New Disaster Preparedness Rule
The Centers for Medicare and Medicare Services (CMS) posted its final rule in September entitled “Emergency Preparedness Requirements for Medicare and Medicaid Participating Providers and Suppliers”. The rule is set to go into effect on November 16, 2016, with health care providers and suppliers affected by this rule required to comply and implement all regulations one year after the effective date, on November 16, 2017, according to the CMS. This rule will affect 72,315 American health care providers and suppliers – from hospitals and nursing homes and other long-term care facilities to home healthcare and hospice to dialysis facilities and care homes for those with intellectual disabilities. The goal of the rule, according to the CMS, is to prevent severe breakdown in patient care that has followed past disasters, including Hurricane Katrina and Super Storm Sandy, and most recently the flooding in Louisiana. The rule is also designed to strengthen the ability to provide services during other types of emergencies, such as pandemics and terrorist attacks. The rule is unusual in that it has provisions for 17 different provider types, among them those that patients rely on to live at home, like outpatient surgery sites, physical therapy offices and home health care agencies. The rule “will make it more likely that facilities will be able to stay open and able to care for patients, and if they need to close or stop work temporarily, get back up and able to care for patients quickly,” said Dr. Nicole Lurie, the assistant secretary for preparedness and response at the Department of Health and Human Services. ”The need for patient care doesn’t stop because streets are flooded or trees are down. In fact disasters often increase the need for health care services.” Although the majority of organizations have had to adhere to at least some emergency preparedness requirements for accreditation, others were not subject to any, including hundreds of residential psychiatric facilities, nearly 200 community mental health centers, dozens of organ procurement organizations and nearly 4,000 outpatient hospices, which treat patients with limited life expectancies. “It’s going to have a big impact on these facilities,” said Emily Lord, the executive director of Healthcare Ready, a nonprofit focused on preparedness that provided feedback to the government on the implications of the rule,” in an article in the New York Times. Some feel the requirements under the new rule will be difficult for smaller facilities. Barbara B. Citarella, president of health care consulting group RBC Limited, was cited in the Times article as saying: ‘My concern is that compliance for some providers, especially home care and hospice, will be financially impossible.” The new as it relates to Medicare and Medicaid participating providers and suppliers specifically requires the following: 1. Emergency plan: Based on a risk assessment, develop an emergency plan using an all-hazards approach focusing on capacities and capabilities critical to preparedness for a full spectrum of emergencies or disasters specific to the location of a provider or supplier. 2. Policies and procedures: Develop and implement policies and procedures based on the plan and risk assessment. 3. Communication plan: Develop and maintain a communication plan that complies with both federal and state law. Patient care must be well coordinated within the facility, across health care providers, and with state and local public health departments and emergency systems. 4. Training and testing program: Develop and maintain training and testing programs, including initial and annual training, and conduct drills and exercises or participate in an actual incident that tests the plan. These standards, according to the CMS, are adjusted to reflect the characteristics of each type of provider and supplier. For example: outpatient providers and suppliers such as ambulatory surgical centers and end-stage renal disease facilities will not be required to have policies and procedures for provision of subsistence needs. Hospitals, critical access hospitals, and long-term care facilities will be required to install and maintain emergency and standby power systems based on their emergency plan. Martial Law Exposed - Oliver North grilled by Congress about Rex 84
Our politicians are strong-armed into passing legislation like this, by having their lives ruined if they don't comply.. http://en.wikipedia.org/wiki/Rex_84 The Miami Herald reported on July 5, 1987: Lieutenant Colonel Oliver North and the Federal Emergency Management Agency ... had drafted a contingency plan providing for the suspension of the Constitution, the imposition of martial law, and the appointment of military commanders to head state and local governments and to detain dissidents and Central American refugees in the event of a national crisis.[1] According to scholar Diana Reynolds: The Rex-84 Alpha Explan (Readiness Exercise 1984, Exercise Plan; otherwise known as a continuity of government plan), indicates that FEMA in association with 34 other federal civil departments and agencies, along with other NATO nations, conducted a civil readiness exercise during April 5-13, 1984.. The exercise anticipated civil disturbances, major demonstrations and strikes that would affect continuity of government and/or resource mobilization. To fight subversive activities, there was authorization for the military to implement government ordered movements of civilian populations at state and regional levels, the arrest of certain unidentified segments of the population, and the imposition of martial law. [2] Rex-84 was written by Lieutenant Colonel Oliver North, who was both the NSC White House Aide and NSC liaison to FEMA, and John Brinkerhoff, the deputy director of "national preparedness" programs for FEMA. They patterned the plan on a 1970 report written by FEMA chief Louis Giuffrida, at the Army War College, which proposed the detention of up to 21 million "American Negroes", if there were a black militant uprising in the United States.[3] Existence of a master military contingency plan, "Garden Plot" and a similar earlier exercise, "Lantern Spike" were originally revealed by journalist Ron Ridenhour, who summarized his findings in "Garden Plot and the New Action Army."[6] Exercises similar to Rex 84 happen regularly.[7] Plans for roundups of large numbers of persons in the United States in times of crisis are constructed during periods of increased political repression such as the Palmer Raids and the McCarthy Era. For example, from 1967 to 1971 the FBI kept a list of over 100,000 persons to be rounded up as subversive, dubbed the "ADEX" list.[8] This list contained many labor leaders, scholars, and public figures of the time. In 2008, for the first time an active military unit has been given a dedicated assignment stateside for civil unrest containment. Humanitarian Financing: Pooled funding – how can NGOs engage?
Pooled funds have enabled more timely and flexible funding for responding to sudden humanitarian crises, and have made it possible to operate in otherwise underfunded emergency settings. In the context of the World Humanitarian Summit, the UN Secretary-General (SG) recommended that member states and other donors should double the Central Emergency Response Fund (CERF) to $1 billion, with the Grand Bargain calling for an exploration of whether NGOs could directly access CERF. The SG also called to increase the aid funneled through the Country-based Pooled Funds (CBPFs) to 15 per cent by 2018. Given the current and growing importance of pooled funding, it is important that humanitarian NGOs understand how these mechanisms function and how they can be accessed in order to be part of improving the overall response to humanitarian crises. NGOs are also managing a growing number of pooled funding mechanisms. The START fund (established and managed by a consortium of NGOs) is providing a quick alternative avenue for NGOs to access timely humanitarian funding. The NEAR Network is exploring options for NGO-run pooled funds at the local level. Subtitles available in English and French. The Safety Management System (SMS) is actively used in industries that manage significant safety risks, including aviation, railway, petroleum, chemical, electricity generation and so on. Almost all staff require basic SMS training in these kind of industries. The educational designs of safety are similar although the human error models (i.e. swiss cheese) improves attention. Therefore, I tried to explain the safety framework with the “baby model” so that audiences can watch without getting bored. I hope that you will find this video useful.
