Stanford Health Policy Forum: Controlling the Cost of Healthcare

01:29:07
The United States health care system is a $3 trillion enterprise, the largest in the developed world. Yet Americans often experience more severe access and quality problems, and spend much more for the same procedures and medications, than patients in other countries. Projections of the future cost of health care are unsustainable, yet many well-intended cost-control efforts have been ineffective. This forum features two renowned experts who will discuss the causes of and potential solutions to the extraordinary cost of American health care. Physician, journalist and Stanford alum Elisabeth Rosenthal has drawn national attention to the issue through her widely praised “Paying Till It Hurts” series in the New York Times. She will be joined by Professor Doug Owens, the director of Stanford’s Center for Health Policy and an expert in health care cost-effectiveness research. Speakers: Doug Owens, Elisabeth Rosenthal, Paul Costello http://med.stanford.edu/healthpolicyforum.html

David Wyper - Drink Deeply, Be Delighted

02:46
Song written by David Wyper and inspired by words from the prophet Isaiah. Let your body, mind and spirit be washed over by the abundance and promises of God, drenched by the Living Water, Jesus Christ. "There is a river that makes glad the city of God." Whoever drinks of this water shall never thirst. This song is part of our latest CD/DVD that marks the 50th year David has been a Gospel singer and songwriter. For more information www.davidwyper.com

Are Biosimilar Drugs the Future of Cancer Treatment?

10:45
Are Biosimilar Drugs the Future of Cancer Treatment? The first biosimilar for cancer treatment has FDA approval and more are in the pipeline.Doctors say it’s too soon to say how it will affect cancer patients. Some of the most exciting advancements in cancer treatment involve biologic therapies.Made with living organisms, biologics prompt the immune system to kill cancer cells.Biosimilars are similar versions of biologics already approved by the U. S. Food and Drug Administration (FDA).The FDA approved the first biosimilar in 2015. Now the agency has approved the first biosimilar to treat cancer. It’s a biosimilar to bevacizumab (Avastin), which gained approval in 2004. Both drugs are approved for treatment of adults with certain colorectal, lung, brain, kidney, and cervical cancers.Mvasi was developed by Amgen, Inc. The company hasn’t announced a launch date. The biosimilar Mvasi is similar to its reference drug Avastin. But it’s not what the FDA classifies as interchangeable. Julie Kennerly, PharmD, assistant director of pharmacy at The Ohio State University Wexner Medical Center, explains.“Generic drugs are, essentially, copies of brand name drugs. You can automatically substitute the generic.Biosimilars are not an exact replica the way a generic is. They’re highly similar to the reference product. But they have some allowable differences due to the fact that they’re made from living organisms and the complexity of the manufacturing process,” Kennerly told Healthline.“They don’t lend themselves to be reproduced exactly. Mvasi can be used for the same indications as Avastin. We can expect clinical outcomes to be the same. But the physician has to indicate one or the other. They can’t be switched or changed without a new prescription,” she continued. She noted that biosimilars are allowed because of the Biologics Price Competition and Innovation Act of 2009.So, they’re fairly new. “We’re just starting to see them come to market,” said Kennerly. To be called interchangeable, the drug must meet the same standards as biosimilars. In addition, they must prove they can produce the same result as the reference product in any patient, even if they switch from one drug to the other.Kennerly believes more biosimilars for cancer treatment will be available in the near future. She said that others in the pipeline include biosimilars for trastuzumab (Herceptin), pegfilgrastim (Neulasta), and rituximab (Rituxan).Getting biosimilars into general use is more complicated than it is for generics. How will doctors choose which drug to prescribe?Much will depend on how medical insurers respond, according to Kennerly.Kennerly suggested it might be particularly challenging for outpatient infusion centers to decide how much of each drug to stock. But cancer centers are likely to have more success in deciding which ones to use. Biologics are expensive and a big driver of escalating healthcare costs. In the United States, generic drugs saved the healthcare system $253 billion in 2016 alone. It remains to be seen if biosimilars will have a similar impact. Dr. Timothy Byun, a medical oncologist with St.Joseph Hospital’s Center for Cancer Prevention and Treatment, said it’s not clear how Mvasi will affect cost of treatment.“Obviously on the surface, this should help to curb the rising cost of cancer treatment. But medical economics does not appear to follow the rules of true market economy,” he told Healthline.“If you have a product that is similar, then that drug’s cost should be substantially less expensive. However, some health insurances have co-pay policy that may end up making generic drugs more expensive for patients. We will have to wait and see whether patients pay more out-of-pocket for Mvasi compared to Avastin,” Byun said.Kennerly believes biosimilars will end up decreasing the cost of cancer care. But estimating how much is a complicated task.“Competition drives prices down. What is unknown is how far. This will largely be determined based on the CMS [Center for Medicare and Medicaid Services] ruling on how to approach reimbursement for biosimilars. What the CMS does, insurers usually follow suit,” said Kennerly. “The current CMS policy requires all biosimilars related to the reference products be given a shared code.Reimbursement is based on the average sell price. The challenge is that it could end up stifling the biosimilar production market,” she continued.“Many people have urged the CMS to reverse its current biosimilar reimbursement policy

Watch: Tony O'Brien's final message to colleagues on his last day as HSE Director General

01:36
'Despite the setback the health service experienced, remain focused on your work' - Tony O'Brien in video message to HSE staff OUTGOING HSE Director General Tony O'Brien has urged staff to "remain focused on their work" despite the "setback" our health service has suffered during the CervicalCheck scandal. Mr O'Brien also said that he is "so proud" of HSE staff and was hoping to visit various hospital and ambulance service nationwide before he resigned last night following building political and public pressure. Two terminally ill mothers - Vicky Phelan and Emma Mhic Mhathuna - who were given incorrect cervical check results had called for Mr O’Brien to resign, a damning internal HSE memo also put pressure on Mr O’Brien to step down. He recorded a video message today, which was posted on the HSE Live Twitter account. Mr O'Brien said: "What's important now is despite the setback that the health service has experienced is that we all, you all in fact in this case, remain focused on the important work that you do. "The HSE, the health services, our health service, delivered by people, by you for people in Ireland, our own families often. "And it is really important that you continue to do the great work that you have been doing in recent years and for which I am so proud of you." He also said that he remains proud of his achievements at the helm of the HSE. "I'm extremely proud of the work you do, in Ireland's health and social care services. "Within the time I have worked in the HSE, I've had the opportunity to be with many of you delivering care, in hospitals, community settings, the ambulance service, all across the country. "It would have been my plan, had time permitted, to make some visits like that, before my planned date of departure in a few weeks time, sadly that's not to be. "What I do know, is that the work that you do, your work, your commitment, your care is a vital part of what makes Ireland, Ireland. It's a vital part of our community infrastructure and I want you to be really proud of everything that you do," he said. Separately, Minister for Health Simon Harris announced that John Connaghan will carry out the role of Director General of the HSE following the decision of Tony O’Brien to step down. "I have now designated John Connaghan to carry out the functions of the Director General of the HSE pending a recruitment process which is to be conducted by the Public Appointments Service," Minister Harris said. "John brings a wealth of senior healthcare management experience at an international level. He will be a tremendous resource in leading the HSE in challenging times. I want to thank John for taking on this interim role." Mr Connaghan joined the HSE last August as the Deputy Director General and Chief Operating Officer having previously worked for NHS Scotland.

Mandy Moore

36:28
Amanda Leigh "Mandy" Moore (born April 10, 1984) is an American singer--songwriter, actress, and fashion designer. Raised in Florida, Moore first came to prominence with her 1999 debut single, "Candy", which peaked at number 41 on the Billboard Hot 100. Her subsequent album, So Real, went on to receive a Platinum certification from the RIAA. Two more singles, "Walk Me Home" and "So Real", were released but failed to have the success of their predecessor. Her 2000 single, "I Wanna Be with You", became her first Top 40 hit in the US, peaking at number 24 on the Hot 100 chart. The parent album, titled the same, was released that same year to generally mixed reviews. The album went on to achieve Gold certification. After revealing her displeasure with her early works, Moore's third album, simply titled Mandy Moore, featured a change of sound that drifted away from her "bubblegum pop" roots. The album spawned the single "In My Pocket", which became her third Top 20 hit in Australia. The album itself was her final album to be certified by the RIAA, receiving a Gold certification. In 2003, Moore released her fourth studio album, Coverage, featuring covers of classic 1970s songs. Following the album's release, Moore parted ways with her record label, due to creative differences. The split prompted the label to release the compilation albums The Best of Mandy Moore and Candy, both of which have sold an estimated 100,000 copies to date. Moore did not return to music until the release of her 2007 album Wild Hope, which failed to have much success. To date, the album has sold an estimated 200,000 copies, and failed to receive an RIAA certification. Similarly, both of the album's singles failed to chart worldwide. In 2009, Moore released her sixth studio album, Amanda Leigh, which peaked at number 25 on the Billboard 200 and sold an estimated 100,000 copies. In 2012, Moore confirmed that she was working on her seventh studio album, currently slated for a 2014 release. As of 2009, Moore has sold more than 12.5 million albums worldwide, according to Billboard. In 2012, Moore was ranked #96 on VH1's list of "100 Greatest Women in Music", as well as #63 on their Sexiest Artists of All Time List. This video is targeted to blind users. El sexo tiene un precio 07

02:09

Get more information at http://go.usa.gov/xWtN4. What is the Cancer Moonshot? During During his State of the Union address on January 12, 2016, President Barack Obama announced the establishment of a new Cancer Moonshot to accelerate cancer research. The initiative—led by Vice President Joe Biden—aims to make more therapies available to more patients, while also improving our ability to prevent cancer and detect it at an early stage. What is the Blue Ribbon Panel? To ensure that the Cancer Moonshot's goals and approaches are grounded in the best science, President Obama directed the Moonshot Task Force to consult with external experts, including the presidentially appointed National Cancer Advisory Board (NCAB). A Blue Ribbon Panel of experts was established as a working group of the NCAB to assist the board in providing this advice.

Nurse Practioner Casteneda discusses that the primary reason for co-founding Orthopaedic and Spine Institute Medical Centers is the caring attitude, bedside manner surgical skill and precision, and medical practices of Dr. Steven J. Cyr, M.D., FAAOS, a Board Certified spine surgeon and Mayo Fellowship trained in spine surgery. http://www.saspine.com The Orthopaedic and Spine Institute Medical Centers, located in San Antonio, Texas, is comprised of a group of orthopedic physicians and practitioners working together to bring you unparalleled healing and wellness using the latest technologies and procedures. LOKALNIE presentation

We built "Lokalnie" at Geeklist #hack4good 3rd place We are representing a Team called “Locals”. On hack4good 0.6 – we have chosen the subject which promotes local and organic produce. Our Challenge was to encourage people to buy food from regional suppliers. Team We had a multidisciplinary team whose member was a biologist, economist, ERP admin, graphic designer and consultants from local “Wawelska” food cooperative. Action plan that we developed Brainstorming session; Research, information gathering and choosing the direction to go: After few hours of research, we have come to a conclusion that there is a big difference in buying food from local organic produce than food from the supermarket. The problem is that many people do not see it. This information opened our eyes, we found the problem and our next task was to propose a solution. Solution: First of all we thought that it would be good to start educating from the beginning. As out target we have selected families with a special focus on children. Solution that we proposed consists of two following deliverables: Create a set of open source education materials that includes: Pack of information from the research gathering. Here parents can find for example the benefits of buying organic food from local produce as well as disadvantages of buying products that are imported especially with a long supply chain. Pack of open source graphical illustrations that can be useful and helpful in creating infographics, educational materials for children and families; Project for the implementation that includes: Visualization and animation for children and parents. By using an apple example we wanted to show in a funny and interesting way long vs. short supply chain with its impact on the environmental and economical factor. Out project is open, published by using creative commons license. We hope it will be developed further by educational institutions, cooperatives and other pro-Eco organizations support by designers. MICRA Victim - Rabbi Aaron Gottesman

Approximately 98,000 Americans die every year because of medical errors. In California, a law that hasn't changed since 1975 takes away the right of injured Californians to hold an incompetent doctor or negligent hospital accountable for medical errors. California's "malpractice cap" limits compensation for pain and suffering because of medical negligence to $250,000. This cap on compensation for injured Californians hasn't been adjusted for inflation since 1975. It would be more than $900,000 in today's dollars. Put that in perspective: The minimum wage would be $2.10 an hour if it hadn't changed since 1975. The cap never changes, no matter how severely you are injured, if you lose your sight, your fertility, or even your child. Because of the cap, people who are injured - and don't make a lot of money - can't find attorneys in California. The cost to prepare and try a case for retired seniors, women who don't work, children or low-income Californians is often more than the case is worth. Without an attorney, health care providers aren't held accountable for hurting people with medical mistakes, neglect and abuse. This and many other videos are part of the site JusticeForPatients.org. JFP is an attempt to illustrate the human cost of this out-moded and arbitrary cap on a victim's financial compensation.

The Great Gildersleeve: Craig's Birthday Party / Peavey Goes Missing / Teacher Problems

01:29:31
Aiding and abetting the periodically frantic life in the Gildersleeve home was family cook and housekeeper Birdie Lee Coggins (Lillian Randolph). Although in the first season, under writer Levinson, Birdie was often portrayed as saliently less than bright, she slowly developed as the real brains and caretaker of the household under writers John Whedon, Sam Moore and Andy White. In many of the later episodes Gildersleeve has to acknowledge Birdie's commonsense approach to some of his predicaments. By the early 1950s, Birdie was heavily depended on by the rest of the family in fulfilling many of the functions of the household matriarch, whether it be giving sound advice to an adolescent Leroy or tending Marjorie's children. By the late 1940s, Marjorie slowly matures to a young woman of marrying age. During the 9th season (September 1949-June 1950) Marjorie meets and marries (May 10) Walter "Bronco" Thompson (Richard Crenna), star football player at the local college. The event was popular enough that Look devoted five pages in its May 23, 1950 issue to the wedding. After living in the same household for a few years with their twin babies Ronnie and Linda, the newlyweds move next door to keep the expanding Gildersleeve clan close together. Leroy, aged 10--11 during most of the 1940s, is the all-American boy who grudgingly practices his piano lessons, gets bad report cards, fights with his friends and cannot remember to not slam the door. Although he is loyal to his Uncle Mort, he is always the first to deflate his ego with a well-placed "Ha!!!" or "What a character!" Beginning in the Spring of 1949, he finds himself in junior high and is at last allowed to grow up, establishing relationships with the girls in the Bullard home across the street. From an awkward adolescent who hangs his head, kicks the ground and giggles whenever Brenda Knickerbocker comes near, he transforms himself overnight (November 28, 1951) into a more mature young man when Babs Winthrop (both girls played by Barbara Whiting) approaches him about studying together. From then on, he branches out with interests in driving, playing the drums and dreaming of a musical career. http://en.wikipedia.org/wiki/Great_Gildersleeve
