Dr. Ebraheim’s educational animated video describes the condition of thoracic outlet syndrome. Thoracic outlet syndrome is a diagnosis of exclusion based on the patient history and symptoms. This can cause pain in the shoulders, neck and numbness in the fingers as the arm is moved. The most common causes: • Structures may be compressed in the thoracic outlet by the cervical rib • Anomalies of the scalenus muscles It is a neurovascular compression neuropathy of the brachial plexus in the thoracic outlet in the retroclavicular region with either a neurogenic or vascular etiology. The thoracic outlet space is created by: •Clavicle •First rib •Subclavius muscle •Costoclavicular ligament •Anterior scalene muscle This space also contains the subclavian vessels and the thoracic duct. It also contains the lower trunk of the brachial plexus (C8, T1). Two types: 1.Neurogenic 2.Vascular The neurogenic type is caused by compression of the neurovascular bundle as it passes over the first rib or through the scalene muscle. Causes of compression: •Cervical rib •Elongated vertebral transverse process (C7) •Anomalies of the scalene muscle insertions •First rib malunion •Abnormal fibrous band on or near the two scalenous muscles •Repetitive shoulder movement •Extreme arm positions •Abnormal pectoralis minor muscle •Weight lifting •Rowing •Swimming Vascular Entity: Caused by a compressed subclavian vessel or by an aneurysm. Where is the compression site? •Where the brachial plexus passes over the first rib. oUsually at site of scalene triangle oThe brachial plexus and subclavian artery pass through the triangle (subclavian vein does not pass through the triangle) •Under the clavicle by the subclavius tendon •Underneath the conjoined tendon inserting into the coracoid process Diagnosis: •Symptoms are usually vague •Pain in the shoulder and neck that usually radiates to the forearm and hand (paresthesia radiating along the arm) •Loss of sensation of the little and ring fingers •May be some vascular symptoms such as arterial ischemia, venous congestion, Raynaud’s phenomenon (changing colors of the hands or chronically reduced pulse) •Look for ulnar nerve sensory changes and intrinsic weakness •Look to see if the patient has intolerance to cold (Raynaud’s phenomenon) Differential Diagnosis: •C8 radiculopathy or ulnar nerve compression at the elbow oCombination of weakness involving the median and ulnar nerve innervated muscles may confirm a more proximal injury to the brachial plexus •Rule out double crush syndrome with carpal tunnel syndrome and thoracic outlet syndrome Compression of the medial antebrachial cutaneous nerve could occur with compression of the thoracic outlet. When observing the shoulder for motion, the patient may experience pain through the range of motion, also known as the arc of pain, which is a positive indicator of a tear. Muscle weakness will come secondary to shoulder pain and you may also observe abnormal motion of the scapula. If a tear is concluded, scar tissue will be built up along the tendon and around the infraspinatus bursa causing a catching or grinding motion of the shoulder. The degree of a rotator cuff tear is difficult to determine without imaging. Some patients may present will decreased range of motion and in a great amount of pain, and some patients may have no pain and have limited range of motion. The first source of imaging is X-ray, however it will not be able to show any soft tissue damage but it will help rule out other conditions. An x-ray will tell you if there is any joint degeneration, impingement from an osseous structure, or arthritis in the joint. Possible treatments for an infraspinatus tear: RICE (rest, ice, compress, elevate) Surgery based on the severity of the tear to reattach the muscle tendon Steroid injections Non-Steroidal Anti-Inflammatory medications Addressing the scar tissue formation in the shoulder joint Stretching Rehabilitative strengthening exercises Addressing the scar tissue and rehabilitation of the shoulder is very important, avoidance of this can lead to further conditions such as frozen shoulder. Warm up of the muscles is important before beginning any strengthening protocol and can be done with stretching such as the doorway stretch or threading the needle yoga pose. Dr. Ebraheim’s educational animated video describes ganglion cysts of the wrist. Ganglion cyst of the wrist is the most common hand mass and it is not a tumor. The cyst is filled with mucinous (gelatinous) fluid and it transilluminates. Ganglion cysts of the wrist can be either dorsal or volar. The dorsal type of ganglion cyst is common and it not as bad. The dorsal type originates from the scapholunate articulation. Occasionally the volar type (rare) occurs and this can be bad. The volar type could compress the median nerve or ulnar nerve and possibly compress the artery which will compromise the circulation in the hand. The volar type originates from the radiocarpal joint, which is the wrist joint itself (Figure 3). Clinical findings A bump or a mass that is well defined, localized, smooth, and not attached to the skin. The dorsal ganglion cyst is obvious with flexion of the wrist. The volar ganglion cyst is obvious with extension of the wrist. The ganglion cyst in general is usually asymptomatic and the patient is more concerned about the look and cosmesis. When the physician examines it, they will see that it is a firm mass that can transilluminate. The physician will do the Allen’s test. The nerves of the hand and the circulation will be checked to make sure that they are not affected. An MRI is usually not done and when it is done, there is usually a fluid signal seen inside the mass. An ultrasound is a good study to check the relationship between the cyst and the vascularity, especially the radial artery with volar cysts. Ultrasound is good during an aspiration when the ganglion cyst is near the artery. Treatment In the majority of cases, treatment is observation. In children, the majority of ganglion cysts resolve themselves in about one year. The physician will either leave the cyst alone, perform an aspiration or perform surgery. An aspiration is usually done for dorsal ganglion cysts. There is a high recurrence rate with aspiration of a dorsal ganglion cyst (50%). There is minimal risk associated with an aspiration. The physician should try to avoid aspiration with volar ganglion cysts because the volar cysts is close to the artery. If the physician performs an aspiration of the volar ganglion cyst, the wrist will be hyperextended to try and see the artery and displace it away from the cyst as the needed is placed in an area away from the artery. It is probably better to do an aspiration of the volar ganglion with ultrasound guidance. Surgery is done for severe symptoms and if there is neurovascular compromise. Excision of the ganglion cyst will be done if it is painful, resect the capsule and the stalk. Excision for a dorsal ganglion cyst has less than 10% recurrence rate. A high recurrence rate excision of the volar ganglion cyst (about 20%). The Allen’s test should be done to check the blood flow before doing excision of the volar ganglion cyst. Lipoma is usually soft and mobile with superficial swelling. If it deep or intramuscular, it is usually large and may be firm, similar to sarcoma. Deep lipoma is usually close to the neurovascular bundle. Most of the lipomas are superficial. Deep lipomas are infrequent and they present a problem on the diagnosis. Lipoma is rarely seen in young patients. It is usually seen in patients older than 40 years old. It is more common in men and occasionally multiple. Differential diagnosis Liposarcoma: a malignant tumor that arises in fat cells. It is usually proximal to the knee and elbow. Consider liposarcoma or any other soft tissue tumor if the tissue mass is deep, more than 5 cm and located in the thigh. MRI will show a homogeneous signal intensity. It has the same signal as fat in the subcutaneous tissue in all pulse sequence. The fatty tumor has the same fatty appearance as the subcutaneous fat. The two other conditions closely related to each other called carpal tunnel syndrome and ulnar tunnel syndrome causes, these are compression of the nerves at the wrist joint though this nerves come from neck region into the shoulder into the elbow and come all the way to the hand to give sensation and also the function of the hand, this nerves can get compress anywhere in the way and on the way from almost neck till the hand and these compression can pain similar to this pains like radiating from wrist joint to the elbow. There are some rare causes like infection without any injury which can happen in the joint or tumors growing inside which are again rare. Now how do you identify and how do you treat, for each and every of this pain there are clinically identifiable Symptoms and Signs which we go through as patients complaints and the type of complaints they come with. we identify the condition so it's not very difficult to identify but we identify very easily. Sometimes these conditions cannot identify clinically then we need certain investigation the basic investigation in x-ray, if the x-ray also comes normal, the second investigation will be a MRI scan of the upper limb so here will definitely get to know if there is anything going on. Very rarely MRI can come normal that is when we have nerve compression like the ulnar tunnel syndrome and carpal tunnel syndrome for that we will need what is called ENMG study that is Electro Neuro Mayo Grams and also nerve conduction studies which is help us to find out which part of the nervous getting compress and what is the future. About the treatment part depends on the particular cause if it is trauma then it as to be rested, it has been taken care of, if fracture that has to be treated. The nerve compression's like carpal tunnel and ulnar syndromes can be treated with medication initially if it doesn't respond to medication then where ever the compression is on the nerves that can be released surgically, that we operate and release the compression there will be a bands of tissues which will compress the nerves that can be released and you get very good relief of that. The remaining causes as we discussed about tennis elbow is again a condition which can be identified which can be treated very well, you will have to give a lot of rest to the elbow sometimes we do give injections into the area where it is maximally painful in the elbow, with that also if it doesn't relent then they may need surgery for that also.

This patient flew in from ALASKA to see me! And she is such a sweetheart and was really excited about getting this large lump in her arm removed! She wasn't nervous at all, her sister who accompanied her isn't a popaholic, so we sent her out of the room when we did this surgery (I think her sis was plenty happy that we did that!). This woman is adorable in so many ways, she is excited to get this removed (and we can all understand why, it's pretty big and really distorts her right arm). You should know that I don't agree to remove everyone's lipoma that I see. The lipoma has to be very mobile under the skin. Usually in the arm, a lipoma is pretty well encapsulated and will pop out whole as you can see here. So this one was truly AMAZING. Also, I train my staff to be very careful about what they say when we are doing surgeries, I think many of you see this and have caught on. It's different when you primarily do surgeries when patients are awake and alert (as opposed to under general anesthesia). I almost always include the patient in my conversation, never say "oops" or express surprise or shock, and this is very important to me because it keeps the patient calm and they understand what's going on and feel included. She obviously is very relaxed and was very interested in what we were doing, in fact she looks over at one time to watch (which I actually try to prevent and discourage, sometimes even if people feel like they can watch themselves undergo surgery, they may realize that they can't handle it during the moment, i much prefer showing them the video directly after surgery instead of having them watch during!) Anyway, Kristi who was filming couldn't help but react with amazement when this lipoma popped out, and in this case, this was absolutely fine... the patient was just as excited as as you can see she really enjoyed this whole procedure probably just as much as we did! What are the dangerous signs with neck pain? My name is Doctor Dan Albright . I'm an orthopedic spine surgeon in Raleigh North Carolina. So what are the danger signs? What should you worry about that should hustle you over to get to the doctor with a neck problem? The first answer would be weakness. Intense pain shooting from your neck down your arm with weakness. You should see a doctor within a day or two. That's typically a pinched nerve problem, a ruptured disc, herniated disc - same thing. So weakness is the big answer there as far as danger signs. Certainly if your arm or leg is not working right you need to get to the doctor. If you have just a generic general crick in the neck, your neck is aching that's less serious. That's not a danger sign and sometimes this has to evolve. So we all get injured in different ways. Say you are in a car accident that's a natural place to worry. And if, a lot of people get whiplash from that. Neck sprain, neck strain, get an ache in the neck. If it just stays in your neck you are probably fine, but if you've hit your head in the car accident that's another reason to see a doctor. Hitting your head can cause more trauma to the neck. Dr. Ebraheim’s educational animated video describes dorsal and volar ganglion cysts of the wrist. Ganglion of the wrist is the number one soft tissue hand mass. If a mass is present in the eiwrts, there a 70% probability that it is a ganglion cyst. The bump or the cyst is fulled with gelatinous material that can change in size and is close to a tendon sheath or the wrist joint. Causes of cysts are unknown. May be caused by trauma or degeneration and weakness of the capsule. Ganglion cysts of the wrist can be either dorsal or volar. A ganglion custs that grows on the top of the wrist is called a dorsal ganglion. Dorsal ganglion is common (70%) and it arises from the scapholunate ligament. A ganglion cyst that grows in the bottom of the wrist is called a volar ganglion. Volar ganglion is about 20% of the ganglion cyusts and it is most common between the flexor carpi radialis and abductor pollicis longus muscles. Volar cysts arise from the radiocarpal joint of the STT joint. Clinically •Dorsal wrist pain with bump or mas defined, localized, smooth, not attached to the underlying skin and mass is translucent. Mass is obvious with flexion of the wrist and MRI is not needed. •Volar wrist pain MRI may be used with volar ganglion, however ultrasound imaging is better for showing the relationship between the artery and the ganglion. Treatment Dorsal ganglion •Aspiration with 50% recurrence rate. Steroids have questionable value. •Excision : resect the stalk and the capsule. 10% recurrence rate. Volar ganglion •Aspiration : volar ganglion is close to the artery. •Excision of the ganglion if it is painful. 20% recurrence after excision in the volar. •Do Allen test to check the blood flow. The examiner applies pressure with the thumbs occluding the ulnar and radial arteries. The patient is asked to make a fist. The patient is then asked to open his hand. When the examiner releases the ulnar artery, a patent ulnar artery will casue blood flow to the hand. Treatment of pediatric ganglion cysts in pediatrics patients is with observation. Most ganglion cysts will disappear within one year. Occult ganglion presents with dorsal wrist pain but the ganglion is not visibile. MRI may show the ganglion. Ganglion VS tumor Aspiration is better than excision of the cyst with a transverse incision-thinking it is a ganglion. Transverse incision is not a good idea if the cyst turns out to be a synovial cell sarcoma.
