What supplement should you take during pregnancy? Formula or breast milk?" As a pediatrician, I always recommend breast milk and breastfeeding, although that's not the right answer for every single family. What I like to recommend is happy baby, happy mother, happy mother, happy baby. I will never force a mother to breastfeed who does not want to breastfeed. We do know that breast milk offers fantastic protection for a baby's immune system. The immunoglobulins that get transferred from the mother to the baby stay with the baby for the rest of their life. The gut is colonized initially and the baby is much less likely to get diarrheal infections, to get ear infections, much less likely to have asthma. It's protected against a lot of common illnesses in childhood. That being said, not all mothers feel breastfeeding is right for them. You don't have to exclusively breastfeed. Some moms breastfeed during the day and they give formula at night. Some moms alternate and some moms don't breastfeed at all. And that's fine, too. The formula that is available on the market now provides a baby with all the vitamins and nutrients as comparable to human breast milk. You have to talk to your doctor - which formula would be right for your baby? We recommend babies who drink formula start with cow's milk formula. If there's something wrong - the baby's not tolerating it, the baby has severe rash or bloody diarrhea - let your doctor know and the doctor will help you choose a different formula for your baby. Babies who are exclusively breastfed at two weeks of age should start Vitamin D drops, 400 units a day. Babies who are on formula do not need to take vitamins because all of the vitamins are supplemented in the formula. Every family has to figure what out is right for them. Society often pressures parents to do things that they might not feel comfortable about, and that's when you need to talk to your doctor. Some moms want to only breastfeed and don't want to pump. Some moms only want to pump and don't want to breastfeed. Whatever is right for you is what you have to do for your baby, because baby will not thrive if the mom or family is not happy.

Vitamins D,E,K PART 2 - Everything You Need To Know - Dr. Nabil Ebraheim
Dr. Ebraheim’s educational animated video describes different vitamins. Vitamin D and PTH (parathyroid hormone) are important for calcium hemostasis. Vitamin D is important for proper maturation and development of the bone. Vitamin D is naturally found in Tuna, Salmon and Cod liver oil. Vitamin D and K are not present in sufficient quantity in breast milk. Production in the body: Precursor for vitamin D is found in the skin. Cholesterol derived precursor (pro-vitamin D3) is formed in the liver and stored in the skin. UV light converts it to pre-vitamin D3. The liver will then convert it to 25 hydroxyvitamin D3. Then the kidney will convert it to 1,25-dihydroxyvitamin D3, which is the active form of vitamin D3. PTH regulates the active form of vitamin D3. Active vitamin D3 stimulates the absorption of calcium and phosphate from the intestine. Low vitamin D levels lead to rickets. Low vitamin D rickets is the most common form of rickets worldwide. In adults, it leads to osteomalacia. Looser zones are stress fractures in patients with osteomalacia. Darker skin individuals should have more sun exposure. Vitamin D resistant rickets: the most common form of rickets in the United States. Also known as familial hypophosphatemic rickets. It is an X-linked inherited disease (dominant). The defective mineralization of bone in osteomalacia leads to a qualitative defect of bone with a high amount of unmineralized osteoid matrix. Osteoporosis, on the other hand, is a quantitative defect of bone. Vitamin D deficiency is considered when blood levels of 25 hydroxyvitamin D3 fall below 20 ng/ml. vitamin D levels are usually found to be low in patients with low energy fractures. Vitamin D deficiency is found in the majority of patients with endocrine or metabolic problems presenting with fracture non-unions. Dilantin (phenytoin) inhibits vitamin D formation in the liver. Decreased exposure to sun light and decreased intake of vitamin D are risk factors for developing Multiple Sclerosis. Recent research has also demonstrated a link between low vitamin D and vitamin B12 levels and the development of Alzheimer’s disease. Daily requirements of vitamin D are 400-800 international units. Vitamin D and calcium are given for female athletic triad. Vitamin E Vitamin E affects peripheral nerves and is a major antioxidant as it stabilizes free radicals and reduces the potential for oxidative damage. Vitamin E prevents peroxidation of fatty acids in cell membranes, helping to maintain their normal fluidity. Vitamin E also has some enzymatic activity. Deficiency of Vitamin E can lead to hemolysis, neurological problems, and retinitis pigmentosa. Vitamin E is found in milk, nuts, corn oil, sunflower oil, avocados, asparagus, and broccoli. Vitamin K Vitamin K is needed to introduce calcium binding sites on Vitamin K dependent clotting factors in order to activate them. Without Vitamin K the blood will not clot. This process is called gamma-carboxylation of glutamic acid. Vitamin K dependent factors are 1972 (factors: 10, 9, 7, 2,). Vitamin K is found in green leafy vegetables. Causes of Vitamin K deficiency include fat malabsorption, prolonged use of broad-spectrum antibiotics, breast-fed newborns, and infants of mothers taking Phenytoin (Dilantin). How to differentiate between Vitamin K and Vitamin C deficiencies? Vitamin K deficiency: easy bruising and bleeding, normal bleeding time and increased prothrombin time, no connective tissue problems. Vitamin C deficiency: easy bruising and bleeding, prolonged bleeding time and normal prothrombin time. Vitamin C deficiency patients have skeletal deformities, gum hyperplasia and bleeding, and poor wound healing. Coumadin is a vitamin K antagonist. Coumadin affects vitamin K metabolism in the liver, leading to the inhibition of vitamin K dependent clotting factors (1972). The therapeutic target level for Coumadin is an INR of 2-3. It takes 3 days to reach that therapeutic level and takes another 3 days to reverse it by vitamin K. fresh frozen plasma would act immediately. Protamine sulfate and fresh frozen plasma would work immediately in the case of heparin. 7 Tips about Vitamins for Infants | Baby Development

We like to make sure that the latch is established and the baby is breastfeeding well, but after that we do like to introduce Vitamin D, because the Vitamin D in breast milk is not transferred well and absorbed well by the baby, so those babies do need extra Vitamin D. It's sold over the counter. It's one dropper full, which is one mL, 400 units per day. Babies who are bottle-fed standard cow's milk or soy milk formula or any of the standardized formulas, actually, they are all supplemented with vitamins, so you do not need to give a baby who is on formula vitamins. That being said, some parents do believe in vitamins, and they like to give supplements. Vitamins for children do not cause harm if used in a small dosage, like the recommended child dosage that's over the counter. After age one, the baby can take the liquid drops. There are certain name brands, but it's a children multi-vitamin that's over the counter. If your doctor recommends that your baby needs iron, you might give a vitamin supplemented with iron as well. At four months, sometimes babies need more iron, especially babies who are not taking solids yet. So at four months, some doctors recommend Vitamin D drops with iron or a multi-vitamin with iron for your baby. Those are not written with a prescription, but you should discuss with your doctor what vitamins your baby needs or does not need. In addition, in different states and different localities, some of the water is fluorinated and some is not, so you need to find out if the water in your child's neighborhood is fluorinated or if you need to give your child a fluoride supplement. In New York City, the water has fluoride, so we do not give our children fluoride in the drops because that would be excess fluoride, and it would cause staining of the teeth. As a child grows, if your child eats a well-balanced diet, they do not need to take a vitamin. I often say that the more expensive the vitamin, the more expensive the urine because they usually just excrete most of it out.

My 2 month old's vitamin D drops keep him awake. What can I do?
The American academy and pediatrics dose recommend that all breast feed babies receive a vitamin D supplement because human milk dose not contain enough vitamin D and a vitamin D deficiency can lead to certain diseases or health conditions one of which is rickets and our bodies naturally synthesize vitamin D by getting sun exposure but obviously we are not taking our babies out and exposing them to the sun they should wear sun screen and for the first few months of life be protected from the sun. So its very important to give this vitamin supplement. Now you noticed that your little one doesn't like it and they seem to be aware more alert after they've gotten it and his could just be the taste, its not a stimulant of any kind, its not something that would actually keep your baby up but just the taste alone would wake your baby wake up and not necessary your feel comfortable. So because its important you need to always weigh out the risk and benefits of different things. There is a few things you could try for example give it when your baby is going to be up and alert not when your baby is trying to go to sleep and also wash it down with a nursing session afterwards or with a bottle and you also could if you wanted to put a couple of drops into pumped breast milk and feed it to your baby in a bottle if you are bottle feeding your baby at times. My 5 month old is only breastfed. Should I also give him vitamin D and iron supplements?
You have a really good question. You want to know if your 5-month-old should be receiving Vitamin D and iron supplements at five months of age and you're exclusively breastfeeding. When it comes to iron, most babies are born with the iron reserve that they need and there is iron in breast milk and there's definitely iron-fortified formula. But some babies don't have enough reserve or don't get enough and they do need supplements. This is usually discovered when the doctor takes a sample of your baby's blood and does a test, finds out the iron levels are low and then recommends a supplement. So unless your doctor said anything, you don't really need to worry about it. But it's always worth bringing it up with the doctor and then they can decide if further investigation or treatment is warranted sooner than when they would have tested it anyway. When it comes to Vitamin D supplements, the American Academy of Pediatrics does recommend that Vitamin D supplements are given to babies, even infants who are breastfed. Vitamin D is naturally synthesized in our bodies when we're exposed to sunlight but we don't expose our infants to sunlight and as they get older, we put sunscreen all over them and so this is one of the reasons why Vitamin D supplements are necessary until about a year of age when babies switch to whole milk. But again, don't give any supplements to your baby until you've talked with your doctor. When is the best time to do detox? Detoxing should be done before and after pregnancy. Doing a cleanse or a reset a few months prior to conception is most essentially recommended as juicing can boost fertility and may also improve the health of the baby, and the parents. Important nutrients vital for Mothers and their babies during pregnancy and breastfeeding: Fulvic Acid – this is one of the needed nutrients that many mothers overlook. This important nutrient helps in the development of the baby helping it ward off toxins and other harmful substances that causes birth deficiencies. Protein – we need an extra 10 grams per day. Mayer Eisenstein MD,JD,MPH** I am a physician, attorney, father, grandfather and husband of a wife whohas not only been a LaLeche Leader for 17 years, has nursed all six of our children and actively helped our daughters and daughter-in-law nurse our nine grandchildren. I was always bothered by the statement of American Academy of Pediatrics that breastmilk was lacking in Vitamin D. However, when you measure the Vitamin D content in most womens breastmilk it was truly lacking in Vitamin D. This lack of Vitamin D could be responsible for the increased incidence of early childhood cavities in some nursing babies.1 Many pediatricians blame the childhood cavities on night nursing, claiming that the breastmilk that remained sitting in the mouth was the cause. They insisted that mothers stop night nursing. In all fairness, they also recommended eliminating night bottles of milk or juice. Even though the cases of early childhood cavities pointed in the direction that breastmilk lacked Vitamin D, I knew there had to be a better explanation. Knowing that all children have adult teeth, I never stopped any mother from nursing at night and told them that is why we all have a second set of teeth. I have been troubled by this for more than 30 years. Last February I attended a lecture by Dr. Bruce Hollis, The Vitamin D Requirements During Pregnancy and Lactation.2 All of a sudden it became clear that the reason breastmilk was lacking in Vitamin D is either the mother was not in the sun enough (just 10-15 mins. a day with 40% body exposure will give you 5,000-10,000 IUD) or because she was not getting enough Vitamin D supplementation. As Dr. Hollis reported, when breastfeeding mothers have sun exposure or adequate supplementation, the Vitamin D levels in their breastmilk skyrockets. At this point Dr. Hollis current recommendation is that pregnant and nursing mothers (because of their increased metabolic rate) need at least 6,000-10,000 IUD daily. His research is ongoing and will be concluded in 2010.3 However, even in the summer we rarely get even 15 minutes a day of adequate sun exposure. Most of us go from our air conditioned house to our air conditioned car to our air conditioned mall or office, never getting even the minimal amount of sun exposure to naturally produce Vitamin D. There is a blood test (25[OH]D - to actually measure the blood level of Vitamin D3 ) that I recommend for everyone, including pregnant and nursing mothers. The normal level should be between 40-60ng/ml. Using this blood level as a baseline, studies have shown that approximately 75% of pregnant and nursing mothers are actually Vitamin D deficient. Sadly to say we have found the same results, i.e. blood levels below 40 ng/ml, in a majority of the Homefirst® population, a group which under normal circumstances is considered above average in health. Causes of Low Vitamin D and Why Your Climate Is Critically Important

Breast-fed infants are also prone to vitamin D deficiency because breast milk does not usually contain a sufficient amount of vitamin D, and infants should not be exposed to direct sunlight. Even if you do get the chance to work on your tan, it's not always advisable to stay out too long under the sun without sunscreen, especially for those who have fairer skin or those who are considered high-risk in getting skin cancer. As you may have guessed, using sunblock, especially those with higher SPFs, can hinder your vitamin D production to as much as 90-99%. The easiest way, and the safest way, is to take vitamin D supplements with your diet.
