Progress Seen in Building a Healthcare System in Rural Myanmar

Doctors have long struggled to reach patients in rural Myanmar, where ethnic insurgencies and political repression have made building a reliable healthcare system virtually impossible. But community health teams in eastern states, near the Thai border, say they are seeing evidence of improvement, in part because of political reforms and cease-fire agreements. Seven ethnic and community organizations recently conducted a survey to assess the overall health and availability of health care for about a half million people living in eastern Myanmar, also known as Burma. The Health Information System Working Group found a decline in conflict and more freedom of movement after the government signed cease-fire agreements with 14 of the 16 main armed groups has led to improvements in a range of health indicators, from the assisted delivery of babies to reduced malaria infections. This network of some 2,600 people includes medics, community health workers, specialists in maternal and child health, and trained traditional birth attendants. Saw Nay Htoo, director of programs, said reduced violence has allowed mobile health worker teams to reach local communities, although some legal restrictions remain. “Three or four years ago they could be arrested anytime, if you are caught by the Burmese government. But after the cease-fire agreement, temporary cease-fire agreement in 2012 more people can more or less have freedom of movement a little bit [more],” said Htoo. The health survey reported gains in reproductive, maternal and child health services. More than 70 per cent of women delivering their latest child had assistance from a trained traditional birth attendant. Lower rates of malnutrition among women were reported, but the survey found rates of malnutrition in children remained high with over 15 per cent of children under the age of five years suffering from acute malnutrition. The survey also found a greater use of bed nets to protect families from mosquito borne diseases such as malaria. Malaria remains a primary cause of death across all age groups at almost 20 percent, and the second main cause of death among children under five years. New drug resistant strains of malaria have been recorded spreading throughout Myanmar. Cynthia Muang, an ethnic Karen doctor and director of the Mae Tao clinic in the Thai border town of Mae Sot, said the reduction in fighting led to opportunities to rebuild local health care services affected by years of violence. She said there is also greater cross-border cooperation between the Thai and Myanmar ministries of health. “So what we have been doing in the community is we try to identify more communities, which we strengthen through collaboration. So we have put a lot of effort on the health system strengthening and we work with international partners and community organizations to upgrade the existing health services and health system,” said Muang. Despite the improvements, there are still enormous challenges in building a more comprehensive health system. The Thai based non-government group overseeing border refugee movements, The Border Consortium (TBC), said that since 1996, the destruction of some 3,700 villages has left about 400,000 internally displaced people in rural areas of eastern Myanmar. Voravit Suwanvanichkij, a research associate at the Center for Public Health and Human Rights at John Hopkins Bloomberg School of Public Health, said that while there is growing collaboration between the government and ethnic groups, leading to improvements in the overall system, more reforms are needed. “Burma desperately needs a health infrastructure. The peoples of Burma, particularly ethnic communities, lack access to essential health programs. Burma should really capitalize on the fact that many ethnic organizations and community organizations have been operating their own health programs for decades. To rapidly expand access there needs to be more collaboration, more decentralization,” said Voravit. Voravit said those first steps to make improvements include ending continuing rights abuses that worsen poverty and impact health care.

I wrote this years ago - I've repeated it and others have repeated it hundreds of times but ONE MORE TIME. "The End is Near" NOT The End is Here! for silver. BIG DIFFERENCE! As long as a bankster can drive the price down an extra penny why would they not do so leaving money on the table?. We all know the future but the future is NOT now. If I was a "bankster boss" and had employees showing mercy to those dumb enough to buy paper silver I would fire them! Their days of owning the price of paper silver are NOT over! They will not relent one day before they are forced to by the Law of Supply & Demand". Besides ONLY owning physical precious metals (no paper EVER!) and NEVER store in a bank box keep it well hidden and spread out over many different SAFE locations! Gold/silver have always been ultra long-term "SHTF" insurance. If the most unlikely happens I'll always REAL money. Gold/silver have been "money" for 5000 years. No fiat currency has ever lasted longer than the US dollar right now! The worth of the USD has dropped 99% since its inception in 1913 when the criminal Federal Reserve gained a monopoly to our money supply. Some sold their homes and cars to buy silver (at like $35 an ounce), put silver on credit cards and so on. NEVER have more than 25% of your liquid assets in any one class )gold/silver are in the same class). You need a house, cars, clothes and necessities of life plus a few money of old fashioned USD fiat currency (people will still accept dollars even if hyperinflation blows up the economy! - it may be a year or two before a gold/silver based system comes into being. Plus expect the government to try to confiscate your metals (ie 99% tax on metal sales) and your prepper supplies (to feed the 98% who were oblivious to the coming catastrophe! HIDE everything! Be stealth in all you do. IF you buy metals today I still prefer silver (too many reasons to list here). But also fill your hidden cache of supplies with tons(?) of barter goods. I buy mostly through Chinese portal ww dot alibaba dot com There are other "Asian Sources for one. Buying 5000 pocket knives for 40 cents each you see sporting goods stores selling for $5 you are not going to lose money! Think WAY outside the box! The seven key "must be X" to be a good barter item list is in my guidebook! ww infinitedangers dot com. You can go from zero to FULLY "prepped" in two weekends! Except for certain training. I purposely wrote the book such that 99% of everything is sold at Walmart as well as sporting goods stores. EASY to do and following my guide you do not end up buying items that "sounded good" but prove useless in a SHTF world. The book will pay for itself many times over. I also do private consulting for those who can afford it. But if the drop in metals has to sweating you should have NEVER put a penny in metals to begin with! Metals are long term SHTF insurance just like car, house and health insurance. Except THIS insurance costs you ZERO. I actually SAVE money by using items (ie canned foods) today I bought at lower prices a year ago. If the lights go out (for good or even a few weeks) while others panic and Walmart etc stores are cleaned out by rampaging mobs my survival group will be safe and sound and provisioned to live a year if need be. AND barter goods to extend that by years as needed. THINK barter! STOP wringing your hands over the daily gyrations of gold and silver. IMO we are near the bottom and about to head up (WAY up with silver). But patience patience! Go back and read the 1st paragraph. School of Medicine class speaker Michael Epstein, who is the son of two doctors, has been interested in science and medicine for nearly his whole life, so medical school was a natural choice. But it wasn't until he went on a medical mission to the Dominican Republic in the spring of his first year at UMass Medical School that he felt like his education had begun. "That trip felt like when medical school really started for me," said Epstein. "It was great to go down to a beautiful community of people and feel like I was able to help out." Dr Norman Swan interviews Professor Des Gorman on the serious challenges facing the distribution and alignment of health services in Australia. The status quo of local health systems is characterised by a variously mal-distributed health workforce, slow, isolated and poorly adaptive training programs, and consequential inequities in health access and quality. Professor Gorman notes that reform is essential if the current health care standards in Australia are to be maintained. Professor Gorman makes the point that long-term solutions to this problem needs to include a better alignment of the health and education systems with each other and patient care needs, better incentives for health professionals to work in rural areas, and, the innovative disruption of current models of health service. This includes using the community health worker as a 'bridging program' to provide articulation between a mal-aligned community and a community in need. Professor Gorman concludes that affirmative and immersion programs are needed throughout the education continuum. Affirmative schemes are the only way to diversify medical student populations sufficiently to create a cohort of practitioners that will address the health needs of indigenous and other minority populations. Introducing incentives, and having a larger emphasis on the duty of a community health care worker, will provide a solution to the significant mal-distribution problems in the Australian health system. He summarises his views as follows: First, the medical workforce is pear shaped. Second, reshaping the medical workforce will require informed and targeted affirmation and immersion. Sebastian Ferrero Foundation News Conference - 3/26/08

Nearly six months ago, Sebastian Ferrero, a happy, healthy three-year-old boy, died following a routine, outpatient test at a local hospital. On March 26, 2008, his parents announced their plans to turn their tragedy into something positive for others. Soon after Sebastian's death, Horst and Luisa Ferrero created the Sebastian Ferrero Foundation to honor their son and to advocate for improvements in pediatric care and patient safety. The ultimate goal of the Foundation is the construction of a full-service children's hospital in Gainesville. At a press conference yesterday, the couple offered to match, dollar-for-dollar up to $1 million, for donations made during 2008 to the Sebastian Ferrero Foundation. The match amount exceeds the $850,000 settlement they received for the loss of their son. The money raised by the Foundation will be used to create and implement a nationally recognized, comprehensive patient safety program at Shands HealthCare and ultimately, to build a state-of-the-art, full-service children's hospital. Regarding their ambitious goals, Luisa Ferrero stated, "All great endeavors must start somewhere, ours starts today." She concluded by saying, "we challenge the citizens, organizations and businesses in this great community to donate to the Sebastian Ferrero Foundation to make a difference. We all need to work together to improve healthcare." Representatives from Shands HealthCare and the University of Florida College of Medicine spoke at the press conference and pledged to work together with the Sebastian Ferrero Foundation to improve pediatric care and implement patient safety programs.
