The presence of pain around the shoulder does not always mean it is a shoulder problem. Pain can come from the structures inside the shoulder or from the structures outside of the shoulder.The pain can be referred from the neck problems ( cervical spine pathology) or from other distant sites. Elbow pain may be rarely transmitted to the shoulder. The shoulder is a common area for referred pain and examination of the shoulder should include an examination of there other areas. One of these areas that can cause pain referred to the shoulder is the neck, such as a herniated cervical disc or cervical spine pathology. The muscles and the nerves that arise from the neck area will pass through and around the shoulder on their way down the arm. The pain may radiate to the shoulder of the scapula. The pain is usually at the superomedial aspect of the scapula. This is an example of the most common area of referred pain from neck pathology and disc herniation. The pain originating from the neck region may overlap the shoulder. Usually, the pain in the neck is felt on top of the shoulder over the area of the trapezius muscle. Pain from the shoulder itself is usually felt at the top of the arm. If the shoulder moves freely without pain, then the neck is the source of the pain. Sometimes these conditions are confusing and difficult to differentiate (they usually overlap), especially when neck related shoulder pain causes weakness of the rotator cuff muscles, causes the shoulder to become stiff or causes shoulder impingement. Pain arising from the acromioclavicular joint can radiate into the neck itself. Sometimes MRI is the only accurate study that helps in differentiating between neck pain and shoulder pain. When cervical spine problems are suspected, the physician should examine the patient for findings of gait disturbance and long tract signs which will indicate that the patient may have cervical myelopathy. A positive Spurling's test will indicate neck pathology, especially nerve root irritation. A positive shoulder abduction test usually indicates neck pathology. The patient’s symptoms are relieved by shoulder abduction, by placing the hand over the head. Heart problems such as myocardial infarction or angina may cause pain radiating to the left shoulder. Irritation of the diaphragm may present itself as shoulder pain. The diaphragm may share the same C4-C5 nerve root irritation. The chest and upper abdomen should be examined carefully for possible pathology. A Pancoast tumor is another condition which causes referred pain in the shoulder region. A Pancoast tumor is a type of lung cancer that forms at the very top of the lung. Shoulder pain may be the first symptom of pancost tumor. The post tumor may invade the brachial plexus, the ribs, the vertebral bodies, the pleura and the chest wall, causing shoulder pain. Sometimes spine fractures may present was shoulder pain. The fracture may irritate the rhomboids muscles and transmit pain to the scapula. This patient flew in from ALASKA to see me! And she is such a sweetheart and was really excited about getting this large lump in her arm removed! She wasn't nervous at all, her sister who accompanied her isn't a popaholic, so we sent her out of the room when we did this surgery (I think her sis was plenty happy that we did that!). This woman is adorable in so many ways, she is excited to get this removed (and we can all understand why, it's pretty big and really distorts her right arm). You should know that I don't agree to remove everyone's lipoma that I see. The lipoma has to be very mobile under the skin. Usually in the arm, a lipoma is pretty well encapsulated and will pop out whole as you can see here. So this one was truly AMAZING. Also, I train my staff to be very careful about what they say when we are doing surgeries, I think many of you see this and have caught on. It's different when you primarily do surgeries when patients are awake and alert (as opposed to under general anesthesia). I almost always include the patient in my conversation, never say "oops" or express surprise or shock, and this is very important to me because it keeps the patient calm and they understand what's going on and feel included. She obviously is very relaxed and was very interested in what we were doing, in fact she looks over at one time to watch (which I actually try to prevent and discourage, sometimes even if people feel like they can watch themselves undergo surgery, they may realize that they can't handle it during the moment, i much prefer showing them the video directly after surgery instead of having them watch during!) Educational video describing the condition of lipoma. Lipoma is the most common type of soft tissue tumor. It is a benign tumor that contains fat. It is slow growing asymptomatic and painless. Lipoma is usually soft and mobile with superficial swelling. If it deep or intramuscular, it is usually large and may be firm, similar to sarcoma. Deep lipoma is usually close to the neurovascular bundle. Most of the lipomas are superficial. Deep lipomas are infrequent and they present a problem on the diagnosis. Lipoma is rarely seen in young patients. It is usually seen in patients older than 40 years old. It is more common in men and occasionally multiple. Differential diagnosis Liposarcoma: a malignant tumor that arises in fat cells. It is usually proximal to the knee and elbow. Consider liposarcoma or any other soft tissue tumor if the tissue mass is deep, more than 5 cm and located in the thigh. MRI will show a homogeneous signal intensity. It has the same signal as fat in the subcutaneous tissue in all pulse sequence. The fatty tumor has the same fatty appearance as the subcutaneous fat. The signal is bright on T1, dark on T2 and in STIR. Liposarcoma will be dark on T! and bright on T2. Liposarcoma is also hot on a bone scan. Lipoma is avascular, cold on a bone scan. Dr Jeff Rebish is a board certified dermatologist, who is in practice with Dr Sandra Lee. He practices general dermatology and some surgical dermatology. Here, he removes a pretty sizable lipoma from the nape / the posterior neck. This patient requested removal because the large bump behind his neck is pretty sizable and is a source of embarrassment for him. The growth was not painful, but he was aware of it when he moved his neck around, and it decreased neck mobility somewhat. Dr. Rebish excised this lipoma carefully, after local anesthesia. This is a procedure commonly done in a dermatology office - there is no need for general anesthesia. As you can see, the patient is comfortable, and there is minor bleeding, and no pain. A lipoma is a benign skin tumor, a growth under the skin, creating a large bump that is usually painless and very mobile. It is a collection of clonal fat cells/adipose cells, and is often somewhat well-circumscribed and can be removed making a fairly small incision on the skin surface. Of course, a larger incision is required for a larger lipoma. A lipoma is not likely to reoccur if it has been excised completely, as in this case. Dr. Ebraheim’s educational animated video describes ganglion cysts of the wrist. Ganglion cyst of the wrist is the most common hand mass and it is not a tumor. The cyst is filled with mucinous (gelatinous) fluid and it transilluminates. Ganglion cysts of the wrist can be either dorsal or volar. The dorsal type of ganglion cyst is common and it not as bad. The dorsal type originates from the scapholunate articulation. Occasionally the volar type (rare) occurs and this can be bad. The volar type could compress the median nerve or ulnar nerve and possibly compress the artery which will compromise the circulation in the hand. The volar type originates from the radiocarpal joint, which is the wrist joint itself (Figure 3). Clinical findings A bump or a mass that is well defined, localized, smooth, and not attached to the skin. The dorsal ganglion cyst is obvious with flexion of the wrist. The volar ganglion cyst is obvious with extension of the wrist. The ganglion cyst in general is usually asymptomatic and the patient is more concerned about the look and cosmesis. When the physician examines it, they will see that it is a firm mass that can transilluminate. The physician will do the Allen’s test. The nerves of the hand and the circulation will be checked to make sure that they are not affected. An MRI is usually not done and when it is done, there is usually a fluid signal seen inside the mass. An ultrasound is a good study to check the relationship between the cyst and the vascularity, especially the radial artery with volar cysts. Ultrasound is good during an aspiration when the ganglion cyst is near the artery. Treatment In the majority of cases, treatment is observation. In children, the majority of ganglion cysts resolve themselves in about one year. The physician will either leave the cyst alone, perform an aspiration or perform surgery. An aspiration is usually done for dorsal ganglion cysts. There is a high recurrence rate with aspiration of a dorsal ganglion cyst (50%). There is minimal risk associated with an aspiration. The physician should try to avoid aspiration with volar ganglion cysts because the volar cysts is close to the artery. If the physician performs an aspiration of the volar ganglion cyst, the wrist will be hyperextended to try and see the artery and displace it away from the cyst as the needed is placed in an area away from the artery. It is probably better to do an aspiration of the volar ganglion with ultrasound guidance. Surgery is done for severe symptoms and if there is neurovascular compromise. Excision of the ganglion cyst will be done if it is painful, resect the capsule and the stalk. Excision for a dorsal ganglion cyst has less than 10% recurrence rate. A high recurrence rate excision of the volar ganglion cyst (about 20%). The Allen’s test should be done to check the blood flow before doing excision of the volar ganglion cyst. A lipoma is slow-growing, benign growth of fat cells. It is contained in a thin, fibrous capsule and found right under the skin. Popaholics I'd fast forward to the 7:35 mark and for those interested in just suturing, that starts around the 11:00 mark. For those trying to learn the procedure, the teaching points I would make are the following. First, the lower back, in and of itself is a bit challenging when you go looking for lipomas (especially) and likewise for cysts. There can be an enormous amount of subcutaneous tissue and you can 'lose' your target fairly easily. For this reason, many physicians opt to start with an elliptical incision as they have a greater chance of localizing their target. I did not do so here as a linear incision is less traumatic and seals up a bit simpler in the end. However, had I had difficulty localizing the lipoma, I would have felt very comfortable extending my incision into a broader elliptical one. Second, if you simply cannot find the cyst or lipoma there is nothing wrong with suturing the patient closed and sending them to someone with more experience. I realize that this is not something anyone, the patient included, wants to hear but it isn't the end of the world either. As opposed to simply digging and digging, if you are out of your depth then simple suture the patient closed and send them along. The reality is that this will almost never happen and even if it did, the next surgeon would likely incorporate your incision into his process without creating a much more significant scar. At any rate, this procedure went fairly smoothly and I must apologize again for my hand getting in the way at times. I have been working on this lately and hopefully the newer videos will be of a higher quality. Thanks and take care.

This video will be on the pathology of lung cancers. This is the most common cause of cancer related deaths. Some risk factors for lung cancer, asbestos, arsenic, and radon among others. Physical symptoms include cough, hemoptysis and weight loss. So a patient comes in and you're suspecting lung cancer. What now? Do a chest xray, which will show a nodule. To make sure it's not a hamartoma, biopsy. Once confirmed, stage the lung cancer to measure it's spread. Metastasis is common to the bone, brain, breasts and adrenals. Now there are some secondary complications to lung cancer. Many stem from it's mass effect. It can compress on structures like your superior vena cava, as SVC syndrome. This leads to congestion in the upper limbs, JVD, and blanching. You can also compress your recurrent laryngeal nerve and cause hoarseness. You can also have a cancer at the superior portion of the lung and this is called Pancoast tumor. This can compress on your superior cervical sympathetic ganglion and cause Horner's syndrome - which manifest itself as ptosis, miosis, and anhydrosis. Lastly, you can compress on the first cervical rib and cause thoracic outlet syndrome. This compresses the brachial plexus as well as the blood vessels and leads to atrophy, weakness and pain in the shoulder and arms. Lastly a complication of any tumor is paraneoplastic syndromes. Now what are the specific types of lung cancers? Neoplasms are broken into two categories: small cell and non-small cell tumors. Let's discuss non-small cell first. 1) Squamous cell carcinoma: loves to cavitate. Microscopically shows keratin pearls on histology and intercellular bridges. Paraneoplastic syndromes include hypercalcemia due to it's release of PTHrP. 2) Large cell: shows large pleomorphic cancer cells. Releases b-Hcg and can cause gynecomastia and galactorrhea. 3) Adenocarcinoma: seen in non-smokers and in women. Instead it's due to activating mutations of proto-onco genes like KRAS, EGFR, and ALK. Histologically you'll see the glandular cells and they are mucin positive. There is a special subtype called bronchoalveolar. From your Clara cells, it grows along your alveolar wall, thickens it and CXR looks like interstitial/atypical pneumonia. 4) Bronchial Carcinoid tumor: not only seen in the gut, it can also be seen in your lungs. It is from the neuroendocrine cells of your lungs and will be small blue cells that are chromogranin A+. Grossly they look like a polyp and rarely mets. They release serotonin, causing vasoconstriction and wheezing. Now let's talk about small cell lung cancers. These cancers are associated with smoking as well as a mutation in L-MYC. This is a neoplasm of your neuroendocrine cells, in particular your Kulchitsky cells. Again neuroendocrine cells are chromogranin A+, as well as neuron specific enolase. Small cell lung cancer is related to many paraneoplastic syndromes. They can release ACTH (Cushings), ADH (SIADH), and antibodies against your brain (encephalitis) or presynaptic calcium channels of your neuromuscular junction (Lambert Eaton syndrome). This guy was so patient waiting for me! I was running way behind on this day, and he was my last patient of the day. I think he waited almost 45 minutes for me, and when I saw him, I had enough time to only remove one of his lipomas. So I promised i would see him again and be sure to remove any others that he wants to have removed. I think he was surprised that the surgery was so simple. And I think he was pretty happy to get this removed. As you can see, it's pretty hard to miss if he's wearing a short sleeved shirt. I think most of us can see why he wanted to have this removed. This can cause pain in the shoulders, neck and numbness in the fingers as the arm is moved. The most common causes: • Structures may be compressed in the thoracic outlet by the cervical rib • Anomalies of the scalenus muscles It is a neurovascular compression neuropathy of the brachial plexus in the thoracic outlet in the retroclavicular region with either a neurogenic or vascular etiology. The thoracic outlet space is created by: •Clavicle •First rib •Subclavius muscle •Costoclavicular ligament •Anterior scalene muscle This space also contains the subclavian vessels and the thoracic duct. It also contains the lower trunk of the brachial plexus (C8, T1). Two types: 1.Neurogenic 2.Vascular The neurogenic type is caused by compression of the neurovascular bundle as it passes over the first rib or through the scalene muscle.
