Asthma is a chronic respiratory condition where the airways in the lungs are inflamed and narrowed causing breathlessness, wheezing, chest tightness and coughing. Symptoms come as recurrent episodes known as asthmatic attacks most commonly at night or early in the morning. Pathology of asthma: The lungs contain millions of air tubes or airways, called bronchi and bronchioles, which bring air in and out of the body. The airways have a layer of smooth muscle in their wall which enables them to constrict or dilate. In people suffering from asthma these airways are inflamed, narrowed and become more sensitive to certain substances. Asthmatic attack, or exacerbation, happens when the airways react to these substances. During an attack, the smooth muscle contracts, squeezing the airways, making them even narrower; mucus secretion is also increased which further obstructs the airways. Asthma is most commonly considered as an inflammatory response disease where the body’s immune system over-reacts to certain environmental agents. Causes of asthma are complex and not fully understood but likely involve a combination of genetic and environmental factors. Family history is a known risk factor for asthma. There are at least over twenty genes associated with asthma of which many are involved in the immune system. Triggers are factors that initiate the attack. Common triggers include: - allergens, such as pollen, animal fur, pet dander, sulfites in preserved food.. - irritants, such as cigarette smoke, industrial chemicals, dust, household chemicals,.. - medications, such as aspirin, beta blockers,.. - physical activities, exercises. There is no cure for asthma. The most effective way to manage symptoms is to identify the triggers of asthmatic attacks and avoid them. There are two main classes of medication: - Bronchodilators - substances that dilate bronchi and bronchioles - are used as short-term relief of symptoms. - Inflammation moderators such as corticosteroids are taken as long-term treatments. Asthma inhalers are used to deliver the medication to the lungs. A number of conditions tend to occur more frequently in people with asthma and should be taken into account when treating asthma: - Allergies, such as eczema and hay fever. These individuals are considered hyper-allergic – they have high tendency to develop allergic reactions. The combination of these conditions is known as atopy or atopic syndrome. Immunotherapy may be recommended for this group of patients. - Gastroesophageal reflux disease or GERD - a condition in which stomach acid backs up and damages the mucosal lining of the esophagus. GERD may worsen asthma symptoms and medications for asthma often worsen GERD symptoms. Treating GERD usually improves asthma and must be included in an asthma treatment plan. - Obstructive sleep apnea or OSA – obstruction of the airway at the throat level during sleep. Asthmatic patients are at higher risks of developing OSA. The mechanism of this association is largely unknown. - Sinusitis: inflammation of paranasal sinuses. Sinusitis commonly worsens asthma symptoms and makes treatments less effective.

Bronchospasm can be reversed with avoidance of triggers or elimination of inflammation. Apart from using medication, 6 randomized controlled clinical trials showed that people with asthma can reduce or eliminate their symptoms of asthma and main bronchodilator medications by about 90% using breathing exercises to slow down breathing back to the medical norms. This method of alternative medicine is called the Buteyko breathing technique. It has rating from the British Thoracic Society showing much higher efficiency of this method for asthma than yoga, meditation, nutritional intervention, or any other tested complementary therapy, as we explained in 3D animation above. Asthma Asthma is a long term condition affecting the airways of the lungs, causing difficulty breathing and this is commonly seen among children. Now the mechanism of asthma is such, our airways are tubes surrounded by muscle and they have an inner lining. Now, upon exposure to an asthma trigger, the muscles contract and tighten thereby narrowing the airway tube. The inner lining then get swollen, secondary to the irritation and also pouring out of mucus into the tube, which further compromises this narrowed airway. The result of such narrowing leads to difficulty with breathing and all the symptoms that we would see in asthma. Causes In most cases, the cause for asthma is not well understood. However, there is a strong genetic predisposition as most of the parents have a strong family history of allergies, eczema or asthma. Environmental exposure to irritants in such patients triggers off an asthma attack. Different triggers affect different children and each child may be affected by one or more factors. Asthma Triggers Now among your triggers, you have allergic triggers and non-allergic triggers. Your allergic triggers include – dust, pests like dust mite, rodents, cockroaches, molds, pollen, the tree and flower pollen, grass and weeds, pets where in the animal fur dander can act as an allergen and strong odour like perfumes, air fresheners, cleaning products act as triggers. Now, your non-allergic triggers include respiratory infections like cold and flu, cigarette’s smoke, pollution, change in weather, exercise, emotional factors like laughing out loud, crying, anger, anxiety and certain medications like non-steroidal, anti-inflammatory medications. Symptoms The symptoms of asthma include prolonged cough that is worsened by colds or upon activity, night time cough leading to disturbed sleep, shortness of breath maybe associated with fast breathing and muscles retractions which are evident under the ribs. Wheezing is the most common symptom, which is a whistling sound mostly on breathing out. There can be a feeling of tightness in the chest, which is described by kids as pain or a funny feeling in the chest. When to see a doctor? Now, when do you need to see your doctor? Should any of the symptoms as described earlier happened to your child, see your doctor who will give appropriate treatment and if need be might refer you to a pediatric pulmonologist for further investigations and control medications. Now prior to the visit to your doctor, pay attention to the story of the cough like what time of the day is it worse, which season brings on the worst cough and if there is any possible food or environmental exposure that’s triggering off the cough. If your child is having an abrupt onset of severe cough and wheeze, a breathlessness, is breathing fast with retractions, is unable to speak in complete sentences, the child appears sleepy or lethargic with either paleness or a bluish discoloration of the lips, you have to definitely call for an ambulance or rush him to the nearest hospital to be seen in the emergency room. Treatment and Management There is no cure for asthma. But it can be treated and managed well, so as to allow your child to participate in all activities of daily living. Recognizing the trigger factors and addressing it to your best ability, reduces asthma flare-ups. Now the medications are different for the different degrees of severity. Asthma can be classified as mild, moderate and severe based on the symptoms by your doctor and the treatment includes some short term and long term medications. Now some medicines are given as pills and others are given in the form of inhaler with spacers or with a nebulizer, which is a machine with a mask and this allows medicines to be delivered by mist droplets to the lungs. You need to formulate an asthma plan with your doctor and this plan needs to include, how to deal with trigger factors, what medicines that need to be given in a specified required situation and what you can do in the event of flare-ups or severe attack until you can reach the hospital for further care. About 1 in 20 adults have chronic obstructive pulmonary disease (COPD), which is the fourth leading cause of death in the U.S. Experts think the two lung conditions are unrelated, although both can cause coughing, shortness of breath, and other symptoms that respond to airway-relaxing drugs, known as bronchodilators. But could there be a link? A recent study by researchers in Australia found that more than 40% of children with severe asthma developed COPD by the age of 50—a 32-fold higher risk compared to asthma-free children. The researchers followed nearly 200 children beginning at age 7. "This certainly raises the point that early and aggressive treatment of asthma may indeed prevent the development of COPD later in life," says Robert Wise, MD, a professor of environmental health sciences at the Johns Hopkins University School of Medicine, in Baltimore. But it's too early to be sure, Dr. Wise says. What is COPD? Experts believe that in some people, asthma—particularly severe asthma—raises the risk of COPD later in life. Asthma, an inflammatory condition, can thicken the walls of lung airways and narrow air passages, a process known as remodeling.
