I was very surprised to get the news, but I knew something was wrong and I knew I had to do something about it. So you go through a series of steps in figuring out 'What do I need to do?' and 'Who do I need to do it with?' That circles back to Weill and the team that is led by Scott Tagawa. He's recognized the times where a higher level of intervention and perhaps something that is not traditional is needed. Study Shows It Really Doesn't Matter How You Treat Prostate Cancer
Experts said Wednesday the first controlled study comparing three different approaches to prostate cancer — radiation versus surgery versus "watchful waiting" — shows there is no truly bad choice for most men. According to NBC News, the British study won't end debates over whether prostate cancer screening is a good idea. However, it should reassure many men that it's OK to make their own decisions about whether and how to treat prostate cancer if they get diagnosed. While tumors were more likely to grow and spread in men who got no treatment compared to those who did, it was still rare. It was even more rare for men to die from prostate cancer. The researchers reported in the New England Journal of Medicine that out of 1,600 men who took part in the 10-year study, just 17 died of prostate cancer. Prostate cancer is very common, showing up in 240,000 U.S. men every year. Prostate cancer is the most common cancer in men in the UK so it's important to know the facts. Dr. Vincent Gnanapragasm from Cambridge University talks us through prostate cancer treatments: Over 80% of men live for 10 or more years after a diagnosis of Prostate Cancer. I’ve come to Cambridge university hospital to speak with Dr. Vincent Gnanapragasam about some of the treatments that are available for this type of Cancer. So Vincent what are the important factors when deciding what type of treatment a person should have? The treatment options for Prostate Cancer are very much dependent on what type of cancer an individual has and also the patient themselves. So if a cancer is confined to within the prostate gland then we might be able to treat it with surgery or radiotherapy, but equally there might be a very slow growing tumour which we can just keep an eye on that. But the kind of cancer is determined by the size of it or the stage of a cancer, the type of cancer it is or the grade and also the PSA test. Now we use those 3 put together to create an idea of the risk of a particular individual dying of prostate cancer. What treatments are available? In general there is surgery to remove the prostate gland, radiotherapy where you deliver high intensity radiation beams or surveillance where we can monitor or just check on someone and defer treatment until later. So a number of different options then Vincent but how do you decide what one is best? That decision is left to the patient in consultation with his consultant or his nurse specialist and comes down to whether the patient feels that a particular type of treatment is best suited for their lifestyle and also the risks and side effects which may come about from that. And what happens when treatment is finished? After treatment is finished the patient enters a phase of very close monitoring and where they’ll be seen regularly in hospital often with a PSA test being done on a regular interval. What about for treatment of cancers that have been spread? When a cancer has spread beyond a prostate then the most common treatment is hormonal therapy and that serves to keep the cancer under control and many men can live for many years with that and if that should fail there are actually other drugs that can be used, chemotherapies which again can be very effective. Dealing with cancer is never easy and this is especially so when deciding about treatments. But of course it helps if you know the facts. ASCO 2018: Novel treatment strategies in prostate cancer therapeutics
Dr Neal Shore speaks to Prof Karim Fizazi about the changing landscape of treatments for prostate cancer, discussing immunotherapies, PARP inhibitors and PSMA testing as future targets and treatments. Dr Fizazi highlights that anti-PD1 treatment (pembrolizumab) may provide benefit for metastatic castration resistant patients following antiandrogen treatment (5 of the 28 patients in the trial had a PSA decline of 50%). Trials building on immune checkpoint therapies with novel combinations to improve response rates have shown promise. Among these, the combination of pembrolizumab and enzalutamide is being explored as a doublet or a triplet with the addition of ADXS-PSA in KEYNOTE-046. Dr Shore raises PARP inhibitors as an avenue of research, with the recent TOPARP trial confirming the activity of olaparib, and Prof Fizazi considers how different genetic abnormalities will inform selection in upcoming trials and eventual care. PSMA testing, already providing new insights when used in combination with PET imaging, may also offer new inroads in developing treatments, and Prof Fizazi summarises current clinical interest in targeted or radiologic medicine.

A Stage 4 prostate cancer diagnosis is reserved for those whose cancer has progressed outside the prostate to other organs. Usually the cancer is no longer responding to hormonal therapy at this point. New prostate cancer treatment- Natural Prostate  Treatment - Natural - Effective- no side effect
For prostate cancer, our treatment consists of several steps. First, we determine the location and size of the prostate cancer lesions by Digital Rectal Examination (DRE), Trans Rectal UltraSound (TRUS), and pelvic Nuclear Magnetic Resonance Imaging (MRI) examination. And then we targeted inject a set of anti-cancer herbal extracts without any side effects into the prostate cancer lesion area in high concentrations by using 3D targeted injection technology, thus directly inhibiting and killing cancer cells, making the prostate cancerous tissue atrophy and necrotize. At the same time, we targeted inject unblocking herbal extracts without any side effects to resolve the pathogenic tissue and prostatic endotoxin, and discharge toxic substances. In addition, we use some other herbal extracts without any side effects to improve and restore prostate immunity and the blood circulation to prevent the recurrence of prostate cancer. Through 3D Prostate Targeted Treatment, the volume of the prostate cancer lesions will shrink or disappear, and the patient’s symptoms should be improved or disappear. At the third clinical stage (III stage), prostate cancer lesion goes beyond the prostate capsule and invades neighboring tissues and organs like seminal vesicle, bladder neck etc. We targeted inject treatment medicines within the prostate capsule to treat the primary lesion, and we also apply 3D targeted injection technology to put a set of anti-cancer treatment medications into adjacent seminal vesicle, the bladder neck and other neighboring tissues and organs for treatment. At the fourth clinical stage (IV stage), prostate cancer lesion spreads beyond the prostate by transferring lesions to distant bones, or even lungs, liver and adrenal gland. We targeted inject treatment medicines within the prostate capsule to treat the primary lesion; at the same time, we couple with systemic treatment and local targeted injection treatment of distant metastasis lesions. The treatment medications include anti-cancer herbal extracts, unblocking herbal extracts, and improving immunity herbal extracts. Our clinical use of medicines have no side effects. Because prostate cancer normally happens in older people, the cancer nodules will press on the prostate tubes, ejaculatory ducts and posterior urethra, prostate cancer patients usually may also have prostatitis and genitourinary tract infection, 3D Prostate Targeted Treatment can treat these diseases at the same time. 3D Prostate Targeted Treatment is very suitable for prostate cancer patients who also have prostatitis and benign prostatic hyperplasia. 3D Prostate Targeted Treatment is an non-surgical method that causes no harm, no side-effects with no ongoing consequences of the original disease, to completely treat prostate cancer. 3D Prostate Targeted Treatment is not a long-term medicinal treatment and so it is the best treatment for prostate cancer as it causes no side-effects. If your diagnosis came as a result of a schedule of appropriate PSA testing, it’s very likely the disease has been found early. This comes as a surprise to most men and they are quite anxious at this point. Being told you have prostate cancer is a frighting message carrying a heavy baggage The normal urge is to plunge into a “crash course” to learn how to manage this unwelcome intrusion into your life, a crash course that you are unlikely to know how to handle objectively. So … what to do?  Stop, take a deep breath, step back a bit, and think carefully. It is extremely unusual that there is any need to rush to a decision. Most cancers have been sitting quietly in your prostate and growing for 10 to 15 years, with the most aggressive ones developing over 10 or so years. So … you haven’t just “gotten” prostate cancer. You just have been “diagnosed” with a cancer that you have been living with for a long time. Likely your diagnosis has been made by a biopsy performed by a urologist, an expert in the surgical management of prostate cancer. It’s extremely important for you to know that any cancer, low grade, intermediate, or high grade can be successfully treated with surgery, augmented if need be by radiation, external beam radiation therapy, permanent radioactive seed placement, for example brachytherapy, combined if indicated by external radiation, and proton therapy. In some cases cryotherapy or short course of focused radiation with Cyberknife technology is chosen.  Active surveillance is a very important option for management of low-risk cancer. It warrants serious consideration and discussion. Take your time and learn about the treatment options available to you. You may want to consider several of these. In most situations, all of these are reasonable options. But for some men their individual  situations makes a particular option the better choice. Nor do all options suit every man’s emotional reaction to the diagnosis or his preferences.  So what should you do? You do not need to become an instant expert. Instead, you need to have consultations with possibly several specialists who can describe their treatments and help you decide which treatment fits you best. In the process you may well have consultations with a urological surgeon and possibly several radiation therapists each skilled in a special type of radiation therapy. If your disease is more complex, you may want an option from medical oncologist, which is what I am. It’s important to be informed, but you don't need to do this alone. That’s what the specialists are there for. This is the time for deliberation, since once treated you cannot reverse the course. After stopping, and thinking, and gaining expert guidance, then take action. Most not all of the mental fuzz of anxiety and consternation will subside. But when you have made a considered decision you will feel very comfortable.  Then you can devote your energy to the goal of successful treatment and recovery. GINGER and CANCER , How Ginger Destroys Prostate,Ovarian and Colon Cancer Better than Chemo. A new study reveals ginger contains a pungent compound that could be up to 10,000 times more effective than conventional chemotherapy in targeting the cancer stem cells at the root of cancer malignancy. A new study published in PLoS reveals a pungent component within ginger known as 6-shogaol is superior to conventional chemotherapy in targeting the root cause of breast cancer malignancy: namely, the breast cancer stem cells. As we have discussed in greater detail in a previous article titled, “Cancer Stem Cells: The Key To Curing Cancer,” cancer stem cells are at the root of a wide range of cancers, not just breast cancer, and are sometimes referred to as “mother cells” because they are responsible for producing all the different “daughter” cell types that makeup the tumor colony. While cancer stem cells only constitute between .2 and 1% of the cells within any given tumor, they have the seeming “immortal” ability to self renew, are capable of continuous differentiation, are resistant to conventional chemotherapeutic agents, and are tumorigenic, i.e. are capable of “splitting off” to create new tumor colonies. Clearly, the cancer stem cells within a tumor must be destroyed if cancer treatment is to affect a lasting cure. The new study titled, “6-Shogaol Inhibits Breast Cancer Cells and Stem Cell-Like Spheroids by Modulation of Notch Signaling Pathway and Induction of Autophagic Cell Death,” identified powerful anti-cancer stem cell activity in 6-shogaol, a pungent constituent of ginger produced when the root is either dried or cooked. The study also found that the cancer-destroying effects occurred at concentrations that were non-toxic to non-cancerous cells – a crucial difference from conventional cancer treatments that do not exhibit this kind of selective cytotoxicity and therefore can do great harm to the patient. The authors of the study further affirm these points: “Cancer stem cells pose serious obstacle to cancer therapy as they can be responsible for poor prognosis and tumour relapse. To add into the misery, very few chemotherapeutic compounds show promise to kill these cells. Several researchers have shown that cancer stem cells are resistant to paclitaxel, doxorubicin, 5-fluorouracil, and platinum drugs [8, 16]. CSCs are thus an almost unreachable population in tumours for chemotherapy. It exhibits cytotoxicity (cell killing properties) against monolayer (1-dimensional cancer model) and spheroid cells (3-dimensional cancer model).

Petrylak, MD, Professor of Medicine, Program Director, Genitourinary Oncology Section, Herbert Irving Comprehensive Cancer Center at Columbia University Medical Center, discusses the research and concerns surrounding cytotoxic agents for the treatment of castration-resistant prostate cancer (CRPC). Numerous phase III trials looking at the combination of docetaxel with novel therapies, bone targeted agents, or angiogenesis inhibitors have all failed to demonstrate a survival benefit when compared to docetaxel alone. Petrylak believes that research may not be looking at the correct targets or the possibilities have been exhausted for initial cytotoxic therapy. The taxane cabazitaxel is currently in randomized trials that compare it to docetaxel in the first-line setting. This agent is currently approved for the second-line in advanced CRPC, following progression on docetaxel. Discovering the best patients for either docetaxel or cabazitaxel is an important area of research, especially when considering newly approved hormonal therapies and possible combinations.

Gold Nanoparticles in Treatment of Prostate Cancer
Current treatment for prostate cancer is expensive, has numerous side-effects, and has a high mortality rate. It is astonishing how long this ailment has affected humanity without providing a reliable solution. Consequently, our team will examine how gold charged nanoparticles are used to aid the treatment of prostate cancer, specifically via drug delivery. These particles have a gold inner core and are coated with polyethylene glycol. Polyethylene glycol and gold are used due to their relatively inert state and non-toxicity. Gold is also used because, at a nanoscale, it is a catalyst for reactions. The surface chemistry of gold nanoparticles can be altered so that they can be coated with small polymers and other therapeutic agents such as cancer-treating drugs (specifically Docetaxel and Doxorubicin). This also gives the patient's DNA the ability to bond to the nanoparticle’s surface, which allows these nanoparticles to travel through the body without being rejected. These nanoparticles are spherical shaped, and while studies are beginning to show that non-spherical particles are more effective than spherical particles, researchers do not yet fully understand the shape effect non-spherical particles have on cancerous cells. The diameter of these nanoparticles is less than 170nm so that they can effectively fit through and diffuse inside cells and capillaries. Additionally, these nanoparticles are charged. Due to their charge, the nanoparticles repel each other, which prevents them from aggregating. Furthermore, it simply helps nanoparticles interact with cells and diffuse through concentration barriers more efficiently. This method of prostate cancer treatment is still being researched and has yet to be approved for medical use; however, it has the potential to replace current treatment methods, as it could be more effective and safer in practice. A study out of the University of Missouri found a certain cholesterol-lowering molecule (RO 48-8071) helped reduce prostate cancer cell growth — essentially causing the cancerous cells to die off. One of the lead researchers said, "Often, cancer patients are treated with toxic chemotherapies. ... We focused on reducing the production of cholesterol in cancer cells, which could kill cancer cells and reduce the need for toxic chemotherapy." The researchers say the study, which has been accepted for publication in the journal OncoTargets and Therapy, could provide a new therapeutic approach to fighting prostate cancer. According to the American Cancer Society, there are more than 180,000 new cases of prostate cancer in the U.S. each year. Prostate cancer causes roughly 26,000 deaths per year. Androgen synthesis inhibitors, such as ketoconazole, aminoglutethamide, and abiraterone acetate. Immunotherapy using a cell-based vaccine called sipuleucel-T. This vaccine uses a man’s own immune cells to fight metastatic prostate cancer that has become resistant to hormone therapy. Chemotherapy, most commonly with the drug docetaxel. Another drug, cabazitaxel, is approved for the treatment of metastatic castration-resistant prostate cancer that was previously treated with docetaxel. Radium 223 dichloride, a radiopharmaceutical approved to treat men with castration-resistant prostate cancer that has metastasized (spread) to the bones and is causing symptoms but has not spread to other organs. This drug collects in certain areas of bone, such as bone metastases, and gives off radiation that kills cancer cells. Men with castration-resistant prostate cancer who receive these treatments will continue to take first-line hormone therapy (e.g., an LHRH agonist) to avoid an increase in testosterone level, which may lead to tumor progression in some men. Furthermore, they have proved that ginger provides the same effects in the case of ovarian cancer as well. Multiple studies have provided evidence that ginger efficiently prevents numerous cancer types and destroys the cancer cells in the colon, prostate, and ovaries. Yet, the best thing about it all is that it does not cause detrimental side effects, as it destroys only the affected cells, and has no effects on the healthy ones. This means that this natural cancer treatment does not poison the body and only improves health. Video abstract of review paper Enzalutamide as a second generation antiandrogen for treatment of advanced prostate cancer published in the open access journal Drug Design, Development and Therapy by Julius Semenas, Nishtman Dizeyi and Jenny Liao Persson. Prostate cancer (PCa) is the most common malignancy, and the third leading cancer-related cause of death among men of the Western world. Upon PCa progression into metastatic disease, androgen deprivation therapy is applied as the first-line treatment, and has been shown to be effective in most patients, leading to a decrease in serum prostate-specific antigen and relief of disease-related symptoms. However, advanced PCa almost inevitably progresses to a castration-resistant state, and is currently regarded as incurable. The large body of evidence indicates that PCa cells remain dependent on androgen receptor (AR) signaling even in an androgen-deprived environment. As such, development of drugs that target AR and AR signaling pathways have become one of the major milestones in treatment of castration-resistant PCa (CRPC). Nevertheless, currently available therapies that target AR signaling are still regarded as palliative and more potent therapies are in great need. Over the past few years, a wide range of novel therapies has entered clinical trial for treatment of CRPC, including androgen synthesis inhibitors (abiraterone acetate), chemotherapeutic agents (docetaxel and cabazitaxel), and immunotherapies (sipuleucel-T). In this context, enzalutamide (previously referred to as MDV3100) is a novel second generation antiandrogen that has been demonstrated to significantly improve survival in men with metastatic CRPC in several clinical trials. In this paper we summarize recently completed and ongoing clinical trials of enzalutamide, and briefly discuss the efficacy of the novel antiandrogen therapy and its limitations for treatment of CRPC.
