Tagalog/Eng/Nat Viagra, the headline grabbing anti-impotency pill, is now on sale in the Philippines. Pfizer, who manufacture the drug, have started delivering to local drugstores in Manila, two weeks after the local Bureau of Food and Drug gave its approval for its sale in the country. Pharmacies in the Philippines have finally begun to receive their stocks of Viagra. The manufacturer Pfizer, started deliveries of the new wonder drug at drugstores in Manila this week. Some stores already began selling the drug on Tuesday. With Viagra becoming the talk of the town, drugstore owners are anticipating a huge demand. Pharmacy owner Bhoyette Vizconde sold five tablets on the first day. SOUNDBITE: (English) "I already hoard (laughs) stocks because I'm really expecting that being the first to have Viagra here in our vicinity, I'm sure a lot of people will come here and inquire or buy Viagra." SUPER CAPTION: Bhoyette Vizconde, Owner of Marcelo's Pharmacy Pharmacy Plus received its first supply early this week. Owner James Sandoval, who owns several stores in the capital, put a few boxes of the pill on the shelf to show clients that the drug is now available. But before the drug was put on sale in his stores, Sandoval issued a memo to all his store managers to ensure that safety precautions are practiced before the drug is dispensed to patients. The B-F-A-D says it is important the pill is only sold to patients who come with a legitimate prescription from their doctors. Each pill comes with a leaflet giving warnings of possible side-effects if the drug is not taken properly. Pfizer warned that the drug should not be taken by people being treated with nitrate-based medication for heart problems. While he expects a huge demand for the drug, Sandoval said Filipino men may be a little shy of asking for the drug themselves. He says the men here will probably not want to admit they need Viagra and so he is arranging a home delivery service to put them out of their misery. SOUNDBITE: (English) "Because maybe at the secrecy of their own home, they can acquire the product because they might be too embarrassed to come forward to admit that they have a sexual problem. Any person that comes forward asking about the product - they're so defensive, they say right away "Hey listen I don't have a problem with this. I'm just inquiring". That goes to tell you how sensitive the issue is." SUPER CAPTION: James Sandoval, Owner of A-B Sandoval Drugstore Dr. Jesus Mendoza is a local doctor who has been checking out the testing of the Viagra drug. SOUNDBITE: (English) "Well, I was pretty excited and wanted to find out whether it was really going to work. Of course, there was some degree of skepticism at first. But when we started seeing back the patients who were on the study, then it was confirmed that my gosh, this works." SUPER CAPTION: Dr. Jesus Mendoza, Urologist In a press conference early this month, Pfizer said the approval from B-F-A-D came after they completed a six-month study of its effects on 500 men from six Asian countries, including the Philippines. The man who discovered Viagra, Dr. Peter Ellis, was in Manila last week and spoke of how his drug differs from the others already available in the market. SOUNDBITE: (English) "And I think the beauty of the drug is that it works to enhance the natural process. All the other products that are out there, you take the product, you get an erection. But Viagra, you take the product, you don't get an erection but it's only when sexually aroused. So it's basically enhancing a perfectly natural function and restoring it back to normal." Urdu/Eng/Nat Doctors in Pakistan are warning of the dangers of men buying the impotence pill Viagra on the black market and taking it without prescription. The sale of the drug in the country is currently being debated by the government - it says it wants to make sure the drug will be made available in the safest possible way. But medical experts say the problem of impotency is high in the country and that men are anxious to try out supplies of Viagra introduced illegally at a high price. Viagra, the impotency pill, is proving to be big business in Pakistan. The drug, first released in the U-S in March 1998, is legally available in more than 50 countries worldwide. Many people consider it the beginning of a new revolution in pharmaceuticals. In Pakistan, the government is currently discussing when and how to introduce the drug. But Viagra has already found its way onto the black market in the country's capital Islamabad and can be bought at a high price. One tablet can cost anything from 20 to 50 U-S dollars - a bottle of Viagra tablets can cost anything from 360 to 800 U-S dollars. Some Viagra users say the drug has been freely available to them for some time now and that they're prepared to pay up. SOUNDBITE: (Urdu) "Viagra is a sex drug and I have been using it for about two years and found it very good, because it adds energy to the body if we use it two hours before sexual intercourse. I have found no other medicine better than Viagra and the demand for it is now growing." SUPER CAPTION: Abdul Qadir, Viagra user The country's health department has so far been reluctant to legalise Viagra - but it is aware of the black market problem and is prosecuting anyone found selling the drug. Government officials say they want to make sure that there are no loopholes before Viagra is legally available in Pakistan. So nobody can buy it without prescription, while in Pakistan we still have some loopholes, some weaknesses where the drug can be sold over the counter and this is my main worry in resisting to register it." But Pfizer, the company that produces Viagra, maintains the drug is a safe medicine for erectile dysfunction when used as recommended in the approved package insert. There are already many legal drugs on sale in Pakistan to help enhance sex. Herbal doctors offer remedies for impotency, claiming they're natural products that have no side effects. The herbal remedy sellers can often be found on roadsides doing a roaring trade - but even they now say that the new wonder drug is threatening their trade. Medical experts say there is no official impotency figure in the country because Pakistani men are reluctant to admit they have a problem in a nation where sex is a taboo subject. But doctors say it is an ever increasing problem and they are warning of the dangers of taking the impotence pill without prescription. Doctor Khalid Saeed said it is even more of a worry as impotency affects men of all ages. SOUNDBITE: (English) "There are two basic main groups in which we can find this who come to us commonly. One is the younger age group - they are really people in their early 20s and the other group is somewhere in the late 40s and 50s and both of them have different presentations. People in the younger age group generally come to you just before they are about to get married." Hindi/English/Nat Viagra may have made a sensational debut on the Western market but a different kind of competition awaits the anti-impotence pill in India. Favourable patent laws have spurred a race among the Indian pharmaceutical companies to produce a Viagra clone. However, India's traditional doctors have snubbed the Viagra hype, claiming their ancient herbal sex potions are far more potent - and cheaper - than any modern day pill. The race for the Indian version of Viagra is on. Just months after the anti-impotency pill hit the Western world, dozens of Indian versions of the drug are waiting for approval by the Indian government. India is one of world's largest drug industries but it only recognises process patents. This has prompted "copycats" to try to develop a slightly different process to make a Viagra clone. Presently, about 10 Indian pharmaceutical companies are in the race to process Sildenafil Citrate - the active ingredient in the Viagra wonder pill. The competition is so fierce that some companies have already started exporting the ingredient - even though they are still waiting for permission to market the product locally. What's driving these companies is a potential market of one (b) billion people - and that means big dollars. And price advantage will be there, it is definite... it is almost certain." On the streets of the Indian capital, hundreds of pavement doctors line the streets peddling traditional cures for impotence. For these roadside doctors, a strong belief in the potency of their magic potions is more important than conducting clinical trials. They claim their remedies offer vigour and vitality - using the rejuvenating properties of well known Indian herbs. These traditional remedies feature heavily in Indian folklore about Mughal emperors and kings who relied on herbal aphrodisiacs to keep their numerous wives satisfied. What's more - they're incredibly cheap. For a country where the problem lies more in ignorance than impotence, traditional alternatives are generally favoured. Sex clinics which offer indigenous cures boast a roaring trade. Run by a family of sexologists who practise traditional medicine, Sablok clinic has been in the business for nearly 70 years. Dr. Sablok scoffs at the hype created by Viagra and says the Indian masses just can't afford the expensive blue pill. Mandarin/Nat A hundred thousand tablets of the impotence drug Viagra have finally entered Taiwan's pharmaceutical market and legislators are worried that Taiwan women would become sex tools as a result. The Pfizer-manufactured potent drugs were flown into Taiwan on Tuesday and hospitals across the island were advised by the Health Department that all Viagra distributed to hospitals must be locked up in the director's safe box. Black market Viagra can be bought for U-S 65 dollars per tablet, but is now available through a doctor's prescription. However, Health Department officials warned hospitals that if the imported Viagra was not locked up, the drug could be stolen. Meanwhile various women's right groups have expressed that with the open availability of the drug, Taiwan women's status in a male dominated society would be more degraded than ever. Doctors and urologists say Taiwan men in general don't need Viagra and only purchase them out of curiosity. SOUNDBITE : (Mandarin) "About 52 per cent of the Taiwan men are impotent but not too seriously. The men that really have impotency problems lie around 20 per cent. But in Taiwan, only one out of ten men need Viagra." The Legislative Council saw lawmakers expressing agony over the fad of Viagra in Taiwan. One lawmaker said the last thing Taiwan needs is Viagra. If you watch any TV then you’re very familiar with the Cialis and Viagra commercials targeted toward men with erectile dysfunction. Nitric Oxide research played a critical role in the development of these two products. Now for many of you I've introduced the word phrase Nitric Oxide, which you've never heard of before or may be vaguely familiar with. Outside your body nitric oxide is considered an environmental pollutant but inside your body it's a signaling molecule. What this means is that it causes something else to occur. And, without it that event cannot take place. For men nitric oxide is absolutely necessary for an erection. In fact, the bottom line is this: No NO No Sex! Little nitric oxide production and you will have a little to weak erection. As the medical and pharmaceutical community came to understand this need for nitric oxide to initiate and maintain an erection they looked for ways to address this problem. To help you understand how ED medications are used to address erectile dysfunction it's helpful to understand a little bit about male anatomy. The male organ contains two cigar-shaped structures called corpora cavernosa that run the length of the organ. Arteries bring blood into these two structures with veins carrying the blood away. In a non-erect state, the blood flow from the arteries is restricted while the veins are open to drain blood away. When a man becomes aroused, the brain sends a signal to reverse this process. The arterial blood vessels open up so that pressurized blood can enter the corpora cavernosa. The veins leaving the male organ are constricted. This action traps the pressurized blood allowing for the size increase and full erection. Unfortunately most ED medications don't address the real cause of your problem, which is a lack of nitric oxide production. Rather than learn how to improve the health of your endothelium and increase its ability to produce nitric oxide, manufacturers of Viagra like products went in a different direction. If you remember from above, there are two competing actions at play. Vascular dilation for improved blood flow and Vascular constriction which limits blood flow. Both are controlled by an enzymatic pathway. Without getting bogged down in the complex interplay between these two action let's put it into its simplest terms. Nitric oxide acts as a signaling molecule to stimulate an enzymatic process that creates cGMP, which then causes the smooth muscles of the arterial blood vessel to relax. This results in increased blood flow to start and maintain an erection. There is another enzyme called phosphodiesterase or PDE that deactivates this cGMP. This results in the smooth muscle returning to its original constricted state. If this action didn't occur then men would walk around all day long with an erection. What the manufacturers of Viagra and Cialis did was figure out a way to disable the PDE part of the cycle by introducing a chemical called sildenafil citrate into your system. This chemical attaches itself to the PDE5 enzyme to disable the Vascular constriction phase allowing the nitric oxide to continue to influence the Vascular dilation allowing you to create enough increased blood flow to have and maintain an erection. This works great if you're producing nitric oxide. But if you're not then this method will not work or work very poorly. As this image clearly shows the aging process results in a significant decrease in nitric oxide production. Most of this decrease is due to damage done to the endothelium resulting in plaque formations and/or calcification.
