Inadequate inhaler technique lowers drug deposition to the lungs, wastes medication and may lead to poor disease control, reduced quality of life, increased emergency hospital admissions and higher treatment costs. COMMON INHALER TECHNIQUE MISTAKES: • Not breathing out first. When you breathe out fully (or as much as you comfortably can) just before taking your inhaler, you create more space in your airways for your next breath in. This means that you can breathe in deeper and for longer when you inhale your asthma medicine - giving it the best chance of reaching the small airways deep inside your lungs, and being most effective. • Not holding your breath after taking your inhaler. If you've been advised to hold your breath after taking in your inhaler, it's very important to do so. When you hold your breath after inhaling the medicine, you are keeping your airways still. This gives more time for the medicine to get into your lungs. If you can hold your breath for 10 seconds, this is ideal but if this isn't possible, you will still benefit by holding it for as long as you feel comfortable. • Not priming the aerosol inhaler device Aerosol inhalers require priming (so you get the right amount of medicine when you use it) before using for the first time, or if they have not been used for a while - always refer to information leaflet. • Not shaking your MDI before use and between puffs. If you don't shake the canister, the asthma medicine and propellant (the substance which helps turn the medicine into aerosol form) will not mix together properly and too much or too little of one will be released. • Inhaling too early before pressing the canister. If you're already half way through breathing in by the time the medicine is released from the inhaler, you won't have enough time to finish breathing in all the medicine because your lungs will already be full. If this happens, some of the medicine will end up being sprayed in your mouth and hitting the back of your throat and not being carried down to your lungs where it's needed. • Inhaling too late after pressing the canister (unless you're using a spacer). It takes less than half a second from the time the canister is pressed for all the medicine inside the inhaler to be released. If you breathe in after this time, some of the medicine will end up being sprayed in your mouth and not to your lungs where it's needed. • Not leaving enough time between doses. You need to give your inhaler a good shake between doses and then wait at least 30 to 60 seconds before taking the next puff. There are many treatment options for children with asthma, largely depending on the age and preference of the child. Metered dose inhalers, which are also commonly called puffers or inhalers, spray the medicine out so that you can breathe it deep into the lungs. Inhalers deliver medicine very quickly and are compact and light so they can easily be taken with you wherever you go. It's very important to use your inhaler the correct way to make sure the medicine gets deposited in the lungs, where it works the best. Incorrect technique can leave some of the particles from the medicine on your tongue or throat, where it won't help at all. A spacer, or holding chamber, is an attachment that should always be used with your inhaler. The spacer holds the medicine in place so you can breathe it in easier. Spacers with a mask on one end are used for smaller kids and babies. First, remove the cap from the bottom of your inhaler. When you get your new inhaler from the pharmacy, you need to "prime" or "waste" it by spraying the medicine into the air four times. Now, fit your inhaler into the opening at the end of the spacer. Shake the inhaler well for 10 seconds. Place mask tightly over the nose and mouth, then push down on the inhaler once. Now the medicine is inside the spacer. Breathe in and out six times through the mask. Rinse your mouth out with water or brush your teeth after using controller medicines such as inhaled steroids. Many times, the correct dose of your medicine will be to inhale two puffs. If this is what is prescribed for you, then wait one minute after the first puff, and then follow all of the steps again Clean the plastic holder of the inhaler each week. Clean the spacer and mask each week by soaking in warm soapy water for 15 minutes and run water through it, shake off water and set overnight to dry. Inhalers are the main source of treatment for asthmatic patients and enables them to lead active lives without the fear of an attack. This article will give some more information about the inhalers for asthma. TYPES Metered Dose Inhalers(MDIs) They utilize a chemical propellant to propel an evaluated dose of medication. They comprise a mouthpiece, a pressurized canister having medication, and a metering valve, which dispenses the right dose of medication. The medicine is released either by inhaling or squeezing the canister. Some of them have counters to know how many doses are remaining. If there is no counter, the patient needs to keep a track of the number of doses used in order to know when the device would be out of medication. The chemical propellant commonly used is chlorofluorocarbon (CFC). But since it damages the ozone layer, other propellants like hydrofluoroalkane (HFA) are now been used. The dose of medicine released by the ones containing HFA is more soft, warm, and reaches directly to the lungs. Dry Powder Inhalers (DPIs) They do not have a gas propellant to propel the drug out of a canister. Each dose contains a small amount of drug in a powder form, which the patient has to suck in. The patient has to breathe in very hard to get the powder into his lungs. Most adults and older children find it easy to operate, but young children may find it difficult to breathe in so hard and suck in the powder. They may be difficult to use, especially during an asthma attack, as during such times, it is hard to catch a deep breath anyway. On the other hand, some find them easier as compared to the previous one, as the hand-lung coordination is not needed. They are categorized as powder disk, dry powder tube, and single-dose dry powder disk inhaler. Spacers are not required to be used with them. Spacers A spacer is a tube, which is attached to the inhaler and holds the medicine until the patient doesn't breathe in completely. It gives more time to inhale slowly, and decreases the amount of medication that deposits on the back of the patient's throat and tongue. It makes the use of the device simpler, and helps in depositing the medicine into the lungs more efficiently. In this, the hand-breath coordination is not important, and deep and fast breathing is not required. This type requires regular cleansing with soap and water. This equipment has transformed the treatment of asthma and other lung diseases. They are the most effective way of providing life-saving medication to the patients. Although there are hardly any side effects, it is always advisable to consult a doctor before using any type of inhaler. Enviro Wacks you KNOW this isn't true, but nontheless push for these chemicals to be banned. Why not ban the chemicals from applications that are proven to cause problems? The new inhalers designed to satisfy the wacks are 4 times more expensive, half the size and require twice as much to achieve the same effect. This is taking something that has worked for decades and reducing it's effectiveness to 1/16 of what is was both in cost and relief. How to Use an Inhaler 1. Shake the inhaler well before use (3 or 4 shakes). 2. Remove the cap. 3. Breathe out, away from your inhaler. 4. Bring the inhaler to your mouth. Place it in your mouth between your teeth and close your lips around it. 5. Press teh top of your inahler once. Immediately take a slow deep breath, pulling the aerosol deep into the lungs. 6. Remove the inhaler from your mouth, close your lips, hold your breath for about 10 seconds, then breathe out. 7. Metered Dose Inhaler (Asmol/Ventolin/Airomir/Qvar/Atrovent) [Without a Spacer] 1) Shake well before use 2) Tale a deep breath in and out 3) Form a seal around puffer with your lips and inhale at the same time as taking a puff 4) Hold your breath for 10 seconds or as long as you feel comfortable [With a spacer] 1) Shake well before use 2) Prime spacer (if using for the first time) by puffing 1 puff into the chamber. 3) Puff 1 dose into the chamber, make a seal around spacer nozzle with your lips and breath in and out 4 times (the spacer has a special 1-way valve so you can breathe directly back into the spacer without taking it away from your mouth). 5) Repeat step 4) if needed for another dose. 6) Once a month wash your spacer in warm soapy water and let it air dry (don't dry it with a towel).
