Today, we're going to talk about men's yeast infections, particularly jock itch or itch around the men's private parts, around the scrotum, around the penis, around those areas. This is a condition I see quite frequently in males. A condition that drives a lot of guys crazy. In fact, I've had two cases in the last two days. One a 44-year-old male not far from where I live here in New Zealand and another guy in his late '40s from Wellington, which is the capital of New Zealand. Both of these cases are quite similar. This is going to be quite an interesting video, particularly for males who've got itch in their groin region and have had no resolution. And this specialist said it was psoriasis. Quite interesting. This doctor gave strong drugs, steroids, which really knocked the patient around quite a lot. The patient improved but wasn't interested in taking steroids and the patient then went to another doctor. This is doctor number four now. And this doctor said, "You've got a fungal skin rash." Finally, we got the correct diagnosis. But what did the doctor do? He put this man on antibiotics, which I find quite interesting. Antibiotics are not something one would generally give for a fungal rash that he or she has diagnosed as a fungal rash. They will normally give an anti-fungal drug, but in this case, the doctor decided to give a penicillin based antibiotic. And the condition exploded and now it went all over his torso, upper back, neck, around the ears, scalp, all over. I'll call him Gary. Gary said a few days after starting treatment, he had a shower and his whole body blew up in a terrible skin rash. Since then, he's been to about 14 different people. You're looking at a guy that's been to about 20 practitioners all up with no resolution. Interesting case. Upon examination of the patient, I had no doubt in my mind that it's a Candida fungal skin infection of the groin and various parts of the body. I found it interesting that nobody offered Gary any advice regarding his diet and lifestyle. Even the natural practitioners he went to offered only a small amount of advice here. My instruction to Gary was to wash the genital region, particularly, twice per day with a good tea tree oil based soap and also to use a water-soluble tea tree oil to wash the private area and to spot treat the tough areas with tea tree oil. This was to be done at least twice per day. I also recommended to Gary to go out and buy about 20 pairs of new cotton boxer shorts and to put on new underwear at least twice per day. This is very important if you're a guy with a fungal yeast infection is to maintain very good hygiene. Change your clothing often. A couple of tips for you are try not to hop in Jacuzzis or hot spas. They're not a very good idea because you'll warm the body up too much. Try to change underclothing frequently and wear loose clothing, particularly cotton clothing. Try to be careful with intimate relations with your partner. Maintain strict hygiene. And make sure your partner also is aware and probably goes on an anti-fungal regime as well. The other patient who's 47, however, is suicidal. This man phoned me up in tears. His wife had left him. His groin rash is so bad. He's eating far too many sweet foods. He can't control it. His business is in ruins. It's wrecking his life. If you thought yeast infections were only something that involved females, think again, because there are a lot of guys out there with fungal skin infections that really have ruined lives because of this terrible problem. The problem cannot be solved purely by dietary means. I'm telling you now; the problem cannot be solved purely by local means, by treating the skin. And the problem cannot be solved purely by lifestyle means. The problem has to be solved by doing different things and combining a proper treatment focused on the local area and systemic treatment. Also addressing underlying stress-related issues which a lot of guys suffer from.

Herpes, Herpes Simplex Virus (HSV) 1 and 2, are responsible for cold sores and genital herpes infections. HSV1 causes about 80% of cold sores, and HSV2 causes about 80% of genital herpes. HSV2 can be in the mouth, and HSV1 can cause genital herpes. About 60% of Americans have oral herpes by the time they reach kindergarten. This is from kissing their Aunt Bertha at their 3rd birthday party. Aunt Bertha had a cold sore when she kissed you, and now for the rest of your life, you carry the virus. If Aunt Bertha had type 2 in her mouth, you will have type 2 in your mouth. Herpes can be spread from mouth to mouth contact, genital to genital contact, and mouth to genital contact. Someone with a cold sore can give another person genital herpes, and likewise, someone with genital herpes and give their partner oral herpes (cold sores).Whichever type of herpes the "donor" has, will be the type the recipient catches. Two interesting spins on infection are herpes gladiatorum, (gladiator) is herpes infection that wrestlers catch, often times on the face or neck. They shave, have 100 tiny cuts in the skin, and when wrestling, their opponents cold sore rubs against their skin, and the virus enters thru a break in the skin. The second, called herpetic whitlow, is herpes infection in other parts of the body. Originally described in nurses who changed dressings of patients with herpes infection, they developed infections on the fingers, where the virus entered thru cuts or cracks in the skin. The current trend is for people to shave their genitals, which, like the wrestler, creates hundreds of tiny breaks in the skin, that should otherwise be protected from infection. We are seeing herpes infections farther from the genitals because of this. A condom does not protect one from infections beyond the area covered by the condom. I often see patients coming in and requesting testing for STDs. There is a blood test for HSV1 and 2. It is an antibody test, and only tells if you have had exposure to the virus (remember, the majority of Americans have), it does not diagnose active infection, nor does it tell the difference between cold sores and genital infection.(HSV1 and HSV2 can be in either location). Only, if one presents with an active sore, can we take a culture, and grow either HSV1 or HSV2. The test will differentiate the 2 types, and obviously, the location of the sore speaks for itself. The only clinical significance would be if you had HSV1 in the genitals, it does not prefer this location, and you would less likely have recurrent outbreaks. We have 3 medications that are used to treat infections. Valtrex, Famvir and Zovirax. None cure the infection, only suppress it. They can be used to decrease an acute outbreak, can be taken continuously to decrease the frequency and severity of attacks, and was recently shown, if taken daily, to decrease the chances of spreading the infection to a non-infected partner.

Hepatitis B Hepatitis B is a serious disease caused by a virus hepatitis B virus (HBV) that attacks the liver. It can cause lifelong infection, cirrhosis (scarring) of the liver, liver cancer, liver failure, and death. Symptoms  Jaundice (yellowing of the skin or whites of the eyes and/or a brownish or orange tint to the urine)  Unusually light-colored stool  Fever  Unexplained fatigue that persists for weeks or months  Gastrointestinal symptoms such as loss of appetite, nausea, and vomiting  Abdominal pain  Frequently there will be no symptoms, and it is only discovered in a blood test Often, symptoms occur one to six months after exposure. An estimated 30% of those infected do not show typical signs or symptoms. e). Hepatitis C In the U.S, 3.2 million people have Hepatitis C. Gonorrhea is a common sexually transmitted infection that often has no symptoms, can leave a woman unable to have children, and can be passed from the mother to child during childbirth. Having an active gonorrhea infection increases the risk you will get HIV or give it to someone else. HIV is incurable and can be fatal. Gonorrhea can be diagnosed with a test and can usually be cured with antibiotics. However, there is currently a worrisome increase in strains of gonorrhea that are resistant to antibiotics. You can have gonorrhea without knowing it. If you think you may have been exposed to or infected with any sexually transmitted infection, see a doctor and get tested whether or not you have any symptoms. All pregnant women should be checked for syphilis, HIV, and other sexually transmitted infections. Gonorrhea is transmitted by vaginal, oral or anal sexual contact. Gonorrhea can also be transmitted from mother to child during delivery. Signs and Symptoms of Gonorrhea Most women with gonorrhea, and some men, will have no symptoms. When a woman has symptoms of a gonorrhea infection in her vagina or urinary tract, they are often mild, may "be mistaken for a bladder or vaginal infection," and may include: • "painful or burning sensation when urinating" • "increased vaginal discharge" • "vaginal bleeding between periods" When a man has a gonorrhea infection in his penis or urinary tract, he may have symptoms that begin from one to fourteen days after infection including: • "burning sensation when urinating" • "a white, yellow, or green discharge from the penis" • "painful or swollen testicles" "Symptoms of rectal [gonorrhea] infection in both men and women may include:" • "discharge" • "anal itching" • "soreness" • "bleeding" • "painful bowel movements" Gonorrhea "infections in the throat may cause a sore throat, but usually" cause no symptoms. Effects of Untreated Gonorrhea in Women When not treated, gonorrhea can cause a woman to develop pelvic inflammatory disease, which in turn can cause: • abdominal pain • fever • "chronic pelvic pain" • infertility (inability to have a baby) • ectopic pregnancy (pregnancy outside the uterus, which can be fatal) Gonorrhea in Pregnant Women If you are a woman who is pregnant or who may become pregnant, it is important that you be tested for sexually transmitted infections. The United States Centers for Disease Control and Prevention recommends that all pregnant women be tested for sexually transmitted infections including HIV, syphilis, chlamydia, and Hepatitis B and that pregnant women who are at risk for gonorrhea or live where it is common be tested for gonorrhea. The World Health Organization recommends that all pregnant women be tested for HIV and syphilis and checked for symptoms of other sexually transmitted infections. If you are pregnant, have gonorrhea, and don't get treated, your baby may contract gonorrhea and have problems including: • blindness • joint infection • blood infection, which can be fatal By getting tested for gonorrhea and treated if you need it, you can protect your own health and your baby's health too. So if you are pregnant, talk to your doctor right away, get tested for sexually transmitted infections, and follow the doctor's instructions. Gonorrhea Treatment Gonorrhea is treated with antibiotics. Anytime your doctor gives you antibiotics, it's vital that you take them on schedule until they are gone. If you don't, it increases the risk that you won't be cured and that you will develop an infection that is harder to cure. But, when candida thrives in a hot humid, airless environment, it can grow in number and cause vaginal infection, although there are many species of candida, candida albicans is the most common. 2.-Bacterial vaginosis Together with yeast, "good" bacteria, called lactobacilli also live in the vagina, when the number of lactobacilli falls too low, it is possible that to trigger a condition called bacterial vaginosis. Bacterial vagninosis can produce a thick discharge and whitish, transparent, slippery discharge in women, which is not likely to cause itching or burning, but it is perceived a fishy smell, especially during sexual intercourse. 3.-Trichomonas vaginalis Of the three most common vaginal infections, trichomonas vaginalis, is the only one that is a sexually transmitted infection, in the medical medium it is known as "trichoic", Is caused by a single-celled parasite trichomonas vaginalis, which is transmitted from partner to partner during sexual act. ***NOT INCLUDE IN VIDEO*** Other types of known vaginal infections are also: Chlamydia vaginitis Chlamydia is a sexually transmitted disease that causes inflammation of the vagina, bleeding may occur on some occasions but not always. Important: an infected individual, with or without HIV symptoms, is at particularly high risk of transmitting HIV to sexual or drug using partners during the early stages of HIV infection because of the very high levels of HIV in the affected individual’s blood stream- it is very important to take proper steps in order to reduce risk of transmission. During the early stages of HIV infection the body is not yet producing HIV antibodies used by HIV tests in order to detect HIV infections. In 2013, a new HIV test was approved that can detect the presence of HIV in human body during the early stage of infection. It is very important to inform the health care provider about a recent HIV infection and to be tested for acute HIV. Once the HIV virus has entered a human body, it starts to invade immune cells, continues to replicate (multiply), destroy the cells, and invade further cells with even more numbers of the virus- the result is a gradual decrease in the immune system potency: immune deficiency. The early HIV symptoms are depending on the infected individual: age, health status, and other factors will mostly affect how soon or late HIV starts to show its symptoms. Stage of the disease is another determining factor of symptom development. HIV Symptoms in Different Stages: EARLY STAGE OF HIV: “Acute Retro-viral Syndrome” (ARS) or “Primary HIV Infection” Flu like symptoms are one of the most commonly experienced symptoms of early HIV infection which I usually described as the “worst flu ever.” These HIV symptoms usually start within 2-4 weeks after HIV infection has happened and is experienced by many, but not all, infected individuals. This symptoms result as the body’s natural response to the HIV infection. Symptoms of ARS may include fever (this is the most common symptom), swollen glands, sore throat, rash, fatigue, muscle and joint aches and pains, and headaches. The Clinical Latency Stage: “Asymptomatic HIV Infection” or “Chronic HIV Infection” The “silent”, “dormant”, or “latency” period of HIV infection is the least symptomatic and sometimes the longest period of HIV infection- it can last an average of 10 years. During the latency period, the virus is leaving in or developing in the infected individual causing only mild to no HIV symptoms. During this clinical latency period, the virus reproduces and increases in numbers only very slowly. Stronger individuals or individuals on anti-retroviral therapy (ART) may remain in this stage for many decades; weak individuals on the other hand, or those without proper treatment, may progress to the last stage of the disease in a much shorter period of time. Important: Individuals in the symptom-free latency period are still able to transmit HIV to others even if on ART. Late/End Stage of HIV: AIDS: Acquired Immunodeficiency Syndrome After the latency period, and with the lack of proper anti-retroviral medication, the HIV infection is most probably going to progress to the fully developed stage of the disease: AIDS. AIDS is the last stage of HIV infection where the virus has damaged and weakened the immune system in so far that it is not anymore able to fight simple infections, or certain types of cancer, which it usually would not be having any troubles to getting rid of. If you think you may have been exposed to or infected with any sexually transmitted infection, see a doctor and get tested whether or not you have any symptoms. All pregnant women should be checked for syphilis, HIV, and other sexually transmitted infections. Syphilis is usually transmitted by vaginal, oral, or anal sexual contact. The second most common way syphilis is transmitted is from mother to child during the pregnancy, or during delivery. You can also get syphilis by kissing on or near a syphilis sore or by touching an infected area on the person. Soon after infection, a sore develops that is usually round, hard, raised, and painless. Usually the sore is on the genitals, hands, or mouth. Usually, there is just one, but sometimes there can be multiple sores. Without treatment, the sore will usually heal within three to six weeks. In many people with syphilis, lymph nodes near the sore will swell up, especially if the sore is near the genitals. Lymph nodes are small balls in your neck, underarms, groin, and knees that help fight infections. Even after the initial sore has healed, your lymph nodes may stay enlarged. Without treatment, the infection will usually develop into secondary syphilis between three and six weeks after the sore appears. At this stage, you may have flu-like symptoms, a sore throat, feel tired, lose your appetite, or have swollen lymph nodes. Most commonly, you will have a rash of red or pink spots on your body. The spots may become similar to a pimple or a mark. The spots will often appear on the sides of your body, your arms, or your genitals or on the palms of your hands or the soles of your feet. You may also get white or gray spots on your mouth or genitals. A few people will lose patches of hair from their scalp, beard or eyebrows. You may also develop problems with your kidneys, eyes, liver, bones, or joints; meningitis; or deafness. In the final stage of syphilis, you may develop spots on your body, most commonly on the skin, bone, or liver. The skin spots may turn into ulcers. You may also develop dementia, paralysis, and damage to the spinal cord and brain. At any stage of syphilis, you may also develop meningitis, headaches, nausea, vision and hearing problems, dizziness, memory and speech problems, irritability, and delusions. If you are a woman who is pregnant or who may become pregnant, it is vital that you be tested for syphilis. If you are pregnant, have syphilis, and don't get treated, more than two-thirds of the time, your pregnancy will have a poor outcome. But feel free to leave a comment if you've tried them to let us know how effective they were. WHAT ARE COLD SORES: About 1 in 5 people in the UK have recurring cold sores. Cold sores usually resolve on their own without treatment in 7-10 days. Cold sores, also known as fever blisters or oral herpes, are very common. They can be easy to recognise as they usually appear as red bumps or blisters around the lips and mouth. Cold sores are caused by herpes simplex virus 1 (HSV-1), the oral form of the virus. In fact, 67% of the world population under age 50 has HSV-1 because it is so contagious Once you are infected, the virus never leaves your body. Most people aren’t exactly sure when they first encountered the virus. It’s usually contracted in early childhood where it may not appear as a visible cold sore. HOW DO COLD SORES SPREAD: The oral herpes virus is spread through direct skin-to-skin contact. It is even contagious when you can’t see a cold sore. Most people first contract the cold sore virus (HSV-1) when they are very young, usually by skin-to-skin contact with an adult carrying the virus. The virus can spread in various ways: through kissing, or by sharing objects like toothbrushes, water bottles, drinking glasses, and silverware. It is also possible to spread the virus from the mouth to the genitals, eyes, and other parts of the body. COLD SORE TRIGGERS WHICH MAY BE PREVENTED: Not everyones cold sores are triggered by the same thing, but these triggers are the ones which could be avoided to prevent a cold sore attack. • Stress can wear down your immune system, giving that dormant cold sore a chance to launch a sneak attack. Instead, when you feel stressed, breathe deeply and relax. This has been found to prevent some bouts of cold sores in some people. Do not share lip balms with other people if you have cold sores. • Prevent chapped lips in the cold by using a hydrating lip balm. COLD SORE TREATMENT: Aciclovir can be bought over-the-counter (OTC) at pharmacies. This does not kill the virus but prevents the virus from multiplying. It has little effect on existing blisters but may prevent them from becoming worse. The cream may provide some protection against cold sores caused by sunlight if it is used before exposure. If you use an antiviral cream as soon as symptoms start then the cold sore may not last as long as usual and may be less severe. There is debate as to how well the cream works. WHEN TO SEE YOUR GP: You should see your GP if you are unsure of the diagnosis, or if the cold sores are not resolving after a week or so. If you have a poor immune system (you are an immunocompromised person) and develop possible cold sores, you should see your GP. You may need tests to confirm the virus, and/or oral antiviral medicines.
