Tramadol vs. Hydrocodone. Overview. Tramadol and hydrocodone are two types of potent pain relievers called opioid analgesics. They’re often used to treat moderate to severe pain, such as long-term pain related to cancer or other chronic conditions. They can also treat pain from an injury or surgery. Hydrocodone is intended for severe chronic pain when nonopioid pain relievers, such as ibuprofen and acetaminophen, haven’t helped. These drugs have a powerful effect on your brain. They’re both highly effective, but they work in different ways. These drugs also come with their own side effects. Here’s what to know about how tramadol and hydrocodone are similar and different. How they work. Both tramadol and hydrocodone attach to receptors in your brain to change your perception of pain. However, tramadol also allows the chemical messengers norepinephrine and serotonin to remain available to your brain for a longer period. It is believed that this helps block pain signals in your spinal cord. Differences at a glance. Tramadol. Generic available—Yes. Brand name—ConZip, Ultram. Forms—Immediate-release tablets and extended-release tablets. Strengths—Immediate release: 50 mg; extended release: 100 mg, 150 mg, 200 mg, 300 mg. Unique side effects: flushing. congestion. sore throat. headache. itching. weakness. seizures. Hydrocodone. Generic available—No Brand name— Zohydro ER, Hysingla ER Forms—Extended-release tablets (Immediate-release hydrocodone is only available in combination products in which it’s combined with other drugs.) Strengths—Extended release: 20–120 mg Unique side effects: confusion. low blood pressure. respiratory depression. gastric obstruction. Side effects Common side effects of both drugs include: dizziness. drowsiness. constipation. loss of appetite. nausea and vomiting. However, tramadol may also cause: flushing. congestion. sore throat. headache. itching. weakness. Most of these mild side effects will resolve within a few days. Serious side effects. More serious side effects of both drugs can include: mood problems. hypersensitivity reactions, including swelling of the tongue or throat, trouble. breathing, and skin rash. Additionally, tramadol can cause seizures. Hydrocodone can also cause: confusion. low blood pressure. respiratory depression. gastric obstruction. Get immediate medical attention or call 911 if you have any serious side effects. from either drug. Risk factors. Certain people may be more at risk of side effects. Side effects of both drugs are more likely or can be more intense if you are a senior. They can also be more intense if you have kidney or liver disease, chronic obstructive pulmonary disease, or other chronic diseases. People with depression who take tramadol may be at increased risk of suicide. Interactions. Tell your doctor and pharmacist about all the medications you take. Many drugs can interact. For example, alcohol and certain over-the-counter drugs that cause sleepiness, including cough or cold formulas, can increase the sedating effect of these drugs. Warnings. Hydrocodone comes with a black box warning about the potential for misuse. According to the Drug Enforcement Administration (DEA), hydrocodone is associated with more misuse than any other opioid. It is possible to develop a tolerance to either drug, especially if you don’t take it exactly as your doctor prescribes it. Building a tolerance to a drug means that the same dosage no longer achieves the same effect. People who develop a tolerance often take more of the drug than recommended to get the same feeling. Tolerance can often lead to dependence. You’re more likely to develop dependence on these drugs if you have a history of drug or alcohol misuse. If you feel yourself becoming dependent, contact your doctor right away. Don’t stop taking the drug, especially if you have taken it for weeks or months. Your doctor will adjust your dosage to slowly taper you off the drug. This helps prevent withdrawal. Tell your doctor right away if you have withdrawal symptoms when you stop taking either of these drugs. Deciding which one to take. Before your doctor can recommend an opioid, it’s important that you discuss all underlying medical conditions. List all your over-the-counter and prescription medications and supplements. Tell your doctor if you’ve ever had a problem with alcohol or substance misuse. Discuss the potential side effects and benefits of these and other opioids with your doctor. ULTRACET Tablet अल्ट्रासेट टैबलेट uses side effect compositon precaution how to use & review ULTRACET® (tramadol hydrochloride/acetaminophen) Tablets combines two analgesics, tramadol 37.5 mg and acetaminophen 325 mg. ULTRACET® is indicated for the short-term (five days or less) management of acute pain. Single Dose Studies for Treatment of Acute Pain In pivotal single-dose studies in acute pain, two tablets of ULTRACET® administered to patients with pain following oral surgical procedures provided greater relief than placebo or either of the individual components given at the same dose. The onset of pain relief after ULTRACET® was faster than tramadol alone. Onset of analgesia occurred in less than one hour. The duration of pain relief after ULTRACET® was longer than acetaminophen alone. Analgesia was generally comparable to that of the comparator, ibuprofen. CONTRAINDICATIONS ULTRACET® should not be administered to patients who have previously demonstrated hypersensitivity to tramadol, acetaminophen, any other component of this product or opioids. ULTRACET® is contraindicated in any situation where opioids are contraindicated, including acute intoxication with any of the following: alcohol, hypnotics, narcotics, centrally acting analgesics, opioids or psychotropic drugs. ULTRACET® may worsen central nervous system and respiratory depression in these patients. WARNINGS Seizure Risk Seizures have been reported in patients receiving tramadol within the recommended dosage range. Spontaneous post-marketing reports indicate that seizure risk is increased with doses of tramadol above the recommended range. Concomitant use of tramadol increases the seizure risk in patients taking: Selective serotonin reuptake inhibitors (SSRI antidepressants or anorectics), Tricyclic antidepressants (TCAs), and other tricyclic compounds (e.g., cyclobenzaprine, promethazine, etc.), or Other opioids. PRECAUTIONS General The recommended dose of ULTRACET® should not be exceeded. Do not co-administer ULTRACET® with other tramadol or acetaminophen-containing products (see WARNINGS, USE WITH OTHER ACETAMINOPHEN-CONTAINING PRODUCTS and RISK OF OVERDOSAGE). Pediatric Use The safety and effectiveness of ULTRACET® has not been studied in the pediatric population. Geriatric Use In general, dose selection for an elderly patient should be cautious, reflecting the greater frequency of decreased hepatic, renal, or cardiac function; of concomitant disease and multiple drug therapy. Acute Abdominal Conditions The administration of ULTRACET® may complicate the clinical assessment of patients with acute abdominal conditions. Use in Renal Disease ULTRACET® has not been studied in patients with impaired renal function. Experience with tramadol suggests that impaired renal function results in a decreased rate and extent of excretion of tramadol and its active metabolite, M1. In patients with creatinine clearances of less than 30 mL/min, it is recommended that the dosing interval of ULTRACET® be increased not to exceed 2 tablets every 12 hours. Use in Hepatic Disease ULTRACET® has not been studied in patients with impaired hepatic function. The use of ULTRACET® in patients with hepatic impairment is not recommended (see WARNINGS, USE WITH ALCOHOL). Information for Patients Patients should be informed that ULTRACET® may cause seizures and/or serotonin syndrome with concomitant use of serotonergic agents (including SSRIs, SNRIs, and triptans) or drugs that significantly reduce the metabolic clearance of tramadol. ULTRACET® may impair mental or physical abilities required for the performance of potentially hazardous tasks such as driving a car or operating machinery. ULTRACET® should not be taken with alcohol containing beverages. The patient should be instructed not to take ULTRACET® in combination with other tramadol or acetaminophen-containing products, including over-the-counter preparations. ULTRACET® should be used with caution when taking medications such as tranquilizers, hypnotics or other opiate containing analgesics. The patient should be instructed to inform the physician if they are pregnant, think they might become pregnant, or are trying to become pregnant (see PRECAUTIONS, LABOR AND DELIVERY). Tramadol is a common analgesic that provides its effects by acting as an opioid and serotonin-norepinephrine reuptake inhibitor (SNRI). The opioid portion of its effects mainly comes from a metabolite, O-Desmethlytramadol (O-DSMT), while tramadol itself has a greater impact on monoaminergic systems. Both the opioid and SNRI effects yield pain relief and mood changes. It does have recreational effects, perhaps more so in those who are efficient at metabolizing tramadol to O-DSMT, but it's generally viewed as less intense than an opioid like morphine or diamorphine (heroin).
