Bronchospasm, due to asthma, is caused by 3 factors: - Swelling of cells in the lining of airways due to chronic inflammation - Constriction of smooth muscles that are located around bronchi and bronchioles (also known as bronchoconstriction) - Production of additional mucus (or phlegm) due to inflammation and triggers of asthma. As a result, asthmatics suffer from these classical symptoms of asthma - wheezing (whistling or rattling sounds in the chest) - coughing - chest tightness - difficulty breathing. Common asthma triggers include following factors: pollen, house dust mites, mold, cigarette smoke, and pet dander. However, there are many other factors that can trigger an asthma attack. These include lower respiratory infections, overeating, anxiety, psychological stress, exercise, and even laughter. Acute asthma exacerbations (asthma attacks) and even deaths are most likely to occur during early morning hours or from about 4 to 7 am during sleep. There are two lifestyle factors that dramatically increase chances of these problems at night. These are: - sleeping on the back (supine sleep) - and mouth breathing. The common approach to asthma is to use bronchodilator medication (such as Ventolin) that relaxes the smooth muscles of airways and dilates them allowing easier air flow. Bronchospasm can be reversed with avoidance of triggers or elimination of inflammation. Apart from using medication, 6 randomized controlled clinical trials showed that people with asthma can reduce or eliminate their symptoms of asthma and main bronchodilator medications by about 90% using breathing exercises to slow down breathing back to the medical norms. This method of alternative medicine is called the Buteyko breathing technique. It has rating from the British Thoracic Society showing much higher efficiency of this method for asthma than yoga, meditation, nutritional intervention, or any other tested complementary therapy, as we explained in 3D animation above. Asthma Asthma is a long term condition affecting the airways of the lungs, causing difficulty breathing and this is commonly seen among children. Now the mechanism of asthma is such, our airways are tubes surrounded by muscle and they have an inner lining. Now, upon exposure to an asthma trigger, the muscles contract and tighten thereby narrowing the airway tube. The inner lining then get swollen, secondary to the irritation and also pouring out of mucus into the tube, which further compromises this narrowed airway. The result of such narrowing leads to difficulty with breathing and all the symptoms that we would see in asthma. Causes In most cases, the cause for asthma is not well understood. However, there is a strong genetic predisposition as most of the parents have a strong family history of allergies, eczema or asthma. Environmental exposure to irritants in such patients triggers off an asthma attack. Different triggers affect different children and each child may be affected by one or more factors. Asthma Triggers Now among your triggers, you have allergic triggers and non-allergic triggers. Your allergic triggers include – dust, pests like dust mite, rodents, cockroaches, molds, pollen, the tree and flower pollen, grass and weeds, pets where in the animal fur dander can act as an allergen and strong odour like perfumes, air fresheners, cleaning products act as triggers. Now, your non-allergic triggers include respiratory infections like cold and flu, cigarette’s smoke, pollution, change in weather, exercise, emotional factors like laughing out loud, crying, anger, anxiety and certain medications like non-steroidal, anti-inflammatory medications. Symptoms The symptoms of asthma include prolonged cough that is worsened by colds or upon activity, night time cough leading to disturbed sleep, shortness of breath maybe associated with fast breathing and muscles retractions which are evident under the ribs. Wheezing is the most common symptom, which is a whistling sound mostly on breathing out. There can be a feeling of tightness in the chest, which is described by kids as pain or a funny feeling in the chest. When to see a doctor? Now, when do you need to see your doctor? Should any of the symptoms as described earlier happened to your child, see your doctor who will give appropriate treatment and if need be might refer you to a pediatric pulmonologist for further investigations and control medications. Now prior to the visit to your doctor, pay attention to the story of the cough like what time of the day is it worse, which season brings on the worst cough and if there is any possible food or environmental exposure that’s triggering off the cough. If your child is having an abrupt onset of severe cough and wheeze, a breathlessness, is breathing fast with retractions, is unable to speak in complete sentences, the child appears sleepy or lethargic with either paleness or a bluish discoloration of the lips, you have to definitely call for an ambulance or rush him to the nearest hospital to be seen in the emergency room. Treatment and Management There is no cure for asthma. But it can be treated and managed well, so as to allow your child to participate in all activities of daily living. Recognizing the trigger factors and addressing it to your best ability, reduces asthma flare-ups. Now the medications are different for the different degrees of severity. Asthma can be classified as mild, moderate and severe based on the symptoms by your doctor and the treatment includes some short term and long term medications. Now some medicines are given as pills and others are given in the form of inhaler with spacers or with a nebulizer, which is a machine with a mask and this allows medicines to be delivered by mist droplets to the lungs. You need to formulate an asthma plan with your doctor and this plan needs to include, how to deal with trigger factors, what medicines that need to be given in a specified required situation and what you can do in the event of flare-ups or severe attack until you can reach the hospital for further care. What is exactly Asthma? Asthma is an inflammatory disorder of the airways, which causes attacks of wheezing, shortness of breath, chest tightness, and coughing.Causes, incidence, and risk factors Asthma is caused by inflammation in the airways. When an asthma attack occurs, the muscles surrounding the airways become tight and the lining of the air passages swells. This reduces the amount of air that can pass by. In sensitive people, asthma symptoms can be triggered by breathing in allergy-causing substances (called allergens or triggers). Common asthma triggers include: 1| Animals (pet hair or dander) 2| Dust 3| Changes in weather (most often cold weather) 4| Chemicals in the air or in food 5| Exercise 6| Mold 7| Pollen 8| Respiratory infections, such as the common cold 9| Strong emotions (stress) 10| Tobacco smoke Symptoms Most people with asthma have attacks separated by symptom-free periods. Some people have long-term shortness of breath with episodes of increased shortness of breath. Either wheezing or a cough may be the main symptom.Asthma attacks can last for minutes to days, and can become dangerous if the airflow is severely restricted. Symptoms include: 1| Cough with or without sputum (phlegm) production. 2| Pulling in of the skin between the ribs when breathing (intercostal retractions). 3|Shortness of breath that gets worse with exercise or activity 4|Wheezing, which: Emergency symptoms: Bluish color to the lips and face Decreased level of alertness, such as severe drowsiness or confusion, during an asthma attack Extreme difficulty breathing Rapid pulse Severe anxiety due to shortness of breath Sweating Other symptoms that may occur with this disease: 1| Abnormal breathing pattern --breathing out takes more than twice as long as breathing in. 2| Breathing temporarily stops. 3| Chest pain. 4| Nasal flaring. 5| Tightness in the chest. Go to the emergency room if: 1| You develop drowsiness or confusion. 2| You have severe shortness of breath at rest. 3| Your peak flow measurement is less than 50% of your personal best. 4| You have severe chest pain. Prevention 1| You can reduce asthma symptoms by avoiding known triggers and substances that irritate the airways. 2| Cover bedding with "allergy-proof" casings to reduce exposure to dust mites. 3| Remove carpets from bedrooms and vacuum regularly. 4| Use only unscented detergents and cleaning materials in the home. 5| Keep humidity levels low and fix leaks to reduce the growth of organisms such as mold. 6| Keep the house clean and keep food in containers and out of bedrooms -- this helps reduce the possibility of cockroaches, which can trigger asthma attacks in some people. 7| If a person is allergic to an animal that cannot be removed from the home, the animal should be kept out of the bedroom. Place filtering material over the heating outlets to trap animal dander. 8| Eliminate tobacco smoke from the home. This is the single most important thing a family can do to help a child with asthma. Smoking outside the house is not enough. Family members and visitors who smoke outside carry smoke residue inside on their clothes and hair -- this can trigger asthma symptoms. 9| Persons with asthma should also avoid air pollution, industrial dusts, and other irritating fumes as much as possible. Now scientists have found a protein within the airways which they believe triggers all asthma attacks. Remarkably, a drug already exists which they think could deactivate the protein, raising hopes for a treatment which may be effective for any asthma patient. The team, led by Cardiff University, has already shown that the drug works in mice and in human tissue samples in the laboratory. They have now designed the first clinical trials, which could start within two years. Lead investigator Professor Daniela Riccardi said: ‘Our findings are incredibly exciting.’ The discovery came quite by chance when Professor Riccardi, formerly a bones specialist, switched from the study of osteoporosis to the study of the lungs five years ago. She realised that a protein that triggers the growth of calcium within bones also plays a role in the airways.

A new study suggest living near fracking sites may make asthma worse. Fracking is a controversial process in which water, sand and chemicals are pumped deep into the ground at high pressure to extract hard-to-reach pockets of natural gas. Dr. Brian Schwartz is a professor of environmental health sciences, epidemiology and medicine at Johns Hopkins Bloomberg School of Public Health in Baltimore. Dr. Schwartz told UPI fracking "has numerous environmental impacts that could affect health." He said the list of impacts includes noise, vibration, strong odors, air pollution, water pollution, soil pollution, heavy truck traffic, conversion of rural areas to industrial ones and stress. There are just as hard to get rid of as dust, yet with a proper filtration system and with constant cleaning of the objects from the house they can be kept away from the house for a long period of time. Mold is also a very dangerous allergen. It usually appears when there is too much humidity inside the house. Thus a dehumidifier and constant cleaning of the house can destroy it and maintain the environment safe even for an asthmatic. During spring and summer, if you suffer from asthma, it is advisable that you stay as much as possible indoors, with the windows closed. It is highly recommended that if you have an asthmatic in the house, you should avoid keeping pets inside because the animal hair is an allergen which can trigger very severe asthma attacks. If you are still going to keep a pet inside make sure that there is as little hair as possible inside. Vacuuming all the house, soft furniture and washing the bedding regularly can maintain a clean environment, thus diminishing the occurrence of asthma attacks. What is Asthma-Asthma psychopathology This week’s episode looks at what is asthma, asthma is a disorder of the respiratory system, in which patients can find it difficult to breath. Asthma is known to have 3 classic characteristics, which are airway flow limitation, airway hyperresponsiveness and bronchial inflammation. There isn’t currently a cure for asthma however we have very effective treatments which I will discuss in the next video. Asthma symptoms are known to get worse at night or in the early morning. The cause of asthma is largely believed to be both environmental and genetic and it often affects children between the ages of 3 and 5. The two types of asthma are extrinsic and intrinsic asthma. Extrinsic asthma is the most frequent form of asthma and is associated with positive skin prick reactions to allergens, this type of asthma is triggered by the environment and is mediated by systemic IgE production. Intrinsic asthma is asthma that is not as a result of an allergen. The bronchioles of someone with asthma differ to a healthy individual, the smooth muscle contracts more leading to a smaller lumen, during asthmatic attacks the mucosal lining also swells up and there is an increased production of mucus, leading to the tubes being narrowed and blocked, this can be very distressing to the patient and in some rare circumstances can even lead to death. some of the bodily factors that are involved in inflammatory response to asthma are IgE antibodies, these are able to bind onto allergens and then interact with mast cells, this leads to the release of histamines from within the mast cells causing contraction of smooth muscles. Dendritic cells are able to pick up allergens and then present them to T lymphocytes, leading to the an up regulation of type 2 t lymphocyte, which allow the production of more IgE antibodies.
