If you decide to appeal an insurance decision, you must appeal to the insurance company. 2. Medicare Advantage plans are typically HMOs or PPOs and on occasion a Private-Fee-for-Service plan. HMO stands for Health Maintenance Organization. PPO stands for Preferred Provider Organization. We will talk more about both of those in a minute. What is important to understand is that Medicare Advantage plans are not standardized, they are regulated to meet a certain minimum coverage. That is an important concept, so let me explain further. Medicare Part A & B is standardized. Medicare Supplement plans are standardized. Everyone in those plans receives the exact same benefits, they can see the exact same doctors and should have the exact same experience. With Medicare Advantage plans, each plan has different benefits, different co-pays, and deductibles, covers different medical services and has a different network of doctors and hospitals. Everyone’s experience is different and depends on which plan they have chosen. In additional; all the benefits, co-pays, deductibles and the doctors and hospitals in the network can and will change each and every year. It will be your responsibility to keep up with the changes via their mandatory Annual Notice of Change. It’s your responsibility to study this annual notice and determine how the changes may impact your healthcare. Medicare Advantage plans are regulated to meet a certain minimum standard. That minimum standard dictates that they must be actuarially equivalent to Medicare Parts A and Part B. That’s an interesting term “actuarially equivalent”. What does that mean? This is another one of the most common questions that we get here at Medicare hero is how do I decide which type of coverage is really best for me. Medicare Supplement: So first let's talk about Medicare supplement plans. The monthly premiums for Medicare supplement plans range from about $85 to $170 dollars a month, actually for people turning 65 I see an average of about $100 to $120 per month for their coverage. The premiums you pay all depends on your age, gender, and where you live. With a Medicare Supplemental plans we can see any doctor or any Hospital in the entire country that accepts Medicare there's absolutely no networks, and when we go to see those doctors there's no co-pays, no deductibles, and no out-of-pocket expenses. All we do is pay our monthly Insurance premiums and we're done. Additionally with a Medicare supplement plan you never need a referral to see a specialist, and again it doesn't matter which insurance company you select such as AARP United Healthcare, Humana, Aetan, Cigna, Mutual of Omaha, or New Era, you can always go see a specialist without a referral when you have a Medigap Plan. Now the last part of Medicare I want to talk about is prescription drug coverage, Part D of Medicare. Medicare Advantage: Medicare Advantage plans were created as a low cost alternative to Original Medicare and Medicare supplement plans. When I say lower-cost what I mean is lower monthly premiums. So the premiums for Medicare advantage plans can be anywhere from $0 in addition to what you're already paying for part b of Medicare up to $60 or $70 and premiums can even be as high as a hundred dollars a month. The key with Medicare Advantage plans is that we want to keep our premiums low because that is the advantage is low monthly premiums. So one of the compromises that Medicare Advantage plans make in order to get those low monthly premiums is they have network so you want to make sure that your doctors and hospitals that you choose to use are in the network for the insurance company you're looking at. This means that Medicare insurance companies such as AARP United Healthcare will have a different network then Humana, Aetna, or bluecross blueshield. So just be careful make sure your doctors, specialists and hospitals are in network. Also your Networks and doctors can change mid-year but we can't necessarily change our Part C coverage. Additionally because we now get those low monthly premiums we now have to pay deductibles co-pays and other charges when we use our insurance coverage. So if you see a specialist you might have a small copay, if you go to the hospital you will have to pay a certain amount of pocket. So we get low monthly premiums with Medicare Advantage plans but then we have to spend money the more we use our healthcare insurance. Additionally Many of the most affordable Part C Medicare Advantage plans do require referrals to see a specialist. Now that we've had all the key points both Medicare advantage and Medicare supplement plans what's the bottom line? Well Medicare supplement plans are best for people who can afford the extra premiums and if you most value your choice of doctors and hospitals as well as a predictable healthcare budget where you're not going to have large healthcare expenses throughout the year. You know if you just pay your monthly premiums you're going to be set. Medicare Advantage plans on the other hand are simply the best if you're looking for low monthly premiums. Both Anthem and Aetna decided to significantly limit your choice of doctors and hospitals. Usually only leaving one major hospital network to choose from in your area. If they happened to pick your prefered hospital system, that might work. If not, than you might have some problems. You don't want to have to try and squint really hard to see a scenario where one of their plans make sense. The biggest risk you run with limited choice is not being able to see a top special if needed. As you'll see in the next section, if this could save you money, I might allow it. But, nine time out of 10 it's just not in the cards. Anthem and Aetna Pricing and Cost This is where things turn into a battle for last place. Both Anthem and Aetna have had little interest in offering competitive rates. There are companies like Medical Mutual and even InHealth Mutual that are cheaper. Also, don’t forget the network limitations both companies have. This has been the case for the last two years, with a few exceptions popping up here and there. Sometimes I would run a quote for a client and Anthem and Aetna would be in the ballpark, but rarely cheaper. If no major adjustments are made by either company in 2016 you can expect this trend to continue. The rate for 2016 should start to show up at the start of open enrollment on November 15.

Aetna to buy Humana as health insurance landscape shifts

(AP) -- Aetna aims to spend about 5 billion to buy rival Humana and become the latest health insurer bulking up on government business as the industry adjusts to the federal health care overhaul. The proposed cash-and-stock deal, announced early Friday, would make Aetna a sizeable player in the rapidly growing Medicare Advantage business, which offers privately run versions of the federally funded health care program for the elderly and some people with disabilities. The combination also would bolster Aetna's presence in the state- and federally funded Medicaid program and Tricare coverage for military personnel and their families. Health insurers are eager to do more business with government payers due in part to a Medicaid expansion fostered by the health care overhaul and Medicare Advantage's surging enrollment. The overhaul is expanding Medicaid coverage in several states as it seeks to provide health coverage for millions of uninsured people. Meanwhile, total enrollment in Medicare Advantage plans has tripled over the past decade to about 16.8 million people and is expected to keep growing as more baby boomers become eligible for the plans. Aetna's acquisition of Humana would make it the largest provider of Medicare Advantage coverage, with 4.4 million members, a figure that could change depending on regulatory review. "Government markets are the most rapidly growing aspect of the system," said Dan Mendelson, CEO of the market research firm Avalere Health. Hartford, Conn.-based Aetna announced its deal a day after the Medicaid coverage provider Centene Corp. said it would spend .3 billion to buy fellow insurer Health Net. That deal would help Centene expand in the nation's biggest Medicaid market, California, and give it a Medicare presence in several western states. In addition to these deals, the Blue Cross-Blue Shield carrier Anthem went public late last month with an offer of more than 7 billion for another insurer, Cigna. Health insurers see more advantages to these big combinations than a chance to build their government portfolios. Major acquisitions can offer an infusion of new business at time when growth has slowed in the biggest part of their business, employer-sponsored health coverage. Plus more employers are opting to pay their own insurance claims and hire insurers to administer the coverage. That's a less lucrative line of work for managed care companies. Big deals also allow companies to quickly diversify their products and cover more territory. They also can yield savings when the companies combine back-office functions and cut overlapping jobs. Both Aetna and Anthem also have cited the potential to improve their technology as a major reason behind their deals. Insurers are working to develop more apps and other tools that customers can use to shop for health care, since plans are exposing those customers to bigger medical bills through high deductibles and other insurance expenses. They also are using technology more to help monitor and improve patient care. The overhaul is accelerating a push in the industry to reimburse doctors and hospitals more based on the quality of care they provide instead of just shelling out a certain amount for each procedure performed. "Any time an industry is changing ... it requires investments to sort of successfully make that change," said Shawn Guertin, Aetna's chief financial officer. The impact these big acquisitions have on consumers can be murky and likely won't be felt for at least a year, because insurers have already finalized most of their plans for coverage that starts in January. A combination may lead to fewer choices and some price changes for consumers, depending on where they live and who already is in their market. Aetna's purchase price for Humana includes a combination of 25 in cash and 05.11 in Aetna shares for each Humana share. The total of about 30 per share, which is based on the closing price of Aetna's stock Thursday, represents a premium of 29 percent to Humana's trading price in late May, before The Wall Street Journal reported that it was an acquisition target. The deal's total value amounts to about 7 billion counting debt. The combined company would be based in Hartford, Conn., led by Aetna Chairman and CEO Mark Bertolini and cover more than 33 million people. On Monday, a U.S. federal judge blocked health insurer Aetna Inc's proposed $34 billion merger with rival Humana. One key factor in comparing Medicare plans is to look for coverages in plans where your out of pocket costs are limited to a specific dollar amount. This makes it easier to determine what you will be paying for services. It is much harder to estimate costs if your out of pocket portion is determined by a percentage of the total Medicare approved amount. Monthly cost is the fourth thing we look at. A very important question is answered here, which is: Are Medicare Supplement Plans Worth It or Should I Choose Advantage? Depending on your county the answer to this question can be very different. Some parts of the country offer very strong Advantage Plans. So in this video, I'm leaving those areas out and really speaking for the majority of the country, when I give my opinion here, which is: If a 64 year old requests my advice, I would tell them to take a Medicare Supplement Plan if they could afford it. My reason is simply this: The coverage is a lot better. Let's be honest- without your health, what do you have? Not a whole lot. So get the best if you can. Make it simple on yourself. One side note- I also like the fact that you have foreign coverage with Supplements, should you like to travel. In 2014, I started doing this after learned that seniors are often offered only one choice in Medicare, depending on where the agent has his loyalty. This is especially true in states that have specific laws reducing the value of Plan G. In addition, some insurance companies are preparing for Medicare Supplement Plan D to be one of the top selling plans in Medicare. To be clear, I am referring to the Medicare Supplement Plan D and NOT Medicare Part D prescription drugs. Medicare supplement Plan D is a direct competitor to Medicare supplement Plan N. Is Medicare Supplement Plan N the right plan for you? Let’s take a look. When comparing Medigap Plan N vs. Plan G, people that prefer the Plan G to the Plan N tend to be people who would rather pay a little extra each month and avoid having to do the extra work to avoid excess charges. The plans are identified by letter across the top row. The category of Medicare services are in the left hand column. In the column below the Plan letter you can see the percentage of insurance coverage offered by that plan. You can find this table on our website and in various Medicare publications. With Medigap Plan N we see that it covers 100% “Medicare Part A coinsurance and hospital costs (up to an additional 365 days after Medicare benefits are used up.). Notice that all the supplements have this coverage. Also, notice down here that Medicare Supplement Plan N pays the Medicare Part A deductible. In 2017 that deductible is $1,380 per event. It will change every year, but with a Medigap Plan N that doesn’t matter. Your supplement will pay it. Without a supplement you pay the deductible up front, then have 60-days of hospital coverage before you start paying a copay. With Medicare supplement Plan N you can be an inpatient in the hospital for more than 365 days with 100% coverage. You will have no out of pocket costs for Part A. The asterisks indicate that you have the Medicare supplement Plan N copay. You will be required to pay up to a $20 co-pay for every doctor visit and a $50 copay for every emergency room visit unless you end up an inpatient in the hospital. Then the $50 copay is waived. The important qualifying words here are “up to”…$20. Here is what you can expect. So notice that with Medicare supplement Plan N you have 100% coverage until you get down to this line item referring to the Medicare Part B deductible and Medicare Part B excess charges. Here Medigap Plan N has no coverage at all. You are expected to pay those fees out of pocket. That’s important. Later in this video I am going to go over a real life example using a common joint replacement that will clear up some of the most common questions and misunderstandings about Medigap Plan N. In order to evaluate if Medicare supplement Plan N is right for you, you need to know what the Medicare Part B deductible is and what it is expected to be in the future. It’s also critical that you understand Medicare Part B excess charges. So let’s focus on these and then go over a real life example to show you how Medicare supplement Plan N works with office visits, hospital stays and physical therapy. If a doctor or hospital does not accept Medicare assigned rates, they are paid 5% less from Medicare, but allowed to charge you 15% more. The amount the doctor or hospital charges you that is more than what Medicare pays the doctor for that service is called an excess charge. Excess charges are only allowed on Medicare Part B services. Those are outpatient and physician services. There are also some states that either forbid or severely restrict Medicare Part B excess charges. These state laws are called Medicare Override Measure or MOM laws.
