the kidneys from reabsorbing sugar into the blood. Instead, the sugar is excreted in the urine. Examples include canagliflozin (Invokana) and dapagliflozin (Farxiga). Side effects may include yeast infections and urinary tract infections, increased urination and hypotension. 7. DPP-4 inhibitors These medications help reduce blood sugar levels, but tend to have a modest effect. They don't cause weight gain. Examples of these medications are sitagliptin (Januvia), saxagliptin (Onglyza) and linagliptin (Tradjenta).

Research now suggests that An injectable class of diabetes medication might one day be available in pill form. Based on the results of a global clinical trial, study authors reported a substantial drop in blood sugar levels for people on the oral medication. They found no significant increase in low blood sugar levels compared to a placebo over six months. Diabetes refers to a group of conditions characterized by a high level of blood glucose, commonly referred to as blood sugar. Too much sugar in the blood can cause serious, sometimes life-threatening health problems. There are two types of chronic diabetic conditions: type 1 diabetes and type 2 diabetes. Pregnant women may acquire a transient form of the disease called “gestational diabetes” which usually resolves after the birth of baby. Pre-diabetes is when the blood sugar level is at the borderline: higher than normal, but lower than in diabetics. Prediabetes may or may not progress to diabetes. During food digestion, carbohydrates - or carb - break down into glucose which is carried by the bloodstream to various organs of the body. Here, it is either consumed as an energy source - in muscles for example - or is stored for later use in the liver. Insulin is a hormone produced by beta cells of the pancreas and is necessary for glucose intake by target cells. In other words, when insulin is deficient, muscle or liver cells are unable to use or store glucose, and as a result, glucose accumulates in the blood. In healthy people, beta cells of the pancreas produce insulin; insulin binds to its receptor on target cells and induces glucose intake. In type 1 diabetes, beta cells of the pancreas are destroyed by the immune system by mistake. The reason why this happens is unclear, but genetic factors are believed to play a major role. Insulin production is reduced; less insulin binds to its receptor on target cells; less glucose is taken into the cells, more glucose stays in the blood. Type 1 is characterized by early onset, symptoms commonly start suddenly and before the age of 20. Type 1 diabetes is normally managed with insulin injection. Type 1 diabetics are therefore “insulin dependent”. In type 2 diabetes, the pancreas produces enough insulin but something goes wrong either with receptor binding or insulin signaling inside the target cells. The cells are not responsive to insulin and therefore cannot import glucose; glucose stays in the blood. In other words, type 2 diabetics are “insulin resistant”. Here again, genetic factors predispose susceptibility to the disease, but it is believed that lifestyle plays a very important role in type 2. Typically, obesity, inactive lifestyle, and unhealthy diet are associated with higher risk of type 2 diabetes. Type 2 is characterized by adult onset; symptoms usually appear gradually and start after the age of 30. Type 2 diabetes accounts for about 80 to 90% of all diabetics. Management focuses on weight loss and includes a low-carb diet.

http://www.PreOp.com Diabetes Patient Education Insulin is the hormone normally made in the pancreas that stimulates the flow of sugar - glucose - from the blood into the cells of the body. Glucose provides the cells with the energy they need to function. There are two main groups of insulins used in the treatment of diabetes: human insulins and analog insulins, made by recombinant DNA technology. The concentration of most insulins available in the United States is 100 units per milliliter. A milliliter is equal to a cubic centimeter. All insulin syringes are graduated to match this insulin concentration. There are four categories of insulins depending on how quickly they start to work in the body after injection: * Very rapid acting insulin * Regular, or Rapid acting insulins * Intermediate acting insulins * Long acting insulin. Diabetes Patient Education * In addition, some insulins are marketed mixed together in different proportions to provide both rapid and long acting effects. Certain insulins can also be mixed together in the same syringe immediately prior to injection. A very rapid acting form of insulin called Lispro insulin is marketed under the trade name of Humalog. A second form of very rapid acting insulin is called Aspart and is marketed under the trade name Novolog. Humalog and Novolog are: * clear liquids. * They begin to work 10 minutes after injection, * peak at 1 hour after injection, * and last 3-4 hours in the body. Diabetes Patient Education * Humalog and Novolog are used as "bolus" insulins to be given 15 minutes before a meal. * Most patients also need a longer-acting insulin to maintain good control of their blood sugar. Humalog and Novolog can be mixed with NPH, Lente and Ultralente insulins. * Check your blood sugar level before giving Humalog or Novalog. * Your doctor or diabetes educator will instruct you in determining your insulin dose based on your blood sugar reading and anticipated meals and exercise. * Always check the bottle before drawing up the insulin. If the solution is cloudy, discard the bottle. * If you are mixing Humalog or Novalog with a longer-acting insulin, always draw up the Humalog or Novalog first to maintain the purity and clarity of the Humalog and Novalog solutions. Another solution of insulin that acts rapidly is called "Regular" or "R" insulin. This insulin does not act as quickly as Humalog or Novalog. Regular insulin is: * a clear, colorless liquid. * It begins to work 30 minutes after injection, * peaks at 3-5 hours * and lasts 6-10 hours in the body. Diabetes Patient Education Regular insulin is: * usually given 30 minutes before a meal. * It can also be mixed with in the same syringe with longer acting NPH, Lente and Ultralente insulins or given separately immediately after each other. * Glargine cannot be mixed with it. Regular insulin is: * the most stable of all the different types of insulin, * but unopened regular insulin is best refrigerated. * Always check the bottle before drawing up the insulin. * If the solution is cloudy, discard the bottle. Diabetes Patient Education * If you are mixing Regular with a longer-acting insulin, always draw up the Regular insulin first to maintain the purity and clarity of the Regular solution. Examples of intermediate acting insulins are NPH and Lente insulins. Intermediate acting insulins are: * cloudy suspensions of crystalline insulin. * They need to be gently rotated between the hands before being used. * They begin to work 1 hour after injection, * peak at 6-12 hours after injection * and last 20-24 hours in the body. The intermediate insulins are: * often given before breakfast. * They may also be given at bedtime, depending on your blood glucose reading. * They can be mixed in the same syringe with Regular, Lispro and Aspart insulins. Ultralente is a long-acting insulin. Ultralente is: * a cloudy suspension. * It begins to work 2-8 hours after injection, * peaks at about 12 hours * and lasts around 18-24 hours in the body. Ultralente insulin is: * often given before breakfast. * It may also be given before dinner in the evenings or at bedtime, on your doctor's instruction. * It can be mixed in the same syringe with regular insulin. * When unopened, it is best stored in the refrigerator. I use an older model for my night time insulin (Levemir) which holds 300 units. There is a new style called the Echo Pen for fast acting. It dials up by points and half points, which is really convenient. What's really cool is, you can also track the last time you took an injection and how much you've taken (because it's really easy to forget). I use this one for my NovoRapid. I've seen some that are longer, but you just need a short needle to pierce the skin, even if you're a larger person. They have different colors (green, yellow) to avoid confusion. I take around 30 units at night, so I need to remember which one is which. The "old faithful" is an extra PenFill that I bring with me when I travel. With these cartridges, you can draw from the top. When these are done, pull the last bits of insulin out to save for later. So you have the PenFills, and faithful 100 unit insulin syringe.

However, the ranges will vary depending on the individual and an individual’s circumstances. Talk to your doctor or Credentialled Diabetes Educator for the range of blood glucose levels that are right and safe for you. Keeping your blood glucose level at the optimum range is a careful balance between what food is eaten, physical activity and medication. Blood glucose levels which are too high, could result in hyperglycaemia or ketoacidosis. Blood glucose levels which are too low, could result in hypoglycaemia. It is important to learn about each reaction and respond appropriately. Ketoacidosis is an emergency and you must call emergency services immediately. Monitor blood glucose levels throughout the day and even at night. Keeping your blood glucose levels on target will help prevent both short-term and long-term complications. A cell-based diabetes treatment has been developed by scientists who say it could eliminate the need for those with the condition to inject insulin. The therapy involves a capsule of genetically engineered cells implanted under the skin that automatically release insulin as required. Diabetic mice that were treated with the cells were found to have normal blood sugar levels for several weeks. Scientists said they hope to obtain a clinical trial licence to test the technology in patients within two years. If successful, the treatment would be relevant for all type 1 diabetes patients, as well as those cases of type 2 diabetes that require insulin injections. Martin Fussenegger, who led the research at the ETH university in Basel, said: “By 2040, every tenth human on the planet will suffer from some kind of diabetes, that’s dramatic. We should be able to do a lot better than people measuring their glucose.” Fussenegger said that, if confirmed as safe and effective in humans, diabetes patients could be given an implant that would need to be replaced three times a year rather than injections, which do not perfectly control blood sugar levels, leading to long-term complications including eye, nerve and heart damage. In Britain, about 400,000 people have type 1 diabetes and three million have type 2 diabetes, about 10% of whom need to inject insulin to control the condition. Type 1 diabetes normally begins in childhood and is an autoimmune disease in which the body kills off all its pancreatic beta cells. The cells respond to the body’s fluctuating glucose levels by releasing insulin, which regulates blood sugar. Without beta cells, patients need to monitor glucose and inject insulin as required – typically several times each day. His team took a different approach, choosing to re-engineer human kidney cells, known as HEK cells, to perform the function normally carried out by the pancreas.
