Doctors say the treatment marks the first time a T-cell immunotherapy application for late-stage breast cancer was successful. The results were published in the journal Nature Medicine. Judy Perkins, a 49-year-old engineer, from Florida was selected for the new therapy after chemotherapy failed to stop the tumor in her right breast from spreading to her liver and other areas, according to the Guardian. She was told she only had three years to live. Doctors from the National Cancer Institute in Maryland first took biopsies from Perkins' tumors, looked at the DNA mutations specific to her cancer. Physicians then extracted immune cells called tumor infiltrating lymphocytes, TILs, from the biopsies. Billions of these immune cells were then grown in the lab. The researchers then injected 80 billion of the TILs into Perkins' body. The treatment was given in tandem with pembrolizumab, a standard cancer drug that helps the immune system. After 42 weeks, tests showed Perkins was completely free of cancer. According to the Guardian, researchers are now planning full scale clinical trials. RUNDOWN SHOWS: 1. IKK Epsilon in T-Cell Anti-Tumour Immunity in Breast Cancer

The majority of deaths from cancer following conventional therapies are a result of metastases. The IKK subunit Epsilon is a key protein in NFkB signalling pathway and it has been found to play a key role in signal transduction in mammalian cells. Several members of the IKK family have been discovered but IKK-Epsilon has been identified as a breast cancer oncogene. Mr Bishop's PhD and Miss Gkanatsiou‘s MSc work will attempt to establish whether pharmacological inhibition of IKK- Epsilon enhances and restores T-cell activation and anti-tumour activity against breast cancer cells. This addresses a huge unmet clinical need, as metastasis and acquired resistance to conventional anti-cancer therapies are major clinical problems in advanced breast cancer patients.

ecancer reporter Peter Goodwin talks to Dr Willard-Gallo at the 2013 IMPAKT conference in Brussels about IDing immune response in breast cancer, helping to asses prognosis. CD4 T cells are critical regulators of immune responses but their functional role in breast cancer is relatively unknown. The goal of the study was to produce an image of CD4 T cells infiltrating breast tumors (TIL) using limited ex vivo manipulation to better understand the in vivo differences associated with patient prognosis. The identification of CD4 T cells as a new key immune element in breast cancer, linked with organized immunity and associated with a higher response rate to chemotherapy and/or excellent long-term clinical outcome, suggests that their presence in some tumours is an important prognostic factor.

Fight cancer, but how? Chemotherapy kills cancer cells but also healthy cells. The use of our immune cells (T-cells) could allow a more targeted attack of cancer cells but are limited. Genetic engineering makes it possible to transform our T-cells into an army of super-killer cells (CAR-T) which reinjected can destroy a tumor. Clinical trials have shown that, but this is just the beginning of a new era where reprogrammed cells can become super sophisticated drugs. A medical revolution begins! André Choulika, Ph.D., is one of the founders of Cellectis and served as Chief Executive Officer since the company's inception in 1999. He is Chairman of the Board of Directors since 2011 and President of Calyxt since August 2010. From 1997 to 1999, Dr. Choulika worked as a post-doctoral fellow in the Division of Molecular Medicine at Boston Children’s Hospital, where he was one of the inventors of nuclease-based genome editing technologies and a pioneer in the analysis and use of meganucleases to modify complex genomes. After receiving his Ph.D. in molecular virology from the University of Paris VI (Pierre et Marie Curie), he completed a research fellowship in the Harvard Medical School Department of Genetics. His management training is from the HEC (Challenge +). This talk was given at a TEDx event using the TED conference format but independently organized by a local community. But new research from Stanford reveals that breast cancers can use physical extensions to tunnel through the body. The main premise is stimulating the patient's immune system to attack the malignant tumor cells that are responsible for the disease. This can be either through immunization of the patient (e.g., by administering a cancer vaccine, such as Dendreon's Provenge), in which case the patient's own immune system is trained to recognize tumor cells as targets to be destroyed, or through the administration of therapeutic antibodies as drugs, in which case the patient's immune system is recruited to destroy tumor cells by the therapeutic antibodies. Cell based immunotherapy is another major entity of cancer immunotherapy. This involves immune cells such as the Natural killer Cells (NK cells), Lymphokine Activated killer cell(LAK), Cytotoxic T Lymphocytes(CTLs), Dendritic Cells (DC), etc., which are either activated in vivo by administering certain cytokines such as Interleukins or they are isolated, enriched and transfused to the patient to fight against cancer. Since the immune system responds to the environmental factors it encounters on the basis of discrimination between self and non-self, many kinds of tumor cells that arise as a result of the onset of cancer are more or less tolerated by the patient's own immune system since the tumor cells are essentially the patient's own cells that are growing, dividing and spreading without proper regulatory control. In spite of this fact, however, many kinds of tumor cells display unusual antigens that are either inappropriate for the cell type and/or its environment, or are only normally present during the organisms' development (e.g. fetal antigens). Examples of such antigens include the glycosphingolipid GD2, a disialoganglioside that is normally only expressed at a significant level on the outer surface membranes of neuronal cells, where its exposure to the immune system is limited by the blood--brain barrier. GD2 is expressed on the surfaces of a wide range of tumor cells including neuroblastoma, medulloblastomas, astrocytomas, melanomas, small-cell lung cancer, osteosarcomas and other soft tissue sarcomas. GD2 is thus a convenient tumor-specific target for immunotherapies. Other kinds of tumor cells display cell surface receptors that are rare or absent on the surfaces of healthy cells, and which are responsible for activating cellular signal transduction pathways that cause the unregulated growth and division of the tumor cell. Examples include ErbB2, a constitutively active cell surface receptor that is produced at abnormally high levels on the surface of breast cancer tumor cells. The treatment, which succeeded after all other conventional treatments had failed, marks the first successful application of T-cell immunotherapy for late-stage breast cancer. While the technique is still in its early days, scientists have welcomed its potential as a future treatment for cancers that have resisted all other forms of therapy. Judy Perkins, a 49-year-old engineer from Florida, was selected to undergo the cutting edge treatment after several chemotherapy sessions had failed to kill the tumour, which had begun to spread to her liver. Before the new treatment, doctors had given her three years to live. She was treated by a team led by Dr Steven Rosenberg at the National Cancer Institute (NCI) in Maryland. The clinical trial, which is still ongoing, used modified T-cells – which make up part the body’s immune response – to tackle the patient’s tumours. Doctors caring for Ms Perkins during her recovery described her return to health as "remarkable". She has now been entirely free of cancer for two years. Immunotherapy, which involves stimulating the body’s natural defences to fight cancer, is already being used to tackle certain cancers, and some forms are already available on the NHS. However, the response rate to even the most successful treatments is relatively low, with one recently trialled therapy showing strong effects in only around 10 per cent of prostate cancer patients. Previous clinical trials using immunotherapy to treat breast cancer have proved largely ineffective. The new approach pioneered by Dr Rosenberg and his team was based on an existing technique called adoptive cell transfer that has proved effective when treating melanoma, but not other forms of the disease. Cancer specialists have described the news as significant evidence that this new approach to immunotherapy could succeed where others have failed. “This research is experimental right now,” said Dr Rosenberg. “But because this new approach to immunotherapy is dependent on mutations, not on cancer type, it is in a sense a blueprint we can use for the treatment of many types of cancer.” The NCI team’s method involved taking T-cells that specifically target cell mutations within patients’ tumours. These cells were then grown in large numbers in the lab and infused back into the patient. To treat the patient in this trial, Dr Rosenberg and his colleagues sequenced genetic material from one of her tumours to identify mutations that were specific to her case. Immunotherapy, where the immune system is harnessed to treat and/or prevent disease, holds great promise in the fight against cancer. One noteworthy facet of cancer immunotherapy is the development of cancer-specific vaccines. In this webinar Dr. Keith L. Knutson from the Mayo Clinic (Jacksonville, FL) and VGTI discusses the use of ACEA’s xCELLigence® system in his efforts to discover a novel epitope for use in breast cancer vaccination. Specific topics covered include: • The concepts of cancer vaccine and immune therapy • Development of an impedance-based assay using the xCELLigence® system to monitor cytotoxic T-cell activity • The advantages of dynamic monitoring of cytotoxic T-cell activity using impedance-based assays vs. the traditional endpoint Chromium-51 Release Assay (CRA) Dr. Keith L. Knutson is currently the director of the Cancer Vaccine and Immune Therapies program at VGTI and is an affiliate associate professor in the Department of Immunology at the Mayo Clinic. He received his Ph.D. in physiology and pharmacology from the University of Georgia, Athens in 1995, and completed two postdoctoral fellowships in immunology at the University of British Columbia and the University of Washington. He was a 2004 recipient of the Howard Temin Award from the National Cancer Institute. Dr. Knutson’s current research focuses on the immunology and immunotherapy of breast and ovarian cancers. His interests are in both the basic immunobiology and clinical translation, including clinical trials. His laboratory is currently conducting research on cancer vaccines that augment CD4 helper T-cell immunity using peptide epitopes. These vaccine strategies are aimed at preventing patient relapse after conventional chemotherapies. Adoptive T-cell therapy, a strategy that involves infusing high numbers of T-cells into patients with active bulky malignancy, is also being examined in Knutson’s lab. Specific adoptive T-cell therapy projects include methods for T-cell expansion and in vitro generation of memory T-cells. Lastly, Dr. Knutson’s lab also seeks to understand how tumors evade the immune system and immune therapies. Several different mechanisms of immune escape are being investigated, including cellular reprogramming (e.g., epithelial to mesenchymal transition) and recruitment of regulatory T-cells and myeloid-derived suppressor cells into the tumor microenvironment. In addition to his research, Dr. Knutson participates in and chairs several study sections. He is currently a full member of the Department of Defense's Breast Cancer Research Program Integration Panel and a full member of the Tumor Microenvironment Study Section at the National Institutes of Health Center for Scientific Review. (CNN) Doctors are sounding the alarm about a possible link between breast cancer and cell phones. Some experts say they're seeing evidence of breast cancer that could be connected to where some women keep their mobile phones. It's convenient, say many young women about putting their cell phone in their bra. Maybe they should talk to Tiffany Frantz. Tiffany got breast cancer at 21. "I put my cell phone right in my bra," Tiffany said. Her mother had some doubts. "We never took it seriously until after she was diagnosed," said Traci Frantz, Tiffany's mother. "It just so happened her tumors were exactly where her cell phone had been against her skin her bare skin for about six years." With no genetic or other risk factors, surgeons removed Tiffany's left breast. "It's kind of coincidental that it's right where I kept my cellphone," said Tiffany. Coincidence? Donna Jaynes also got breast cancer at 39 --also no risk factors. Her doctor said it was an unusual distribution exactly matching her cell phone. The tumors were just under the surface of her skin. "All in this area right here, which is where I tucked my cellphone," Donna Jaynes said. She says she did that for ten years. She had to have a mastectomy. "Yeah, I thought cell phones were safe," said Jaynes. Breast cancer specialists are now sounding a warning. They say men are also getting breast cancer under cell phones in shirt pockets. The wireless industry denies any problem --citing a lack of scientific evidence that cell phones cause breast cancer." There's no evidence, but that's because we haven't studied it," breast surgeon Dr. Lisa Bailey said. "Until further data either supports it or disproves it, I would keep cell phones away from the body, in particular the breasts," breast cancer radiologist Dr. June Chen said. Immunotherapy Combination in HER2-Positive Breast Cancer

Kevin Kalinsky, MD, MS, assistant professor of medicine at Columbia University, discusses the results from a trial in HER2-positive breast cancer that was was presented in December 2017. The study looked at the use of trastuzumab (Herceptin) in combination with a checkpoint inhibitor in patients that were either PD-L1 positive or negative. In patients that were PD-L1 positive, there was a significant difference in overall survival compared to PD-L1 negative patients. Kalinksky says that this poses the question of whether there is a small amount of patients that really respond to immunotherapy in this population. Kalinsky also noted that it was interesting to see higher rates of stomal tumor-infiltrating lymphocytes in the PD-L1 positive cohort, where these lymphocytes seemed to be able to dichotomize those would would respond. While the focus in breast cancer research has been primarily triple-negative disease, there are some ongoing trials like this one evaluating immunotherapeutic strategies in HER2-positive breast cancer. As is common with breast cancer, Judy Perkins had recovered from a primary malignancy only to have her cancer return, more aggressively, 10 years later. After she failed to respond to multiple therapies, Perkins, riddled with tennis-ball-sized lesions throughout her body and facing a terminal diagnosis, enrolled in a National Institutes of Health (NIH) trial aimed at adapting a well-known approach, called autologous tumor-infiltrating lymphocytes (TILs), to new cancers. This tailored treatment paradigm consists of extracting immune cells, typically T-cells, from within or nearby each patient’s tumor(s). These activated cells are trying to identify and destroy cancerous cells but are ineffective against the disease on their own. So, using several methods of genetic and protein analysis, clinicians sort through the sample seeking cells that have programmed themselves to recognize mutated proteins that are made by cancerous cells but not by healthy cells. Infusing large cultures of these targeted TILs back into the patient has been shown to be quite successful at treating several types of cancer wherein the cells bear high numbers of mutated proteins. Many of the most common cancers do not fall into this category. Yet thanks to recent advances in DNA and RNA sequencing and the aforementioned sorting techniques, it has become easier for researchers to both examine the entire genome of tumor cells and find TILs capable of recognizing the small handful of tumor-specific proteins present, spurring a new wave of clinical trials such as the ongoing one at the NIH. According to NPR, the team has treated 45 total patients with a variety of stage IV metastatic cancers. Seven have responded thus far. The case of patient 4136 – aka Ms Perkins – represents a breakthrough. As described in their report and an accompanying editorial in the journal Nature, her tumor-infiltrating T-cell population contained a line that was reactive against just four mutant proteins produced by her breast cancer cells. Like other TIL protocols, Perkins’ other white blood cells were knocked out prior to the infusion and she received a high dose of a signaling molecule called interleukin 2 to encourage the special TIL line to take over and expand throughout her body. Chemo and immunotherapy together may help triple-negative breast cancer patients
