Part A, B, C and D? Don’t panic. I’ll explain what all this alphabet means! There are four main “parts” of Medicare insurance: Part A, Part B, Part C, and Part D. Medicare Part A (hospital insurance) and Medicare Part B (medical insurance) together make up Original Medicare. Medicare Part C, also known as Medicare Advantage, and Medicare Part D (prescription drug coverage) programs that let you get Medicare plans through private insurance companies that contract with Medicare. Let’s look at each part individually. Medicare Part A typically pays for your inpatient hospital expenses, hospice services, home health care, and care in a skilled nursing facility. You don’t have to pay extra for Part A if you have worked and paid Social Security taxes for at least 40 calendar quarters ( or 10 years); Medicare Part B typically covers most medically necessary doctors’ services, preventive care, durable medical equipment, hospital outpatient services, lab tests, x-rays, mental health care, and some home health and ambulance services. You pay a monthly premium for this coverage. Medicare Part C (Medicare Advantage) are private health plans These policies, must cover at least the same benefits covered under Medicare Part A and Part B; however, your costs may be different, and you may have extra benefits, such as coverage for prescription drugs or extra days in the hospital. If you want, you can choose to get your Medicare coverage through a Medicare Advantage Plan instead of Original Medicare. Some plans also cover Part D as part of your Medicare Advantage benefits package. You may pay a monthly premium for this coverage, in addition to your Part B premium. Part D is the part of Medicare that provides outpatient prescription drug coverage. Part D is provided only through private insurance companies that have contracts with the government — it is never provided directly by the government (like Original Medicare is). If you want Part D, it is very important to select Part D coverage that works with your Medicare health benefits. If you have Original Medicare, choose a stand-alone Part D plan (PDP). It is very important to choose a plan that covers your medications. Every Medicare Prescription Drug Plan has a formulary that lists the drugs it covers. A plan’s formulary may change at any time, so make sure to check it periodically. I hope this information was helpful! Selecting between Medicare Supplement and Medicare Advantage plans is an important decision for those turning 65 or retiring and selecting their Medicare coverage for the first time. This video explains the difference between Medicare Advantage and Medicare Supplement plans, also known as Medigap. We will cover things like the premium, cost, copay, deductible, out of pocket expenses, and networks of these two popular supplement insurance plans. This video will help in your decision making process as you select between Medicare Advantage and Medicare supplement plans. This is another one of the most common questions that we get here at Medicare hero is how do I decide which type of coverage is really best for me. Medicare Supplement: So first let's talk about Medicare supplement plans. The monthly premiums for Medicare supplement plans range from about $85 to $170 dollars a month, actually for people turning 65 I see an average of about $100 to $120 per month for their coverage. The premiums you pay all depends on your age, gender, and where you live. With a Medicare Supplemental plans we can see any doctor or any Hospital in the entire country that accepts Medicare there's absolutely no networks, and when we go to see those doctors there's no co-pays, no deductibles, and no out-of-pocket expenses. All we do is pay our monthly Insurance premiums and we're done. Additionally with a Medicare supplement plan you never need a referral to see a specialist, and again it doesn't matter which insurance company you select such as AARP United Healthcare, Humana, Aetan, Cigna, Mutual of Omaha, or New Era, you can always go see a specialist without a referral when you have a Medigap Plan. Now the last part of Medicare I want to talk about is prescription drug coverage, Part D of Medicare. With a Medicare supplement plan you can select independently of your Medicare supplement insurance company a prescription drug RX plan. Medicare Advantage: Medicare Advantage plans were created as a low cost alternative to Original Medicare and Medicare supplement plans. When I say lower-cost what I mean is lower monthly premiums. So the premiums for Medicare advantage plans can be anywhere from $0 in addition to what you're already paying for part b of Medicare up to $60 or $70 and premiums can even be as high as a hundred dollars a month. The key with Medicare Advantage plans is that we want to keep our premiums low because that is the advantage is low monthly premiums. So one of the compromises that Medicare Advantage plans make in order to get those low monthly premiums is they have network so you want to make sure that your doctors and hospitals that you choose to use are in the network for the insurance company you're looking at. This means that Medicare insurance companies such as AARP United Healthcare will have a different network then Humana, Aetna, or bluecross blueshield. So just be careful make sure your doctors, specialists and hospitals are in network. Also your Networks and doctors can change mid-year but we can't necessarily change our Part C coverage. Additionally because we now get those low monthly premiums we now have to pay deductibles co-pays and other charges when we use our insurance coverage. So if you see a specialist you might have a small copay, if you go to the hospital you will have to pay a certain amount of pocket. So we get low monthly premiums with Medicare Advantage plans but then we have to spend money the more we use our healthcare insurance. Additionally Many of the most affordable Part C Medicare Advantage plans do require referrals to see a specialist. Now that we've had all the key points both Medicare advantage and Medicare supplement plans what's the bottom line? Well Medicare supplement plans are best for people who can afford the extra premiums and if you most value your choice of doctors and hospitals as well as a predictable healthcare budget where you're not going to have large healthcare expenses throughout the year. You know if you just pay your monthly premiums you're going to be set. Medicare Advantage plans on the other hand are simply the best if you're looking for low monthly premiums. But often times people are surprised to learn that they will actually pay MORE for their medications when they are on Medicare. Medicare drug coverage can be obtained either through a stand alone Part D drug plan, or through a Medicare Advantage Plan that includes Part D drug coverage. Every year, Medicare has a set formulary, or list of drugs that all Medicare Part D drug plans must cover. Aside from that, plans can differ in which medications they include in their particular formulary. Medicare has no “cap” on your out of pocket drug costs, and if you’re taking expensive medications you may fall into the “donut hole” or coverage gap. This means that when you reach a certain spending threshold, you’ll be responsible for a greater portion of the cost of your drugs. If you find that your Medication costs are getting too expensive, there are a few alternatives I like to recommend. 1. Pay cash without using your insurance card, and use coupons for discounts. My favorite website to recommend is goodrx.com — this site will let you know which pharmacies in your areas have the lowest prices for certain medications, and the site provides free coupons for many drugs as well. If you are at a risk for or close to reaching the Medicare “donut hole” this year, sometimes paying cash for some your less expensive generic medications can help you avoid the coverage gap. 2. Apply for Extra Help through the government. Certain Medicare beneficiaries can qualify for Extra Help with their Medicare prescription drug plan costs. This Extra Help is estimated to be worth about $4,000 per year. To qualify for the Extra Help a person must be receiving Medicare, have limited resources and income. In 2018, in order to qualify for extra help your combined savings, investments, must not be worth more than $28,150 if you are married, or $14,100 if you are not currently married or not living with your spouse. Resources like your home and your car don’t count toward this qualifying amount. To apply for extra help, you can go online to ssa.gov, or call social security at 1–800–772–1213. 3. Look into Co-pay cards, or Pharmaceutical assistance programs. Many pharmaceutical companies offer financial assistance programs for the drugs they manufacture. For folks talking expensive specialty medications for chronic conditions, you can often save thousands of dollars on your medications if you qualify. Qualification criteria will vary based on the drug manufacturer. To see if you qualify, visit https://www.medicare.gov/pharmaceutical-assistance-program/. Lastly, many folks tend to go on auto pilot when it comes to their stand alone Part D coverage. It is so important to shop your Part D drug plan annually during the Medicare Annual Election Period, which runs every fall from Oct. 15th — Dec. 7th. Each year your Part D Plan benefits can change, the formulary and tiers can change, and it’s important to review your policy annually to maximize your savings. What is the best Medicare Advantage Plan? This is a question I get almost every day. When I help someone choose a Medicare Advantage Plan that is best for their needs there are 5 main things we look for in the Medicare Advantage Plan (Medicare Part C Plan). The first thing you want to look for to find a good Medicare Advantage Plan is the Provider Network. In order to be a good plan for you it must have a large choice or network of providers. So when you choose the best Medicare Plan for you it is very important to make sure all of your Medical and Pharmacy providers are in the network. You may have a plan that allows you to see providers outside the network, but you will pay a higher cost share so it is best to stay in network if possible. The second thing I look for when helping someone select the best Medicare Advantage Plan for their needs is the Medicare Part D prescription drug coverage. We always make sure to look up all the medications a person has to help determine which plan will cover them all and give the lowest out of pocket costs. You also want to be sure that the plan will cover prescriptions that may pop up in the future as well. Medical Coverage is the third thing to look for. We always compare the medical coverage on the Medicare Advantage plans available in a clients area. One key factor in comparing Medicare plans is to look for coverages in plans where your out of pocket costs are limited to a specific dollar amount. This makes it easier to determine what you will be paying for services. It is much harder to estimate costs if your out of pocket portion is determined by a percentage of the total Medicare approved amount. Monthly cost is the fourth thing we look at. Some may say that the monthly cost is one of the most important factors in choosing the best Medicare Advantage Plan. This can be true because there are some plans out there that while they may have better coverage than some others, they can be way overpriced. You want to make sure the difference benefits make up for the difference in monthly premium. There is a large misconception that there are FREE Medicare Advantage Plans, it is true that there are $0 premium Medicare Advantage Plans out there, but there are out of pocket costs associated with the plans. If there is a higher cost Medicare Advantage Plan that you are considering then before you sign up for that plan it is a good idea to compare that with the best Medicare Supplement plan in the area. There are many Advantage Plans that are so high in monthly cost that it is better to choose a Medicare Supplement Plan that would give you much better Medical coverage. In my opinion one of the most important factors to determine the Medicare Advantage Plan that is right for someone is the Company they go with. You want to look for a plan with a larger reputable company that has been in the Medicare Advantage business for a while. There are a lot of small companies that pop up everywhere just to get a piece of the pie. They may be good for you for a year and then bad the next and you'd have to go through the whole process of finding a plan again. You want to avoid switching Medicare Advantage Plans every year. It can lead to a lot of issues for you because every Medicare Advantage plan has different networks, different prescription coverage, different health coverage, and many other things that could cause problems. You could end up needing to change medications or providers. If you do change plans just make sure you prepare for those differences and also prepare to get new referrals if your plan requires them. Another big tip I have for people with a Medicare Advantage Plan is "Don't get sold"! A lot of agents will convince you that you should change your plan over a 5 or 10 dollar difference in copays, premiums, or some other minor difference. This can cause problems like I mentioned before, unless there is a major change in your plan then "don't fix what isn't broken"! Don't get sold a plan, always make sure it is 100% in your best interest. It is important to know how this works with other insurance you have in order to get the best coverage. Here is what you need to know about this option. How Do You Get Coverage You have two options to get drug coverage including through Medicare Part C Advantage Plan or through Part D Prescription Drug Plan. The most common way is through Part D. This plan can be used with original Medicare and other plan option such as a Medical Savings Account (MSA) or Medicare Cost Plans. An example may be 20 or 25 percent. When you pay for drugs you are paying for a month supply unless stated otherwise. There is potential for out-of-pocket costs to be paid by the beneficiary if the insurance does not cover the cost, but in few cases this is rare. Additional Tips on Drug Coverage You can get help paying for Medicare drug coverage if you qualify. You may need to provide proof of documentation to show your eligibility for assistance. Such options may allow you to pay just a few dollars for medicine. You can obtain help when you are receiving medication and before your supply runs out for the month. The amount you pay for your plan varies based on drugs used, plan option, where you go to obtain prescriptions, if the drug is on the formulary approved by Medicare, and whether you qualify for assistance in paying for drug costs. You may qualify for assistance by your state if you receive Medicare Part A and/or Part B coverage. Original Medicare is the Medicare parts A & B and D that we have detailed in previous videos. In this video we will talk about Medicare Advantage plans. In our next video of this series we will discuss Medicare Supplement or Medigap plans. You are only allowed a Medicare Supplement plan if you keep your Original Medicare. You cannot have a Medicare Supplement or Medigap plan and a Medicare Advantage plan at the same time. So, let’s talk about Medicare Advantage plans. 1. Medicare Advantage plans replace your Original Medicare. They are not supplements. They do not supplement or enhance your Medicare coverage, they replace it. (show book) When you choose a Medicare Advantage plan the benefits and appeals processes outlined in this guide are no longer relevant. You can throw this book away. Your benefits will be detailed in an “Evidence of Coverage” provided by the insurance company you choose to replace your Medicare, along with their “Annual Notice of Change”. You no longer need to show your doctor your red, white and blue Medicare card or Medicare number, you will have a new card provided to you by the insurance company. You can no longer appeal your benefits to Medicare. Medicare is out of the picture as far as your personal service in concerned. If you decide to appeal an insurance decision, you must appeal to the insurance company. 2. Medicare Advantage plans are typically HMOs or PPOs and on occasion a Private-Fee-for-Service plan. HMO stands for Health Maintenance Organization. PPO stands for Preferred Provider Organization. We will talk more about both of those in a minute. What is important to understand is that Medicare Advantage plans are not standardized, they are regulated to meet a certain minimum coverage. That is an important concept, so let me explain further. Medicare Part A & B is standardized. Medicare Supplement plans are standardized. Everyone in those plans receives the exact same benefits, they can see the exact same doctors and should have the exact same experience. With Medicare Advantage plans, each plan has different benefits, different co-pays, and deductibles, covers different medical services and has a different network of doctors and hospitals. Everyone’s experience is different and depends on which plan they have chosen. In additional; all the benefits, co-pays, deductibles and the doctors and hospitals in the network can and will change each and every year. It will be your responsibility to keep up with the changes via their mandatory Annual Notice of Change. It’s your responsibility to study this annual notice and determine how the changes may impact your healthcare. Medicare Advantage plans are regulated to meet a certain minimum standard. That minimum standard dictates that they must be actuarially equivalent to Medicare Parts A and Part B. That’s an interesting term “actuarially equivalent”. What does that mean? How often do Medicare supplement rates increase? How much have they gone up in the past? Which carrier has the lowest rate increases? Where can I find this information. If you are turning 65 and enrolling in Medicare, this may be your only chance to purchase a Medigap plan. Here is your guide to understanding GA Medicare. You don't have to receive Social Security to enroll in Medicare insurance. CMS is the agency that oversees Medicare. Senior healthcare insurance is commonly used to describe Medicare. How to enroll in Medicare? You may sign up for Medicare 3 months before your 65th birthday, the month you turn 65, and 3 months following your 65th birthday. You may enroll online (highly recommended), by mail or by visiting a Social Security office. How does Medicare work? Bob Vineyard explains in this Georgia Medicare Minute video Original Medicare has 4 parts. Medicare Part A covers hospital inpatient charges and has a per admission deductible Medicare Part B is for outpatient health care treatment. Doctor visits, labs, X-rays, MRI, CT scan (CAT scan), ambulance, ER, outpatient clinics, etc. Essentially any Medicare approved health care received outside the hospital setting. Treatment considered medically necessary under Medicare guidelines. Medicare Part B has a calendar year deductible. Once satisfied, Medicare pays 80% of the approved charge, you are responsible for the remaining 20%. Most seniors worry about hospital stays, but your major expenses will more than likely fall under Medicare Part B outpatient expenses. Original Medicare stand alone has no cap on your out of pocket expenses. You pay until you run out of money, get well or die. Medicare Part C is now referred to as Medicare Advantage. More on that later. Medicare Part D covers outpatient prescription drugs. Some Advantage plans have drug coverage built into the plan. How are Georgia Medicare supplement plans priced? Almost all GA Medigap carriers have entry age pricing. The highest premium for any particular plan could be as much as 3x the lowest premium. Prior to that supplement plans were issued under the BCBSGA label. Since 2018 they have offered plan G and HDF in addition to other plans. Both plans were retired a few years ago and then G was reintroduced in 2017. Since 2010 Blue has used unisex rates and did not charge extra for tobacco use. They currently have male/female rates and do rate for tobacco use. These changes make rate increase history meaningless. Original Medicare does not have networks. You can use any doctor or hospital anywhere in the US. Another plus is the ABN form which is only used with Original Medicare. Advantage plans are managed care plans. Some are PPO, some are HMO. In 2018 about half the plans are HMO with the balance being PPO. If you receive non-emergency out of network care under a PPO you may have higher copay’s and deductibles. Your max OOP may also be higher. Some plans do not cap out of network charges. Non-emergency out of network care charges may be denied if you have an HMO. In 2018 most Advantage plans issued in Georgia limit your out of pocket for approved in network health care expenses to $6700. The American Cancer Society tells us that more than half the 1.4 million new cancer diagnoses occur in people age 65 and older. Roughly 20% of retiree deaths are a result of cancer. Medicare pays for almost half of the $74 billion spent on cancer treatment. The elderly account for 70% of cancer deaths each year. Some cancer centers, such as Mayo Clinic, Sloan Kettering and MD Anderson may not participate in certain Medicare Advantage plans. Not all cancer responds to chemotherapy, but 80% of cancers are treated with chemo. Those treatments occur in an outpatient setting and are covered under Medicare Part B. Chemotherapy drugs administered in a doctor's office or clinic normally fall under Part B. Medication from a retail pharmacy or by mail fall under your PDP. This is why most of us chose a Medicare supplement plan in 2018. A Medicare Advantage Plan is a health plan, you will have to go to certain Doctors / Providers and you will have copays as you use the Plan. What are Medicare Part D Drug Plans? Medicare Part D is a federal program administered through private insurance companies. These companies offer retail prescription drug coverage to Medicare beneficiaries. Prior to 2006, when the Medicare Part D began, tens of thousands of Medicare beneficiaries in America had little help with retail drug costs. They would often spend thousands of dollars each year paying for their medications out of pocket. Fortunately, today’s Medicare beneficiaries have better coverage with Part D. How do Medicare Part D drug plans work? Medicare Part D is simply insurance for your medication needs. You pay a monthly premium to an insurance carrier for your Part D plan. In return, you use the insurance carrier’s network of pharmacies to purchase your prescription medications. Instead of paying full price, you will pay a copay or percentage of the drug’s cost. The insurance company will pay the rest. Your Part D insurance card will be separate from your Medigap plan. Medicare Part D plans all follow federal guidelines. Each insurance carrier must submit its plan outline to the Centers for Medicare and Medicaid Services annually for approval. To improve your understanding of Medicare Part D, let’s look at the basic way that each Part D plan works. There are 4 stages to a Part D drug plan, as follows: Annual Deductible – in 2018, the allowable Medicare Part D deductible is $405. Plans may charge the full Part D deductible, a partial deductible, or waive the deductible entirely. You will pay the network discounted price for your medications until your plan tallies that you have satisfied the deductible. After that, you enter initial coverage. Initial Coverage – during this stage of Part D drug coverage, you will pay a copay for your medications based on the drug formulary. Each drug plan will separate its medications into tiers. Each tiers has a copy amount that you will pay. For example, a plan might assign a $7 copay for a Tier 1 generic medication. Maybe a Tier 3 is a preferred brand name for a $40 copay, and so on. The insurance company tracks the spending by both you and the insurance company until you have together spent a total of $3750 in 2018. The Coverage Gap – after you’ve reached the initial coverage limit for the year, you enter the coverage gap. During the gap, you will still generally have significant discounts for generic medications. You will pay only 35% of your brand name medications, and 44% of generics. (This is so much better than in 2006 when many people had to pay 100% of their drugs in the gap.) Your gap spending will continue until your total out of pocket drug costs have reached $5000 in 2018. Catastrophic Coverage – after you’ve reached the end of the coverage gap, your plan will kick in to pay 95% of the costs of your formulary medications for the rest of the year. This feature in Part D drug plans helps you limit your potential spending if you have expensive medications.
