More information on pediatric asthma: https://www.nationaljewish.org/conditions/pediatric-asthma/overview Learn more about Dr. Lanser: https://www.nationaljewish.org/doctors-departments/providers/physicians/bruce-j-lanser DENVER -- Common mistakes taking inhaled medications for asthma leave many patients with uncontrolled asthma, leading to unnecessary asthma attacks, emergency room visits and hospital admissions among the seven million children with asthma in the United States. “We see a lot of children, in fact, who just don’t know how to use their inhalers at all,” said BJ Lanser, MD, a pediatrician and asthma specialist at National Jewish Health in Denver. “For whatever reason, they were given the inhalers but were never taught the proper techniques in how to use them, and that can lead to serious problems.” Poor inhaler technique fails to deliver medications deep into the airways where they are needed. Among the more common mistakes children make is using an inhaler without a spacer, a plastic tube attached to the mouthpiece. When children activate the inhaler, the spacer lets the medication mix with air so that it can be inhaled more effectively. For smaller children, spacers often are paired with masks to make the process easier. “Without a spacer, 70 to 80 percent of the medicine ends up in the child’s mouth and never gets deep into the lungs where it needs to be,” said Lanser. “If they continue to make those mistakes dose after dose, their symptoms begin to worsen and often those children end up in the hospital.” That’s just what happened to 10-year-old Amanda Grabel of Santa Fe, New Mexico. “She was about 7 when we noticed that she would have really bad asthma attacks whenever she got a cold,” said Rori Grabel, Amanda’s mother. “We saw doctor after doctor and they threw more and more medicine at her, but nothing was working.” In addition to their family doctor, the Grabels saw an allergy specialist, a pediatric pulmonologist and made several frantic trips to the ER. “Amanda was afraid to go to sleep at night, because she was afraid she was going to die,” Grabel said. “That’s how bad things had gotten.” After seeing doctors in three states, the Grabels took Amanda to see Dr. Lanser at National Jewish Health in Denver and were relieved by his evaluation. “Amanda didn’t need any more medicine,” said Lanser. “She had the right medication, she just wasn’t taking it properly. So, we really worked with her on technique when using her inhaler and today, she’s thriving.” If your child uses an inhaler to control asthma, here are some tips to help them get the most out of their medicine: Always stand when using an inhaler. Standing allows the lungs to fully expand so the medication can get where it is needed most. Look straight ahead. You want to make sure your head is in a neutral position, not leaning forward or backward. This will help direct the medicine into your airways and prevent it from collecting in your mouth. Before you inhale, exhale. Have your child take some normal breaths and then a big, deep breath, then fully exhale so the lungs are empty. Then, when your child inhales, the medicine gets deep into the lungs. Inhale slowly. Even if you are having trouble breathing at the time, be sure to inhale your medicine slowly. If you hear a whistling sound from your spacer, you know you are inhaling too quickly. Close your lips around the mouthpiece. Because the medicine is aerosolized, it can easily escape the mouthpiece, so be sure to make a tight seal with your lips to get all the medication into your lungs. Just a little training made a big difference for Amanda. Today, she runs, plays soccer and tennis and plays the trumpet in the band. “I feel like I don’t even have asthma anymore,” she said. “I know that I do, but now I can just be happy and not worry about anything because it’s under control.”

Asthma is a chronic respiratory condition where the airways in the lungs are inflamed and narrowed causing breathlessness, wheezing, chest tightness and coughing. Symptoms come as recurrent episodes known as asthmatic attacks most commonly at night or early in the morning. Pathology of asthma: The lungs contain millions of air tubes or airways, called bronchi and bronchioles, which bring air in and out of the body. The airways have a layer of smooth muscle in their wall which enables them to constrict or dilate. In people suffering from asthma these airways are inflamed, narrowed and become more sensitive to certain substances. Asthmatic attack, or exacerbation, happens when the airways react to these substances. During an attack, the smooth muscle contracts, squeezing the airways, making them even narrower; mucus secretion is also increased which further obstructs the airways. Asthma is most commonly considered as an inflammatory response disease where the body’s immune system over-reacts to certain environmental agents. Causes of asthma are complex and not fully understood but likely involve a combination of genetic and environmental factors. Family history is a known risk factor for asthma. There are at least over twenty genes associated with asthma of which many are involved in the immune system. Triggers are factors that initiate the attack. Common triggers include: - allergens, such as pollen, animal fur, pet dander, sulfites in preserved food.. - irritants, such as cigarette smoke, industrial chemicals, dust, household chemicals,.. - medications, such as aspirin, beta blockers,.. - physical activities, exercises. There is no cure for asthma. The most effective way to manage symptoms is to identify the triggers of asthmatic attacks and avoid them. There are two main classes of medication: - Bronchodilators - substances that dilate bronchi and bronchioles - are used as short-term relief of symptoms. - Inflammation moderators such as corticosteroids are taken as long-term treatments. Asthma inhalers are used to deliver the medication to the lungs. A number of conditions tend to occur more frequently in people with asthma and should be taken into account when treating asthma: - Allergies, such as eczema and hay fever. These individuals are considered hyper-allergic – they have high tendency to develop allergic reactions. The combination of these conditions is known as atopy or atopic syndrome. Immunotherapy may be recommended for this group of patients. - Gastroesophageal reflux disease or GERD - a condition in which stomach acid backs up and damages the mucosal lining of the esophagus. GERD may worsen asthma symptoms and medications for asthma often worsen GERD symptoms. Treating GERD usually improves asthma and must be included in an asthma treatment plan. - Obstructive sleep apnea or OSA – obstruction of the airway at the throat level during sleep. Asthmatic patients are at higher risks of developing OSA. The mechanism of this association is largely unknown. - Sinusitis: inflammation of paranasal sinuses. Sinusitis commonly worsens asthma symptoms and makes treatments less effective.

PORTLAND, Tenn.- The community is helping a teen tackle an asthma attack that nearly took her life. According to the Centers for Disease Control and Prevention, nearly 26 million Americans suffer from asthma. More than 7 million are under the age of 18, like Ashley Beasley. The Portland High School sophomore suffered complications with an asthma attack and rhinovirus six weeks ago. More than a hundred people are at Richland Park on this bright, Sunday helping the teen get though one of her darkest times. Jacki Beasley, Ashleys mother says, "On August 29th she called me from a friends house and said she was having an asthma attack, They called an ambulance because she had passed out." Jacki Beasley says emergency crews flew her daughter to Centennial Childrens Hospital in Nashville. She had a brain injury due to losing oxygen for more than 10 minutes. Doctors put the 16-year-old in a medically induced coma for a week. Beasley says, Rhinovirus is really fast acting, you're deprived of oxygen that asthma triggersit's definitely a struggle." Not only for Ashley, but also her brother who suffers from the disease also. Each day 9 Americans die from asthma. There are more than 3,600 deaths due to asthma each year, many of which are avoidable with proper treatment and care (Asthma Allergy Foundation of America). Beasley says Ashley is improving, We can see something different about every day. She blinks and squeezes. Sarah Brown says, "She's always been a fighter and we have faith that she is going to keep getting stronger . Ashley is just a wonderful person, so I wanted to do something ." Sarah Brown is a family friend and organizer of today's fundraiser/walk. Brown says, Supporting her will make her stronger and the family stronger. I want her to graduate and do the things she loves. Beasley says, "The community support has been amazing. I never dreamed that my family or she would mean so much to this town that we have." Ashley Beasley is expected to be released from the hospital by Tuesday. Her family says she will do a combination of physical, occupational, and speech therapy. CINCINNATI (Liz Bonis) -- Theres a rise in breathing complications going around the Tri-State. Family medicine experts at TriHealth said it was not peoples imaginations. Complaints of shortness of breath were way up bringing people into area doctors offices and emergency rooms. Blame it on the sudden surge of allergies and allergens in full bloom. Doctor Scott Woods told Local 12 News he saw a rise in complications at Bethesda North Hospital and also at his practice in Norwood, Bethesda Family Medicine. He said it could also party be the humidity. This time of year though is very difficult on people with lung disease; the high humidity, emphysema, COPD puts a lot of stress on the heart and lungs so we see a lot more people coming in for shortness of breath, said Dr. Woods. His main concern was that especially kids as they head back to school get proper medications so they don't chase symptoms as the allergy and asthma season gets worse. It's clearly been the worst allergy season I have seen in 20 years. It's been an intense year and especially if your child has had bad allergies historically, it would be good to get ahead of it. Particularly if your child has asthma; get them on medicine now before September takes hold and those things can really flare intensively, advised Dr. Woods. Dr. Woods said if people wait to gut it out, as kids get back into classes they get even more miserable when the fall allergies flourish. I just want to make it really clear that I don't blame anyone for the surgery not going as planned. I am very thankful to the whole team who's job it was to take care of me and keep me safe in the operating room. They did their jobs. Yes, I did feel a bit upset afterwards when I felt like my concerns weren't being addressed by the anesthesiologist, but I am in no way blaming him for what happened (didn't happen), this is a very common complication for patients like me. I have since spoken to the anesthesiologist again and I apologize for calling him snarky lol. He was actually very kind. Hopefully on Monday we will able to take the time to fully address my concerns and come up with a real surgical plan for how my airways will be managed. CLEVES, Ohio (WKRC) - Friends and neighbors are rallying around the family of a 12-year-old boy hospitalized from complications of an asthma attack. Brandon Hawkins is the youngest of four children. He's a very bright student and loves dinosaurs. He's had asthma all his life, but on May 15, he had his worst episode yet; one that send him to Cincinnati Children's Hospital Medical Center (CCHMC) in a helicopter. On the way to the hospital he went into cardiac arrest and suffered a stroke. "His initial prognosis was not good, but he's actually beating the odds," said Brian Hawkins, Brandon's father. He and the boy's mother, Jackie, have been by his side every day. Brian is a Cincinnati firefighter and Julie works at CCHMC. They've put their lives on hold to be with their son. Friends and neighbors are filling in the cracks. "The phrase it takes a village- it really does. Our small little village and our families, they are- they're helping with our kids, they're giving them rides, I mean they're just- I can't even tell you how grateful I am," Jackie said. Tuesday night, May 31, that village- Cleves to be specific- held a fundraiser to help the family get through this tough time. Skyline Chili donated a portion of their sales to the family and friends sold baked good and buttons with the message #BelieveInBrandon. "It was automatic support. Childhood Asthma May Increase Risks of Shingles

Nearly 1 million incidences of herpes zoster, which is also known as shingles, occur every year in the U.S., with an estimated one-third of all adults affected by age 80. Despite its prevalence, particularly between ages 50 and 59, it is still unclear why some individuals will develop shingles, and others will not. In a population-based study published in the Journal of Allergy and Clinical Immunology (JACI), Mayo Clinic researchers build on their previous research from 2013, which linked asthma in childhood with an increased risk of shingles. “Asthma represents one of the five most burdensome chronic diseases in the U.S., affecting up to 17 percent of the population,” says lead author Young Juhn, M.D., who is a general academic pediatrician and asthma epidemiologist at the Mayo Clinic Children’s Research Center. “The effect of asthma on the risk of infection or immune dysfunction might very well go beyond the airways.”

Children with asthma are more prone to develop pneumonia after getting sick with flu than children who do not have asthma. Flu infection can be more serious for children with asthma, even if their asthma is mild or their symptoms are well-controlled by medication. In childhood asthma, the lungs and airways become easily inflamed when exposed to certain triggers and influenza can cause further inflammation of the airways and lungs. Influenza infection in the lungs can trigger asthma attacks and a worsening of asthma symptoms. Pulmonary complications include pneumonia and other acute respiratory diseases. Asthma is the most common disease among children hospitalized with flu.

(DENVER, Colorado) – If you’re the parent of a child with asthma, you know it can be a challenge to find ways to help him or her exercise safely and comfortably. But experts say there is one activity that can help keep your child fit, and actually improve asthma symptoms. “When it comes to cardiovascular activities that are well-tolerated, swimming, specifically, is highly recommended, particularly in indoor swimming pools,” said Tod Olin, M.D., a pediatric pulmonary specialist at National Jewish Health. “We know that the kids with asthma are less likely to trigger attacks if they’re in a really humid environment, and the water forces them to be more conscious and controlled with their breathing.” Not far from Dr. Olin’s office is Morgridge Academy, a school for children with chronic conditions who require daily medical attention on the campus of National Jewish Health. Administrators there consider swimming so therapeutic that it’s a part of the school curriculum. “Before coming to us, many of our kids didn’t run around and play like other kids,” said Jennifer McCullough, director of Education at Morgridge Academy. “But when you get them into that pool with that warm air, and teach them how to regulate their breathing, they can do a lot more physical activity than they would be able to do otherwise.” Over time, students who regularly play and exercise in the indoor pool have gained better control of their asthma and improved their cardiovascular health. “We track the progress of each student, and we’ve found that swimming in the pool helps reduce their symptoms. Swimming is a large part of our fitness program that has been key in scaling back emergency room visits and hospital stays.” said McCullough.
