Serotonin Syndrome: Causes, Symptoms, and Diagnosis

11:08
Serotonin Syndrome: Causes, Symptoms, and Diagnosis What is serotonin syndrome? Serotonin syndrome is a potentially serious negative drug reaction. It’s believed to occur when too much serotonin builds up in your body. Nerve cells normally produce serotonin. Serotonin is a neurotransmitter, which is a chemical. It helps regulate: digestion blood flow body temperature breathing It also plays an important role in the proper functioning of nerve and brain cells and is believed to impact mood. If you take different prescribed medications together, you may end up with too much serotonin in your body. The types of medication that could lead to serotonin syndrome include those used to treat depression and migraine headaches, and manage pain. Too much serotonin can cause a variety of mild to severe symptoms. These symptoms can affect the brain, muscles, and other parts of the body. Serotonin syndrome can occur when you start a new medication that interferes with serotonin. It can also occur if you increase the dosage of a medication you’re already taking. The condition is most likely to occur when two or more drugs are taken together. Serotonin syndrome can be fatal if you don’t receive prompt treatment. What are the symptoms of serotonin syndrome? You may have symptoms within minutes or hours of taking a new medication or increasing the dose of an existing medication. The symptoms may include: confusion disorientation irritability anxiety muscle spasms muscle rigidity tremors shivering diarrhea rapid heartbeat, or tachycardia high blood pressure nausea hallucinations overactive reflexes, or hyperreflexia dilated pupils In more severe cases, the symptoms may include: unresponsiveness coma seizures irregular heartbeat What are the causes of serotonin syndrome? Typically, the condition occurs when you combine two or more medications, illicit drugs, or nutritional supplements that increase serotonin levels. For example, you might take medicine to help with a migraine after already taking an antidepressant. Certain types of prescription medications, such as antibiotics, antivirals used to treat HIV and AIDS, and some prescription medications for nausea and pain may also increase serotonin levels. Examples of drugs and supplements associated with serotonin syndrome include: Antidepressants Antidepressants associated with serotonin syndrome include: selective serotonin reuptake inhibitors (SSRIs), such as Celexa and Zoloft serotonin and norepinephrine reuptake inhibitors (SNRIs), such as Effexor tricyclic antidepressants, such as nortriptyline and amitriptyline monoamine oxidase inhibitors (MAOIs), such as Nardil and Marplan certain other antidepressants Migraine medications (triptan category) Migraine medications in a drug category called “triptans” are also associated with serotonin syndrome. These include: almotriptan (Axert) naratriptan (Amerge) sumatriptan (Imitrex) Illegal drugs Certain illegal drugs are associated with serotonin syndrome. These include: LSD ecstasy (MDMA) cocaine amphetamines Herbal supplements Certain herbal supplements are associated with serotonin syndrome. These include: St. John’s wort ginseng Cold and cough medications Certain over-the-counter cold and cough medications that contain dextromethorphan are associated with serotonin syndrome. These include: Robitussin DM Delsym How is serotonin syndrome diagnosed? There’s no specific laboratory test for serotonin syndrome. Your doctor may begin by reviewing your medical history and symptoms. Be sure to tell your doctor if you’re taking any medications or have used illegal drugs in recent weeks. This information can help your doctor make a more accurate diagnosis. Your doctor will usually perform several other tests. These will help your doctor find out if certain organs or body functions have been affected. They can also help your doctor rule out other conditions. Some conditions have similar symptoms to serotonin syndrome. These include infections, drug overdose, and hormonal problems. A condition known as neuroleptic malignant syndrome also has similar symptoms. It’s an adverse reaction to medications used to treat psychotic diseases. Tests your doctor may order include: a complete blood count (CBC) a blood culture thyroid function tests drug screens kidney function tests liver function tests What are the treatments for serotonin syndrome? If you have a very mild case of serotonin syndrome, your doctor may only advise you to immediately stop taking the medication causing the problem. If you have severe symptoms, you’ll need to go to the hospital. At the hospital, your doctor will closely monitor your condition.

Doctors shocked as man Starts bleeding green blood

02:10
Doctors were shocked after seeing a patient that was bleeding green blood, according to reports. A team of Canadian surgeons got a shock when the patient they were operating on began shedding dark greenish-black blood, the local newspaper reported. He emulated science officer Mr. Spock of Star Trek Enterprise, who supposedly had green Vulcan blood. In this case, the unusual blood color of the 42-year-old was the result of the medication he was taking for migraine headaches. The surgery of the man's leg went ahead successfully and his blood returned to normal once the drug had tapered off. The patient had been taking large doses of sumatriptan, 200 milligrams a day. This had caused a rare condition called sulfhaemoglobinaemia, wherein sulfur is incorporated in the hemoglobin oxygen transport compound in the red blood cells. In describing the case to the local newspaper, doctors, led by Dr. Alana Flexman of St. Paul's Hospital in Vancouver wrote: "The patient recovered uneventfully and stopped taking sumatriptan after discharge. "When seen five weeks after the last dose, he was found to have no sulfhaemoglobin in his blood." The man had needed urgent surgery because he had developed a dangerous condition in the legs after falling asleep in a sitting position. The surgeons performed urgent fasciotomies, a limb saving procedure involving surgical incisions to relieve pressure and swelling caused by the condition known as compartment syndrome. In compartment syndrome, inflammation and pressure in a confined space limits the flow of blood to tissue that causes localized damage to the nerves. It is commonly caused by trauma, internal bleeding or a wound dressing or cast being too tight.

Ketosis and Fasting: Do BCAAs Break a Fast or Ketosis: Thomas DeLauer

Simply put, a ketogenic diet results in your body using and burning fat, rather than carbs, for fuel (energy) BCAA’s Overview Amino acids are the building blocks of protein. Branched chain amino acids (BCAAs) are so called because of their structure, which includes a “side chain” of one carbon atom and three hydrogen atoms. BCAAs are the only amino acids not degraded in the liver. All other amino acids are regulated by the gut and the liver before being circulated elsewhere in the body. This means that dietary intake of BCAAs directly influences plasma levels and concentrations in muscle tissue (Layman DK 2003). Interestingly, BCAAs are burned for energy (oxidized) during exercise, so they’re also an important exercise fuel. So this poses the question…. If it can be used as fuel, can it break a fast? BCAAs, however, head directly into the bloodstream. This means that dietary intake of BCAAs directly influences plasma levels and concentrations in muscle tissue There are three BCAAs: leucine, isoleucine, and valine BCAA supplementation may reduce muscle soreness, accelerate recovery, promotes muscle protein synthesis and supports fat loss (1,2) Ketosis and Insulin Insulin’s primary purpose is to regulate the metabolism of fats and carbohydrates. The digestive system breaks down carbohydrates, such as sugars and starches, into a molecule called glucose. Insulin allows cells in the body absorb glucose so your body can use it for fuel, ultimately lowering levels of glucose in the blood stream Ketosis and Insulin Ketosis will take place when the body needs energy and there is not sufficient glucose available for the body. This happens when the body is lacking insulin and blood glucose levels become high. BCAAs can Spike Insulin Studies have shown that branched-chain amino acid supplementation increases insulin levels and concentrations of FFAs Leucine, specifically, stimulates glucose uptake for up to 45 minutes (3,4) Insulin and Ketosis Study Despite there being some benefits to supplementing BCAAs, it may not be wise to do so while in a ketogenic state In one study, a man voluntarily fasted for 50 days. His ketones were high (14 mmol/l) and he was injected with insulin (around 0.1 IU/kg, so around 7-8 IU). For 30 min, his ketones and blood sugar dropped and there was a massive uptake of ketones by the brain and ketone production was halted. However, within 30 min his ketones started shooting back up In short, an insulin spike can kick you out of ketosis, albeit temporarily, as insulin clearance is seemingly fast (5) BCAAs While in Ketosis (Increase in circulating BCAAs) In one study dietary replacement of carbohydrate by fats were studied in six healthy subjects, which was preceded and followed by a 3-day period of balanced diet. During the ketogenic diet, levels of free fatty acids, glycerol and 3-hydroxybutyrate rose significantly and glucose levels decreased by 16.5 +/- 3.2% The hormonal pattern switched towards a catabolic mode with a fall in insulin levels (-44.0 +/- 6.3%) and a rise in glucagon concentration (+39.0 +/- 10.4%). https://www.ncbi.nlm.nih.gov/pubmed/6761185 The average levels of gluconeogenic amino acids (alanine, glutamine, glycine, serine and threonine) were reduced by 8-34% while those of the branched chain amino acids increased by more than 50%. Concluded that a ketogenic diet may result in an increase of circulating BCAAs within the body - so supplementing with BCAAs may be even less necessary while in ketosis (6) References 1) All About BCAAs | Precision Nutrition.
