Fibrous dysplasia of bone
Fibrous dysplasia is an abnormal bone growth where normal bone is replaced with fibrous bone tissue. It causes abnormal growth or swelling of bone. It can occur in any part of the skeleton but the bones of the skull, thigh, shin, ribs, upper arm and pelvis are most commonly affected. Fibrous dysplasia is very rare, and there is no known cure. Fibrous dysplasia is not a form of cancer. Most lesions are monostotic, asymptomatic and identified incidentally and can be treated with clinical observation and patient education. Facial Sculpting in Fibrous Dysplasial - Craniofacial Surgery Fibrous dysplasia is a rare genetic disorder. It causes an abnormal bone growth where normal bone is replaced with fibrous bone tissue. It causes abnormal growth or swelling of bone, sometimes leading to gross enlargement. Surgical camouflage is the only treatment for established case along with medical intervention to prevent future transitions. The case presented here is a 20 plus years old female suffering from this condition for the past 8 years. Her entire right skull and face was involved creating gross enlargement. The right skull bones, forehead, right orbit, maxilla and mandible were involved. Surgical plan for removing extra bone layers in the skull and face was done. Bicoronal parietal incision was placed and the whole of scalp was excised till the infraorbital rim. The right orbit was trimmed to requirement which prevented bulging out of eyes. The bone in skull, forehead and obit was removed in layers by combination of burs and chisel by "stamp cuts". After achieving the desired reduction, the flap was closed in layers. Through an intra-oral approach, set-back osteotomy was performed with trimming of the excess maxilla and mandible bones. The layers of bone were sculpted in the right half of the jaws. The jaw bones were managed and opening closed in layers. Post-operatively patient was happy with reduction and being currently followed up.

Aneurysmal Bone Cyst - Everything You Need To Know - Dr. Nabil Ebraheim
Dr. Ebraheim's animated educational video describing Aneurysmal Bone Cysts. The typical locations of aneurysmal bone cysts are the posterior elements of the vertebrae or in the long bones, such as the distal femur and proximal tibia. Aneurysmal bone cysts are destructive expansile lytic lesions that contain cavities filled with blood. This expansile lytic lesion could expand greater than the width of the growth plate or the width of the bone. Patients with aneurysmal bone cysts are usually younger than 20 years old. The lesion is usually located in the metaphysis. There is usually a periosteal rim around the lesion, however the lesion can expand into the soft tissue. MRI or CT scan will probably show fluid lines or levels. The lesion can occur as a primary lesion and it will express USP6 translocation or it can be secondary and can be associated with underlying lesion in 30% of the time, such as giant cell tumor, fibrous dysplasia, chondromyxoid fibroma and chondroblastoma. Pathology: blood filled cysts and lakes of blood with fibrous stroma and may have giant cells. Differential Diagnosis: Aneurysmal bone cysts can be confused with two entities on x-rays: 1.Simple bone cyst 2.Telangiectatic osteosarcoma. Aneurysmal bone cysts may mimic a giant cell tumor pathologically because it contains giant cells. Clinical Examination •Pain •Swelling •May have a fracture (rare) •Rarely the patient may have neurological deficit if the lesion occurs in the spine and is pushing or pressing onto the nerves. Treatment •Start with nonoperative treatment if there is a fracture •The treatment is usually curettage of the cyst and bone graft •Recurrence of aneurysmal bone cysts occurs in approximately 25% of the cases. oThe highest rate of recurrence occurs in patient with open growth plates. Polyostotic Fibrous Dysplasia
Centered within the proximal left humeral diaphysis, there is an eccentrically located, cortically based T1-hypointense lesion with lobular margins. On the axial GRE weighted images, additional lesions are identified within the glenoid neck. The lesions are T2/STIR hyperintense and heterogeneously enhance. Imaging findings are classic for polyostotic fibrous dysplasia. Fibrous dysplasia is a non-neoplastic tumor like congenital process resulting in a localized absence of osteoblast differentiation maturation with replacement of normal bone with large fibrous stroma and islets of immature woven bone. The disease occurs predominantly in children and young adults. In the polyostotic form, patients are usually diagnosed by 10 years of age. The osseous structures affected by fibrous dysplasia are weaker than normal bone is susceptible to pathologic fracture. Sarcomatous degeneration to osteosarcoma, fibrosarcoma, chondrosarcoma or malignant fibrous histiocytoma may occasionally be seen. NMR289

Fibrous Dysplasia Intraoral Excision
A 56 year old female patient who has been earlier diagnosed with fibrous dysplasia along the left mandible reported for correction of the asymmetry. Previously, elsewhere she had been treated via an external approach which was anesthetical in appearance. This time cosmetic, asymmetry correction of the affected areas of left half of mandible was performed through intra-oral approach. A large gingivomucoperiosteal flap was raised along the left mandibular buccal sulcus from the molar to the posterior most region. Using a reciprocating saw, the affected areas were carefully cut in the buccal aspect. Similarly, in the lingual side too, the involved, extra neo-bone-fibrous tissue was cut and dissected out. Care was exercised to prevent damage to vital structures such as nerves. The impacted third molar, that was positioned high was identified, and extracted during the procedure. After removal of desired level of bone, the entire involved area was debrided, all small pieces of bones flushed, and sharp residual edges were trimmed. The flap was approximated. Suturing was performed in layers.

Adamantinoma - Everything You Need To Know - Dr. Nabil Ebraheim
Dr. Ebraheim’s educational animated video describing Adamantinoma tumor of the tibia. Adamantinoma affects young adults usually over 20 years of age. It usually affects the tibia. It is a low grade malignant tumor. When it is a low grade bony tumor, you treat it by wide excision alone, because chemotherapy and radiation therapy will not work for low grade malignant bony tumors such as adamantinoma and parosteal osteosarcoma. Chemotherapy and radiation therapy will not work with chondrosarcoma. Treatment of chondrosarcoma is wide resection. For the treatment of adamantinoma, parosteal osteosarcoma and chondrosarcoma, do a wide resection. There is doubt that the adamantinoma tumor arises from the osteofibrous dysplasia (OFD). Osteofibrous dysplasia (OFD) occurs in children and is benign. The treatment of osteofibrous dysplasia is observation and not wide resection. Osteofibrous dysplasia (OFD) looks similar to fibrous dysplasia, but it has an osteoblastic rimming. Presentation of Adamantinoma: • Usually is pain for a long time. • There may be tenderness over the tibia or a mass. • There may be bowing of the tibia. • It occurs within the tibial diaphysis. X-rays: • Classic with multiple lucent and sclerotic defects. • Soap bubble appearance. • No periosteal reaction. Differential Diagnosis: • Chondromyxoid fibroma. • Fibrous dysplasia. • Osteofibrous dysplasia. Pathology: • Adamantinoma is a biphasic tumor. • There will be a cell nest of epithelial cells in a fibrous stroma. • Keratin-positive. Treatment: • Wide resection and reconstruction. • Lung metastasis occurs in about 25% of the patients. • Need to do patient follow-up for a long period of time. In the exam, they may give you an x-ray of a tibia and the lesion is located in the diaphysis of the tibia. The slides may show epithelium-like tissue embedded in fibrous stroma. What is the treatment? The treatment of adamantinoma is wide resection. These images demonstrate a slightly expansile osseous lesion centered in the left aspect clivus with central lucency and peripheral ground glass appearance. The cortex is intact. These findings are characteristic of fibrous dysplasia. It can have a variable appearance which can look misleadingly aggressive on MRI. This is a congenital process where fibrous stroma replaces bone. It is predominantly found in younger patients and can result in deformity. Fibrous Dysplasia, Left Frontal Bone
35-year-old female presenting for evaluation of facial asymmetry. Centered within the left frontal bone, there is an expansile, T1 hypointense, T2 hypointense, heterogeneously enhancing lesion with both FLAIR hypointense and slightly hyperintense components which demonstrate mild diffusion restriction. The lesion expands the diploic space of the left frontal bone with extension along the roof of the left orbit. There is no invasion of the underlying dural surface or extraocular musculature. Imaging findings are classic for craniofacial fibrous dysplasia. Typical presentations include cranial asymmetry, facial deformity, nasal stuffiness, proptosis and/or visual impairment. The anterior craniofacial structures are more frequently involved. Heterogeneous signal predominates in the T1 and T2 weighted images. Treatment is typically for cases where the airway or orbital structures are compromised. 15NMR269

Jeffrey Kim Dr Kim is an otologist (ear specialist) at the National Institute on Deafness and Other Communication Disorders, NIH, and Georgetown University Hospital, Washington, DC. He graduated from the University of Cincinnati College of Medicine. His Otolaryngology-Head and Neck Surgery training was completed at Baylor College of Medicine in Houston, Texas. This was followed by a Fellowship in Otology- Neurotology at the University of Virginia Health Sciences Center. Dr. Kim has collaborated with Dr. Michael Collins in evaluating the patients with McCune-Albright Syndrome at NIH for the last 15 years.
